We did an episode about Mucormycosis a while back, and here we are touching on it again, but this time about the risk factors. This is an emergent disease in marine mammals of the Salish Sea, and particularly harbor porpoises who it seems to affect more than other species. In addition, porpoises are a good surrogate for how this disease can affect Southern Resident Killer Whales that are critically endangered. But in order to understand what the risk is, we need to know how they get the fungus, how likely are they to be exposed to it, how severe is the infection, what does it look like, and what are the cofactors that may affect if a porpoise gets the disease? In this paper the authors look at a lot of different factors, studies and what we know from other species and combine them into a qualitative risk assessment for harbor porpoises that is the first step to better understanding this disease and its effect on Salish Sea marine mammals. Mucormycosis has a death rate of about 54% according to the The United States Centers for Disease Control and Prevention. Gujarat and Maharashtra are the worst affected with almost 3,000 cases each of mucormycosis so far. The Delhi High Court has ordered duty free import of mucormycosis medication Amphotericin B, while US-based Gilead Sciences is preparing to supply 1 million doses to India. But availability is not the only issue. The steep price of mucormycosis treatment is costing between five to fifteen lakhs per patient. Among the survivors of mucormycosis, some are losing their eyes due to the fungal infection. On this episode of The Suno India Show, our reporter Suryatapa Mukherjee speaks to Dr Akshay Nair, an oculoplastic surgeon in Mumbai. Fungal diseases are rare but, once diagnosed, incredibly hard to treat and often fatal. The overwhelming surge of COVID-19 cases in India has given rise to mucormycosis, also called “black fungus” for the appearance of the lesions caused by the infection. Dr. Arturo Casadevall talks with Stephanie Desmon about this and other fungal diseases, why COVID patients in India are particularly vulnerable, why treatments are slow and often ineffective, and why the pharmaceutical industry hasn't invested more in treating these often deadly infections.

India has so far recorded close to 12,000 cases of mucormycosis, or black fungus infection as it is commonly known. The Central government has now declared it a notifiable disease. Mucormycosis was not unknown in India, but it is now, with the second wave of the COVID-19 pandemic continuing to rage, that the country has seen a surge in these infections. While the increased use of steroids, needed for the treatment of severely ill COVID-19 patients is being cited as one possible reason, experts have also said that a majority of cases are seen in those with poorly-controlled diabetes. Mucormycosis can affect many organs in the body, but what is being seen now, is the rhino-orbito cerebral form -- the infection affects the sinuses, nose, eyes and then brain. Doctors have reported that patients come to them a few weeks after recovering from COVID-19, with symptoms of mucormycosis. Amphotericin B is the main antifungal drug used in treatment, though stocks are running low now in the country due to high demand. The Centre recently said five new pharma companies had been approved to produce Amphotericin-B. Earlier, only six firms were manufacturing it. The Centre on Thursday directed all state and UT governments to declare Mucormycosis, known as ‘Black Fungus', a notifiable disease under the Epidemic Diseases Act, after reports came about 126 deaths due to this deadly disease till now across India. Nearly 5,500 cases have been officially reported till now from different states.

Over the past few days, we've been reading a lot of reports of a rare but fatal fungal infection that is being detected amongst Covid survivors and recovering Covid patients. Mucormycosis or the Black Fungal infection is said to spread rapidly through the sinuses and lungs leading to a loss of eye sight and in certain instances even affecting brain functions. Sri Ganga Ram Hospital in Delhi reported at least six cases of the Black Fungal infection on 6 May. Two days later on 8 May, Maharashtra reported that 200 such patients were being treated in the state, amongst whom at least 8 people have lost their lives. In Gujarat which has at least 100 reported cases of mucormycosis as of now, the state government has announced separate wards in Covid hospitals to treat such patients.While this sudden upsurge in cases of the infection is raising alarms, doctors say that it's not a new infection at all. Although it's not a very commonly occurring one, black fungal infections have been detected amongst immunodeficient patients mostly but what seems to be triggering this infection amongst certain Covid survivors now, is an injudicious or perhaps untimely use of steroids that are being used to treat coronavirus.
