I wrote thousands of posts and started sprinkling in audio interviews with healthcare CEOs in 2007 –long before the podcast boom.  My blog became very popular and as a result I even switched the name of my healthcare consulting firm to Health Business Group to take advantage of the blog's brand equity.The biggest podcast highlight of that era came in 2014, when I interviewed all 9 candidates for Governor of Massachusetts about their plans for healthcare. It was my goal to help elevate the political debate.In 2017, I started co-hosting the CareTalk podcast with John Driscoll, then CEO of CareCentrix and now Chairman of the UConn Health System. We recorded monthly, in-person and experimented with a variety of formats.When the pandemic arrived, John and I switched to remote recording and began taping weekly. There was so much to discuss about COVID-19 and we made the most of it. Our producer, Vincent Grippi masterfully built up our show to one of the top few on Apple Podcasts and YouTube.Meanwhile, I also shifted the Health Business Blog to an all-podcast format in September 2020, about six months into the pandemic, with Dafna Williams taking over as producer. I called it the HealthBiz podcast and we released a new episode every week on all the major audio channels plus YouTube. We're now well over 200 episodes.In 2024, we tested out the idea of melding HealthBiz and CareTalk by adding HealthBiz Briefs to the CareTalk channel. The experiment was a success, generating substantial engagement.In 2025, we're going all the way by moving all of HealthBiz over to CareTalk.     An Enlightened Approach to Hernia Surgery with Dr. Reinhorn and Dr. Fullington

    I had a great experience with my inguinal hernia surgery this summer, so I just had to invite my doctor, Nora Fullington and her  partner, Dr. Michael Reinhorn from Boston Hernia onto the podcast!  A few takeaways:The vast majority of hernia repairs are performed by general surgeons in hospital operating rooms. But better outcomes, lower cost, and greater convenience are possible if you can find a specialized practice like I did and go to an ambulatory surgery center. (Only 3% of inguinal hernias in Massachusetts are repaired in ASCs, which is nuts.)Despite the superiority of the Boston Hernia model, insurance companies and big health systems make it hard for patients to go this route.You shouldn't be scared of mesh as long as your surgeon knows what they're doingPatients are often mis-diagnosed with hernias as a result of medical imaging.     As a physician and healthcare CEO, Russ Richmond already knew a lot about healthcare. But when a bad accident put him into the hospital for three months he developed a much more profound understanding of caregiving.In particular, he learned that many of the nurses and therapists who provided him with such great care were thinking of quitting their jobs!Once he was released he did some research and discovered that a root cause of burnout was a lack of effective management and high administrative burden.     Ending polycystic kidney disease. Interview with PKDO founder Richard Kellner

    Richard Kellner is on a mission to end polycystic kidney disease (PKD), a leading cause of kidney failure in younger people. The key is combining  pre-implantation genetic testing with in vitro fertilization to prevent the disease from being passed from parent to child. The PKD Outreach Foundation, run by Richard is raising awareness so that physicians and parents-to-be know about this option. PKD Outreach is also making testing and IVF more affordable and accessible through a partnership with US Fertility and by making cash grants to families.In addition, PKD Outreach is  publicizing ways to slowing the progression in young adults with lifestyle and drugs, and supporting live kidney donation.Richard has a compelling personal story. His late wife passed away from PKD, even after receiving a kidney donation from Richard. His adult kids have it, too.I like the organization's mission and determination so I decided to donate.     PatientPay and ClearGage are merging. I interviewed CEO Tom Furr to get his take. The combined offering is intended to deliver a comprehensive billing and payment platform that includes estimation and eligibility along with streamlined patient payments.Doctors offices are generally set up to bill insurance, but patients are now responsible for a lot more of the bill. Offices that manage patient payments effectively perform much better financially and reduce friction. That's what PatientPay enables.Patients (i.e., consumers) have become accustomed to smooth transactions through Amazon and other e-commerce and in-person experiences.     Ironically, patient care coordination itself is often uncoordinated. It's spread across multiple providers and administrators, causing frustration for care teams and patients.Lumeon founder and CEO Robbie Hughes has developed an approach that makes the care journey more reliable rather than settling for the current craziness, where patients are expected to be satisfied as long as they get some time with a doctor.One of the really interesting elements of this interview was the discussion of whether we should be willing to trade off some efficacy for reliability.     Matthias Hoffman first became conscious of his vision as a teenager. He and his friends in Switzerland dreamed of becoming pilots (none actually did) but Matthias's hopes were dashed early on due to inadequate vision.While pursuing electrical engineering degrees, he realized his strength was in building things. So he left academia and worked in a variety of vision and optical startups, eventually leading significant teams.Many people who could benefit skip their eye exams, and for more than a decade entrepreneurs have been trying smartphone based eye exams and online eyeglass sales, often with virtual try-ons. But those have been failures. The exam interface is too complex and burdensome, and people want to try on their glasses at retail.Eyebot is an automated eye exam designed to produce a prescription at the point of sale, i.e., retail stores. It takes 90 seconds and doesn't require the person being examined to do anything special. Eyebot has developed its own technology and eventually plans to expand beyond the basic exam to more advanced screening for things like glaucoma.But Matthias stresses that Eyebot has no intention of taking away the jobs of eye care professionals.Host David E.
