In this podcast episode we want to introduce you to our BCEN Friend, Adam Sorenson. Graduating in 2012 with his BSN, Adam Sorenson began his career as a pediatric nurse before transitioning to a role as a pediatric intensive care nurse. After a few years in the PICU, Adam shifted to working in an adult surgical intensive care unit (SICU) at a Level 1 trauma center, where he continues to work today. Adam's professional development includes advanced training, starting with Trauma Nursing Core Course (TNCC) and Advanced Trauma Care Nursing (ATCN) to obtaining his certification as a Trauma Certified Registered Nurse (TCRN). In 2014, during the Ebola outbreak in Central and West Africa, Adam joined the Highly Infectious Treatment Team (HITeam) at Denver Health Hospital, an all-voluntary group established to handle such crises. Denver Health was designated as an Ebola regional treatment center in 2015. In 2021, Adam took on a part-time role as a Biocontainment Unit Nurse Educator, focusing on training, education, and preparedness for viral hemorrhagic fevers, rare respiratory viruses, and novel viruses. Additionally, Adam has served as a nurse in the Air Force Reserves for the past five years. Pat Hirsch, a nurse who has a depth of experience in critical care and PACU units, describes some of the common types of liability in this setting and how PACU personnel handle them. The possibility of falling always looms large in a PACU. Patients may be disoriented, have pre-existing cognitive issues, visual disturbances, language barriers, and other issues. Where Pat works, low-, medium-, and high-risk patients are pre-identified. High-risk patients are given yellow socks and wrist bands, and other precautions are set into place. Breathing issues may be common for those with COPD, lung restrictive conditions, and sleep apnea. CO2 levels must be monitored. Many simple physical techniques, like having a conscious patient cough, can resolve the issues, but sometimes more complex maneuvers are required. Being sure that pulse ox monitors are working correctly is an important part of the equation. PACU personnel need to be particularly aware of the possibility of internal bleeding by recognizing its signs. This serious condition may sometimes require a return to surgery. He's been to all 50 states and loves to travel outside the US, but, for now, Covid has put a damper on that. Allen also loves college sports and has been to the NCAA Men's basketball games every year since 1998. His new love is watching Women's College gymnastics – it's just amazing. Allen a Critical Care Nurse who worked in the hospital for about 7 years in the CVICU, SICU and ED. Allen completed his Masters in Critical Care Clinical Specialist from Marymount University in VA. Dr. Joel Topf joins us on Rounds to discuss and review key concepts in the recognition and management of acute kidney injury (AKI) in the SICU. AKI is a common and morbid complication among hospitalized patients. Further, trauma and surgical patients, in particular, are at an increased risk for AKI due to the myriad of pre-, intra-, and postrenal insults that commonly occur at the time of injury, during resuscitation, surgery,, as well as from iatrogenic insults including IV contrast, NSAIDs, antibiotics (aminoglycosides and the infamous Pip/Tazo/Vanco ice cream sandwich). From the use of a DDAVP clamp in patients with severe hyponatremia to the use of balanced solutions in critically ill patients, kidney_boy breaks it down for us as only a true salt whisperer can!

A left groin AV loop graft was pursued due to multiple access point failures in the past secondary to severe peripheral artery disease. Pre-op evaluation included risk stratification with RHC which was consistent with WHO Group 2 pulmonary HTN and diffuse atherosclerosis in the RCA on LHC. Intra-op, patient had an episode of significant hypotension after administration of protamine that required phenylephrine and ephedrine. In the PACU, his BPs continued to be low (70s/40s mmHg), requiring admission to the SICU where cardiology was consulted.  In the SICU, patient had ongoing hypotension despite pressors and fluids. Exam demonstrated a systolic murmur consistent with TR and elevated JVP. Labs were notable for a mild elevation in liver enzymes, elevated troponin, high NT-proBNP and elevated lactate. TTE demonstrated a moderately dilated and hypokinetic RV, elevated RVSP and evidence of pressure/volume overload. CTA abdomen/pelvis demonstrated extensive mesenteric atherosclerosis and signs of gastric ischemia. Patient was treated for RV failure with norepinephrine, inhaled epoprostenol, and CVVHD for volume removal. He became febrile and was treated empirically with broad spectrum antibiotics. Due to concern for the new loop graft causing high output heart failure vs RV failure, it was temporarily occluded for testing and then permanently ligated by vascular surgery with significant improvement in his BPs and RV function on repeat TTE.  Case Media ABCDEClick to Enlarge A. Heather Hart, Nurse Practitioner at Vanderbilt University Medical Center, comes on to talk about her journey through Nursing and Nurse Practitioner school, what her education from Nursing and Nurse Practitioner entailed, the similarities and differences between these 2 programs,  some of the troubling issues currently facing nursing education, where she sees the future of nursing/ nurse practitioner education moving towards, her take on "work/life balance", characteristics of successful nursing educators, and more! Biography: Heather Hart received her BSN from Belmont University in 2009 and began her nursing career at Skyline Medical Center on the Cardiology Step-Down floor. There she advanced to a charge nurse position where she developed a love for critical care nursing. She accepted a position at the Vanderbilt Surgical ICU in 2012 where she also worked as a charge nurse and later took on the role of Clinical Staff Leader.  She has chaired several committees at Vanderbilt including the SICU Performance Improvement Committee, SICU Quality Subcommittee, and Nurse Champion Subcommittee for the Organ Donation Advisory Council at Vanderbilt. During her tenure in the SICU she was awarded the Rosamund Gabrielsson VUMC Staff Nurse of the Year in May 2016. In this EAST Traumacast, the state of Early Goal Directed Therapy is discussed with two dynamic speakers, Dr. Donald Yealy who is the lead author on the PROCESS trial , and Dr. Ruby Skinner who is a busy SICU director.  This and two other studies prompted changes in the Surviving Sepsis Campaign Guidelines.  Drs.
