Acute kidney injury is a frequent complication of critical illness, and a subgroup of these patients will require renal replacement therapy. In this episode. Acute Kidney Injury is common, complicated and holds significant morbidity and mortality. But...if we recognise it, we can make a real difference to our patients' outcomes. In this episode we run through the anatomy, physiology and aetiologies. We have a think about the multitude of definitions of AKI and then take each of the pre renal, renal and post renal categories and think about the ways we can optimise our care in each. We also have a think about who needs to be admitted and who can be safely managed in the community. This was a hugely valuable episode for us all to research and bring clarity to a complicated topic, we hope it does the same for you too! At Mission Therapeutics, Sarah Almond serves as their Head of Pharmacology, but she wears many hats to help develop potential treatments for neurodegenerative conditions. Their main area of focus centers around research into deubiquitinating enzymes (DUBs), which may impact neurodegeneration. By studying DUBs, Sarah and her team have been able to generate highly targeted and potent molecules that contribute to developing safe and effective therapeutics. In fact, one of these compounds (MTX325) is currently enrolled in a clinical trial to test its safety and pharmacokinetics ahead of effectiveness in treating Parkinson's disease, a neurodegenerative condition impacting the central nervous system. With the first patient dosing scheduled for later this year, they are hopeful it could be a gamechanger for treating these debilitating conditions. A new study examines the health utility of patients at 60 days after dialysis-requiring acute kidney injury and identifies 3 subgroups of AKI survivors with distinct health utility profiles.

Sepsis‐associated acute kidney injury (SA‐AKI) is a common, increasingly prevalent problem in the intensive care unit (ICU). The association between sepsis and AKI has been studied previously. However, the lack of a reproducible and standardized consensus definition has limited the interpretability of available knowledge. In order to assess SA-AKI incidence, patient characteristics, timing, trajectory, treatment, and associated outcomes a multicenter, observational study was conducted. We discuss renal replacement therapy, compare modalities, and review the history of CRRT as an introduction. Then we review two studies: “Intensity of Continuous Renal-Replacement Therapy in Critically Ill Patients: RENAL study” published in 2009 and “Timing of Renal-Replacement Therapy in Patients with Acute Kidney Injury and Sepsis: IDEAL-ICU study” published in 2018, both in the New England Journal of Medicine.
