366 Midair Collision at Reagan National - Tragedy Over the Potomac

Aviation News Talk podcast

Play Episode Listen Later Jan 31, 2025 16:51

Max discusses the tragic midair collision at Reagan National Airport between a PSA Airlines CRJ700 regional jet and a U.S. Army Black Hawk helicopter. The midair crash over the Potomac River was captured by a Kennedy Center webcam. ATC instructed the Army helicopter to follow the airliner to Runway 33, but it did not comply. The helicopter was not transmitting ADS-B Out, though its position was tracked using Mode S transponders. TCAS alerts are disabled below 400 feet, meaning the pilots had no automated warning. Nighttime perception challenges likely contributed to the crew's failure to see and avoid each other. Trescott reviews ATC audio and past midair collisions, emphasizing the Swiss Cheese Model of accident causation—multiple failures aligned to cause the crash. A reported FAA radar screen showed collision alerts 30 seconds prior, but no action was taken. He urges pilots to use traffic displays with relative vectors for better situational awareness, particularly at night. The episode ends with a call to fly defensively and use all available tools to prevent midair collisions. In this replay of episode #398 of the Aviation Careers Podcast, guest Max Trescott discusses strategies for pilots to stay safe while building flight time toward the 1,500-hour requirement for airline positions. Emphasizing the higher risk associated with general aviation compared to commercial airline flying, Trescott highlights key factors such as the "killing zone" for low-time pilots and the importance of accumulating time in type rather than just total flight hours. He underscores the necessity of a professional attitude, akin to that expected in airline operations, and adherence to Standard Operating Procedures (SOPs). Trescott warns against complacency and overconfidence, stressing continuous learning and thorough understanding of aircraft limitations. He advises maintaining safe margins, avoiding risky behaviors, and being aware of the heightened dangers of night flying, particularly the somatogravic illusion and black hole approaches. Trescott also touches on the hazards of fatigue, the importance of avoiding FAR violations, and the risks associated with certain over-the-counter medications. He concludes by recommending vigilance in identifying hazards and fostering a strong safety culture, whether in general aviation jobs or as a flight instructor. 337 Yeti Airlines flight 691 crash – Human Factors for General Aviation Pilots

Aviation News Talk podcast

Max discusses the critical role of human factors in aviation safety, prompted by the tragic crash of Yeti Airlines flight 691 in Nepal in 2023. The flight crashed during a visual approach due to pilot errors and high workload, resulting in the loss of all 72 people on board. Trescott explains how human factors like high workload, distractions, and ineffective Crew Resource Management (CRM) played a critical role in the accident. The crew's use of noise-canceling headsets and the challenging new airport approach added to their workload, leading to mistakes. Trescott uses the SHELL model (Software, Hardware, Environment, Liveware) to analyze the accident, emphasizing how the interaction of these elements can lead to errors. He discusses the effects of high workload on pilot performance, including attentional narrowing and task shedding, and offers strategies to manage it, such as starting descent planning earlier and effective CRM practices. Recognizing high workload is crucial, but pilots often don't notice it in real-time, necessitating proactive measures and specific training. Trescott concludes by highlighting the dangers of overconfidence and complacency, even among experienced pilots, and stresses the importance of developing strategies to catch and correct mistakes to enhance flight safety. They are complex neurologic disorders with diverse origins and causes. Headaches are often viewed as the primary pathology, but they are fundamentally a symptom. Understanding them is an evolving science, and their patterns can be recognized for effective diagnosis and treatment. In 2003, Pareja et al proposed the term "epicrania" for headaches triggered by extracranial causes, suggesting a link between intracranial components and extracranial nerves. Peripheral Triggers and Plastic Surgery: Plastic surgeons noted relief of migraines through corrugator muscle resection and botulinum toxin injection, indicating peripheral headache triggers. Severe migraines post-head or neck injury may have an extracranial origin, suggesting peripheral nerve irritation. Traditional migraine medications may offer modest relief. Primary treatment involves inhibiting nerve irritation through interventional pain techniques, turning off the pain origin and associated migraine centers. Ultrasound Advantages: Evaluation and injection of nerves have traditionally used landmarks and fluoroscopic images, but ultrasound offers unique advantages. Nerves often travel with arteries, potentially contributing to "throbbing" pain. Ultrasound provides a more precise and dynamic visualization of these structures. Exploring Extracranial Causes: The International Classification of Headache Disorders (ICHD) categorizes headaches as primary or secondary. While valuable, it broadly defines peripheral nerve contributions as "Other Terminal Branch Neuralgias." The study of extracranial peripheral nerve entrapments and dysfunction reveals overlap between ICHD-defined headaches and potential nerve entrapments causing these pain patterns. Collaboration with disciplines like pain management enriches the understanding and treatment options for headaches. Conclusion: Headaches, often seen as isolated intracranial phenomena, may have extracranial triggers. Understanding and treating these triggers, especially through ultrasound-guided techniques, present a promising avenue in headache management. Functional Medicine Research with Dr. Nikolas Hedberg
Does the Autoimmune Paleo Diet Work?

Functional Medicine Research with Dr. Nikolas Hedberg

The autoimmune paleo diet (AIP) also known as the autoimmune protocol diet is an extremely popular diet these days for autoimmune disease but there has never been any studies done to evaluate if it actually works.  Although testimonials should be taken into account, and there are many all over the internet,  the ultimate test for validity is a clinical trial. As of August 29th, 2017, we now have a study that looked at this diet and how effective it was for autoimmune disease.  The paper is entitled, “Efficacy of the Autoimmune Protocol Diet for Inflammatory Bowel Disease.”  Dr. Konijeti is the lead author and it was published in the journal Inflammatory Bowel Disease. Let’s dig into the study and see what they came up with.  The study included 18 adults but three of them withdrew because they couldn’t commit to the diet so there were only 15 participants.  9 of them had Crohn’s disease and 6 had ulcerative colitis.  Crohn’s disease and ulcerative colitis are autoimmune inflammatory bowel diseases. What causes these diseases?  The authors note that Crohn’s disease and ulcerative colitis are due to genetic factors, gut dysbiosis, dietary factors, environmental exposures, and immune system dysregulation.  This is mostly true for all types of autoimmune diseases but I would also include infections in this list such as viruses and bacteria. To be more specific regarding dietary factors that increase the risk of inflammatory bowel disease, it is noted that a typical Western diet high in refined carbohydrates, omega-6 fatty acids, saturated fat, low in fiber, vitamins, and nutrient-dense foods are part of the problem. What is the autoimmune paleo diet (AIP)? The autoimmune protocol diet is also known as the autoimmune paleo diet since it is an offshoot of the classic paleo diet.  The following foods are eliminated from this diet: Gluten-containing grains Gluten-free grains Dairy Eggs Nuts Seeds Refined carbohydrates and sugar Legumes Nightshades Oils Food additives Coffee Alcohol Why are these particular items avoided in the autoimmune paleo diet? The goal is to remove any foods that tend to be inflammatory in nature and can lead to dysbiosis.  Approximately 70% of the immune system is in the gut and the microbiota have key connections with the immune system so it makes sense to reduce inflammatory stress in this area as much as possible. The autoimmune paleo diet consists of the following: Fruit Vegetables Lean meats Fish Healthy Fats Coconut Sweet potatoes A few additional items that are emphasized are fermented foods to help maintain a healthy microbiota and bone broth for intestinal healing and nutrient density. How did the subjects eat in this study? All subjects followed a 6-week elimination phase and then a 5-week maintenance phase where no foods could be reintroduced.  The participants had a lot of support including a private Facebook page where they could interact with each other and share recipes and ideas. Additionally, they worked with a health coach and a registered dietician.  The subjects were educated on proper sleep hygiene, stress management, exercise instruction, grocery shopping and food preparation including how to make bone broth and the benefits of fermented foods.  Education against the use of NSAIDs was also provided since these lead to bowel inflammation and contribute to dysbiosis. They were also given two books:  “The Paleo Approach:  Reverse Autoimmune Disease and Heal Your Body” by Ballantyne and “The Autoimmune Paleo Cookbook:  An Allergen-Free Approach to Managing Chronic Illness” by Trescott.  These are two excellent books on the autoimmune paleo diet. C-reactive protein and fecal calprotectin were measured before and after the diet.  If you are a patient of mine you know that we always run c-reactive protein levels because it is an excellent marker of inflammation.
