Jsem čtyři sta kilometrů před Ósakou, což je teoreticky tak půlden, pět až deset aut. Teoreticky. V Japonsku od včerejšího, tedy pondělního večera lije, stopovat se nedá. Sedím v nějaké autodílně, kde jsem dostal už dvě kávy - sice plechovkové, ale teplé - a podle všeho mám dost času na článek.Nevím přesně, kde se nacházím, musím mrknout do mapy, takže moment. Jakási vesnice Kushirocho kousek nad městem Masuda, mezi rýžovými poli na břehu Japonského moře: jihozápad ostrova Honšú.Do Masudy mě přivezl mládenec jménem Sakamoto Takumi, který jel přesně opačným směrem, ale otočil to a dal si se mnou čtyřicet kilometrů jenom tak - z radosti, že má v autě cizice. A za společnou fotku.V té Masudě jsem si konečně koupil boty, protože celé poslední dva měsíce chodím v jedněch a těch samých, ostudně jetých sandálech. S novými křuskami na nohou jsem potom kráčel tmou a ptal se sebe sama, co je vlastně cílem. “Netuším,” odpovídala stopařská část mého já té příčetnější. Ve skutečnosti jsem šel vstříc nějakému poslednímu zážitku dne, který se ovšem nedostavil - tato část Japonska je klidná, oproti celé mé předchozí Asii úplně mrtvá, je tady božské ticho. Lidé najednou nežijí na ulici, po setmění jsou všichni zavření doma.Můj nevyřčený plán, že se vnutím k někomu aspoň pod pergolu, kde dostanu drink a nabiju veškerou elektroniku, neměl šanci vyjít; během své hodinové túry jsem vůbec nikoho nepotkal.Naštěstí je Japonsko cestovatelsky luxusní, nádherně jednoduchou zemí. Takové autobusové zastávky, jaké stojí tady na zapadlé vsi, nemají ve světě obdoby, jsou to vlastně čisťounké chatky. Takže tam. Nemaje žádnou funkční elektroniku, poslepu jsem vytáhl spacák a dlouho nemohl usnout, protože to takhle bylo naposledy. Už za pár hodin začne, vypadá to, obyčejný život.Po svítání jsem netušil, jakým směrem se vydat, ale nové boty mě loužemi dovedly až ke garáží lomeno autodílně, kde už dvě hodiny sedím, pozoruju nekončící déšť a vzpomínám na předchozí dny.Zajímavé je, že co bylo včera, připadá mi jako před týdnem, a co předevčírem, to jako před rokem.

More than a third of patients released from the hospital never fill their discharge prescriptions, but “meds-to-beds” programs can help improve that statistic. That is the approach Renown Health in Reno, Nev., took with the help of its 340B savings. Renown Vice President of Pharmacy Services Adam Porath joins us to describe how this meds-to-beds program improves patient care.Hospital readmissions down, patient convenience upRenown Health's 340B-funded program offers medication to patients who are being discharged from the hospital, either through bedside delivery, pneumatic tube, or a unique discharge lounge. The effort began as a pilot for Medicaid patients in 2016, and it demonstrated patients in the program were 25% less likely to be readmitted to the hospital once discharged. These health improvements plus the added convenience of medication access for patients convinced the system to expand the program.340B pricing to patients who cannot payPorath says Renown Health refers patients who cannot afford discharge medications to its social services team, which can authorize providing the drugs to those patients at the 340B-discounted rate. The team also will work with patients to see if they qualify for coverage such as Medicaid or other programs to reduce their out-of-pocket costs. Porath said Renown's meds-to-beds program provides drugs free of charge to about 30 patients per month.The keys to successRenown Health's meds-to-beds program has been a success, with more than 80% of eligible patients participating as of the end of 2024. The hospital expanded the services to all patients and started operating it 24/7 in April 2024. Porath said the keys to success include regular reporting to stakeholders and innovations to handle a large volume of patients discharging at once. „Nevěřili byste, jak jsem zjistil u svých spolutrpitelů, jak po rozsudku smrti člověk začíná lpět na životě. Stopovat se musí umět, že ano. Stopař by měl zvládat naprostou nejistotu, která se před ním rozprostírá. Kde budu za hodinu a kde zítra? Nevím, netuším, nezajímá, nazdar.Stopař nemůže lpět na hygieně, protože té se mu často z objektivních důvodů nedostává; ani na pohodlí. Matně si ještě vzpomínám na ony vypolstrované, čisťounké kamiony v Rusku – to bejvávaly na silnici časy! Tady na Borneu obvykle nemůžu dát nohy pod sebe, protože tam je už napasovaný sud s nějakým mazutem, a tak si je strkám různě za hlavu. Přitom jsem antijogínský typ.Stopař se nesmí bát lidí, i když třeba vypadají jako lovci lebek – právě naopak, měl by je mít rád. Což taky zvládám, z dětské důvěry v širý svět okolo jsem naštěstí nikdy nevyrostl… No a potom není k zahození, když stopař dovede v průběhu cesty improvizovat.Dám příklad z posledních dvou dnů tady na ostrově Borneo, v jeho východní, indonéské části. V noci na čtvrtek jsem se jakžtakž vyspal, respektive byl jsem v posteli a jedním přivřeným okem sledoval velký fotbalový příběh milovaného Tottenhamu; to mi za ospalky stálo. Jinak na Borneu obvykle jedu celou noc v kamionu – už jsem to praktikoval třikrát.Pokud se podíváte na mapu, těžko tomu uvěříte: sice jde o obrovský ostrov, ale přeci jen, od severu na jih je to jenom dva tisíce kilometrů.No právě, dva tisíce. A já tu ještě nikdy nezvládl víc než 350 za den.

Vzpomínám, jak jsem před týdnem přeplul do Indonésie, konkrétně na Sumatru, a podíval se do mapy, kudy jet dál. Nejprve mě překvapila vzdálenost: na jih ostrova to mám hodně přes tisíc kilometrů. Východní cesta byla širší, trochu kratší a nejspíš výrazně rychlejší, ale zjevně taky nudnější, tak jsem to vzal středem ostrova, abych si před koncem cesty užil nějakou další krásu, ne-li dobrodružství.Těžko ovšem říct, zda jsem udělal dobře. I když jsem neustále jel, Sumatra zabrala pět dnů. Hypnotizoval jsem okolí, jestli nezahlédnu orangutana nebo aspoň paviána, ale ne, pořád jen palmová džungle, rýžová pole, rybáři veslující na řekách. Pěkné – ale dlouhé.Trochu únavné.Řidiči jsou vesměs milí, ale i ti vás nakonec unaví, protože do střední Sumatry zavítá málokterý cizinec, a tak je každé setkání, neřkuli společná jízda, obrovským zážitkem. V praxi to vypadá tak, že skoro každý začne vytáčet videohovory směrem k příbuzným, a pak na mě namíří mobil, abych se mamince, synkovi, kamarádům a bůhví komu všemu ukázal. Nikdo z těch lidí nehovoří anglicky, tak musím šaškovat a dělat blbiny. Ti neznámí lidé na displeji jsou většinou naprosto strnulí a v šoku, i já zhruba po minutě tuhnu a mlčíme. Já rozpačitě, oni spokojeně…Poněkud únavný je i fakt, že většina Sumatřanů nedokáže vstřebat koncept stopování. Nevěří tomu, že bych mohl dojet díky zdvižené ruce byť jen do příštího města, natožpak stovky kilometrů za den. Myslí si, že se pletu, za každou cenu mě chtějí vyhodit někde na autobusovém nádraží.Trochu pomáhá, když si projedou mé sociální sítě a vidí, že jsem za ty poslední dva měsíce pár jiných řidičů vystřídal (konkrétně jich za sebou mám 224), ale i tak kroutí hlavou, že takhle náš svět přece nefunguje, na zdviženou ruku se po něm nejezdí. Když už nevím kudy kam, řeknu prostě, že nemám peníze. Nehorázná lež, ale v takové chvíli umlknou a začnou sahat do kapes.„Vlastně mám,“ zastavím je, „ale jenom na jídlo a na nocleh.“Načež si musíme vyměnit čísla a oni mi několik dalších dnů píší, jestli jsem opravdu někam dojel. No samozřejmě! Klidně bych jim mohl sdělit pravdu, že jsem odjel po deseti minutách, a že to bylo jasné, vůbec jsem o tom nepochyboval, ale většinou ty jejich zprávy jenom olajkuju a pošlu jednu fotku, protože já si přece nemůžu korespondovat s padesáti lidmi denně, to bych se zhroutil.Uprostřed Sumatry jsem se dostal na zatím nejhorší silnici mé cesty, to byla jedna díra za druhou. Když nasednu do kamionu, tak jenom sundám sešlapané a smradlavé sandály, jediné boty, které mám k dispozici, a bosá chodidla hodím na palubní desku.Řidič se usměje, zvedne palec, a pak si zapálí cigáro, protože zatím každý z mých třiceti indonéských řidičů kouřil jednu za druhou.Jsou to velké změny. Nikde jinde se zdaleka tolik nekouřilo. A v Rusku jsem se musel před nástupem do kamionu zout povinně, tam se všechno lesklo čistotou, měli koberečky. V Indii to v řidičově kabině vypadalo jak v menším chrámu, no a tady na jihu, tady to vypadá jak za časů mého mládí.

When someone dies, our relationship with them doesn't just disappear. Sometimes the relationship changes in ways we never expected, allowing us to feel closer to them than we did when they were alive. This can leave us learning to grieve not just for what we had, but for what never got the chance to have with them. In this episode we talk with Never Faull about grieving for their father, who died in 2018 from cirrhosis of the liver. Nev shares how their relationship with their father was distant during his life and how they've found ways to create a deeper connection with him after his death. JEN NA ⁠HTTPS://HEROHERO.CO/CESTMIR⁠⁠⁠⁠ A ⁠HTTPS://FORENDORS.CZ/CESTMIR „Vždycky, když se vrátím, uvědomím si, jak moc se máme dobře a jak nám chybí nadhled nad tím, jak žijeme,“ říká farmaceut Stanislav Havlíček, který s Lékaři bez hranic jezdí na humanitární mise – byl například v Jemenu, Etiopii, Bangladéši nebo Palestině. Jako příklad toho, jak si v Česku neumíme představit situace v jiných zemích, uvádí, jak nenásilná u nás byla sametová revoluce. Je to podle něj rarita, a když lidé ve světě bojují o demokracii nebo svá práva, většinou riskují své životy. Na prahu občanské války je teď například Jižní Súdán, ze kterého se Havlíček naposledy vrátil, a připomíná také aktuální situaci v Gaze, která vzbuzuje velké společenské debaty. „Lidé tam nevědí, co bude. Nevědí, jak dlouho to bude trvat. Jsou závislí na pomoci zvenčí. Pro mě je těžko představitelné, že by se tady mohlo stát něco podobného,“ popisuje farmaceut. K současnému dění v oblasti říká, že v Pásmu Gazy žijí dva miliony lidí, na které nelze uplatnit kolektivní vinu a trest. Pokud je šestiměsíční dítě, které hladoví dva měsíce, nelze se na něj podle Havlíčka dívat jako na teroristu. Podle něj to, co lidé vidí v médiích, je naprosto jiná zkušenost než ta, kterou získá člověk na vlastní oči. Na debatách se ho dokonce často lidé ptají, jestli v Gaze viděl někoho z Hamásu. Havlíček na to ale prý jen těžko odpovídá, když vedle militantního křídla organizace existuje i politické uskupení, jehož součástí jsou například i úředníci pracující ve zdravotnictví. Na tamější dvoumilionovou populaci podle něj nelze nahlížet jen jako na dav fanatických džihádistů – jsou to normální lidé. „Je tam spousta lidí, kteří trpí špatnou politikou svého vedení, ale nemůžou s ním nic dělat. Nemůžou dojít na Václavák a zacinkat klíčema,“ vysvětluje farmaceut. Havlíček byl naposledy v Pásmu Gazy loni na začátku roku a neumí si představit, jak náročná může být situace v oblasti teď, když dochází k blokování humanitární pomoci. Na Fouskovi mu nejvíce vadí, že po slovech nepřicházejí činy. „Kromě slibů, vizí a řečí okolo, jak se vše bude měnit a fungovat, jsem čekal i činy. Za sebe říkám, že jsem ty činy zásadním způsobem nezaregistroval. Dokonce na některých valných hromadách jsem byl i celkem kritický, protože je tam spousta věcí, které považuji za zbytečně netransparentní, zbytečně nezodpovězené,“ vypráví.

Známe se několik let, maximálně deset, a detaily se dozvíte při poslechu podcastu. Myslím, že jsme se poprvé viděli v divadle Troníček, kam přišel (buď omylem, nebo nějakou hloupou náhodou) ma standupové představení, ve kterém jsem vystupoval.Protože jsme měli nějaké společné kamarády, tak jsme se o přestávce nebo po představení dali do řeči. Zaujalo mě, že má přes rameno něco jako kabelku. Jasně, muži to občas nosí, ale přiznejme si, že je to divné. Nevím, jestli jsem mu k tomu tehdy něco řekl (blbec a hulvát jsem na to dost), ale to je už jedno.Až později jsem se od našich společných kamarádů dozvěděl, že to nebyl nějaký nepovedený módní výstřelek. Petr Král měl v té kabelce své “výpomocné” srdce. A potřeboval ho proto, aby - náhodou a třeba i během našeho vystoupení - v divadle neumřel.Dnes už má úplně nové srdce a jak jsem napsal, vše se dozvíte v tomhle pocastu. Jen ještě podotknu: mám Petra Krále opravdu rád, považuju ho za fajn člověka a muže, který umí dělat svou práci, a myslím, že se nám povedl dobrý rozhovor.Avšak to už posuďte sami. Příjemný poslech!
