The founder and managing director of 123Carbon explains carbon insetting, SAF certificates, and SAF registries. Guest Jeroen van Heiningen is the founder and managing director of 123Carbon. The company works with fuel suppliers and carriers to issue transparent and externally verified environmental attribute certificates, including SAF certificates, used by airlines and cargo operators to reduce their transport-related Scope 3 emissions. Jeroen discusses the sustainable aviation fuels (SAF) market, focusing on carbon insetting, the role of SAF registries, and the importance of transparency and standardization in the industry. He explains how carbon insetting differs from carbon offsetting, emphasizing the need for companies to invest in their value chain de-carbonization. The discussion also covers the digital certificate process, the collaboration between 123Carbon and IATA, and future trends in the SAF market. 123Carbon's registry is integrated with other IT systems and allows an airline to upload documents and manage its inventory of digital certificates online. It's a multi-modal agnostic platform that can be branded. With the use of sustainable bio-fuel growing in the land and sea modes of transportation, the accounting needs to be integrated with air. 123Carbon recently introduced Carboninsets.com, a free service for forwarders and cargo owners that connects them with issuers on the registry. Jeroen set-up and led Accenture's Sustainability Services Benelux and wrote the CDP NL 50 report in 2010. He has 15 years of experience with multi-modal carbon insetting programs, covering both Air (KLM corporate biofuels Program) and Marine (the GoodShipping Program). He holds a master's in business administration from Tilburg University. Takeaways: Carbon insetting is a more effective long-term approach than offsetting. Companies are increasingly linked to net-zero targets managed by SBTi. (Science Based Targets initiative) Standardization in the SAF market is crucial for transparency. The SAF market is evolving with a focus on multimodal transportation. Demand and supply must be connected to stabilize SAF prices. New services like CarbonInsets.com facilitate connections between issuers and buyers. The aviation industry is leading in SAF adoption compared to other transport modes. Future initiatives will focus on integrating various transportation modalities. Max discusses the tragic midair collision at Reagan National Airport between a PSA Airlines CRJ700 regional jet and a U.S. Army Black Hawk helicopter. The midair crash over the Potomac River was captured by a Kennedy Center webcam. ATC instructed the Army helicopter to follow the airliner to Runway 33, but it did not comply. The helicopter was not transmitting ADS-B Out, though its position was tracked using Mode S transponders. TCAS alerts are disabled below 400 feet, meaning the pilots had no automated warning. Nighttime perception challenges likely contributed to the crew's failure to see and avoid each other. Trescott reviews ATC audio and past midair collisions, emphasizing the Swiss Cheese Model of accident causation—multiple failures aligned to cause the crash. A reported FAA radar screen showed collision alerts 30 seconds prior, but no action was taken. He urges pilots to use traffic displays with relative vectors for better situational awareness, particularly at night. The episode ends with a call to fly defensively and use all available tools to prevent midair collisions. Dans l'épisode d'aujourd'hui je vous partage les informations et les analyses suite à ma participation, en tant qu'invitée, au séminaire annuel du G.R.O.S (Groupe de Réflexion sur l'Obésité et le Surpoids). C'est une organisation de professionnels de santé (psy, nutritionniste, diététicien.ne, chercheures, généraliste) qui prend le temps de réfléchir aux soins apportés aux personnes grosses. Ils ont développé une méthode, et un annuaire des professionnel.le.s formés à cette méthode. C'est une approche non stigmatisante, qui va dans le sens de la prise en charge des patients en accompagnement de chirurgie bariatrique et/ou traitements médicamenteux, mais aussi guérison des TCAs. Le G.R.O.S intervient aussi auprès des acteurs décisionnaires de la santé pour portée une voix moins stigmatisante que la médecine actuelle plutôt grossophobe. La conférence annuelle 2024 était sur le thème "Les traitements de l'obésité ; bilan, limites et perspectives". Les invervenant.e.s ne sont pas toustes du G.R.O.S. Je vous laisse découvrir les détails sur leur site. N'hésitez pas à parler du G.R.O.S à vos professionnel.le.s de santé ! Pour ma part, en tant que personne concernée, et militante anti-grossophobie, je suis ravie et reconnaissance de pouvoir assister à cette conférence annuelle, car cela me permet d'apprendre beaucoup de chose sur la partie médicale de ces sujets. Je ne suis pas toujours en accord avec certaines interventions qui, pour moi, vont trop souvent vers l'équation obèse = malade = traitement. Je pense qu'être juste gros, sans aucune maladie associé n'est pas une maladie et ne nécessite pas de traitement. On peut soigner les TCAs, et le rapport au corps, mais pas le poids qui n'est pas un problème en soi. Je pense aussi, que même obèse, ce qui compte c'est de soigner les maladies déclarées, tel que le diabète, l'hypertension, les problèmes cardiaques, le cholestérol, comme on les soignerait pour des personnes minces. Soigner le poids, n'est pas, à mon sens, et au sens de beaucoup de littérature scientifique, la priorité ou une assurance d'une meilleure santé. Je vous partage tout cela dans cet épisode où j'espère que j'ai bien retranscrits les informations portées par les intervenant.e.s tout en développant mon avis et mes espoirs de sortir de l'équation obèse = malade (donc ne les stigmatisons pas, ils sont juste malade) = traitement, pour en arriver à une vraie déstigmatisation, ce qui pour moi veut dire une vraie lutte contre la grossophobie de la société et de la médecine, car non, on ne devrait pas continuer de croire que toutes les personnes grosses ont besoin d'être soignées pour leur grosseur. Non. Elles ont besoin d'être soignées, pour les maladies et uniquement au moment où ces maladies apparaissent. En matière de prévention, pareil, on n'a pas besoin de faire de la prévention anti poids, donc grossophobe, mais de la prévention pour une bonne hygiène de vie de façon mesurée : le moins de conduite à risque possible, une bonne gestion du stress et du sommeil, de l'activité physique, tout type d'activité physique, régulière (à l'échelle de sa vie pas de chaque journée), et une alimentation variée et diversifiée (non pas équilibrée, non pas avec la culture des régimes et la classification moralisatrice des aliments). Et en n'oubliant jamais que les personnes grosses, comme tout le monde, vivent dans une société capitaliste qui voue un culte sans limite à la performance et la productivité, y compris en valorisant le corps mince, athlétique, beau, et le santéïsme.

Richard Blumenthal stated “After whistleblower John Barnett raised his concerns about missing parts, he reported that his supervisor called him 19 times in one day and 21 times another day. And when Barnett asked his supervisor about those calls, he was told, ‘I'm going to push you until you break.'” Blumenthal said that in his opinion, the Department of Justice should criminally prosecute Boeing for violating its 2021 deferred prosecution agreement. Last week we talked about why you often feel hungry and some ways you can feel more satisfied. And we briefly mentioned that one of the reasons you always feel hungry is that you're taking a medication that increases your appetite.  Today I want to focus on those medications that can cause weight gain and what you can do about it. It's important to remember that certain medications may cause weight gain as a side effect. But remember that not everyone taking certain medications will have the same issues. Some antidepressants and antipsychotics are more likely to cause weight gain than others. For example, a selective serotonin reuptake inhibitor (SSRI) like Paxil is expected to cause weight gain more than Zoloft or Prozac. Also, tricyclic antidepressants (TCAs) like amitriptyline and nortriptyline and medications like Remeron and Lithium cause weight gain.  It's difficult to determine precisely how much weight a person could potentially gain with each of these medications as the amount of weight people gain seems to vary between studies.  If you're experiencing weight gain with your antidepressant or antipsychotic medication, make sure to discuss your concerns with your healthcare provider. Never stop taking your medication without talking to your provider first. Diabetes medications like insulin, glipizide, and Actos can also cause weight gain. This is because these medications cause your body to absorb more sugar from your blood. This extra sugar is then stored as fat. As a result, people on these diabetes medications typically gain 5 lbs to 10 lbs of weight. Steroids like prednisone used to help lower inflammation, can also cause weight gain. However, this weight gain is typically seen when the medication is used for long periods. The good news is most people who use prednisone typically use the medication for a short time, so weight gain shouldn't be an issue.  It's also important to know that weight gain isn't an issue with topical steroid use (e.g., creams or ointments), nose sprays, or eye drops. Typically weight gain is seen when people use steroids for certain inflammatory conditions (e.g., rheumatoid arthritis, cancer patients, lupus).  The medication causes weight gain by increasing your appetite, changing how the body processes sugar and fat, and causing fluid retention. Seizure medications like gabapentin, Lyrica, and Depakote can also cause weight gain. It's unclear why this happens, but it may be due to increased appetite.  Antihistamines used to treat allergy symptoms can also cause weight gain. Medications like  Benadryl and hydroxyzine are thought to make you hungrier and tired, making you less active and gaining weight. What can I do to manage weight gain from medications? Several things to help manage weight gain caused by medications involve healthy lifestyle changes. You can start by following a healthy diet and regular exercise. Your healthcare provider may also suggest switching you to an alternative medication or lower dose of your current medication to see if this helps with your weight gain. What about semaglutide? Let's say you've followed a healthy diet and regularly exercised. And you've talked to your healthcare provider about switching medications or lowering your dose, but you continue to keep the weight on. When these lifestyle changes (diet and exercise) aren't enough to lose the weight you desire, weight-loss medications like semaglutide may be helpful.  Semaglutide can help suppress your appetite and food intake.  Thanks again for listening to The Peptide Podcast, we love having you as part of our community. In this episode, we will discuss the most important updates from the American Diabetes Association's 2023 Standards of Care in Diabetes. Key Concepts The first-line therapy for type II diabetes is based on whether the primary goal of therapy is cardiorenal benefit (reduced risk of ASCVD, heart failure, or CKD) or glycemic and weight goals. For cardiorenal benefit, GLP1 receptor agonists and SGLT2 inhibitors are heavily emphasized. For glycemic control and weight gain, GLP1 receptor agonists (or GLP1/GIP in the case of tirzepatide) have a very favorable effect on weight loss and glycemic control. While metformin is still mentioned, it is no longer the sole, first-line therapy for type II diabetes. For patients with diabetes and a high risk of ASCVD (20% or higher), high-intensity statins, ezetimibe, and/or PCSK9 inhibitors are recommended to achieve an LDL less than 70 mg/dL. In patients with a history of ASCVD events, these same therapies are used to achieve a recommended LDL goal of less than 55 mg/dL. Among selected patients with diabetes and CKD with albuminuria, finerenone (a new mineralocorticoid receptor antagonist) is recommended to improve renal and cardiovascular outcomes. A variety of different therapies are now recommended for neuropathic pain, including gabapentinoids, SNRIs, TCAs, and several antiseizure medications (lamotrigine, lacosamide, oxcarbazepine, and valproic acid). A wide variety of other new recommendations are discussed in the episode, including NASH/NAFLD, obesity and weight management, special populations, diabetes technology, and health behavior changes. El término de alimentación intuitiva fue creado por Evelyn Tribole y Elyse Resch en 1995, los cuales escribieron un libro llamado “Alimentación intuitiva: El retorno a los hábitos alimentarios naturales” en los establecieron diez principios esenciales de la nutrición intuitiva. La dieta intuitiva consiste en comer de todo, opciones saludables, en función del hambre que se sienta, se trata de conectar con uno mismo.Esta no es una dieta como tal si no un tipo de alimentación ya que no restringe ningún alimento sino que se basa en conseguir un bienestar físico y mental mediante la escucha de las señales internas que transmite el cuerpo; hambre, saciedad, preferencias, satisfacción, etc.El hecho de que con esta alimentación no se prohíba ningún alimento no significa que sea adecuado alimentarse la mayor de veces a base de alimentos procesados sino que se deberá encontrar un equilibrio.¿CUÁL ES SU OBJETIVO PRINCIPAL?El principal objetivo de este nuevo método es aprender a comer más saludable de manera en que basemos aquellas ingestas diarias en lo que nos está pidiendo nuestro cuerpo y no en lo que nos dicen de manera externa (terceras personas o dietas). ¿CÓMO PRACTICARLA?No existe una pauta específica para tener en cuenta en el momento de realizar esta dieta, aunque algunos puntos básicos son:Buscar información de fuentes fiables, como sería el caso del libro oficial de los creadores de los diez principios esenciales. How motivates is the patient to get better? - Are they coasting (teenagers)? May need thoughtful/ careful deprescribing. Nontraditional/novel treatment options- Nontraditional/novel options for treatment resistant depression, PTSD treatment, chronic pain, etc. - Ketamine - Psilocybe Psychiatry & Primary Care- Incorporating psychiatry into our own primary care practices is anxiety provoking but inevitable in this day and age of healthcare - We can learn new things and leverage our existing resources to better help our patients - Time with our patients is a barrier - Ask the patient: what is the most pressing issue for you today? Episode 105: Antidotes to toxidromes. Some poisonings share common signs and symptoms and may be treated with antidotes without laboratory confirmation of the offending agent. Dr. Francis discussed with Dr. Arreaza some of those toxidromes and how to treat them. Written by Aida Francis, MD. Participation by Hector Arreaza, MD. Definitions: • Antidotes are substances given as a remedy that inhibit the effects of another drug of abuse or poison. Most are not 100% effective and fatality is still possible after administration. • Toxidrome is a constellation of signs and symptoms caused by an overdose or exposure to chemicals or drugs that interact with neuroreceptors. Toxidrome is the combination of the word “toxin” and “syndrome”. Management strategies of toxidromes are determined by the signs and symptoms even when the causative agent has not been identified. A little bit of Background: The World Health Organization reported that 13% of deaths caused by poisonings are children and young adults. Intentional poisoning attempts are more frequent among adolescent women than men. It is difficult to evaluate poisoned patients because they are too altered to provide history and there is often not enough time to perform a physical exam or obtain serum studies prior to life-saving interventions. To diagnose a toxidrome clinically, you need three elements: pupil size, temperature, and bowel sounds. For example: Pinpoint pupils with hyperactive bowel sounds point to cholinergic toxidrome, and dilated pupils with high temperature, and hypoactive bowel sounds point to anticholinergic (see details below). Pinpoint pupils -> Bowel sounds -> Hyperactive: CHOLINERGIC -> Hypoactive: OPIOIDS Normal or dilated pupils -> Temperature -> High -> Bowel sounds -> Hyperactive: SYMPATHOMIMETIC -> Hypoactive: ANTICHOLINERGIC -> Normal or Low -> Bowel sounds -> Hyperactive: HALLOCUNOGENIC -> Hypoactive: SEDATIVE-HYPNOTICS Anticholinergic Toxidrome and the Physostigmine antidote: • Anticholinergics inhibit the binding of acetylcholine to the muscarinic receptors in the central nervous system and the parasympathetic nervous system. Examples of anticholinergics include atropine and tiotropium. Other substances that may cause anticholinergic toxidrome include antihistamines (especially first-generation: diphenhydramine), antipsychotics (quetiapine), antidepressants (TCAs, paroxetine), and antiparkinsonian drugs (benztropine). Symptoms of toxicity include tachycardia, non-reactive mydriasis, anhidrosis, dry mucous membranes, skin flushing, decreased bowel sounds, and urinary retention. Neurological symptoms include delirium, confusion, anxiety, agitation, mumbling, visual hallucination, and strange behavior. Neurological symptoms last longer because of the anticholinergic lipophilic properties which cause them to distribute into fatty organs and tissues like the brain. “Mad as a hatter, red as a beet, blind as a bat, hot as a hare, dry as a bone” [Spanish: loco como una cabra, rojo como un tomate, ciego como un topo, seco como una piedra, caliente como el infierno] • The antidote for anticholinergic toxidrome is physostigmine. It is an acetylcholinesterase inhibitor and prevents the metabolism of acetylcholine. This increases the level of acetylcholine in both the central nervous system and peripheral nervous system. Physostigmine can cause seizures and arrhythmia, so close monitoring in the hospital is required during treatment. Cholinergic toxidrome and its antidotes atropine and pralidoxime: Acetylcholine is part of the parasympathetic nervous system and cholinergic substances can induce a parasympathetic response. Some of these substances include pesticides, organophosphates, carbamate, and nerve gas. Chlorpyrifos had been used to control insects in homes and fields since 1965. It has been used in our crops in Bakersfield, and the most recent mass exposure was in May 2017. it was banned on food crops in the US in August 2021. It has been banned for residential use for a longer period. Repeated exposure to chlorpyrifos causes autoimmune disorders and developmental delays in children and fetuses. The symptoms of cholinergic toxidrome can be summarized with the SLUDGE/ “triple” BBB acronym. This includes salivation, lacrimation, urination, defecation, gastrointestinal cramping, emesis, bradycardia, bronchorrhea, and bronchospasm. There can also be muscle fasciculations and paralysis. • The antidote is Atropine. Pralidoxime is used for organophosphates only because it cleaves the organophosphate-acetylcholinesterase complex to release the enzyme to degrade acetylcholine. Pralidoxime should be used in combination with atropine, not as monotherapy. It requires hospital admission, and a note for organophosphate, remember that the patient needs external decontamination (shower). Let's go to part 2 of our discussion, environmental exposure. Carbon Monoxide Toxidrome and the antidote oxygen: Carbon monoxide intoxication is usually due to smoke inhalation injury. Carbon monoxide is a silent gas produced by carbon-containing fuel or charcoal. Carboxyhemoglobin (COHb) forms in red blood cells when hemoglobin combines with carbon monoxide, reducing the binding and availability of oxygen at the tissue level. It's like CO falls in love with hemoglobin and hemoglobin cheats on Oxygen by binding to CO instead, and neglects oxygen delivery to tissues. Carbon monoxide also causes direct cellular toxicity. The symptoms and signs of poisoning include headache, altered mental status, nausea, vomiting, visual disturbance, Cherry-red lips, coma, and seizure. You can also see lactic acidosis and pulmonary edema. Neurological symptoms can be chronic, so it's important to follow up. The blood COHb level must be used to confirm the diagnosis because standard pulse oximetry (SpO2) and arterial partial oxygen pressure (PaO2) cannot differentiate COHb from normal oxygenated hemoglobin. You must obtain a serum COHb level. • The antidote is 100% oxygen or hyperbaric oxygen therapy and close follow-up. Consider intubating if there is edema of the airways due to inhalation injury. Cyanide Toxidrome which include sodium nitrite, sodium thiosulfate, and hydroxocobalamin In combination with Carbon Monoxide poisoning Cyanide poisoning can simultaneously be caused by inhalation of smoke or colorless hydrogen cyanide or ingestion of cyanide salts or prolonged use of sodium nitroprusside (ICU for hypertensive emergency). Symptoms are very similar to carbon monoxide poisoning. There may be long-term neurologic deficits and Parkinsonism. Diagnosis is clinical and waiting for serum cyanide levels can cause treatment delay. However, serum lactate levels over 10 mmol/L suggest cyanide poisoning. • Since cyanide poisoning resembles carbon monoxide poisoning and both toxidromes typically present simultaneously in the pathognomonic fire victim, treat simultaneously with sodium nitrite, sodium thiosulfate, and hydroxocobalamin as well as oxygen as mentioned with carbon monoxide poisoning. Hypnotic and sedative substances (antidote: flumazenil) Examples of hypnotic or sedative substances are alcohol, benzodiazepines, or zolpidem. Signs and symptoms of toxicity include slurred speech, ataxia, incoordination, disorientation, stupor, and coma with mild and rare hypoventilation and bradycardia. • The antidote is flumazenil which is a competitive antagonist at the benzodiazepine receptor. After treatment monitor patients for seizures in case of TCA poisoning, arrhythmia, or epilepsy. Opioid toxidrome (antidote: naloxone) Examples of opioid intoxication in children would be heroine in adolescents or accidental ingestion of pain medication in young children. Signs and symptoms are similar to the sedative toxidrome except for the pathognomonic finding of miosis or “pinpoint pupils” on physical exam. There will also be respiratory depression, hyporeflexia, bradycardia, muscle rigidity, and absent bowel sounds or constipation. Hypoventilation is severe and can cause death. • The antidote is naloxone which is a synthetic opioid receptor antagonist that can diagnose and treat opioid poisoning. It is indicated if the respiratory rate is less than 12. It has a short half-life and is repeatedly administered every 3-5 minutes until the respiratory drive is restored in order to avoid rebound respiratory depression and intubation. It has a rapid onset so the patient must be observed for 24 hours for opioid withdrawal symptoms. Summary: It is important to be able to recognize a toxidrome and antidote early. Once the antidote is administered, you should observe the patient 24 hours for symptoms of rebound toxicity or withdrawal. Consider repeat administration of the antidote if rebound symptoms occur and treat withdrawal symptoms as needed. Don't forget to consider multidrug poisoning if symptoms are non-specific. I am a night shift nurse and I am having a tough time with timing my meals/food during the nights i'm working. I am trying be as healthy as I can working night shift (7p-7a). I know the number one best thing for me to do for my health would be to switch to day shift but unfortunately with the staffing shortage at the hospital I am unable to switch as of now and need to stay on nights. I work 3 nights of 12hr shifts a week. Would you be able to recommend the best meal timing for the nights i'm working? I have listened to your podcasts about meal timing for a normal sleep cycle with being awake during the day which I follow on my days off! :) Thank you so much! -Stephanie, a fellow New Englander   Anita: Thank you for your podcasts and all you do to promote health and wellness. I appreciate you. I am not able to tolerate any of supplements or even the detox protocol. I do ok for one or two days and then I am terribly sick and cannot even tolerate hardly any foods. I have CYP2C19-2 copies of variant. Overall functional effect- no enzyme function (drugs less effective, increased chance side effects, and unable to clear from liver- blood thinners, PPIs, anti fungals, anti seizure drugs, SSRI'S, and TCAs. I also have SLCO1B1- one copy. Predicted decreased function. Overall functional effect- Decreased protein function. One copy of Hemochromatosis HFE variant also, may not increase risk. My question is: is there an enzyme I can take something I can do to reverse the effects of these genes? Thank you so much!   Erica: How does daylight savings time affect our biological processes and rhythms?   Erica: Would you ever consider doing an episode that sums up all the recommendations that you would make specifically for professional athletes and how this differs from the average client? For example, through trial and error I feel that my body tends to need more vitamin C, zinc, and magnesium; I've also found ashwaganda to help, as well as higher protein, and I also tend to feel best after around 9 hours of sleep even though my genotype is kapha-pitta (for reference, I'm a professional boxer so there's around 2.5-3 hours of training and about another 1 of walking every day, so it's high demand). I'm a newbie to the podcast but I wanted to thank you because it's already become a constant part of my routine to listen to every morning, which is really comforting and helpful so I really appreciate you putting all the time and effort into doing these episodes and connecting with your audience. Also, I was wondering if you've heard of the hypnotherapist Marisa Peer? You give so many great recommendations, and after listening to a few of your Motivation Monday podcasts (which are some of my favorite), I thought her YouTube videos on mindset and taking control of inner narrative might be something you (or other members of the audience) might enjoy so I wanted to pay it forward :)   Terry: My wife thinks I have nocturnal convulsions. She said I have the symptoms as described by Dr. Google. I searched to see if you have ever discussed nocturnal convulsions on your podcast and did find any hits. If that is what I have, what can be done? I have run the big 5 labs and I have a good idea of my mineral deficiencies that I am working on improving. The symptoms seems to have gotten worse since I have been walking 10,000+ steps per day.   Jamie: Thank you again for everything that you do and your help to help us all be a little better every day. Really appreciate you and your team. We have a bit of a quandary at home as we have a toddler aged just over 2.5 years and a new baby around 16 weeks old. Whilst my wife and i do not have any dairy in the house (i've largely fixed my gut and know i have a sensitivity and she is very much on baord and will do big 5 later this year). EVery medical practicioner is pushing dairy, her nursery has dairy every day. She is exposed to some and we are ok with it as we control what we can at home but there is a worry about her receiving enough calcium for her growth. What specifically would you suggest food / drink wise if you were to cut dairy completely to ensure your kids had enough calcium when growing?
