Leah Hechtman, well respected naturopathic practitioner, and reproductive health expert, explores the pain associated with endometriosis and the emerging understanding of adjusted neurological pathways through a woman's life experiences that can shapes her perception of pain. She is joined by FX Medicine ambassador, Emma Sutherland, who delves into the concepts of the neurocircuitry that occurs in women with endometrial pain, and explores why some women may or may not feel pain depending on the physical severity of tissue proliferation. There's a deep connection between lived experiences, emotions and trauma which impacts a woman's feeling of safety, self-worth and empowerment which requires ongoing work to untangle in order to recover from experiencing ongoing pain - even when the clinical signs of endo have ceased. The importance of support and providing a safe space for women to discuss all the underlying factors to their pain is emphasised. Importantly, knowing when to refer is explained, particularly if there are aspects of psychological, physical or sexual health that requires another experts skills to facilitate the road to recovery. Emma and Leah discuss various tests that prove to be valuable for measuring inflammatory and coagulation makers, both hallmarks of endometriosis, the use of medications and how they assist with management. Dr. Jade Teta, Naturopath, author and podcast host, joins our ambassador Emma Sutherland, as he describes the connection between metabolism, hormones and weight management. Working to define metabolism, Jade debunks the calorie-in vs calorie-out paradigm often relied on for weight management, replacing it with a holistic view of metabolism as a sensory apparatus within the body operating to respond to multiple stimuli including stress, hormones and temperature. Describing the difference between metabolic flexibility and inflexibility, Jade highlights the benefits of both a short and extreme diet and a gentle and prolonged diet for weight management. Turning to hormonal influences, Jade and Emma discuss the role of leptin, insulin, oestrogen and progesterone on metabolism and in turn, weight management. Join Integrated GP Dr. Nicole Nelson and fellow integrative medicine GP and Ambassador, Dr. Michelle Woolhouse, on this detailed podcast on breast cancer prevention. In this episode, they explore the multifaceted landscape of modifiable aspects that can significantly impact breast cancer development. Researchers have outlined a host of modifiable factors, including early pregnancy before the age of 30, breastfeeding support, regular exercise, alcohol consumption, breast density screening, minimizing exposure to endocrine disruptors, and supporting oestrogen processing and excretion. Alcohol consumption, in particular, is dose-dependent and shown to increase breast cancer risk, making zero intake the safest option. Dietary choices play a pivotal role, with fibre, indole-3-carbinol-rich foods like cruciferous vegetables, vitamin A, iodine, selenium, and gut microbiome health all influencing breast cancer risk. Additionally, understanding and influencing beta-glucuronidase activity, maintaining bowel regularity, and supporting detoxification phases (Phase 1, Phase 2, and Phase 3) are key strategies. They also explore how adopting a diet aligned with genetic heritage, or the Mediterranean Diet can offer protective benefits. An interesting and often overlooked aspect, insulin regulation, and how it acts as a tumour growth factor, is covered in detail. Strategies encompass choosing foods with a lower and sustained insulin response, intermittent fasting, selecting sugar alternatives, and closely monitoring sugar levels in food. Endocrine disruptors, with their wide-ranging negative effects on oestrogen transport and hormone receptors, are discussed in-depth. Functional tests are also outlined such as saliva, serum, and urinary oestrogen metabolites, gut beta-glucuronidase activity, genetic SNPs, and morning spot urine iodine to provide a comprehensive understanding. Lastly, they explore the significant impact of exercise on reducing mortality risk post-breast cancer and emphasise the importance of prioritising sleep quality as an essential lifestyle aspect. Gastroenterologist Dr. Will Bulseiwicz and our ambassador Emma Sutherland explore the holistic method of supporting the gut microbiome and reducing risk of developing chronic disease. Dr. Bulseiwicz explains that we're uniquely bio-individual - all the way down to the level of our gut microbiome and a person's health is intrinsically connected to its own unique gut terrain a.k.a. gut microbiome. He also discusses the benefits of maintaining a healthy gut lining by supporting mucus secretions such as secretory IgA and the usefulness of probiotics, and provides us with recommendations on how to choose the “right” probiotic for our patients. Naturopath Keonie Moore shares in-depth clinical experience on managing Paediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal infections (PANDAS) with Ambassador Emma Sutherland. Together they define the key differences between PANDAS and PANS, as well as the the common triggers, signs and symptoms to help clinicians pinpoint the condition - including an overview of the research history backing this relatively newly understood condition. They also discuss the immune-based pathophysiology that not only involves the nervous system but centralises around gastrointestinal health, triggering systemic inflammation. Keonie shares several components of her Moore method; a treatment plan where specific microbiome, nutrigenomic and pathology tests are recommended to help clinicians determine PANDAS and their therapeutic recommendations. Finally, Keonie details an in-depth, informative, and practical case study of a ten-year-old boy who had severe tic disorder, outlining her strategic approach to the case, therapeutic considerations and dosages, and ultimate outcome. The case study shows the determination of the patient, his family and the clinician in order to bring him back to health. At 37 percent, the rate of Australian caesarean section procedures is already one of the highest globally, and is projected to hit 45 percent by the year 2030. Join fx medicine Ambassador Emma Sutherland and guests Dr. Kate Levitt and Kerry Sutcliffe for an evidence-based discussion on the current birthing landscape in Australia. Kerry first takes us into the fascinating history of childbirth to help us understand the drastic changes from women-led, community-based birthing to the modern medicalised and seemingly disempowered process of birthing. She discusses the main drivers leading to C-section, recognising the prevalence of trauma around birth and how clinicians can provide knowledge and tools for an overall positive birthing experience. Dr. Levett layers in results from her current research which provides insight into the importance of working with a woman's support structures (birth partner, family, allied healthcare practitioners) and following their preferences that can ultimately lead to profound birth and health benefits. She provides important information on how birthing guidelines are developed and how giving frank, open and honest information driven by the woman's birth experience requirements helps to support a more effective (in both cost and personal experience) labour. Integrative GP Dr. Lily Thomas dives into the many complexities involved in fibromyalgia, a condition underpinned by chronic, systemic inflammation and neuroinflammation. Our Ambasssor Dr. Michelle Woolhouse opens the conversation discussing the many aetiologies of fibromyalgia which include oxidative stress, mitochondrial dysfunction, mast cell activation, microglia activation, peripheral and central sensitisation. They discuss common risk factors such as Vitamin D deficiency, gut health, and abnormal peripheral and central pain pathways, as well as others that can play a part in the development of fibromyalgia - which mainly impacts women aged 20 to 60 years of age. The gut is a major first focus for Dr. Thomas, who details connections between those with fibromyalgia and IBS, as well as how to investigate and treat through the gut. There's also a deep discussion surrounding mitochondrial health and the development of fibromyalgia, due to the high concentration of mitochondria in skeletal muscle tissue as well as their impacts on damaging the nervous system and their ability to produce melatonin. Further to this, there is a fascinating technical discussion around gendered mast cells and microglia, and how they are shown to act differently depending on their sex, exposure to environmental toxins, dietary triggers, and sex hormones, all of which may be impacting patients with fibromyalgia. The brain and nervous system aspect can't be forgotten as various areas of the brain known as the pain matrix contribute to both peripheral and central sensitisation with the focus on trigger points on reducing peripheral sensitisation having an upstream effect.
