Gastroenterologist Dr. Michael Owens did a thought experiment during the pandemic period. He asked himself, what would I do with my time if I'm not doing as many colonoscopies? That question led him to leave his large GI group to start up a multi-specialty GI group that takes advantage of the trends shaping the future of GI. For example, trends such as value based care, the microbiome, AI, colonics, single-use devices, genomic tests, liquid biopsy and so on. As he implies, Mike literally implemented the central premise of the book Scope Forward to build future on his own terms. He stands as a great example for GI 2.0. My guest today is integrative gastroenterologist Dr. Robynne Chutkan. Studying at Yale University and then Columbia medical school, Dr. Chutkan took a job with Georgetown University Hospital. Then in 2004, she created the digestive center for wellness. Dr. Chutkan continues to help patients heal various health issues with proper nutrition and lifestyle choices from autoimmune disease, skin disease, eczema, asthma and so much more. She even noticed that some underdeveloped countries have far fewer autoimmune diseases than our developed country. What was this based on? Early in her practice, Dr. Chutkan saw patients improving their health drastically and finally asked them, “what are you doing differently?” Inspired by their improved results through healthy eating she started to explore this path. She is not opposed to medication, but she says there are quite a few options to getting better besides using medication as a first and only solution. Virtua Health Gastroenterologist Dr Krysta Johns-Keating, MD, discussed gut health, keeping our digestive track moving as we overindulge Gas is normal, but any change should be reported. She stressed these symptoms should always be brought to a health professional: blood in stool, unexplained weight loss and chronic acid reflux starting after 60. She offered ways to stay healthy, don't over eat, avoid processed foods and eat vegetables and fruit, don't lie down after a big meal, exercise and get a colonoscopy. She adds that 45 is the new recommended age to begin and that African Americans are at higher risk at a younger age. Dr Johns-Keating promised that preparation for the test is not as extreme as it used to be, and it's the gold standard for discovering and removing pre-cancerous polyps. We also discussed women who present different issues, thanks to hormones and sharing space with reproductive organs. In this episode, I interview a board certified gastroenterologist, Dr. Matt Blankenship from Tulsa, Oklahoma. Graham is a gastroenterologist with extensive clinical (consultative and procedural) experience across a wide range of GI disorders. He specializes in the fields of Inflammatory Bowel Diseases (IBD – Crohn's disease, ulcerative colitis) and non- inflammatory gut disorders. Graham developed the Crohn's and Colitis centre at the Royal Brisbane and Women's Hospital (RBWH) together with Australia's first series of IBD specialist clinics and a Telehealth program for regional Queensland. He introduced IBD specialist nursing during this period. Graham is an honorary group leader at the QIMR Berghofer Medical Research Institute where he works with his team in the Gut Health research group, Associate Professor within the Faculty of Medicine, University of Queensland, Chair of the ANZ IBD consortium of clinicians, and Chair of Crohn's Colitis Cure (a charity supporting research and clinical practice for clients with IBD). He's had countless interests over the course of his career as a gastroenterologist and professor at the University of Massachusetts Medical School, but Dr. David Cave's most current interest is an approach in gastroenterology called video capsule endoscopy. This procedure involves a pill not much larger than a pill of aspirin that has the capability of taking pictures all throughout the gastrointestinal (GI) tract. By providing an unprecedented way of looking at the GI tract, this technology has resulted in a treasure trove of new, incredibly useful diagnostic information. Dr. Cave was the first in the U.S. to utilize this technology and was the only one using it for over a year since the first device was introduced in 2001. He joins the podcast today to discuss how the technology had advanced and evolved over the years, the information that's been discovered as a result of this technology, the recent development of a magnetically controlled capsule, the possibility of using artificial intelligence in reading the images captured by these capsules, the results of a randomized controlled trial that compared capsule endoscopy with the standard of care in detecting active bleeding in patients, and so much more.
In Episode 70 I sit down with Gastroenterologist Dr Will Bulsiewicz to talk about Ex Vegans. Specifically we discuss the typical journey of these folks who have bailed on Veganism and the gut health issues they have described. Dr B and I drill down on what is happening with the aim of helping you if you ever find yourself in a similar position. By way of background this episode was inspired after a handful of Vegan influencers bailed on Veganism and reintroduced animal products back into their diet. One after the other, in what seemed like a 3-6 month period, it became a ripple effect where some of the most prominent influencers jumped on board and also reintroduced animal products into their diet. I listened to all of their stories and noted many similar patterns. I also noted that despite none of them being qualified in Medicine or the field of Nutrition/Dietetics many people in the comments were second guessing their own diet. It became apparent that the size of their following almost made their qualifications irrelevant. I'm all for influencers and I don't think everyone needs a degree to talk about a subject. However, the devil is in the detail of what's being spoken about and with life threatening eating disorders absolutely rife, we need to be careful how certain information is shared and who we, as individuals, go to for qualified information. I realised the vulnerability of these people and the vulnerability of their community, who had unknowingly been getting advice about nutrition from people who were privately going through pretty significant battles with food & their health. So with that in mind I thought it was really important for me to get a world leading Gastroenterologist on the show to make sense of what's going on here. Its really important to note that the case study we discuss in this is 100% hypothetical. It's the typical journey spoken about by these influencers but isn't specifically about any single person in particular and I think you will see by the way the conversation goes that we both have absolute empathy for anyone struggling with their diet.
