It can feel like it happens overnight. Suddenly there is fear where there wasn't fear before. Anxiety that doesn't make sense. Obsessive thoughts. Out of nowhere rituals and OCD like behaviors. Maybe tics. Rage that feels completely out of character. Sleep goes to heck. Separation becomes unbearable. And you're left standing there wondering what just happened to my child.If you are listening to this and your chest feels tight because this is your story, I need you to hear me. You are not imagining this. Your child did not just decide to become someone else. And you are not dramatic for knowing something deeper is going on. There is a growing awareness in psychiatry that not all psychiatric illness is psychiatric— some percentage of what presents in psychiatric settings is actually triggered by autoimmune responses to various pathogens and insults.  In the late '90s a syndrome called PANDAS (pediatric autoimmune neuropsychiatric disorders associated with strep) came on the scene with cases of sudden onset OCD and tics that appeared in conjunction with strep infections. The theory behind PANDAS was that parts of the basal ganglia of the brain had exterior cell proteins which were very similar to those of the strep bacteria, and as the immune system began to attack the strep bacteria, it also began an autoimmune assault on parts of the midbrain, including the basal ganglia, thus producing a sudden onset of OCD or tics.In recent years PANDAS has evolved into a broader concept called PANS— pediatric acute onset neuropsychiatric syndrome— which includes a vast array of neurological and psychiatric symptoms and syndromes that can potentially be triggered by autoimmune reactions. It can be tricky to diagnose PANS….this is probably why it is still a somewhat controversial diagnosis in some arenas. We don't have a clear and consistent set of labs or biomarkers to positively determine PANS, and because the syndrome is so broad in how it can present, clinicians struggle to come up with a modal or average presentation that they can put in their pattern recognition systems. One feature that seems fairly classic for PANS is that the symptoms tend to increase during and/or right after a concomitant infectious illness, presumably because the immune system activation is reigniting the autoimmune attack on the central nervous system.Today's story features a mom, Kari, and her daughter, Zoe. About My Guest: My guest for this episode is Dr. Neil Nathan.  Neil Nathan, MD has been practicing medicine for over 50 years and has been Board Certified in Family Practice and Pain Management and is a Founding Diplomate of the American Board of Integrative Holistic Medicine and a Founding Diplomate of ISEAI. This week on the show Maine and Miranda recap of UFC BJJ 3 headline by two title fights and some incredible performances. We also recap the 2025 IBJJF NoGi Pan championships. Plus Preview Grapplefest 19. In the recap section of the show we kicked off with the UFC BJJ 3. Kicking it off with the 135 pound title fight between Mikey Musumeci vs Kevin Carasco that saw mikey win impressively in the first round with his signature Mikey lock. In the Co-main event we discuss Carlos Henrique vs Matheus Gabriel where Carlos retained the 155 pound title by decision. We also recap the rest of the card including an exciting match with title implications between Sam Nagai vs Kennedy Maciel that saw Nagai take the win by decision. Cassia Moura vs Alex Enriquez where Cassia takes the match by decision, and we talk ADCC Trials implications. We discuss the rematch between Jansen Gomes vs Gustavo Batista that saw Jansen once again take the decision win. In Kris Olivo vs Andy Murasaki we discuss Andy Kimura win in round 3. In Andy Varela vs Daniel Sathler we discussed the timing of Andy tricep drag to the back take and Rear Naked Choke. We discussed Shye Lilly vs Ana Mayordomo where Ana won both at UFC and IBJJF in the same weekend and Jalen Fonacier vs Pedro Nakano that saw an impressive fast heel hook for Jalen to take the match. In our recap of the 2025 IBFFJ NoGi Pans we broke down and recapped every Adult Black Belt finals match and the result, including some odd scoring and refereeing. In the preview section we discuss Grapplefest 19 card. Recorded 10-6-2025

I'm Dr. Daniel Cameron, and today I'm addressing a question I often see in my practice: What are the risk factors for chronic Lyme disease?Chronic Lyme SymptomsSome patients do not fully recover from Lyme disease. They experience a broad range of symptoms, including ongoing fatigue, pain, sleep problems, neurologic changes, emotional strain, and disruption of daily life. These challenges can affect school, parenting, and work responsibilities.Common chronic Lyme manifestations include:Musculoskeletal: chronic arthritis, muscle pain, stiffness, and tendon inflammation.Neurologic and psychiatric: brain fog, memory issues, neuropathy, sensory changes, depression, irritability, mood swings, and PANS. Post-treatment Lyme disease syndrome (PTLDS) is often debated, but I view it as a potential ongoing infection rather than simply a syndrome.Cardiovascular/dysautonomia: POTS, Lyme carditis, arrhythmias, chest pain, and dizziness.Other manifestations: sensory overload (light, sound, heat, cold, or smell sensitivity), sometimes related to dysautonomic issues.Risk Factors for Chronic Lyme DiseaseWhile formal assessments are ongoing, in my practice I see several consistent contributors:Severe initial infection such as neurologic Lyme meningitis or carditis.Treatment delays, sometimes months or years.Early systemic involvement at onset—widespread fatigue, pain, neurological symptoms, or functional loss.Co-infections such as Babesia and other tick-borne pathogens.Reinfections and relapses, which can increase the likelihood of chronic complications.Key Takeaways for CliniciansScreen patients carefully for these risk factors.Monitor for co-infections, especially in high-risk or relapsing patients.Do not dismiss persistent symptoms, even if a formal diagnosis has not yet been established.Advice for PatientsSeek early treatment—timing matters.If symptoms persist, pursue a second opinion or find a physician experienced in managing chronic manifestations of Lyme disease.Watch for co-infections, especially Babesia, which may complicate recovery and even mimic other conditions (e.g., menopause).Advocate for comprehensive care for yourself and your family.Thank you for joining me.
