Is progesterone the most misunderstood hormone in women's health? For decades, women have been told that progesterone is only necessary to protect the uterus. But what if that's only a small part of the story? In this fascinating and thought-provoking episode of The Girlfriend Doctor Show, Dr. Anna Cabeca welcomes renowned OB-GYN and integrative medicine pioneer Dr. Felice Gersh for a deep dive into the science, controversy, and clinical nuances of progesterone therapy in menopause. Drawing on more than 45 years of clinical experience, Dr. Gersh explains why she believes progesterone is far more than a hormone for pregnancy and uterine protection. Together, she and Dr. Anna explore progesterone's role in brain health, inflammation, metabolism, cardiovascular wellness, sleep, cognition, and healthy aging. They discuss why hormone therapy should never be one-size-fits-all, the differences between progesterone and synthetic progestins, the growing evidence supporting cyclic hormone therapy, and how delivery methods—including oral, vaginal, transdermal, and rectal progesterone—can produce dramatically different effects in the body. The conversation also tackles one of the most controversial topics in women's health: hormone therapy after breast cancer. In this episode, you'll learn: • Why progesterone is a "life hormone," not just a reproductive hormone • The surprising neuroprotective effects of progesterone • How oral progesterone differs from vaginal and transdermal delivery • Why cyclic hormone therapy may more closely mimic natural physiology • The role of allopregnanolone in sleep, mood, cognition, and brain health • How inflammation influences breast cancer risk more than many women realize • Why individualized hormone therapy remains essential for optimal aging If you've ever wondered whether you're using progesterone correctly—or if you've been told you don't need it after menopause—this episode may completely change how you think about hormone health.   Key Timestamps 00:00 – Introduction to Dr. Felice Gersh and why progesterone remains one of the most misunderstood hormones in medicine. 04:30 – Why menopause is a metabolic transition, not simply the end of fertility. 08:45 – Dr. Gersh explains why progesterone should not be viewed solely as uterine protection. 14:20 – The concept of progesterone as a "life hormone" and its effects throughout the body. 21:30 – Cyclic versus continuous hormone therapy and what nature can teach us about hormone rhythms. 28:15 – Neuroprotection, traumatic brain injury research, cognition, and progesterone's impact on brain health. 36:40 – Progesterone versus synthetic progestins: why the distinction matters. 43:55 – Oral progesterone, allopregnanolone, and the surprising effects on sleep, mood, and cognition. 58:10 – The science behind vaginal, transdermal, and rectal progesterone delivery. 1:09:20 – Postmenopausal bleeding, endometrial protection, and important clinical considerations. 1:18:45 – Progesterone, breast cancer, inflammation, and informed consent. 1:31:00 – Final thoughts on individualized hormone therapy and empowering women through education.   Memorable Quotes "I don't call them sex hormones. I am delighted to welcome Dr. Felice Gersh for part two of our conversation on progesterone, brain health, and cognition. Dr. Gersh is a valued colleague, friend, and mentor. She is a board-certified OBGYN in integrative medicine. She taught at the USC Keck School of Medicine for 12 years and is the founder and director of the Integrative Medical Group of Irvine. Today, we continue our conversation by exploring the liver's role in detoxification, dosing, and alternative routes of administration for progesterone. We cover the effects of allopregnanolone, the timing hypothesis, how to optimize hormones during perimenopause and menopause, the impact of cycling hormones in menopause, optimal estradiol levels, progesterone intolerance, and paradoxical effects. We also discuss growth factors in estrogen cycling, mitochondrial health and inflammation, how shift work disrupts circadian rhythms, and immune system changes in menopause. This is a conversation you will definitely want to revisit. Dr. Gersh truly challenged my thinking about hormone therapy with her simple but powerful message about the importance of considering the bigger picture when it comes to hormone replacement therapy. IN THIS EPISODE, YOU WILL LEARN: How reproductive hormones influence cognition, mood, and focus The cognitive changes, brain fog, and memory lapses that occur during menopause How allopregnanolone, progesterone's metabolite, can either support calm and sleep or potentially accelerate cognitive decline over time How different routes of progesterone administration (oral, vaginal, rectal) produce very different outcomes Dr. Gersh challenges the assumption that static hormone dosing aligns with the evolution of female physiology.  How localized symptoms act as indicators of what could be happening in the brain, bones, and cardiovascular system The benefits of using progesterone in cycles rather than continuously How estradiol and progesterone decline in menopause can lead to low-grade inflammation, shifts in immune function, and reduced mitochondrial efficiency How shift work disrupts circadian rhythms Connect with Cynthia Thurlow   Follow on X, Instagram & LinkedIn Check out Cynthia's website Submit your questions to support@cynthiathurlow.com. I'm thrilled to have Dr. Felice Gersh join me for a two-part conversation focusing on menopause and HRT. She is a prolific voice in the integrative gynecology space, a true mentor, and someone I respect enormously. In the first part today, we dive into progesterone and its wide-ranging systemic effects. We unpack the differences between synthetic and bioidentical hormones, discuss the Women's Health Initiative, examine the role of progesterone in cardiovascular disease, and look at the importance of nitric oxide production. We explore ovarian senescence and how we accelerate our ovarian aging. Dr. Gersh explains how declining nitric oxide production relates to aging and shares ways to achieve optimal health during menopause. We also discuss how PCOS impacts fertility and longevity, how estradiol and progesterone modulate neurotransmitters, and how they impact brain health and cognition in perimenopause and menopause. This is truly a conversation you will not want to miss. Be sure to stay tuned for my second discussion with the insightful Dr. Felice Gersh. IN THIS EPISODE, YOU WILL LEARN: The value of progesterone beyond simply protecting the uterine lining What progesterone does that synthetic progestins cannot do How the risks and side effects of synthetic progestins unfairly tarnished the reputation of progesterone How progesterone supports nitric oxide signaling and vascular health The essential role progesterone plays in stabilizing the nervous system The role of progesterone in supporting mitochondrial function, musculoskeletal health, and connective tissue repair Symptoms that could appear or worsen when progesterone levels are low Dr. Gersh reframes ovarian aging as a core marker of aging in women  Factors that could impair ovarian function and hormone production Connect with Cynthia Thurlow   Follow on X, Instagram & LinkedIn Check out Cynthia's website Submit your questions to support@cynthiathurlow.com Join other like-minded women in a supportive, nurturing community: The Midlife Pause/Cynthia Thurlow  Cynthia's Menopause Gut Book is on presale now! In this IMMH 2023 conference presentation on "Sex Hormones and their Impact on Mental Health", Dr. Gersh explains why ovarian hormones influence so much more than reproduction – and why they are a missing ingredient in many conversations about brain health, mood disorders and cognitive decline. She highlights the disproportionate burden of depression and anxiety in women and notes that nearly a quarter of women in their 40s and 50s (peri/early menopause) are being prescribed antidepressants, arguing that hormone health must be considered as part of the bigger clinical picture. Dr. Gersh is a dual board-certified OB-GYN and integrative physician, founder and director of the Integrative Medical Group of Irvine, and a leading voice in women's hormonal and metabolic health. In this session she combines clinical and scientific data and decades of experience to demonstrate that estradiol (E2) plays a central, protective role in the brain, supporting energy production, regulating inflammation and maintaining neurological resilience. In this very informative and practical talk, Dr. Gersh outlines how hormonal decline and disruption can increase vulnerability to mood disorders, migraines, cognitive symptoms and neurodegeneration – and how optimizing hormones through lifestyle, nutrition and psychological therapies can help protect long-term brain health. The overall message is both preventative and hopeful: many hormone-related mental health risks are modifiable and treatable. In this episode, you will learn: Why sex hormones function as whole-body "life/health hormones," supporting brain, metabolic and immune health – not just reproduction. How hormones are a missing piece in many discussions of depression, anxiety and cognitive decline in women. That almost 25% of peri- and early-menopausal women are prescribed antidepressants, highlighting the scale of midlife mood symptoms. The association between teen oral contraceptive use and higher lifetime cardiovascular events, including heart attacks and strokes. How estradiol (E2) is strongly neuroprotective, supporting brain energy metabolism and cellular resilience. Why women have approximately 2.5× the incidence of Alzheimer's disease, and how hormone loss may contribute to that risk. How estradiol helps regulate microglia, astrocytes and neuroinflammation, keeping the brain's immune response balanced. The role of estradiol in maintaining the blood–brain barrier, and how deficiency may increase inflammatory vulnerability. Emerging evidence that low-dose estradiol therapy may support mood stability, including in postpartum depression and certain bipolar presentations. Why progesterone is also neuroprotective and anti-inflammatory, with important roles in mood, mitochondrial function and overall brain health.

The same with estrogen.” That's right, she is back to challenge the conventional "lowest dose for the shortest time" approach to menopausal hormone therapy, explore why menopause creates a default pro-inflammatory state, how different estrogens activate different receptors, and why understanding these distinctions matters for cardiovascular health, bone density, brain function, immune response, and metabolic health.Episode Overview (timestamps are approximate):(0:00) Intro/Teaser(2:00) Welcome to an Estrogen Masterclass(6:00) The Estrogen Family: E1, E2, E3, E4 Explained(15:00) Alpha vs. Beta Receptors & Why It Matters(27:00) Menopause, Inflammation & Gut Health(33:00) Menopause as a Hormone Deficiency State(45:00) Why the Women's Health Initiative Got It Wrong(59:00) When to Start Hormone Therapy (Age 35?)(1:13:00) Bioidentical vs. Synthetic Hormones(1:22:00) Vaginal Estrogen: Canary in the Coal Mine(1:32:00) BiEst & Topical Estrogen for Skin(1:40:00) After Party: Dr. Stephanie's TakeawaysResources mentioned in this episode can be found at: https://drstephanieestima.com/podcasts/ep453We are grateful to our sponsors:KENETIK - You think carefully about how you fuel your body but are you fueling your brain? We connected once before in Episode 221, where we spoke about PCOS and metabolic health. Today, Dr. Gersh joins me to dive into perimenopause and menopause. We discuss various issues surrounding the Women's Health Initiative and talk about estrogen as a family of hormones, estrogen-mimicking chemicals, synthetic hormones, and oral contraceptives. We get into the importance of thyroid function, the pro-inflammatory state of a loss of estrogen in perimenopause and menopause, and the importance of nutrition and exercise for metabolism. We also speak about the lesser-known changes that occur in menopause, including changes in body odor, constipation, sarcopenia, osteoporosis, and vocal changes.  I intend to invite Dr. Gersh back again to discuss women and mitochondrial health, and then once again to talk about her latest book, Menopause: The Fifty Things You Need to Know.  Stay tuned to find out what menopause really is, and what it does to a woman's metabolic system. In this second part of my conversation with Dr. Felice Gersh, we shift the focus to progesterone and what it really means for your health. You've probably heard that progesterone is only important if you have a uterus, or that it's just about preventing uterine cancer, but that's far from the whole story. You'll hear why oral progesterone, the most common form prescribed, may actually be doing more harm than good, how it creates unnaturally high levels of certain metabolites that can sedate the brain, and why this could even impact memory long-term. We also talk about safer and more effective ways to use progesterone that actually work with your body. And if you're in menopause, or even decades past it, you'll discover why cycling hormones still matters and how it can support your brain, bones, immune system, and thyroid health. This conversation will challenge so much of what you've been told and open the door to thinking about hormones in a whole new way.For full show notes, please see:  https://innatopiler.com/podcasts/connection-progesterone-estrogen-hashimotos-dr-felice-gersh-2/Get ThyroLove - the first all in one bottle multi-nutrient comprehensive formula designed specifically for those with Hashimoto's at ThyroLove.com - use code “Podcast” to get 10% off and free shipping For more information about everything Hashimoto's please visit InnaTopiler.comIf you are struggling to lose weight with Hashimoto's, Inna has a 10 day plan just for you at InnaTopiler.com/jumpstartIf you need help with fatigue or brain fog with Hashimoto's, please check out Inna's 9 Day Exhaustion Solution at innatopiler.com/energyIf you don't yet know your thyroid type, please be sure you sign up for Inna's next free training at InnaTopiler.com/zoomcall

We have a mashup episode today, exploring heart health during menopause and the benefits of hormone replacement therapy.  In this episode, Dr. Felice Gersh dives into the role of estrogen in immune function, inflammation, and heart health. Dr. Deb Matthew highlights the importance of assessing cardiovascular risks and hormone balance in perimenopausal and menopausal women to optimize heart health and minimize reliance on statins. Join us as Dr. Matthew and Dr. Gersh share their insights on heart health, hormones, and menopause. Ep. 356: Menopause, Heart Health, and HRT with Dr. Felice Gersh [00:01:53] Dr. Gersh guides listeners in understanding and appreciating the impact of estrogen on immune function, inflammation, and the healing processes within the body. [49:33] Estradiol is vital for heart health, and its decline after menopause can lead to issues like stiffening of the heart, heart failure, and arrhythmias in women. Ep. 325 Menopausal Toolkit: Heart, Brain and Hormonal Health with Dr. Deb Matthew [00:01:30] Most women fear breast cancer more than heart disease, even though cardiovascular disease is the leading cause of death in women. Yet, clinicians fail to talk enough about the effectiveness of hormone replacement therapy in mitigating the risk of heart disease during perimenopause and menopause.  [00:04:04] While estrogen can benefit the cardiovascular health of women, the risks associated with oral estrogen and synthetic forms, including blood clots and artery plaque instability, have led to hesitancy in using hormone replacement therapy despite its potential long-term benefits when administered correctly. [00:11:26] The balance of hormones like testosterone, DHEA, and cortisol plays a crucial role in cardiovascular health. Imbalances, such as flat cortisol patterns due to stress, potentially increase the risk of heart disease. Dr. Matthew uses tests like coronary artery scans and coronary intima-media thickness to assess cardiovascular risk in patients before considering hormone replacement therapy. [00:20:22] Dr. Matthew emphasizes the importance of advanced lipid testing, addressing the nuances of cholesterol particle size and inflammation, balancing hormones, and optimizing thyroid and blood sugar levels to manage cardiovascular risk instead of immediately resorting to statins.
