Megan Gaul, a weight loss coach from Staten Island, discusses her journey and approach to helping women over 30 with 200 pounds or more achieve their weight loss goals. With a background in education, Megan emphasizes the behavioral aspects of weight loss, drawing from her own 100-pound weight loss in 2018. She highlights common mental barriers like unstructured weekends and the "it shouldn't be hard" mindset. For the transcript and full story go to: https://www.drmanonbolliger.com/magan-gaul Highlights from today's episode include: Megan states weekends with no structure often derail weekday progress. Megan explains that long-term weight loss is won or lost in the mind, not the diet. Manon talks about Naturopathic doctors aren't automatically coaches—and patients often expect both. ABOUT MEGAN GAUL: Megan is a weight loss coach from Staten Island. Her mission is to support women age 30 and up, 200 lbs and up, achieve their weight-loss goals without shrinking their lives. She came to this mission through her own 100-lb weight loss in 2018, where she discovered food freedom, healthier emotional boundaries, self-trust, and a richer life. She's now in her 5th year of business, and she and her clients focus on small habit-change, self-compassion, and authenticity. Core purpose/passion: I'm most passionate about helping women live more in-tune with what they want. So many of us live in fear for a huge percentage of our adult lives. We get stuck in people-pleasing, anxiety, and making choices only to get away from what we dislike. I want more women to feel awesome about making choices to go TOWARDS what they desire. What is cultural distress? It is a negative response rooted in a cultural conflict where the patient lacks control over their situation. It results in more physiologic effects on the body resulting in allostatic overload. To prevent this, healthcare practitioners must use strategies such as cultural humility to help patients navigate healthcare. Come find the best ways to deliver culturally sensitive care in any setting.

· Naturopathic holistic model:· Integrates lifestyle optimization (sleep, stress, diet, exercise), symptom management, and pharmacologic therapy where indicated.· Emphasis on patient empowerment and sustained habit formation recognizing the effort required for meaningful change.· Preventive care:· Screening labs for cardiometabolic risk (e.g., diabetes, heart disease) based on age and symptoms.· Body composition focus (muscle mass maintenance), and standard preventive screening (mammograms, Pap tests).· Cycle tracking enhances staging awareness and guides care decisions.· Interprofessional collaboration:· Patients benefit from combined support of family physicians and naturopathic doctors; coordination emphasizes patient-centered care rather than provider turf.· Nursing practice alignment with holistic ethos: empathetic listening, normalization of experiences, and comprehensive symptom support.

In this episode, I break down the major categories of medicine— allopathic, functional, naturopathic, ayurvedic, Chinese, homeopathy, and integrative—explaining what each does well and where each falls short. I then show why longevity medicine goes further by treating aging itself as the root cause. You'll learn how the 12–16 hallmarks of aging accelerate each other, why symptom management or root-cause treatment alone can't stop the process, and why measuring and addressing the hallmarks directly is essential. I share why we hold ourselves to the standard of a 30-year-old body and a 300-year-old mind, how stress speeds aging, and why the questions your practitioner asks determine your results. If you've been bouncing between doctors without answers, this episode will show you what to look for next. The chat really dives into a deep philosophical and super-practical look at healing, all sparked by Peter Galle's firsthand experience with a pretty serious health hurdle—a herpetic cold sore infection in his eye. He ran the full gamut, hitting it with conventional treatments like Valtrex alongside a ton of classic antivirals like Coptis (Huanglian) and Skullcap. But nothing was truly curative or stopping the progression of the ulcer. He was ready to give up and hit the emergency room for steroids when, as a last-ditch "recesses of my mind" shot, he took a large dose of St. John's wort internally. Peter describes a near-instant, profound cooling effect, and the infection began to resolve. This miraculous, boundary-pushing moment opens up the main thesis of the conversation: the deep, messy intersection of medicine, politics, and personal philosophy. Peter and you talk about the frustration of the "magical gap" that exists between highly institutionalized, textbook-based medicine (TCM, Naturopathic, or conventional) and the unique, individual healing path. You both stress that medicine is at its best when it is pragmatic, clear, and focused on the patient's best outcome—calculating the risk of a harsh intervention (like short-term steroids for joint destruction) against the long-term consequences. The biggest hurdles aren't just the pathogens; they're the practitioners' internal politics and the ego-driven desire to make their own worldview or model right. The conversation broadens to the macro-level: how our non-integrated, moralizing medical system—and even humanity's inappropriate relationship with the Earth—prevents us from truly seeing and honoring the unique path to healing for the suffering individual. Ultimately, the path forward is rooted in humility and a non-judgmental approach to meeting the patient where they are. Peter & Mason discuss: St. John's Wort "Magic": High-dose St. John's wort resolved a stubborn eye infection when other treatments failed. Pragmatism Over Dogma: Choose the most effective, lowest-risk treatment—even if it mixes approaches. Flaw in the Model: No medical system is complete; all are shaped by cultural limits. Healing "Gap": Standard treatments don't always work due to individual differences. Individual Uniqueness: Treat each patient as unique; personal response matters as much as studies. Internal Politics: Ego and bias can hinder effective treatment decisions. Humility & Integration: True healing requires humility and blending different medical approaches. Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Sarah: Hey doc! Ive been regularly gua sha'ing my face and I noticed that my face feels 'crunchy' especially at my jawline and at my lymph nodes by my ear. It doesnt seem to be improving despite regular use, is this normal or anything to be concerned? I feel it more in my left side which I seem to get more breakouts on my cheek/jawline/neck. Thank you! Margie: I just read all your cancer information because of a recent diagnosis. I have a Naturopathic doctor in addition to conventional oncologist. I'm curious your take on Esaic tea and or soursop? I told not to take it during treatment because it's a detox and we want the targeted chemo and immunotherapy to work… this is from my ND, however, research on both suggest otherwise. I'm very curious your thoughts. Thank you! Alicja: Hello Dr.Cabral, I recently run by blood work and urine test. I have pretty good results from my blood work (except high cholesterol) but I am concern about results from my urine test. It shows I have trace amount of blood in my urine and also albumin in my urine was detected (slightly above the optimal cut-off) My urine albumin is 0.3 mg/dL What can I do about this? Does that indicate early kidney damage? (BTW test was done on day 16th of my cycle) Thank you for answering my question. I appreciate all you do. Tyler: I'm a 37 year old male and I was diagnosed with a pilocytic astrocytoma brain tumor in my hypothalamus in December 2022. It was a benign tumor the size of a large egg, and I had a 15 hour surgery to remove 90% of it. 3 years later, I still have erectile dysfunction and low libido. Why is that and what can I do to fix it? I'm on Clomid for my testosterone, which is now around 500. I was on testosterone gel for a couple months but quit because of sperm impact. I test my sodium level every other week because it was low at times after my surgeries. I have a programmable shunt to help drain CSF. I don't have morning erections. Could this be from damage during the brain surgeries? Is it hormonal-related even though I'm on Clomid? Is it psychological? Please help. I listen to your podcast daily. Trish: Hi Dr C. - hoping to get some advice on your 7-day detox. I'm in my 50's and most of my life my weight has gone up and down by 30lbs. I have a problem with binging. As I've gotten older, I've learned how to control it and my triggers. I've tried the detox 3 times and I have found that it causes me to binge. Becoming too strict it throws me for a loop. I don't do the two fasting days. I fast 20 hours. I avoid all the inflammatory foods gluten, dairy, corns, eggs & nuts as I do this on a daily basis so that isn't hard for me. Giving up berries/fruit in my smoothie & some quinoa or brown rice is what starts to create me wanting to binge. I usually eat 1 sq a day 90% cacao choc too. Any advice on making the detox beneficial without following the guidelines 100%? Longevity, Cash PT, and Skating Where the Puck Is Going In this episode of the PT Entrepreneur Podcast, Doc Danny talks about why he keeps coming back to one big theme: longevity. He looks at how the market around proactive health, functional medicine, and long-term performance is exploding and why cash-based clinics are perfectly positioned to play a major role. If you want to move beyond "fix the injury and discharge" and build an ongoing longevity offer, this episode lays out the opportunity and the mindset behind it. In This Episode, You'll Learn: Why patient experience is a competitive edge in cash-based practices How Claire gives you an operational advantage your patients can actually feel Why Danny has always tried to "skate where the puck is going" in healthcare How cash-based PT went from rare to common in a decade Why functional medicine and longevity clinics are booming The role PTs can play as movement-focused, accountability-driven "quarterbacks" How one training partner's transformation turned into a walking case study Why generational health change makes this work bigger than a single patient Ways to start building or partnering into a longevity offer inside your clinic Claire: The Patient-Experience Edge in a Cash Practice Danny opens by talking about what really matters in a cash-based clinic: patient experience. When people are paying out of pocket, they notice everything. He makes a simple comparison: While your competitors step out mid-session to catch up on notes, you stay fully engaged. While they stay late at the clinic finishing documentation, you are following up with patients and planning their next visits. That is the competitive edge Claire gives you. Claire is PT Biz's AI scribe, trained specifically for physical therapists. It handles your documentation instantly in the background, so your time and attention stay on your patient, not on your EMR. The result: Better in-room experience Better retention and follow-up Smoother, more efficient operations Try Claire free for 7 days: https://meetclaire.ai Skating Where the Puck Is Going Danny has always tried to pay attention to where health and wellness are headed, not just where they are today. Back in 2014, when he and his wife opened Athlete's Potential in Atlanta, cash-based PT clinics were rare. He only knew of one other in the city, but he saw more and more of them popping up on the West Coast, especially in California. That was his signal that a trend was forming. Fast forward more than a decade and there are now dozens of cash-based clinics in Atlanta alone. Many of them are true businesses with teams, multiple locations, and the kind of systems that support seven-figure revenue and even sales to private equity or hospital groups. That bet — skating to where the puck was going — paid off. The Next Wave: Longevity and Proactive Health Now, Danny sees a similar wave building around longevity and proactive healthcare. He shares the story of a training partner he has worked out with for the past couple of years. Together they have tracked: Blood panels year over year Body composition with tools like InBody Sleep and recovery data using wearables like Whoop The changes in that friend's biomarkers, physical capacity, and day-to-day energy have been dramatic. Friends who have known him for years almost do not recognize how much healthier and more capable he is. That kind of transformation is exactly what more people are starting to want. And the broader market is responding. Functional Medicine and Longevity Are Booming Danny points to the rapid growth of functional medicine, lifestyle medicine, and longevity-focused services as a sign this is not a fad. He has seen: Naturopathic and functional medicine clinics expanding quickly Providers leaving hospital systems to start proactive, integrative practices High-end gyms and programs charging tens of thousands per year for bundled health, testing, training, and recovery When he first looked for a functional medicine provider in Atlanta, there was one very expensive option. Today there are multiple. Even family members of his who were deeply rooted in traditional medical systems have shifted into functional and lifestyle medicine because they want to help people earlier, not just when they show up critically ill. The PT's Role in the Longevity Ecosystem Danny is clear: he is not saying physical therapists should try to become functional medicine doctors. Instead, he sees a natural lane where PTs can win: Movement and musculoskeletal health experts Accountability partners who help people actually implement changes Educators who can translate research and trends into safe, practical steps He has already tested this in small ways at Athlete's Potential — reviewing blood panels, talking through sleep data, adjusting training, and updating exercise programs over months and years as patients move from "out of pain" to "performing and staying healthy." For some people, that relationship has lasted for years, shifting from acute rehab to long-term physical and lifestyle coaching. Blue Ocean: Ongoing Longevity Coaching for the Right People Danny describes this longevity space as a "blue ocean" for the right clinics: There are more and more people who want proactive help with their health. There are relatively few trustworthy, movement-focused providers offering it in a structured way. He draws a line between evidence-based functional and lifestyle medicine providers and more fringe offerings that are heavy on hype and light on science. A clinical background, understanding of research, and experience with musculoskeletal care give PTs a strong foundation to cut through the noise for their patients. And you do not have to do it alone. You can: Build your own longevity-style continuity offer inside your clinic, or Partner with functional medicine or lifestyle medicine providers and stay focused on movement, strength, and physical capacity. Generational Health Change One of the most powerful parts of Danny's story is the ripple effect he has seen in his training partner's life. By changing his own habits — training, sleep, stress management, nutrition — that friend has also influenced his entire family and friend group. Kids see what their parents do and assume it is normal. Friends see what someone has done for their health and start asking questions. Danny calls this "generational health change." You are not just helping one person feel better. You are changing what feels normal for the people around them, including their kids. From "Your Knee Feels Better" to "What Do You Want Life to Look Like at 80?" So what does this look like in a practical way inside your clinic? Danny suggests starting with a simple shift in conversation once an injury is under control: Talk about how long they want to be functional and independent. Ask what they want life to look like in their 70s and 80s. Use the older adults you have seen on both ends of the spectrum as examples. From there, you can start to build ongoing support — programming, check-ins, movement testing, and education — that helps them move toward that long-term vision instead of just away from short-term pain. Is Longevity a Fit for Your Clinic? Danny is not saying every clinic has to add a longevity offer. If you like what you are doing now and your business is healthy, that is okay. But he does believe this is where a big part of the market is heading. People are more aware, more curious, and more willing to invest in staying capable longer. For clinics that want to play in that space, now is the time to start paying attention and experimenting. Ep. 27 | The Naturopathic Midwife - Michele Sayball by Sofia Scheuerman
Guest: Dr. Hannah Lawler, Naturopathic Doctor (ND)Core Theme: Moving beyond the "pill for every ill" mentality to embrace nutrition (The Bean Protocol™), individualized hormone support, and the powerful signaling of the female cycle.Key Takeaways & Highlights1. The Luteal Phase "Truth Serum"Uni and Hannah discuss the emotional intensity often dismissed as "just PMS."The Luteal Narrative: The luteal phase (the week before your period) acts as a "BS filter." While it can feel like "lunacy," it often highlights real issues in life or relationships that we suppress during the rest of the month.Actionable Advice: Don't make life-altering decisions (like divorce) during this phase, but document the feelings. Use them as data points to address once your hormones settle.2. Naturopathic vs. Conventional MedicineDr. Lawler clarifies the rigorous path of an ND:Training: Four years of medical school, including biochemistry, anatomy, and pharmaceuticals, plus extensive training in nutrition, homeopathy, and hydrotherapy. The "Village Doctor" Approach: Hannah operates a concierge practice, making house calls. This allows her to see a patient's environment—poking around the pantry to see what they think is healthy versus what actually supports their biology.3. The Foundation of Nutrition: The Bean ProtocolBoth Unique and Hannah emphasize that nutrition is not a "fad" but the literal building blocks of cells.Soluble Fiber (Beans & Psyllium): Essential for clearing recycled estrogen and toxins from the liver. This is the "insurance policy" for anyone on Hormone Replacement Therapy (HRT).Personal Responsibility: You cannot drive a "Ferrari" (your body) and put in the cheapest fuel. High-performance health requires high-quality, consistent inputs.4. Advanced Optimizations: Peptides & GeneticsWhen the foundation of nutrition is set, Dr. Lawler looks at "levers" to move the needle:Peptides: Signaling molecules that tell cells to perform specific tasks (e.g., recovery, skin health, or mood). Hannah highlights GHK-Cu (copper peptide) for anti-aging and Thymosin Beta-4 for recovery.Genetics (3x4 Genetics): Understanding "bottlenecks," such as the COMT gene, which affects how you process caffeine and adrenaline. Knowing your genetics removes the "DIY" guesswork from health.5. Thyroid Health & The "Canary in the Coal Mine"The discussion touches on why Hashimoto's is so prevalent today:Environmental Toxins: Halogens like fluoride, chlorine, and bromine can "plug" iodine receptors in the thyroid, leading to dysfunction.Root Cause: It's never "just" the thyroid; it's a response to stress, viruses, or environmental toxicity (like mold).

In this episode Matthew Hicks, ND, MS joins to dive into the topic of psilocybin-assisted group therapy for depression. Dr. Hicks is a research Investigator at the National University of Natural Medicine as well as a Naturopathic doctor and licensed psilocybin facilitator at Synaptic Institute. In this conversation, Dr. Hicks shares findings from one of the first studies investigating psilocybin-assisted group therapy for depression, conducted in Oregon's new legal psilocybin framework. He explains how the high cost and labor-intensive nature of psychedelic therapy inspired him to explore a group model that could make treatment more financially accessible while preserving - and even enhancing - its therapeutic potential. Dr. Hicks describes the structure of the study and discusses how initial participant hesitancy about group work transformed into deep connection and shared healing. He also highlights the study's significant reductions in depression scores, improvements across quality-of-life measures, and the potential for group-based approaches to pave the way toward insurance reimbursement and broader access to psychedelic care. In this episode, you'll hear: Why affordability and accessibility were central motivations for developing a group-based psilocybin protocol The benefits and challenges of conducting psilocybin sessions in a shared group setting How Dr. Hicks' study balanced inclusion of low-income participants with safety and stability criteria The details of the group treatment structure for Dr. Hicks' study Why Dr. Hicks believes group formats may be inherently therapeutic in addition to their economic efficiency Dr. Hicks's vision for future efficacy and cost-effectiveness studies that could enable insurance coverage Quotes: "In terms of the group dynamic, almost everyone in the intake process was very reluctant. They were trepidatious. They were a little worried about the group part of it. And almost everybody at the end of it, when we did the follow ups at the group, was amazing. People made friends. They felt really supported. They felt really seen by the process of hearing other people's journeys and the growth that they went through—and seeing some other examples of transformation was really powerful and was really encouraging to me." [10:29] "So [there are] really positive aspects to doing this in a group format that's not just economic—it's not just cheaper to do this in group, it actually has therapeutic benefits that you miss out on when you only do this one on one." [11:12] "That was always my question in the follow up sessions: 'did your participation in this study change the way you engaged in psychotherapy? Did it change the relationship with your therapist?' And a lot of people reported that it did. They felt they were able to open up and engage more deeply, be more introspective. And it did, in many cases, not all, improve their psychotherapy outcomes as well." [18:24] "Some people reported that hearing someone else in the group crying for a bit really opened up something in them and they almost felt grateful for that. This other person is having a meaningful experience over there, and that's something they wouldn't have gotten on their own if they hadn't heard that person crying."
