Dinesh reflects on his early years growing up in Byron Bay, Australia, recalling fond memories of the idyllic coastal town where he spent his formative years. From bike rides to flying lessons, Byron Bay was a place of joy and discovery for him. However, his path to medicine wasn't straightforward. Initially considering a career in law, Dinesh's life took a different turn when he experienced depression during his time in law school. Inspired by the doctor who helped him through this challenging period, Dinesh found his calling in medicine, driven by a desire to support others in the way he had been supported. His journey took a dramatic turn when he was involved in a devastating car accident that left him paralysed. Despite the immense challenges he faced, Dinesh persevered and continued his medical studies, eventually becoming an emergency department doctor at Gold Coast University Hospital. Through his work, he found a way to give back to others, drawing on his own experiences to provide compassionate care to those in need. Dinesh was the first quadriplegic medical intern in Queensland and the second person to graduate medical school with quadriplegia in Australia. Dinesh is a doctor, lawyer, disability advocate, and researcher. Halfway through medical school, he was involved in a motor vehicle accident that caused a cervical spinal cord injury. Dinesh has completed an Advanced Clerkship in Radiology at Harvard University. As a result of his injury and experiences, Dinesh has been an advocate for inclusivity. He is a founding member of Doctors with Disabilities Australia. Dinesh works in the emergency department at the Gold Coast University Hospital. He is a senior lecturer at the Griffith University and adjunct research fellow at the Menzies Health Institute of Queensland. Dinesh is a researcher in spinal cord injury. He is a doctor for the Gold Coast Titans physical disability rugby team. Dinesh is a senior advisor to the Disability Royal Commission. Dinesh was the Gold Coast Hospital and Health Service's Junior Doctor of the Year in 2018. He was awarded the Medal of the Order of Australia in 2019. Today's guest was actually a yayborhood nomination from one of our most active contributors affectionately known as Dana Saurus or Dana Pham. Dana so generously sent a whole list of suggestions for guests a while back and this is one that has finally come to fruition. She is the founder of Junior Doctor's Corner and is a doctor herself working to provide much needed support to junior doctors entering the world of medicine and discuss important topics that are often dismissed or taboo. JDC also has its own podcast so make sure to check that out if there are any other young doctors listening in. Her nomination also hails from the medical world but is not only a doctor, he is one of those rare species who has completed degrees in both medicine AND law – the holy grain combination that very few globally dare to tackle. And if that weren't enough of a superhuman feat, Dinesh Palipana is also a quadriplegic and disability advocate becoming only the second person with quadriplegia to graduate as a doctor in Australia and the first with a spinal cord injury. At just 25 years old, after a horrific car accident, Dinesh Palipana became a quadriplegic. Since then, Dinesh has become one of the most accomplished, recognised and admired figures in Australian medicine.  Dinesh lost the use of his lower body, and a large degree of function in his arms and hands. It was the beginning of a new phase of life, one that would be filled with extraordinary challenge and struggle. A phase of life that demanded resilience on a scale few of us can imagine. On this week's episode of The Risk Equation, Dr Chris Maguire explores how Dinesh recovered from the accident and began to face the realities of life through a trip to Sri Lanka, what prejudices he faced upon his return to medical school, and the amazing new innovation in the science and treatment of spinal cord injuries in recent clinical trials.   In 2020, Dinesh has been nominated as Australian of the Year,, he's a recipient of the Medal of the Order of Australia for service in Medicine, and was the 2019 Junior Doctor of the Year at Gold Coast University Hospital. Professor Gary D. Rogers is a health professions educational leader, public health researcher and GP with a focus on HIV medicine. He commenced as Professor and Dean of the School of Medicine at Deakin University in June 2020. Immediately prior to joining Deakin, he was Professor of Medical Education and Deputy Head of School (Learning & Teaching) at the Griffith University School of Medicine, as well as undertaking clinical work in the Infectious Diseases Unit at Gold Coast University Hospital. Gary gained his medical degree from the University of Adelaide 1984. In the early 1990s, he formulated and led an interprofessional primary health care team focused on HIV care and prevention in the city of Adelaide. Whist still in full-time clinical practice, he undertook the Monash University Master of General Practice Psychiatry in order to gain the skills he needed to support the survivors of the HIV pandemic following the implementation of effective treatment for the condition in the 1996. In the mid-2000s he completed a PhD in public health utilising a health inequity framework and then worked for two years across the 21 Pacific Island countries and territories in HIV care mentorship and sexual health promotion. Since joining Griffith University in 2008, Gary's scholarly work has focused on health professional education. He is a former President of the Australian and New Zealand Association for Health Professional Educators (ANZAHPE) and continues to serve on the Association's Fellowship Committee. He was a member of the Executive Committee of AMEE, the international association for health professional education, from 2013 to 2019. In 2017, he was recognised as a Principal Fellow of the global Higher Education Academy. Gary's current research focuses on the affective learning associated with the acquisition of professional and interprofessional values among health students.

An unborn NSW twin whose mother was unable get approval to enter the state in time for emergency surgery has died. The pregnant Ballina woman waited to be granted an exemption for surgery at the Gold Coast University Hospital 125km away, but ended up having to wait for 16 hours in Lismore for a flight to Sydney on Thursday.  Oliver Peterson says he's "disgusted" the border issue has had anything to do with loss of life in an emergency medical situation. Ten years ago, halfway through completing his medical degree, Dinesh Palipana's car crashed, leaving him a quadriplegic. Dinesh lost all sensory and motor function below his chest, affecting his fingers and half his arm. It took him seven months of recovering in hospital, but he emerged still determined to become a doctor. He went on to complete his medical degree, and now works as a resident at Gold Coast University Hospital in one of the busiest emergency departments in the country. Dinesh's refusal to accept the loss of his dream is paving the way not only for him, but for people with a disability around the world. He tells me on this episode of Streets of Your Town how he found his life’s purpose in medicine, and that he’s so relieved he didn’t listen to the naysayers who told him he could no longer become a doctor after his accident.

In today’s episode I chat with Chloe Mackie. Chloe has two children - Sunny (3) and Maya (2 weeks) - and is a midwife with a passion for antenatal care. “I want to empower women with knowledge and education. Ultimately I want to work in Midwife Group Practice or private midwifery because I believe that every woman should have the opportunity to have a known care provider through pregnancy and birth,” she says. Chloe was working at the Gold Coast University Hospital when she and husband, Jason, started trying for a baby. After two years and a few miscarriages, they sought the advice of a fertility specialist who told them they had unexplained infertility. In the months following they did three rounds of intrauterine insemination (IUI) but to no avail. Chloe found the experience to be emotionally taxing so on the fourth month she opted to have a break. She decided to go back to the basics - yoga, acupuncture and improving her holistic health - and she fell pregnant naturally on her next cycle.  “When we were trying to conceive people would often tell me that if I stopped thinking about it, it would happen. That was the hardest thing to hear because you can’t stop thinking about it; every minute of every day you just want a baby. And every time I got my period I’d feel like a failure. Coincidentally though, when I did start thinking about it…thinking about me and my husband and our health…we fell pregnant.” Chloe opted to have a private midwife who would support her throughout the pregnancy, attended her home birth and care for her six week postpartum. Like her mother and her cousin before her Chloe suffered from hyperemesis and was so sick in the first two trimesters that she needed constant care as she was unable to get out of bed by herself. She enlisted the help of her naturopath and acupuncturist and by 30 weeks she improved. She experienced a typical first labour with contractions over three to four afternoons and nights that seemed to stop come morning. After her fourth round of acupuncture in as many days, labour  established quickly and her contractions were strong and steady. She settled in at home where she paced and listened to music and later that evening her midwife arrived and discovered she was 5cm dilated. Not long after her membranes ruptured and there was meconium present which indicates that the baby had done a poo in utero. It’s one of the reasons that a woman must transfer to hospital from home and so Chloe and Jason made their way to Gold Coast University Hospital with their private midwife. On the car journey Chloe noticed increased back pain and when she got to the birthing suite she discovered that baby was posterior with a asynclitism presentation (his head was off to the side). “It’s like trying to push a triangle through a circle,” she says. She opted to have Sterile Water Injections - non-pharmacological pain relief which is administered by two midwives mid-contraction. Ultimately they trick your brain into thinking there’s no localised pain in that area and allows you to remain upright so you can stay active and help rotate your baby into a more optimal birth position. They only last 1-2 hours and they are very painful when they’re administered - like hundreds of bee stings - but the pain only lasts a few minutes. Chloe spent the following hours sumo stomping, walking up and down stairs and practising side-lying releases (you lay on the bed and drop one foot off to the side as someone supports your hips; it changes the diameter of the pelvis and encourages baby into better position). Unfortunately all of Chloe’s work hadn’t encouraged her baby into a better position so she opted for a second round of water injections (not a common occurrence) and went through the motions again. At 7cm she talked to the Obstetrician who was empathetic to her situation and her birth plan. They discussed an emergency cesarean and whilst Chloe accepted that it was a safe option for both her and her baby, she wanted to buy an extra hour of time and opted to have an epidural. Thirty minutes after it was administered she felt the urge to push.   “When my midwife checked me she asked me to put my finger inside to feel my baby. And he was right there! It was a perfect example of an epidural working beautifully. It released my pelvic floor muscles and allowed my baby to come down on his own. We did an hour of passive descent and then I pushed for 15 minutes and he was born!” They went home 6 hours later and Chloe was having excruciating after pains but no increased bleeding. Eighteen hours later she had taken her quota in paracetamol and Nurofen and started developing a fever so they returned to hospital thinking that she may have retained placenta but nothing was found. She was administered IV antibiotics and went home with oral antibiotics but within hours, Chloe declined rapidly.  “The pain was getting worse by the minute and I had this overwhelming feeling that something was very wrong,” she says. “I was slipping in and out of consciousness and I lost control of my bladder so Jason called the ambulance and I was given broad spectrum antibiotics on arrival.” She couldn’t feed Sunny or access donor milk so her best friend came to the hospital to feed him and continued expressing during Chloe’s hospital stay. Chloe was diagnosed with Group A Strep that had colonised in her genital tract and developed into Septicemia. It was a traumatic time but  thanks to fast acting antibiotics, she fully recovered within a few weeks.  When Sunny was two, Chloe had a miscarriage and then fell pregnant the following month. Bracing for the worst of hyperemesis, she enlisted the help of her naturopath and acupuncturist early on and whilst she definitely was sick, it wasn’t nearly as severe as her first pregnancy. At 41 + 1 she went into labour and headed to the hospital shortly afterward so she could settle into her birth space with its dimmed lights, music and birth pool.  Once again the baby was posterior with an asynclitism presentation so Chloe opted for water injections and side-lying release which worked instantly. “I felt her turn and then I slid off the bed and had the sudden urge to bear down. Jason picked me up and put me in the bath and Maya was born 12 minutes later. Dinesh was the first quadriplegic medical intern in Queensland, and the second person to graduate medical school with quadriplegia in Australia.Dinesh earned a Bachelor of Laws (LLB), prior to completing his Doctor of Medicine (MD) at the Griffith University. He has completed an Advanced Clerkship in Radiology at the Harvard University. Halfway through medical school, he was involved in a catastrophic motor vehicle accident that caused a cervical spinal cord injury.As a result of his injury and experiences, Dinesh has been an advocate for inclusivity in medicine and the workplace generally. He is a founding member of Doctors with Disabilities Australia.Dinesh is currently a resident medical officer at the Gold Coast University Hospital. He is a lecturer at the Griffith University and adjunct research fellow at the Menzies Health Institute of Queensland. He has research interests in spinal cord injury, particularly with novel rehabilitation techniques.Dinesh is the Gold Coast University Hospital’s representative in the Australian Medical Association Queensland’s Council of Doctors in Training. The former national champion suffered a medical episode in the smash on Monday, when his vehicle ploughed through a fence. He was taken to Gold Coast University Hospital but couldn't be saved. Sarah & Paddy spoke with Chris Parker, of Central Coast Lifesaving New South and reflected on the amazing life of Dean Mercer.
