He recognized that healthcare had so many operational inefficiencies that applying these principles in the emergency department could yield tremendous results through solving basic problems and capturing low-hanging fruit. The conversation explores how Greg's medical background both helps and hinders systematic thinking about business problems. He explains that physicians are trained with a scientific mindset where every patient encounter resembles an experiment - gathering evidence, forming hypotheses, running tests, and evaluating outcomes. This mirrors the problem-solving methodology used in Lean thinking, making the transition natural for some medical professionals. However, the competitive nature required to succeed in medical school and residency can create fixed mindsets and reduce curiosity, as many doctors become accustomed to being the "alpha dog" who always has the right answers. A significant portion of the discussion focuses on psychological safety and its critical role in enabling improvement. Greg defines psychological safety as "being rewarded for being vulnerable" - whether that's admitting you don't know something, raising concerns about broken processes, or challenging existing systems. Through his experience working in emergency departments across the United States and even New Zealand, he observed that departments where people felt safe to speak up consistently had better outcomes than those where the culture encouraged keeping quiet and just getting the job done. Jamie and Greg explore how technology systems can actually enhance psychological safety by creating a buffer between individuals and problems. When issues are logged in a system rather than raised face-to-face, it shifts the dynamic from personal confrontation to collaborative problem-solving. The issue becomes the common enemy that everyone works together to address, rather than a source of interpersonal tension. Greg notes that rather than reducing human interaction, electronic systems actually increase communication by creating visibility and fostering engagement around improvement opportunities. The conversation turns to habit science and its application to continuous improvement culture. Greg credits reading "The Power of Habit" by Charles Duhigg, "Atomic Habits" by James Clear, and "Tiny Habits" by BJ Fogg with transforming both his personal life and his understanding of organizational change. He explains how KaiNexus applies the habit loop concept - cue, routine, reward - to create interconnected behaviors across different organizational levels. The routine of one person becomes the trigger for another person's habit, creating a web of positive behaviors that sustain improvement culture. You can reach him at dberrymd@hotmail.com We must remember, when working up these patients with addiction, they can have poor health baseline anyway, so they are at higher risk for something emergent to be going on Dr. Berry talks about this approach to opiate OD We have a small window between the ED and definitive care to help addicted patients move forward with treatment – a warm handoff to a treatment center or primary care that can monitor recovery is key We have specific rules for most emergencies but often psychiatry is not on a set system Holds may not always accomplish what we hope they will, it will not force someone to changeBut there is certainly a role for the patient who is genuinely suicidal We talk about good structure of evaluation and treatment of the psychiatric emergencies David talks about some patients he has been able to help with addiction recovery from his experience Don't underestimate the difference we can and do make with patients just because we don't always see the result Dr. Berry talks about the message he has for clinicians:We deal with a lot of emergencies, people actively dying, so when we hear a psych complaint or substance abuse complaint, we tend to mentally shuffle them into a lower priority. He was conceived following a one-night stand and was almost aborted before being put up for adoption to a loving family. He had his first child and felt the gift of knowing his first genetically related family member for the first time.Tragically, his first daughter was diagnosed with spinal muscular atrophy, a diagnosis not compatible with life and she later died around 9 months of age.This led David to turn to alcohol to help deal with the pain. His family took notice of this, so he transitioned to opiates instead of alcohol, something that he could hide more easily. He was eventually found out and spent some time in jail and tried some treatment centers. He lost his medical license, his family left him and he became homeless.His rock bottom was when he found himself homeless living under a bridge with no ID and realized that no one would even realize if he had died. With the help of another doctor, David started taking Suboxone, which helped him to overcome opiate addiction.This opened a path to Dr. Berry getting his medical license back and eventually led to a role as chief of staff of his hospital as well as opening up a rehab clinic in Colorado. Hearing David's story helps put in perspective the fact that none of us are that far removed from the homeless, drug addicted patient we care for in the ER. In this podcast episode, we talk with Dr. Ruby Long about gun violence Dr. Ruby J. Long has been an ER doctor for a long time and has seen a lot of gun violence come through her doors. She is reaching out to help providers understand the specifics, provides a helpful acronym and is sharing her resourceful book. The Reality of Gun Violence in the U.S. 54% of U.S. adults have been directly affected by firearm violence. Gun violence has surpassed other leading causes of death for children and adolescents. Firearms are the leading cause of death in suicides. It is important that providers are able to identify risk factors, particularly for senior males and veterans, and promoting secure gun storage. How can gun violence impact the medical and mental health providers who are supporting victims? The moral injuries and trauma healthcare professionals face from witnessing the physical and mental health impacts of gun violence. Dr. Long shares her own experiences as an ER doctor and the profound emotional impact of treating gunshot victims. The importance of responsible gun storage and using the "Be SMART" approach: Secure guns. Model responsible behavior. Ask about unsecured guns. Recognize the role of guns in suicide.
