    Quality Coordinator / Management Representative, Quality Manager, what's the difference?  Well, we're going to tell you in this episode.  ISO 9001 really doesn't give much direction other than leadership and top management, but generally accepted is the role of the "Quality Manager". This means and is important that your Quality Manager has management level authority.  They need to be taken seriously and have the ability to carry out tasks and trainings as a manager would.     Ep 131 - Medical devices gone wrong with the Dalkon Shield

    The Dalkon Shield was an IUD developed in the 1960s by OB-GYN Hugh Davis with electrical engineer Irwin Lerner.  They brought in a lawyer, Robert Cohn, to help them form The Dalkon Corp.  Because The Dalkon Corp had no sales force, they decided to sell the product to another company.  That company was AH Robins, who made Robitussin and chapstick, among other things.  After more than a decade, many findings came out about the Dalkon Shield.  Instead of using a monofilament string, this IUD used hundreds of tiny filaments covered by an open-ended sheath.  The tiny spaces between the filaments provided a ladder for bacteria to climb from the vagina to the sterile uterus.     The U.S. Naval Ship Repair Facility and Japan Regional Maintenance Center (SRF/JRMC), Yokosuka detachment has thousands of gallons of oil routinely drained from ships coming into dry dock.  Special teams have to be on standby and ready at any given time to remedy a spill.  Shop X-99 consists of 65 members, 28 of which are oil specialists.  Shop X-99 also holds the seven member ‘Mada Yatte Tanda (MYT) team.  This team, in keeping with SRF/JRMC's organizational goals, decided to improve technology, specifically their burdensome, unwieldy equipment used for oil clean up. After identify stakeholders, the team used a tree diagram to describe methods and tools used in the clean up process.  After answering questions, they determined the root causes of their difficulty of clean up was mostly the machinery they used.  The team used hand-operated skimmers to chase the oil, they had to use dozens of hoses.  Set up and transportation was time-consuming and required many workers.  The team decided to completely reverse the system of clean up and since there was no funding, they had to create their own equipment with what they had on hand.  The new process and equipment exceeded the goals of the MYT team and provided many intangible benefits.     EP 129 – DUMPY LEVEL & GRADIENT MEASURING TOOLS

    In 2006, Singapore's Housing and Development Board wanted to improve work processes and better support it's own mission.  They enlisted the help of Accessway V2 Innovation & Quality Circle to help.  Accessway V2 used the Lotus Blossom Technique to identify problems and solutions.  They decided the problem with the most potential solutions would be the easiest to solve.  This revealed that the tool to check accessible ramp gradients would be the easiest problem to solve.  Also in 2006, at this time, Singapore's Ministry of National Development announce that the entire island would have to be fully barrier free by the year 2011.  So, improving the tool to accurate measure the ramps made sense.     In our last episode of the Top 5 Quality Management System Failures of all time according to Domenic Tramontana, we discuss the Ford Pinto.  Most people are familiar with the disaster that was the Pinto.  I had heard jokes about it but didn't really know what was funny.  I was shocked when I learned more about the Ford Pinto.     We continue the Top 5 Quality Management System Failures of All Time-a review of an article written by Domenic Tramontana.  We quickly discuss a design flaw with the iPhone 4-an antenna in the wrong place.  Kyle tells us about the process of testing and validation.  Included in this episode is the Teton Dam.  Construction began on the dam in 1972, and was completed in November of 1975.  On June 3, 1976, two small seeps were observed.  On June 5, 1976 the embankment breached and there was a rapid release of the entire contents of the reservoir.  This flooded 5 counties, 300 square miles and traveled 155 miles downstream.     Happy Independence Day. Part 2 of today's 2 part episode we examine the Emancipation Proclamation in both its historical context and as an example of what a mission and vision for an organization can and aught to be. Let this be a lesson on leadership, and the price to be paid when an organization looses sight of its vision. Proverbs 29:18 says Where there is no vision, the people perish. The Emancipation Proclamation President Abraham Lincoln issued the Emancipation Proclamation on January 1, 1863, as the nation approached its third year of bloody civil war. The proclamation declared "that all persons held as slaves" within the rebellious states "are, and henceforward shall be free." Despite this expansive wording, the Emancipation Proclamation was limited in many ways. It applied only to states that had seceded from the United States, leaving slavery untouched in the loyal border states. It also expressly exempted parts of the Confederacy (the Southern secessionist states) that had already come under Northern control. Most important, the freedom it promised depended upon Union (United States) military victory. Although the Emancipation Proclamation did not end slavery in the nation, it captured the hearts and imagination of millions of Americans and fundamentally transformed the character of the war. After January 1, 1863, every advance of federal troops expanded the domain of freedom. Moreover, the Proclamation announced the acceptance of black men into the Union Army and Navy, enabling the liberated to become liberators. By the end of the war, almost 200,000 black soldiers and sailors had fought for the Union and freedom. Juneteenth (short for “June Nineteenth”) marks the day when federal troops arrived in Galveston, Texas in 1865 to take control of the state and ensure that all enslaved people be freed. The troops' arrival came a full two and a half years after the signing of the Emancipation Proclamation. Juneteenth honors the end to slavery in the United States and is considered the longest-running African American holiday. On June 17, 2021, it officially became a federal holiday.  January 1, 1863 By the President of the United States of America: A Proclamation. Whereas, on the twenty-second day of September, in the year of our Lord one thousand eight hundred and sixty-two, a proclamation was issued by the President of the United States, containing, among other things, the following, to wit: "That on the first day of January, in the year of our Lord one thousand eight hundred and sixty-three, all persons held as slaves within any State or designated part of a State, the people whereof shall then be in rebellion against the United States, shall be then, thenceforward, and forever free; and the Executive Government of the United States, including the military and naval authority thereof, will recognize and maintain the freedom of such persons, and will do no act or acts to repress such persons, or any of them, in any efforts they may make for their actual freedom. "That the Executive will, on the first day of January aforesaid, by proclamation, designate the States and parts of States, if any, in which the people thereof, respectively, shall then be in rebellion against the United States; and the fact that any State, or the people thereof, shall on that day be, in good faith, represented in the Congress of the United States by members chosen thereto at elections wherein a majority of the qualified voters of such State shall have participated, shall, in the absence of strong countervailing testimony, be deemed conclusive evidence that such State, and the people thereof, are not then in rebellion against the United States."     Ep 107 - They handed back their certifications...? Trading Accreditation for Performance Model

    A health organization in New Zealand hands in its standard accreditations for a more strenuous criteria program. This is a very interesting case study in which Spectrum Care in New Zealand decided to trade in their standard accreditation for a performance-based model.  Being a government funded organizations, Spectrum had certifications that they were required to keep and accreditations that they achieved voluntarily to show a standard of excellence.  They consistently achieved the maximum three-year certification for the voluntary accreditation.  After evaluation the accreditation, Spectrum felt it only assessed the health and hospital side of things.  The organization wanted something more strenuous that really assessed how they managed their business. After some consideration, Spectrum handed in their accreditation certificate and went for the New Zealand Business Excellence Criteria.  This is closely aligned with the Malcolm Baldrige Criteria for Performance Excellence.  This Criteria accreditation assesses seven organizational elements including leadership, strategic planning, business results, process analysis, etc.  This gives a better overall evaluation of the business and goes further than just assessing what they do.  This evaluation system assesses how they manage and plan for what they do, how they manage the people and the information, and their customers.  See, a company can be good at turning out widgets, but they could be even better if they assessed these seven other elements. After working towards the Baldrige-based criteria, Spectrum decided to apply for the New Zealand Business Excellence Award.  The organization received the Silver Award after evaluation and said their obvious next step is to go for the gold. I love organizations that look for improvement.  Spectrum was doing great but looked at the system that said they were doing great and decided they could do better.  Once they achieved better, they knew there was still a higher level and strived to achieve that.  There's always, always room for improvement.     Ep 106 - Common Quality Management Approaches

    There's more than one way to skin a cat, or to achieve Quality Management.  That's one of the best things about Quality Management, it's not a one size fits all.  In this episode, we discuss several different approaches, as laid out by Carter McNamara on managementhelp.org.  All of these approaches are beneficial to different companies depending on your needs.  WE kind of had fun with this episode, with Darci quizzing Kyle on the different approaches listed to see if he agreed with the description I had before me, but it's a good example of the different ways you can achieve a Quality Management System. In this article, McNamara mentions the ISO 9000 system, which is what I (Darci) have become familiar with.  I know from listening to Kyle that the ISO 9001 standard does not pigeonhole you into a once and done system, which is a common misconception.  The 9001 standards simply asks you to write down the way you do things, and do it that way.  If something changes or does not work, record that, too.  I know that is simply stated, but it's also that simple.  For example, when I started helping Kyle with Texas Quality assurance and then I took on accounting and payroll, he made me type up procedures for those aspects of the business.  I was the uncooperative employee.  I was so mad I had to type it up and kept asking how much detail I needed.  Ever the patient auditor, Kyle would simply reply, “Someone that has never done this before needs to be able to read your procedure and get the job done correctly.”  UGH!!  But I tell you what, I've used those procedures myself every month since then to make sure I'm doing it correctly and there are currently changes I need to make that will simply be saved as a new revision.  I do not have to scrap the whole thing.
