We wrapped up with a woman named Kathy McDaniel who had a near-death experience and instead of the peaceful light and deceased relatives everybody talks about, found herself surrounded by fog, shrieking, and a booming voice asking her if she knew where she was.

İngiltere'de asistan doktorlar, 7-13 Nisan 2026'da altı günlük greve çıktı. İşçi sınıfına düşman İşçi Partisi hükümetinin başındaki Keir Starmer, sorunu çözmek yerine tehdidi seçti. Britanya Tabipler Birliğinin (BTB) yürüttüğü görüşmelerde hükümetin 2026–27 için önerdiği ücret artışı yalnızca yüzde 3,5 oldu. Asistan doktorların yanıtı ise eriyen ücretlerinin telafisi, daha fazla asistan doktor kadrosu açılması, zorunlu sınav ücretlerinin karşılanması ve insanca çalışma-eğitim koşulları talebiyle greve gitmek oldu.İngiltere Ulusal Sağlık Sistemi, 1948'de sağlık hizmetinin esas olarak vergilerle finanse edildiği bir model olarak kurulmuştu. Ancak 1979'dan itibaren Thatcher hükümetleriyle simgeleşen neoliberal politikalar, sağlık alanını da piyasa mantığına göre yeniden düzenledi. İMF ve Dünya Bankası'nın programlarıyla pek çok ülkede kamu harcamaları kısıldı, sağlık hizmetleri piyasalaştırıldı, emekçiler daha güvencesiz hale getirildi. İngiltere'de ve pek çok ülkede asistan doktorların bugün yaşadığı sorunlar da bu uzun sermaye saldırısının sağlık alanındaki sonuçlarından biri.İngiltere'de asistan doktorlar bugün ilk kez greve çıkmıyor. Bu, 2023'ten beri çıktıkları 15. grev. Talepler uzun süredir aynı başlıklarda düğümleniyor: reel ücret kaybının telafisi, eğitim kadrolarının artırılması, sınav ücretlerinin karşılanması, insanca çalışma ve nitelikli eğitim koşulları. Hükümet ise yüzde 3,5'lik artışı, sınav ücretlerinin geri ödenmesini ve üç yıla yayılan 4.000–4.500 ek asistan doktor kadrosunu “tarihî teklif” gibi sundu. Bu tablo Türkiye'deki asistan doktorlara hiç yabancı değil. Türkiye'de de asistan doktorlar performans sistemi, angarya, günaşırı nöbet, eğitim hakkının hizmet baskısı altında ezilmesi ve düşük ücretlere karşı önemli mücadeleler yürüttüler. 2011'de Dokuz Eylül Üniversitesi Tıp Fakültesinde başlayan asistan grevi bu açıdan özel bir yere sahipti. Bu grev, asistan doktorların kendi emeklerine ve eğitim haklarına sahip çıkmasının simgesel örneklerinden biri olmuştu. Öyle ki grevin kazanımla sonuçlandığı 5 Nisan, TTB tarafından Asistan Hekimler Günü olarak sahiplenildi. Trakya, Ege ve Çukurova'daki üniversitelere haftalar içinde yayılan bu grev dalgası, sorunların tek tek hastanelere özgü değil sistemsel olduğunu ve mücadelelerin patlayıcı biçimde yayılabileceğini göstermişti.Bugün İngiltere'de asistan doktorların ücret, eğitim kadrosu ve çalışma koşulları için greve çıkmasını, Türkiye'deki bu deneyimlerle birlikte düşünmek gerekir. Aradaki önemli farklardan biri örgütsel zemin. İngiltere'de BTB, asistan doktorlar adına hükümetle doğrudan toplu pazarlık yürüten bir güçken, Türkiye'de TTB ve tabip odaları böyle bir yetkiden yoksun bırakılmış, çoğu zaman muhatap bile kabul edilmemekte. Bu nedenle doktorların ücretleri, çalışma koşulları ve uzmanlık eğitimi sorunları, TTB'nin taraf olduğu gerçek bir toplu pazarlığın konusu yapılamamakta.İngiltere'de greve çıkan asistan doktorlar, dünyadaki meslektaşlarına önemli bir mücadele örneği sunuyor. Hakları kazanmanın ve korumanın yolu kararlı ve örgütlü mücadeleden geçiyor. Türkiye'deki deneyim de bunu gösteriyor. 2011 grev dalgası ve sonrasında yürütülen asistan doktor mücadeleleri, nöbet ertesi izin hakkı başta olmak üzere önemli kazanımlar elde etmişti. Ancak bugün asistan doktorlar, nöbet ertesi izin hakkı dahil pek çok kazanılmış hakkı dahi kullanırken engellerle karşılaşmakta. Ücret, eğitim ve çalışma koşulları gibi temel başlıklarda ciddi sorunlar yaşamaya devam etmekte.Bugün Türkiye'de asistan doktorlar alabildiğine örgütsüz durumdalar. Tabip odaları/TTB, örgütlenmek için eldeki en önemli mevzilerden biri. Asistan doktorların daha iyi koşullarda çalışabilmesi ve eğitim alabilmesi için tabip odalarına üye olmaları, sahip çıkmaları, denetlemeleri ve kendi talepleri etrafında örgütlenmeleri gerekmekte. Ücretsiz, nitelikli, eşit ve kamu eliyle planlanan bir sağlık sistemi de ancak böylesi mücadelelerin ulusal ve enternasyonal ölçekte büyümesiyle kurulabilir.

The ENG implant has data placing it as the most reversible, hormonal contraceptive agent available with a typical use failure rate of 0.05%. Unfavorable bleeding patterns, such as frequent or prolonged bleeding, affect approximately 40% of ENG implant users within the first 3 months but typically improve over time. Nonetheless, it is the main reason for patient discontinuation. In the past, various medications have shown to have at least some short-term reduction in bothersome breakthrough bleeding (BTB). These include doxycycline, ethinyl estradiol (EE), mefenamic acid, combined oral contraceptives (COCs), short term tamoxifen, norethindrone, and ulipristal acetate.
