Maria Esguerra escaped the Children of God at the age of 22 with her two children who have disabilities. Maria's firsthand experience drives her passion to support fellow survivors of cults and institutions, advocating for specialised understanding, interventions and access to governmental schemes such as Redress. Maria has actively engaged in media and advocacy efforts to raise awareness about the challenges faced by people escaping coercive environments. She founded a support group for second and multi-generational survivors (also referred to as SGAs or MGAs) from all cults. It addresses the devastating impacts from these groups, including abuse, denial of basic rights and forced labour. Description:There is evidence that clinician experience with intubation is associated with improved success rates and evidence that missed intubation attempts are associated with worse survival, at least in cardiac arrest. The recent Airway EBG paper recommends EMS agencies with low intubation proficiency should use SGAs instead of intubation in cardiac arrest. This all begs the question of whether agencies who intubate more often have higher success rates than those who do not. That's exactly the question a new paper from Annals of Emergency Medicine attempts to answer. Advanced airway management in EMS systems around the country ranges from surgical airways to laryngoscopy, either video or direct, to the use of a supraglottic airway (SGA) device. As a “blind” airway device, the King Airway has largely replaced the Combitube as the civilian and military SGA of choice due to its design and ease of use. However, over the past decade, there have been many other SGAs, particularly iterations of the laryngeal mask airway (LMA), that have come to market. It is the wide and varied options for an SGA that have necessitated this evaluation as we hope to provide guidance to EMS agencies in choosing a device for their service. Read the full article here.

This week's EYE ON NPI will "Speak Friend and Enter" your wireless connectivity toolbox, it's TE's LEMBAS LTE/GNSS USB Modem (https://www.digikey.com/en/product-highlight/t/te-connectivity-amp/lembas-lte-gnss-usb-modem) an easy-to-use full powered cellular and GPS modem designed specifically to be used with single-board Linux computers (SBCs) to add networking and location tracking with a simple plug-and-play USB connection. We bought one and popped it open - not recommended because it will void your warranty - and found inside is a Quectel EC25-AFXD module (https://www.digikey.com/en/products/detail/quectel/EC25AFXGA-128-SGAS/13278280) - an LTE Cat 4 data-only module with high speed uplink and downlink when LTE is available, and GSM/GPRS backward-compatibility for remote areas. Quectel EC25 is a series of LTE Cat 4 modules optimized specially for M2M and IoT applications. Adopting the 3GPP Rel-11 LTE technology, it delivers maximum data rates up to 150 Mbps downlink and 50 Mbps uplink. Designed in the compact and in a unified form factor, the EC25 series is compatible with Quectel multi-mode LTE Standard EC21 series/EC20-CE/EG25-G/EG21-G modules and UMTS/HSPA+ UC200A-GL module, which allows for flexible migration among them in design and manufacturing... backward-compatible with existing EDGE and GSM/GPRS networks, ensuring that it can be connected even in remote areas devoid of 4G or 3G coverage. EC25 series supports Qualcomm® IZat™ location technology Gen8C Lite (GPS, GLONASS, BDS, Galileo and QZSS). The integrated GNSS greatly simplifies product design, and provides quicker, more accurate and more dependable positioning. The LEMBAS has a lot of little details that shows the design team focused on a high quality build and removing any obstacles to use. For example, instead of a 'USB stick' style modem that blocks all of the USB ports on your raspi, the modem uses any USB Type C cable - this also allows the user to place the LEMBAS wherever the antenna will get best reception. When we opened it up, we noticed that the board was conformal coated (https://www.digikey.com/en/products/filter/coating-grease-repair/642), so it will handle being exposed to humid air, although please note it's not rated for outdoor use. The SIM card is pre-installed but can be removed / replaced easily if a particular data service is desired. We also thought the status LEDs that appear as part of the TE logo showed a cute designer touch. One handy design decision that makes the LEMBAS easy to get started with: inside is a 2-port USB hub (https://www.digikey.com/en/products/detail/texas-instruments/TUSB4020BIPHP/5724124) so that, when plugged into USB, it shows up as both the modem interface and a small USB storage key. On that storage are all the files you need to run to install the modem interface on an Arm32 or Arm64 computer like the Raspberry Pi. This definitely solves the "to get on the internet I have to install my wireless drivers but I can't get onto the internet because I haven't installed my wireless drivers" problem. And of course, there's a GNSS inside as well, which makes this perfect for asset tracking or geo-locating projects where you need to know where the code is running from. It's ready to go in an instant and with native Linux support for cellular modems, you don't need to do any AT command noodling - use Python or node.js or whatever language your application is and open secure sockets like normal. A few years ago someone commented on one of my social media posts, asking me to explain why I felt that growing up in a cult was such a bad thing. So I wanted to take the time to answer the question fully. And to be clear, I don't think that it should be the survivor's job to take on the emotional labor of educating the public about childhood trauma in cults. Use the two-hand ventilation technique whenever possible:   (See Adventures in RSI for more)     Supraglottic Airways: for difficult bag-valve-mask ventilation or a difficult airway (details in audio) LMA Classic Pros: Best studied; sizes for all ages Cons: Cannot intubate through aperture   LMA Supreme Pros: Better ergonomics with updated design; bite bloc; port for decompression Cons: Cannot pass appropriate-sized ETT through tube   King Laryngeal Tube Pros: Little training needed; high success rate; single inflation port Cons: Flexion of tube can impede ventilation or cause leaks; only sized down to 12 kg (not for infants and most toddlers)   Air-Q Pros: Easy to place; can intubate through aperture Cons: Not for neonates less than 4 kg   iGel Pros: Molds more accurately to supraglottis; no need to inflate; good seal pressures Cons: Cannot intubate through (without fiberoscopy)   Summary • If you can bag the patient, you're winning. • If you have difficulty bagging, or anticipate or encounter a difficult airway, then don't forget your friend the supraglottic airway (SGA). • Ego is the enemy of safety: SGAs are simple, fast, and reliable. • Just do it.   References Ahn EJ et al.
