Published 2025 Jun 2. doi:10.1097/CND.0000000000000502  Show transcript:  Dr. Aaron Zelikovich: Welcome to today's Neurology Minute. My name is Aaron Zelikovich, a neuromuscular specialist at Lenox Hill Hospital in New York City. Today, we will discuss a recent article, How Well Do We Evaluate Small Fiber Neuropathy? A survey of The American Academy of Neurology members, which evaluates small fiber neuropathy in clinical practice. The current landscape of evaluating and testing for small fiber neuropathy remains highly variable in regards to serum testing, skin biopsy, and nerve conduction studies. In this survey study, 800 members of The American Academy of Neurology were randomly selected and emailed a survey. 400 neuromuscular physicians and 400 non-neuromuscular physicians were selected. The overall response rate was 30% with half of the completed surveys coming from neuromuscular physicians. The most common overall initial blood work for this patient population was a CBC, vitamin B12, basic metabolic profile, TSH, and hemoglobin A1C. Other high yield blood tests included ESR, SPEP, immunofixation, and ANA. 70% of responders would also order a nerve conduction study as part of the initial workup. Second line evaluation had less consensus and included skin biopsies for intraepidermal nerve fiber density, hepatitis panel, HIV, and paraneoplastic testing. Responders noted that if the patient had acute onset of symptoms, had symptoms that were asymmetric, or being under 30 years old, they would order a more extensive workup. The authors discussed the importance of both clinical exam, history, and diagnostic workup in patients with symptoms compatible with small fiber neuropathy. They highlight that there is no current objective gold standard for a diagnosis of small fiber neuropathy. The current diagnostic recommendation by the AAN for distal symmetric polyneuropathy includes serum blood sampling for glucose, vitamin B12, SPEP, and immunofixation. Clinical practice in the diagnosis of small fiber neuropathy remains highly variable based on the provider and clinical context of the patient. At Hollins University in Roanoke, Virginia, she studied political science and creative writing. Her first book, The Man from the Train (coauthored with her father) chronicled the serial killer behind the Villisca axe murders. Published in 2017, The Man from the Train was nominated for an Edgar award for best fact crime, and won the Kansas Notable Book award.     The Katherine Massey Book Club @ The C.O.W.S. hosts the 4th study session on Char Adams' Black-Owned: The Revolutionary Life of the Black Bookstore. Published at the end of 2025, Adams' new book made a number of lists for top books of the year. Touted as an "award-winning journalist, editor, and storyteller for NBC News, known for her work on race, identity, and inequitable systems," Adams is a lovely black female, Victim of Racism. This may be the first to seriously investigate the history of bookstores operated by people classified as black in the area of the world known as the US. Specifically, Adams investigates how black people have responded to the System of White Supremacy through the lens of bookstores ownership. C.O.W.S. listeners should know that White Supremacists have invested immense time and energy to ensure that black people do not read or have access to books. So, black bookstores must be heavily targeted places. Last week, we discussed the Tree of Life Bookstore in Harlem and the Hue-Man Experience Bookstore in Denver. A central theme of this text is that black bookstore operators were motivated to provide books for black children. Importantly, last week Adams told us that a significant pattern is that black females often read fictional romance novels - like Terry McMillan's Waiting To Exhale, while black males mostly read non-fiction works of history.     As Indonesia grapples with increasingly frequent climate disasters—from the devastating floods in Sumatra and Aceh to prolonged droughts affecting food security—a new book is rejecting the usual solutions. No carbon credits. No waiting for the next Elon Musk. Instead, Bacaan Bumi asks: what if the answers lie in Indonesia's own revolutionary history, its constitutional foundations, and its diverse philosophical traditions? Published by Yayasan Obor Pustaka Indonesia last year, Bacaan Bumi began as a monthly supplement for Inside Indonesia magazine—where, we should acknowledge, several Talking Indonesia hosts are also involved. (Yes, this is a slightly nepotistic episode, but we promise the ideas are worth it.) The supplement was initiated by Gerry van Klinken, a longtime Indonesia scholar and one of the board members of Inside Indonesia, and brought together 17 Indonesian academics, activists, and thinkers who argue that technology and market mechanisms alone won't save us. Instead, they propose something more radical: an eco-socialist manifesto rooted in Indonesian soil. The book emerged from conversations sparked by a groundbreaking summer school on critical environmental history at Gadjah Mada University—Indonesia's first university program of its kind. The response has been striking: packed book launches across Java, students demanding more courses, and activists finding new language to connect Marxist commodity analysis with Javanese mysticism, Islamic green theology with feminist readings of adat traditions, and Sukarno's Marhaenism with 21st-century ecological citizenship. The editors don't call it an academic collection. They call it a manifesto. In his introduction, Farabi Fakih writes that Indonesia's environmental movement in the 21st century is “the natural continuation of the Southern revolution imagined by Sukarno.” He explicitly rejects what he calls the “techno-magical narrative” of Silicon Valley billionaires and the “declensionist narrative” of inevitable doom—both of which, he argues, serve to disable collective action against capitalism. But what does an environmental manifesto look like in the Indonesian context? How do you connect Marx's theory of metabolic rift to flood disasters in Sumatra? Why do young Indonesians find hope in pan-psychism and Kendeng mountain feminism? And what happens when you discover that Indonesia's 1945 constitution already contains ecological philosophy that's been largely forgotten? In this episode, we had a conversation with two of Bacaan Bumi's key contributors: Farabi Fakih, who heads the Master's program in History at Gadjah Mada University where the critical environmental history curriculum was born, and Fathun Karib, a historical sociologist, postdoctoral fellow at the Asia Research Institute, and founding member of punk band Critical Death. Together they explore why genuine solutions must come from within Indonesia, why book tours revealed both hope and anxiety among younger generations, and how a 1960s Indonesian constitutional provision about the earth might offer more wisdom than all of Silicon Valley's promises combined.     A large UK study of nearly 125,000 women has found that the menopause can cause a loss of grey matter in women's brains affecting memory and emotions, similar to that seen in Alzheimer's. Published in the journal Psychological Medicine, the researchers say it may help explain why we see almost twice as many cases of dementia in women than in men. Professor of Clinical Neuropsychology Barbara Sahakian, from the University of Cambridge and the senior researcher on the study and Dr Paula Briggs, Consultant in Sexual and Reproductive Health at Liverpool Woman's NHS Foundation Trust, discuss the findings with Nuala McGovern.There's been much discussion about the benefits of singing, but with less singing happening in schools - over half of state secondary school teachers have said that their pupils never sang together in assembly - how do you encourage children and young people to sing and what are the benefits?
