Psychotic bipolar depression (PBD) is a prevalent yet understudied psychiatric illness with no specific guidelines or Food and Drug Administration-approved medications for its treatment. Recent studies suggest that some antipsychotics and mood stabilizers may be effective in managing bipolar depression; however, their effectiveness for PBD remains unclear. With an urgent need for more focused research for managing PBD, several authors conducted a literature review to piece together existing literature on the topic. In this podcast, Dr. Julia Ann Koretski, Digital Editor of the Journal of Clinical Psychopharmacology (JCP), discusses the review with two of its authors, Dr. Maité Cintrón Pastrana and Dr. Anthony Rothschild, who is Editor-in-Chief of the Journal of Clinical Psychopharmacology. Both authors are from the University of Massachusetts Chan Medical School. A third author, Jessica C. Irizarry Flores, is from the Ponce Health Science University Medical School in Puerto Rico. The article appears in the July/August 2024 issue of the journal.

According to the World Health Organization, bipolar disorder is one of the top three causes of hospitalization in people aged 15 to 44. It's estimated that 5% of the world's population is on the bipolar spectrum and of that 5%, only 1 to 2% are diagnosed. And while those percentages may seem small, with over 8 billion people currently in the world, we are talking about over 400 million people with bipolar brains. It's far more common than you think. In the US alone, around 4.4% of adults deal with or develop bipolar disorder, and the numbers show it impacts more females than males. I sat down with author and filmmaker Sara Schley to talk about her experience with bipolar disorder and how she has turned what she calls her broken into a blessing as she serves the world by spreading knowledge and resources for those who believe there's little hope out there. In fact, that's the furthest from the truth. Moreover, these patients are at the risk of being misdiagnosed as having unipolar depression rather than bipolar depression, causing them to be at greater risk for being mismanaged. Effective management of patients with bipolar 1 and 2 depression is vital to enhance their quality of life and reduce functional disability. Health care providers (HCPs) need to be aware of the shortcomings of traditional monotherapy and adjunctive therapies; considering the ramifications associated with their side effects and efficacy, they have both adherence challenges and significant relapse rates. Dr. Cynthia Calkin's study using Metformin in treatment-resistant Bipolar Disorder showed that improving insulin resistance dramatically improved depression outcomes. This research is helping shift the paradigm for how we treat bipolar disorder. Her clinical practice and research studies demonstrate that metabolic dysfunction significantly contributes to mental illness and, perhaps even more importantly, that treating metabolic dysfunction results in significant symptomatic improvements. This is a dramatic clinical shift in psychiatry, and as you will hear, it has been slow to catch on within the psychiatry community.
