Is an elevated prostate-specific antigen (PSA) the beginning or the end of the prostate cancer workup? On this episode of BackTable Urology, Dr. Ruchika Talwar interviews Dr. Tarik Benidir to explore the “PSA problem,” explaining why PSA is biologically useful but clinically imprecise, often leading to false positives, unnecessary biopsies, and detection of indolent disease. They discuss a rational, patient-centered approach to elevated PSA, including when to repeat the test, how to interpret results in the context of age, race, family history, and prostate size, and the use of PSA density and age-adjusted norms instead of rigid cutoffs. --- Get the BackTable apphttps://www.backtable.com/app --- This podcast is supported byCleveland Diagnosticshttps://www.isopsa.com/ --- Timestamps 00:00 - Introduction04:03 - Why PSA Can Be Misleading06:43 - Approach to Elevated PSA and Initial Workup09:51 - MRI, PSA Density, and Biomarker Testing14:31 - Guidelines, Diagnostics, and When to Biopsy20:16 - Comparing 4K, IsoPSA, and Other Biomarkers25:23 - Choosing Between Transperineal and Transrectal Biopsy28:42 - Management Considerations for Elderly and Comorbid Patients33:09 - Active Surveillance, Recurrence, and Long-Term Monitoring40:47 - Key Takeaways and Closing Remarks --- More about this episode The conversation highlights the role of reflex biomarkers, MRI scans, and selective biopsy strategies to reduce unnecessary procedures and focus on identifying clinically significant cancers. In today's episode, I sit down with former NBA players Tarik Black and CJ Miles to discuss storytelling, authenticity, mentorship, and the transition from professional basketball into new pursuits. CJ explains why the best storytellers must live the story first and how years on the court shaped his creative work as a writer and photographer. Tarik shares lessons from Kobe Bryant, including how purpose, passion, and strong mentors can guide the next chapter. We also talk about the humility required to become a beginner again, the value of integrity in business, and how curiosity transforms fear, challenges old assumptions, and creates a path toward growth and lasting expertise.See Privacy Policy at https://art19.com/privacy and California Privacy Notice at https://art19.com/privacy#do-not-sell-my-info.
The Dodgers' handling of Tarik Skubal has Evan Roberts convinced they have no intention of signing him long term. After Los Angeles pushed Skubal to two of the highest pitch counts of his career in consecutive August starts, Evan argues the Dodgers are treating him like a rental and riding him as hard as possible before he leaves. Shaun Morash pushes back on declaring it a done deal, but agrees the workload is strange. Evan takes the argument one step further, predicting that if the Dodgers are truly out, the Mets could become the clear favorite to land Skubal. Plus, the Yankees' future comes into focus with George Lombard Jr., Anthony Volpe's potential role at second base, and questions about how the infield could look in 2027.
The Dodgers' handling of Tarik Skubal has Evan Roberts convinced they have no intention of keeping the ace long term. After back-to-back starts featuring two of the highest pitch counts of Skubal's career, Evan argues Los Angeles is riding him hard because they expect him to leave, opening the door for the Mets to become the obvious landing spot. Shaun Morash is in for Tiki Barber alongside Evan Roberts and pushes back on just how certain that conclusion really is. The hour also turns into a heated Deshaun Watson debate after the Browns quarterback fires back at Cleveland fans for booing him. Evan calls the Browns more dysfunctional than the Jets, while Shaun argues the reaction is partly fueled by the fan obsession with Sanders. Plus, George Lombard Jr.'s Yankees future, Anthony Volpe's role, Francisco Alvarez frustration, and a birthday-text feud with Tommy Lugauer.
The Los Angeles Dodgers lost to the Milwaukee Brewers in the final game of a four-game series, losing the season series and costing themselves any playoff tiebreakers. While Tarik Skubal still doesn't seem to have settled into life as a Dodger, it was the offense that continues to be a mess. Jeff Spiegel and Jeff Snider are live breaking it all down! Plus, Justin Wrobleski was placed on the injured list prior to the game and Kyle Hurt was recalled. Is this scheduled rest time for Wrobleski or is the forearm injury something to be worried about? Grant Mona is joined by Arash Markazi, John Browner and Josh Tariff for a packed edition of The Sporting Tribune Today. The crew breaks down Aaron Donald's latest workouts with the Rams and whether the future Hall of Famer is really preparing to come out of retirement. They also debate what Donald could add to an already loaded defense featuring Myles Garrett and whether the Rams should be viewed as clear Super Bowl favorites. Then, the conversation turns to the investigation surrounding Kawhi Leonard and the stalled trade that would send him to the Toronto Raptors for Brandon Ingram. Could Leonard end up back with the Los Angeles Clippers? What happens to the trade if the investigation stretches into training camp? The panel looks at the difficult decisions facing both organizations and why Clippers fans may quickly welcome Leonard back if he remains in Los Angeles. Arash also discusses the excitement surrounding Tarik Skubal after the reigning back-to-back Cy Young Award winner joined the Dodgers, while Grant explains why he isn't ready to panic over the team's recent struggles. In this episode: Tarik Skubal pitched for the first time at Dodger Stadium since joining the Los Angeles Dodgers at the trade deadline, with mixed results. Skubal showed flashes of dominance but also allowed two big hits to Jac Caglianone, accounting for three runs allowed over five innings. But LA was able to overcome the deficit by chipping away, setting the stage for a huge three-run double by bobblehead hero Max Muncy in the bottom of the sixth inning to give them the lead. After Jack Dreyer coughed the lead back up, Freddie Freeman drove in Shohei Ohtani for the second time in the game to give the Dodgers a lead they wouldn't relinquish, thanks to solid innings from Tanner Scott and Edwin Diaz. Underscoring all of this was a rough game for home-plate umpire John Bacon and the Dodgers not able to do anything about it because Andy Pages and Miguel Rojas both lost ill-advised challenges early in the game. The lack of challenges probably took a run off the board in the bottom of the seventh when Pages struck out looking at ball four, but Freeman came through and made it a non-factor. But we talk about the Dodgers' issues with the ABS system. Jeff Snider and Clint Pasillas are here to talk about it all! #Dodgers Tube in all season long! The biggest get of the trade deadline season made his debut for the Los Angeles Dodgers on Tuesday night, with hopes that Tarik Skubal could end the mini-skid they had been on. Unfortunately, things didn't go as planned, as the Chicago Cubs defeated the Dodgers and extended LA's losing streak to five. However, there's no need to panic about the defending back-to-back champs. On this episode of Baseball Bar-B-Cast, Jake Mintz and Jordan Shusterman recap Skubal's pretty good debut for his new team and why him landing in Los Angeles ultimately wasn't a shocking outcome. They then dive a little deeper into the actual deal, including the players the Detroit Tigers received in return for Skubal, whether they could've gotten a better deal by waiting until closer to the trade deadline and why there's even more intrigue surrounding the Dodgers this postseason. Later in the show, Jake and Jordan are joined by Boston Red Sox MassLive beat reporter Chris Cotillo to examine how the Red Sox did at the deadline, including the shocking acquisition of Adley Rutschman, the impressive winning stretch they've been on and what turned Craig Breslow into being a little more aggressive than we've been used to seeing. The guys then close the show by highlighting George Lombard Jr.'s impressive debut for the New York Yankees, the "new" look Philadelphia Phillies post-deadline and Jacob deGrom reportedly blocking a trade to the Atlanta Braves. ] 1:14 - The Opener: Skubal's first Dodgers start 20:48 - The return for the Tigers 30:10 - Chris Cotillo joins the show 38:02 - How good are the Red Sox? 49:45 - End of the Marcello Mayer era in Boston 55:51 - We Need To Talk About: Lombard Jr.'s debut 1:06:38 - Around The League Subscribe to Baseball Bar-B-Cast on your favorite podcast app:
