A lot of us try to build a private practice or virtual clinic on the side while working per diem or picking up shifts. But the truth is, that path usually doesn't work. In this episode, I talk about why moonlighting slows you down, how to know if you're on the right track, and when to take a loan or raid your 401k to buy yourself time. I'm not telling you what to do, just sharing how I've done it and what I've seen other docs go through. If you want your practice up and running in under 5 years, this is the kind of stuff we have to talk about. Is a concierge medical practice the key to more freedom in medicine or is it just another version of being tied down? In this episode, I break down the real-world pros and cons of concierge models for physicians, nurse practitioners, and other clinicians considering a pivot from the traditional healthcare system. We talk about: What a concierge model actually is (and how it's different from DPC or cash-pay telemedicine) The benefits: smaller patient panels, consistent revenue, deeper patient relationships The challenges: 24/7 access expectations, higher-touch service, and Medicare restrictions Why some concierge doctors still feel burned out What I've learned running a virtual cash-pay clinic and why it may offer more autonomy, flexibility, and sustainability If you're thinking about building a direct care model or scaling back from insurance and volume-based medicine, this episode will help you figure out if concierge is really the right path for you. Malpractice happens—even to good doctors. In this episode, I break down the three biggest drivers of lawsuits against physicians and nurse practitioners, based on what I've seen and studied in this space. Whether you're practicing virtually or in-person, these risks are real—and avoidable. 1. Communication + Documentation: Patients sue when they feel blindsided. Explain things clearly and document well. 2. Missing Acute Conditions: If something might be urgent, don't miss it. Create safety nets in your workflow. 3. Loss to Follow-Up: Never assume the patient will return. Have a system in place to close the loop. I share practical tips on how to improve communication and documentation, avoid missing acute issues, and prevent patients from falling through the cracks. This is about reducing risk without practicing defensive medicine. ➡️ Want to protect your practice and future-proof your career? In this episode, I get real about my experiences hiring freelancers on Fiverr, Upwork, and OnlineJobs.ph. As a physician trying to grow a cash-based, online medical brand, outsourcing seemed like the obvious move—but I learned the hard way that it takes more than clicking "buy" on a gig. I talk about the most common pitfalls, how to communicate better with freelancers, and how outsourcing can actually become a superpower if done right. If you're running a virtual practice, cash-based clinic, or building your online brand as a physician — you need to figure out your branding voice. This episode breaks down exactly what that means — and no, it's not just how you sound. It's about how you show up, how your audience feels about your content, and whether they trust you enough to book a visit or buy your service. I share why consistency across YouTube, social, and your website is non-negotiable — and how mismatching your online vibe with your real-life practice can cost you patients (or worse). I also talk about how I'm working through this right now with my own brands and social media manager — figuring out the right balance between being myself and crafting a clear message.
If you're building a cash-based medical practice, you're probably dreading the billing headaches — especially when it comes to Medicare. In this episode, I break down exactly what it takes to bill Medicare Part B if you decide to be a non-participating provider (and why it might still be a pain). I also cover the difference between Part B and Part C (Medicare Advantage), what you legally have to do, and how to decide if outsourcing your Medicare billing makes sense. I share two companies I researched — CollaborateMD and AdvancedMD — plus tips on super bills, EHRs, and how to avoid wasting time on insurance messes. CollaborateMD – https://www.collaboratemd.com AdvancedMD – https://www.advancedmd.com
In this episode, I break down the messy world of Medicare and cash-based practices — because if you're building a private medical practice or a walk-in clinic like me, you're gonna run into this headache. I cover the 3 main options every physician faces: Ignore Medicare patients completely Officially opt out (and what that really means) Become a non-participating provider (and the billing mess that comes with it) It's complicated — even if you hire a lawyer. I explain how Part B vs. Part C (Medicare Advantage) changes the game, what you legally HAVE to bill, and what you can skip if you stay in the cash lane. If you're thinking about starting your own practice or walk-in clinic, this episode is a must-listen. You'll avoid costly mistakes and know exactly what's coming.
Most private practice doctors are just copying the traditional healthcare model—but why? The mainstream system is broken, inefficient, and frustrating for both doctors and patients. If you're launching your cash-based or virtual medical practice, you don't have to do things the old way. In this episode, I challenge you to rethink every part of your practice, including: How patients actually find you (hint: it's not through your website) Scheduling & payments (you don't need a front desk or an EHR) Intake forms (do patients really want them?) Lab & imaging results (how to present them in a way patients actually understand) The future of medical records (voice notes, AI, and custom software) The best practices in medicine aren't just the ones we've always used—they're the ones that actually work.
A physician recently shared a post about how medicine is a minefield—you can do everything right and still get sued, blamed, or burned out. The system makes it easier to destroy doctors than to support them, and many are leaving for good. But instead of waiting for change, what if you made a shift? Some medical settings are more supportive than others Switching specialties or clinical settings can make a huge difference There are many non-clinical career options Cash-based care (concierge, DPC, or fee-for-service) provides more freedom I also share my personal experience: How I handled a medical board investigation My perspective on lawsuits and the reality of getting sued Why cash-based medicine is a viable way to stay in medicine on your terms Listen in for real talk on the challenges of being a doctor today—and how to find a better path. I just signed the lease on my 500 sq ft clinic space in Long Beach, and I'm thinking a lot about what kind of practice I want to build. Instead of locking myself into a rigid model, I'm keeping things flexible—giving myself the option to shift between a cash-based walk-in clinic, urgent care, or even direct primary care. Why flexibility matters when starting a medical practice: You can adjust based on patient demand You're not stuck with a model that doesn't work for you You can scale up or down depending on how much patient care you want to provide I chose a retail space because I want to be visible and accessible. But if I don't get enough traction, I'll consider moving into a medical office building. Even in a small space, you can do a lot—bring in an NP or PA, offer virtual visits, host group coaching, and adapt your practice over time. Listen in as I share how I'm thinking about this new clinic and why you don't need to have everything figured out before you start.
I've been getting this question a lot—why start a brick-and-mortar cash-based walk-in clinic when I already have a virtual practice? Here's my honest answer—money, ego, testing a model, and building something meaningful for my local community in Long Beach. I talk about the risks, the overhead, and why I'm not abandoning virtual medicine or the digital nomad life. My virtual practice will still focus on health coaching and healthy aging—but this clinic is me testing a model that might be scalable (or might just lose me a bunch of money
What's up, everyone? It's Dr. Mo from Digital Nomad Physician. Quick update—consults are paused for the next two months while I dive into something new and super exciting. I'm opening a cash-pay walk-in clinic in Long Beach, California. No insurance. Simple pricing. Quick access. And honestly… I couldn't find another clinic like it out there. In this episode, I'm walking you through why I'm doing it, the steps so far (permits, contractors, medical supplies, ADA compliance), and all the headaches of setting up a private practice from scratch. If you've ever thought about starting your own clinic or wondered if a cash-based walk-in model could work, you'll want to hear this. I'm testing it so you don't have to. If you're thinking about starting a cash-based, physical medical practice, there's one term you should know: GPO (Group Purchasing Organization). A GPO helps you save money by negotiating bulk discounts on medical supplies, medications, office essentials, waste disposal, and even malpractice insurance. Instead of setting up contracts with multiple vendors yourself, you get instant access to lower prices through these organizations. Pros: Streamlined purchasing Discounted rates on supplies & services Access to vendors that typically work with larger organizations Cons: Some GPOs have membership fees (though most don't) Less flexibility in product selection Popular GPOs to check out: Medline Premier Inc. McKesson Cardinal Health Vizient MPPG (Medical Practice Purchasing Group) Should you use a GPO? It depends! If you're just starting out, you may be fine ordering directly from a major supplier like McKesson. But as your practice grows, a GPO can simplify operations and cut costs. Have you used a GPO before? Let me know your thoughts! Email me at drmo@digitalnomadphysician.com.
Managing a medical practice shouldn't feel like an uphill battle. One of the best ways to minimize burnout, reduce malpractice risk, and improve patient satisfaction is by setting clear expectations from the start. In this episode, I'll break down practical strategies for streamlining patient interactions, improving communication, and making care more seamless—for both you and your patients. We'll cover: How to set boundaries without feeling like the bad guy Why clear policies help both you and your patients The role of informed consent in reducing liability How to structure your workflow to avoid frustration and miscommunication By the end of this episode, you'll have actionable steps to create a more efficient, stress-free practice that protects your time and delivers high-quality care. Tune in and make patient interactions work for you—not against you. Thanks for tuning in to the Digital Nomad Physician podcast! Telemedicine has changed how physicians interact with patients, but can you legally run a virtual-only practice using text-based (asynchronous) communication? Some states allow it, while others require audio or video visits before you can establish a doctor-patient relationship. In this episode, I'll break down: ✔️ Which states allow full asynchronous telemedicine ✔️ Which states require an audio/video visit before texting patients ✔️ How to structure your virtual practice to stay compliant and efficient ✔️ Common pitfalls to avoid when offering text-based medical care If you're a doctor looking to build a low-burnout, high-efficiency virtual practice, this episode will help you navigate the legal landscape and set up a workflow that works for you.
Can a cash-based medical practice really match or exceed your income? Many doctors think insurance is the only way to stay profitable—but that's not true. Patients are actively looking for cash-pay physicians, and with the right marketing, you can build a thriving practice on your terms. In this episode, I cover: Why patients are frustrated with insurance-based healthcare and willing to pay cash The biggest myths about cash-based medicine (and why they're wrong) How to choose a profitable niche that patients will pay for The importance of marketing—more than just “being online” Whether to DIY your marketing or invest in professional help Cash-based medicine isn't just possible—it's profitable. Ever had a difficult patient who seemed like more trouble than they were worth? It happens to all of us and it's not that the patient is bad, it's that we don't have the right processes in place to avoid customer related headaches. One of the biggest advantages of running a private, cash-based practice is the freedom to choose how you handle patient payments—including the ability to offer refunds strategically. In this episode, I share a real-world example where a refund could have prevented major headaches, negative reviews, and even a potential malpractice lawsuit. I also break down: Why refunding a patient's money is sometimes the best business decision How this flexibility helps protect your practice from bad faith disputes How refunds build long-term trust and improve your reputation If you're a physician considering a cash-based or direct-care practice, this episode will help you understand how having control over financial policies can protect your career and reduce liability. ---------------------------- Thanks for tuning in to the Digital Nomad Physician podcast! It's understandable to be frustrated practicing medicine in the mainstream insurance model because it is designed around profits. But the alternative is working in the cahs-based system which is still quite young and not very well evolved. Fortunately, health insurance and healthcare is becoming so unaffordable that cash-based practices are gaining traction. But you must know what you're offering the patient and keep practicing your pitch. In the insurance based system the patient knows exactly whtat they are getting but it's often unclear in the cash-based world. Are you getting one vist? Multiple visits? How much is the follow-up? This was my final meeting with Rabeea Hasan and I've learned a lot and have many decisions still to make. The overall consensus is that I have to be consistent and get my message across in a comprehensible way. Patients shouldn't have to guess what they are getting from me. ------ -Dr. Mo Mohammad Ashori LinkedIn Digital Nomad Physicians Website Telemedicine: Digital Nomad Health Website Physician Coaching & Consulting Dr. Mo Goodreads Build Your Virtual Medical Practice Online Course
For the past few years have I've had a physician coaching client who wanted to become a digital nomad. Though he did all the hard work I was instrumental in helping him make the right decisions and plan things out. He is now successfully working from another country as a specialist doing vritual medicine and making a very healthy salary. ------ -Dr. Mo Mohammad Ashori LinkedIn Digital Nomad Physicians Website Telemedicine: Digital Nomad Health Website Physician Coaching & Consulting Dr. Mo Goodreads Build Your Virtual Medical Practice Online Course
It's easy to say that the US healthcare system is broken and of no value. I disagree. I think it is motivated by profits which makes it tougher for the patient to navigate but there is defintiely value in it for the patient who knows how to navigate it. One oof the most important things we can do in this system is to figure out where we belong. Do we want to click in and out and just collect a paycheck? Do we want to have more meanfinful patient doctors relationshios? Do we need to completely exit it? ------ -Dr. Mo Mohammad Ashori LinkedIn Digital Nomad Physicians Website Telemedicine: Digital Nomad Health Website Physician Coaching & Consulting Dr. Mo Goodreads Build Your Virtual Medical Practice Online Course
I love the ability to work remotely but as a digital nomad I'm not always in the same place and getting established somewhere new can be frustrating. In this episode you'll hear the loud streets of Tehran which is just another normal day for me as I go run errands. I'll tell you about navigating internet connection issues and buying SIM cards and just trying to not get swindled in day to day interactions. All part of traveling! ------ -Dr. Mo Mohammad Ashori LinkedIn Digital Nomad Physicians Website Telemedicine: Digital Nomad Health Website Physician Coaching & Consulting Dr. Mo Goodreads Sign up for my online course! It was relatively easy getting set up in Europe and running my virtual medical practice. But here in Tehran, it's a different story. From the Internet to the VPN to even opening a bank account, it's all complicated and frustrating. The frustration is worth it if you can enjoy the process and you have realistic expectations. What usually would take me 1 day in Spain has taken me nearly 2 weeks here in Iran. Then there is the time zone difference and the slow internet speeds. It's been fun, however. In this episdoe I'll give you my initial impressions and my prief interaction with the Iranian healthcare system. ------ -Dr. Mo Mohammad Ashori LinkedIn Digital Nomad Physicians Website Telemedicine: Digital Nomad Health Website Physician Coaching & Consulting Dr. Mo Goodreads My online course: Build Your Virtual Medical Practice Online Course
A successful medical career doesn't have to be easy and without hurdles. As with any place I stay long enough in, I like to answer the question of whether I could see myself moving there and practicing medicine. We all need help at some point in our career - the best way to get that is to know how to ask for it. It's worth putting some effort into it and understand why and how other clinicians can help us in our careers. ------ -Dr. Mo Mohammad Ashori LinkedIn Digital Nomad Physicians Website Telemedicine: Digital Nomad Health Website Physician Coaching & Consulting Dr. Mo Goodreads Heart Health Reddit
A medical practice doesn't have to be complicated. Physicians are in high enough demand that if you hang up a shingle you'll get plenty of customers knocking on your door. Sure, to ruin a super optimized medical practice where you optimize for profits takes a lot more work, but that's not what all of us need. ------ -Dr. Mo Mohammad Ashori LinkedIn Digital Nomad Physicians Website Telemedicine: Digital Nomad Health Website Physician Coaching & Consulting Dr. Mo Goodreads Heart Health Reddit
