Burnout has been a major concern for healthcare providers for years, with clinicians pointing to administrative work being a chief contributing factor. Now, some organizations are testing new tools powered by artificial intelligence to help assist with that workload. On today's episode of Gist Healthcare Daily, Kristine Lee, MD, Associate Executive Director of Virtual Medicine, Technology, and Innovation at The Permanente Medical Group, joins the podcast to share what the 10,000-physician group is seeing from its current pilot of an ambient AI scribe. Hosted on Acast. See acast.com/privacy for more information.
For years, people in the medical field have been exploring how to streamline our healthcare system - to help attract and keep staff, connect more of us with family doctors, and do it all for less money. The goal seems impossible at times, but researchers believe Artificial Intelligence - or so-called "large language models" - might be a way to do it. Randy Giffen is a solutions architect with IBM and a former physician. This week he'll be part of a webinar called "Large Language Models and Their Potential Role in a Learning Health System."

Over Coffee® is on post-CES hiatus. Please enjoy this re-posting of one of our top episodes of 2022! This post and podcast are for informational purposes only, and are not intended as medical advice. Please contact a healthcare professional with any health-related questions. Clinical neuropsychologist Dr. Albert "Skip" Rizzo is a pioneer in the field of medical virtual reality. Since the 1990s, Skip, as he prefers to be called, has been using immersive technology to treat his students. (He doesn't call them "patients".) Today Skip is the Director of Medical Virtual Reality at USC's Institute for Creative Technologies. He first began working in research at ICT in 1995, according to his LinkedIn profile. One of his multiple titles is "Ayatollah of VR", listed from 1995 to the present! (We first had the pleasure of hearing him speak at Cedars-Sinai's inaugural Virtual Medicine conference in 2018.) Skip's ground-breaking work has garnered multiple awards. He's researched, designed and evaluated VR-based therapy systems for traumatic brain injury, autism and post-stroke rehabilitation. He is also the creator of BraveMind, a VR-based exposure therapy program for the treatment of PTSD. As of December, 2022, according to their website, the U.S. FDA has endorsed the use of AR and VR devices in medical care--decades after Skip's discoveries in the field. But when he first started talking about using virtual reality applications at psychology conferences, he says, "(My colleagues thought I was crazy.)" Today, he's looking at an apparently-limitless future for mental-health VR apps. "There's no wall here," Skip says, when considering future directions in which he'd like to innovate. Skip, who will be speaking at Cedars-Sinai's 2023 Virtual Medicine conference in March, shared the story of his creative journey with medical virtual reality in this interview we recorded in February, 2022. On this edition of Over Coffee® we cover: Skip's introduction to virtual reality; His process, from there, in implementing VR simulations for brain-injury rehabilitation; What he remembers from the early days of treating his students with VR simulation-based therapy; How he and his team currently use VR simulations with virtual humans to help patients on the high end of the autism spectrum; A closer look at some of the virtual-reality applications that can enhance empathy across various areas of mental health; Some of the results Skip has seen; His vision for the future of therapeutic virtual reality.
Dr. Mitch Shulman joined Ken to talk about virtual medicine.
I think it's important to stay ahead of the curve. In some ways healthcare moves slowly but the spaces where physicians can make money change rapidly. Finding your own client base or finding your own patients, whether as a physician or a health coach, is an important skill to develop. I don't think it has to be that complicated. In a recent episode, I talked about a Healthy Aging program you can create. I think understanding where you would find these clients and how you execute this is important. The goal is to build your own client based and genuinely love what you do as a physician. That has been missing in our profession to a large extent. https://digitalnomadphysician.com/ https://www.cardiachealthcoach.com/ https://digitalnomadhealth.com/
Let's talk about taking a break from medicine. Whether you're fully burnt out and want to leave western medicine or just want to take a break. There is the emotional aspect and the financial aspect of it. In order to make money, you likely will want to earn some income. Here are all the different ways in which you can earn a good income while taking a break from traditional mainstream medicine. This post and podcast are for informational purposes only, and are not intended as medical advice. Please contact a healthcare professional with any health-related questions. "It's all about the patients," says pioneering psychologist/neuropsychologist Dr. Albert "Skip" Rizzo. He's discussing his use of medical virtual reality in clinical psychology. And for him, it all started in the 1990s, with an observation he made while watching one of his patients absorbed in a popular game. Fast-forward to today. With numerous awards for his groundbreaking work, Dr. Rizzo, or Skip, as he prefers to be called, is the Director of Medical Virtual Reality at the University of Southern California's Institute for Creative Technologies. He also serves as Research Professor and Scientist for both USC's Department of Psychiatry and Behavioral Sciences and their School of Gerontology. A range of applications Among the applications he has created is BraveMind: a VR-based exposure therapy program for PTSD. Traumatic brain injury, autism, and ADHD are among the other clinical conditions for which Skip researches. designs and evaluates VR-based therapy systems. We first heard one of his presentations during Cedars-Sinai's inaugural Virtual Medicine conference in 2018. The VR applications he discussed included one which measured classroom involvement among students with ADD, and game-based systems to assist in post-stroke rehabilitation. But Skip says there's much more he wants to do, using simulations to serve clinical patients, first responders and members of the medical community. Skip shared the story of his work with medical virtual reality, explored some of the ways in which this therapy can be helpful to patients on the autism spectrum and offered a look at the areas in which he'd like to expand his work in the future. On this edition of Over Coffee® we cover: Skip's introduction to virtual reality; His process, from there, in implementing VR simulations for brain-injury rehabilitation; What he remembers from the early days of treating his patients with VR simulation-based therapy; How he and his team currently use VR simulations with virtual humans to help patients on the high end of the autism spectrum; A closer look at some of the virtual-reality applications that can enhance empathy across various areas of mental health; How some programs developed for patients with autism may also be effective in reducing social anxiety among children and the general public, post-COVID; Safety considerations for children in the use of VR in a clinical setting; Some of the results Skip has seen; His vision for the future of therapeutic virtual reality. And, heads-up: Skip is speaking at two upcoming conferences. He's a featured speaker at the 6th Annual Virtual Reality and Healthcare Symposium, happening in Nashville this Wednesday and Thursday, March 3rd and 4th. Then, on Thursday, March 24th, he will be an early-morning speaker at Cedars-SInai's all-virtual vMed22 Conference.

Our Over Coffee® Valentine special will be posted later this week! Meanwhile, as Cedars-Sinai prepares for their annual "Virtual Medicine" conference in March 2022, here is our earlier interview with conference founder Dr. Brennan Spiegel, on his book, "VRx: How Virtual Therapeutics Will Revolutionize Medicine". In March, 2018, we watched history being made in Los Angeles. Medical professionals from thirteen countries had come to Cedars-Sinai Medical Center for the world's first "Virtual Medicine" conference. An incredible two days followed. Led by Cedars-Sinai Director of Health Services Research Dr. Brennan Spiegel, a series of medical professionals took the stage for presentations on pain management, therapy and preventive medicine--using virtual reality. A standing ovation followed the final session. Unbelievably, four years have passed since that groundbreaking conference. The fifth, now-annual Virtual Medicine conference, "vMed22", is coming up March 24-25 of this year, and will be entirely virtual. (Here's the link for information, and we're looking forward to attending, virtually!) Meanwhile, in 2020, we had the honor of getting to talk with Dr. Spiegel about his findings, and his experiences, as we discussed his book, VRx: How Virtual Therapeutics Will Revolutionize Medicine". Here is a rebroadcast of our interview--and we're looking forward to March 24th! On this edition of Over Coffee®, we cover: What first inspired Dr. Spiegel to begin using virtual reality in his medical practice; One of the early experiences which he had with VR, and describes in his book; How VR is currently being used in therapy to treat anxiety; A look at one doctor's use of virtual reality to help veterans with PTSD; How VR works to combat eating disorders, such as obesity and anorexia; A neuroscience explanation of why it works–to “fool” the brain!; A look at how VR can work in the brain alleviate pain; The story of one patient who used VR to control pain and wean himself off opiods; Some of the obstacles to widespread adoption of medical VR–and how the field is growing; An exciting new program, currently in the planning stages at Cedars-Sinai; One program which Dr. Spiegel developed with a congregation, in VRx, to help parishioners lower their blood pressure through VR; AND, how to find out more about upcoming free webinars and resources through Cedars-Sinai.

Dr. Brennan Spiegel presents at inaugural "Virtual Medicine" conference, Los Angeles, 2018. Could virtual reality be effective as a treatment for chronic pain? Or as therapy for PTSD? As a way to combat phobias? Or even as a means of preventive medicine? According to Dr. Brennan Spiegel's new book, VRx: How Virtual Therapeutics Will Revolutionize Medicine, the answer is, "very possibly". It may also be, "very soon." In VRx, Dr. Spiegel, who is Director of Health Services Research at Cedars-Sinai Medical Center in Los Angeles, explores numerous areas of medicine in which he and his fellow medical professionals are implementing the technology into the healing process. Cedars-Sinai currently has one of the largest virtual-medicine programs in the world, with new innovations forthcoming. And some of these uses are absolutely mind-blowing. Dr. Brennan Spiegel presents some of his findings during the inaugural Virtual Medicine Conference, Los Angeles, 2018. Psychologists are looking at helping patients combat depression through the use of digital avatars. Obstetricians are starting to offer VR experiencesan alternative to an epidural. And Dr. Spiegel's own experience includes management of chronic pain--both for his patients and for himself--through virtual reality. Early in his book, Dr. Spiegel points out that virtual reality is not a panacea. But he takes his readers on a journey, starting with his own first introduction to the technology in 2014. From there, VRx explores the cases and research in the areas in which he's seen it work effectively. Dr. Spiegel (who is the founder of Cedars-Sinai's annual Virtual Medicine conference, which we've had the pleasure of covering in the past) talked about his own journey from skeptic to virtualist, offered a look at some of the use cases in his book and previewed some exciting developments on the horizon. in the field of virtual medicine at Cedars-Sinai Medical Center. On this edition of Over Coffee®, we cover: What first inspired Dr. Spiegel to begin using virtual reality in his medical practice; One of the early experiences which he had with VR, and describes in his book; How VR is currently being used in therapy to treat anxiety; A look at one doctor's use of virtual reality to help veterans with PTSD; How VR works to combat eating disorders, such as obesity and anorexia; And, a neuroscience explanation of why it works--to "fool" the brain!; A look at how VR can work in the brain alleviate pain; The story of one patient who used VR to control pain and wean himself off opiods; Some of the obstacles to widespread adoption of medical VR--and how the field is growing; An exciting new program, currently in the planning stages at Cedars-Sinai; One program which Dr. Spiegel developed with a congregation, covered in VRx, to help parishioners lower their blood pressure through VR; AND, how to find out more about upcoming free webinars and resources through Cedars-Sinai. Would you like to hear medical-VR findings from the top four winners in Cedars-Sinai's international "Best Clinical VR Research of 2020" competition? They'll be presenting their findings, live, on Tuesday, December 8th at 9:00 am, Pacific time. Here's the link for more information.

Dr George Lee returns to discuss the latest medical news. Expect deep insights delivered with generous humour. COVID-19 poses major risks to healthcare workers in neurocritical care. While efforts are made to limit the number of medical personnel attending patients in neurosurgical wards, there is also a need to find a sustainable solution. Could virtual medicine be the answer? Prof Dr Vicknes Waran Mathaneswaran, from the Division of Neurosurgery, Faculty of Medicine, University of Malaya shares the findings of a study that tested the use of smart glasses in ward rounds during this pandemic. "Telehealth has come into focus during the COVID-19 pandemic as physicians face an immediate need to reduce exposure by providing care—or at least triage—remotely when appropriate. Under usual circumstances, telemedicine is comparatively low risk. That said, telemedicine does bring specific risks to patient safety and physician/practice liability. Minimizing those risks calls for adapting daily practice routines around informed consent, documentation, and other standard components of a patient encounter, as well as adjusting the practice’s insurance coverage." David L. Feldman is chief medical officer, The Doctors Company and Healthcare Risk Advisors. He shares his story and discusses his KevinMD article, "7 tips for telehealth during COVID-19." (https://www.kevinmd.com/blog/2020/04/7-tips-for-telehealth-during-covid-19.html) This episode is sponsored by The Nuance Dragon Ambient eXperience (DAX). The Nuance DAX solution makes it possible to forget the tech-toggling and reduces documentation burdens no matter how or where care is being provided. (www.kevinmd.com/nuancedax)
On this week's The Sci-Files, your hosts Chelsie and Danny interview Katie Mitchell. Katie is a doctoral student in the Information and Media program at MSU. She is interested in how media and technology influence interpersonal relationships and health. She is currently studying telemedicine and its impacts on patients as well as providers. Katie received both her Bachelor's and Master's degrees from the Department of Communication at Michigan State University. Healthcare providers actually have a strong interest in providing telemedicine to their patients, and the patients' interest also appears to be growing. The big question for Katie is how to provide virtual care that includes the interpersonal elements that are critical to patient satisfaction. If you're interested in talking about your MSU research on the radio or nominating a student, please email Chelsie and Danny at scifiles@impact89fm.org. You can ask questions about future episodes here. Check The Sci-Files out on Twitter, Facebook, Instagram, and YouTube!

with @vintweeta @pbcancerdoc @sumitshahmd @omnivorousreadCoronavirus is now disrupting the entire health care system, not just because of the burden of dealing with the actual disease itself, but because of everything else that's had to grind to a halt. One of the areas where we really worry about things coming to a total stop like that is, of course, cancer treatment, which can often feel like a race against the clock even under the best conditions. In this episode, Dr. Bobby Green, MD (Community Oncologist and Chief Medical Officer, Flatiron Health) and Dr. Sumit Shah (Oncologist and Head of Digital Health, Stanford Cancer Center) join a16z's Vineeta Agarwala (physician and general partner) and Hanne Tidnam to talk about what is happening to oncology during the outbreak—how treatment is affected; what kind of clinical decisions oncologists and patients are having to make, and how they're making them; the tech tools that specialists are using, and how they could improve; and what happens to oncology as a whole when it's forced to go virtual.
While the technology to be able to see a doctor virtually has been around for awhile, the novel coronavirus pandemic has opened the doors to virtual medicine in a way that has not been seen before. Dr. Ed Brown, CEO of Ontario Telemedicine Network joins Mike Stubbs to discuss accessing medical professionals and advice, virtually.
We’re at a moment where we are now seeing medicine go virtual faster, and at a scale that it has never done before. In this conversation, a16z bio general partners Vijay Pande and Julie Yoo, who come from the worlds of bio, technology and care delivery, talk with Hanne Tidnam all about what exactly virtual care and “telemedicine” is... and what it isn’t; what it works well for, what doesn’t (yet), and where there’s potential; and finally, the current pain points (including regulation), and what we’ll learn from this current moment for the next generation of virtual medicine tools.

Virtual reality is an exciting new medical implement. And Cedars-Sinai Medical Center, in Los Angeles, continues to explore its effectiveness. pioneer new uses--and share their knowledge with the community. In March 2018, Cedars-Sinai Medical Center, in Los Angeles, hosted the first-ever Virtual Medicine conference, which was nothing short of mind-blowing! This week, Dr. Brandon Birckhead, Co-Director of the Virtual Medicine Conference, gave us a look inside the event--and a preview of the 2019 edition, which is just about to happen! Dr. Brandon Birckhead (far right) moderates a "Roundtable for Medical Virtualists" panel during Cedars-Sinai's inaugural "Virtual Medicine" conference For two days during the inaugural 2018 conference, attendees from 13 countries explored different aspects of the use of VR and AR in medicine. Just a few of the areas speakers addressed: VR in phobia treatment, pain management and physician training. Here's Cedars-Sinai's link to their videotaped talks. (We had the honor of covering the conference, which was live-streamed in virtual reality. Here is a link to our coverage.) With Virtual Medicine 2019 coming up, Dr. Birckhead talked about his experiences planning the inaugural conference, shared some of Cedars-Sinai's research on the use of VR in medicine, and offered a preview of this year's conference. (UPDATE: Virtual Medicine 2019 has sold out. Here is the link on which the conference was live-streamed last year We haven't yet heard whether it will be live-streamed on March 27th and 28th.) On this edition of Over Coffee®, you will hear: What the experience of planning Cedars-Sinai's first "Virtual Medicine" Conference was like; A preview of Dr. Birckhead's upcoming talk at the 2019 conference; One application of a VR program in the medical field that may not have occurred to you (but makes perfect sense!); A summary of Cedars-Sinai's recent research findings on the use of VR in pain management; A preview of the sessions scheduled for Cedars-Sinai's 2019 "Virtual Medicine" conference; Dr. Birckhead's favorite takeaways from last year's "Virtual Medicine" conference. While arrangements are still being finalized, there's a chance they might do that again!
Alexis Smirnov, the co-founder and CTO of Dialogue (dialogue.co), discusses the changing times in regard to healthcare and how virtual healthcare options are becoming more commonplace. Dialogue is the top Canadian telehealth service and Smirnov is passionate about the future of healthcare. Smirnov has many years of experience in the design and development of social analytics systems, search engine technology, conversational mobile apps, and more. He has played an integral role in the building of products and services for various startups and Fortune 100 companies. Smirnov is continually intrigued by transformational technologies and the practical methods to bring them to market, and his passion for tech led him to his work as CTO at Dialogue. Smirnov discusses how technology is changing the way we receive healthcare. His company, Dialogue, was founded in 2016 by co-founders who saw an incredible need for access to better health care, and they set out to solve the problem via technology. Dialogue offers virtual healthcare solutions for employees, and impactful benefits for businesses and their platform provides forward-thinking, premium, affordable health care options for Canadian companies and individuals. The Dialogue team is comprised of many skilled entrepreneurs, developers, sales experts, designers, customer service specialists, engineers, and quality assurance experts who work collaboratively to devise the best strategies to tackle modern healthcare issues. And their medical team features some of the most highly respected healthcare professionals in Canada who practice virtual healthcare and seek to bring the best care to everyone. Smirnov describes the problems of traditional healthcare settings in regard to treating patients, and how a virtual care team can alleviate some of the burdens on the system, long waiting times, and more. He describes how many companies will choose to offer virtual healthcare as a corporate benefit, as many employees may find it helpful to have treatment and advice from qualified healthcare workers right on their smartphone… in an app. He discusses the use of photos in the treatment process, such as a skin issue, for example, in which a user could photograph their particular skin issue, send the photo via the app, and get a doctor or health care worker's advice on treatment right away. He states this method could sometimes allow care to come within minutes, as opposed to days or weeks in a traditional setting. And by cutting overhead through the use of technology, doctors can often reduce their time with paperwork and give more time to patients, which as Smirnov states, is what everyone wants. Smirnov details how many companies are seeing the benefits of offering this virtual healthcare option to their employees, and as such Dialogue is growing at an incredible rate. He expects that the transition from physical first to digital first healthcare will create a booming industry for virtual care providers. As doctors and hospital administrators learn more about the service, Smirnov mentions that they are always surprised that the system has not been put in place earlier, and they quickly gravitate toward the service as they see that the benefits are boundless.

If you follow the work being done in Virtual Reality (VR) and Augmented Reality (AR) - you probably have already seen the work being done by today's guest: Dr. Brennan Spiegel. In this episode, Brennan gets real about the applications for virtual reality in healthcare, and what is needed to drive adoption throughout the healthcare system and around the world. Brennan Spiegel is the Director of Health Services Research at Cedars-Sinai Health System. He’s also a Professor of Medicine and Public Health at UCLA, and Co-Chair of the VR/AR Association Digital Health Committee. He has published numerous best-selling medical textbooks and has more than 170 articles in peer-reviewed journals. His work has been featured in Bloomberg, Forbes, Huffington Post, LA Times, NBC News, NPR, PBS, Reuters, and the Wall Street Journal and now we’re adding Digital Health Today to that esteemed list of media outlets! You can often find Brennan at major conferences around the world including HIMSS, Connected Health, Exponential Med, Health 2.0 and CES - actually it was CES where I heard him speak for the first time. And now, he’s founded a new conference call Virtual Medicine which was held last week at Cedars-Sinai Medical Center in Los Angeles. It was the first international symposium dedicated to medical virtual reality. Tickets were sold out for this debut event, and if you weren't there, fear not, because in true pioneering fashion, Brennan worked with the team over at Samsung to get the event filmed and streamed live in 360 - the recordings aren't up yet, but you can grab your goggles and transport yourself back in time and over to Los Angeles and watch the event as if you were there.
