Guest: Richard M. Anderson Date: 3-27 LIVE 5-7 pm EST TOPIC: Humanity's Clock is Ticking: Are We Doomed or Destined for the Stars? About the author Richard Anderson is a San Jose State University alumnus with a master's degree in microbiology. His studies also emphasized molecular biology and biochemistry. Several years after earning his MA, Richard obtained a Clinical Laboratory Bioanalyst license. In subsequent years, he worked in various administrative and technical management roles in laboratory settings, beginning his first assignment as a clinical laboratory director in 1993. In 2006, he began working in a reference laboratory for Quest Diagnostics; he continued working in various Quest laboratories as a laboratory administrative director and technical director. Shortly before the worldwide COVID-19 pandemic, Richard began exploring research areas that had caught his attention during his studies and employment years. As a result of his research, new information came to light that sparked his enthusiasm. As the pandemic engulfed the world, research and writing his first non-fiction book, "The Evolution of Life," filled the voids left by relative isolation. The completion of this book prompted the creation of his fiction series, "Outbound," looking into the future of humanity off-planet. Richard lives with his wife in California's south San Francisco Bay area, close to their children and grandchildren.
In this episode we are exploring rare and ultra-rare disease in honor of Rare Disease Day on February 28 which raises awareness for the 300 million people worldwide living with a rare disease. Steve Becker is committed to improving human health by unlocking the power of molecular diagnostics. In the world of medical devices and diagnostics, you might think the latter falls under the purview of the former, says Steve, yet: “from a regulatory standpoint, diagnostics is covered by a lot of the medical device rules.” Unlike medical devices like implants, however, “diagnostics is really the ability to measure your body, whether you're healthy or sick; and if you are sick, what's making you sick. Helping to uncover your health status and what you can do to improve is really the heart of diagnostics.” In this episode, Steve shares the lasting lessons learned during his stints at Thermo Fisher, Quest Diagnostics, Agilent, and GE Healthcare, where he directed sales, global marketing, business development, and licensing. Learn how to navigate within a constantly changing healthcare business landscape, from the first steps toward launch to the regulatory requirements and cultural differences that can affect development, distribution, and product adoption. Going to market on one's own is a big task, says Steve – find the right partners to mitigate the commercial regulatory tasks and technical risks. Ultimately, the company whose name is on the product assumes the liability – “they're the ones on the hook” – but relying on specialists along the way improves your chances of a streamlined launch. During his tenure at Quest Diagnostics, the company built a “global diagnostics network,” pooling the capabilities, diagnostics insights, samples, testing and sharing of leading companies into one place. In the end, Quest brought the first Zika test to market, reliant upon colleagues. In fact, choosing your initial target audiences can come into play even during development, as “diseases become more localized and companies are made for those markets,” Steve adds. Pay attention to “how to get paid for what you do,” your reimbursement strategy. In diagnostics, for example, ask yourself: Is it going to measure what we say it's going to measure? (Analytic or clinical validity) Can we do anything with the information? (Utility) Interestingly, reimbursement can often become political; for example, in the US it's widely accepted that breast cancer can come with a genetic indicator (the BRCA gene) – and the patient can take prophylactic steps or opt for watchful waiting. In contrast, Steve says that: In a single-payer system, if you were 49 you could take the [BRCA] test; if 50, no test. The calculation includes risk, quality of life, total cost – someone makes a decision, snapped the line, and that's where it landed. Add to that differences stemming from culture and tradition, and it makes sense to work with local, in-country partners to navigate your go-to-market strategy, especially with regard to regulatory requirements. To date, Steve says that European companies would often launch first domestically and then in the US because it was easier, but that's starting to change as the EU changes its regulatory stance. Regulations are stricter now in Europe, with liability going as far back as the manufacturer – for translation quality, for example. Diagnostic tests are now treated more like a medical device, including review by regulatory bodies, etc. – it's a lot more work but it's a good thing. Think globally but act locally, Steve adds. You may use the same assay (investigative procedure for qualitatively or quantitatively measuring the presence, amount, or functional activity of an element) as in the US but a local company can handle the technology, packaging, regulatory requirements, approvals, and clinical evidence and market under THEIR brand, with their own branding and local language. This happens consistently, across IV diagnostic companies – Abbott, Siemens, Roche, Thermo Fisher – and all of them have to treat each of their launches “locally,” working with in-country regulatory bodies to get approvals. And vice versa: one of the first Covid tests distributed in the US was manufactured by a Korean company but marketed as a known American brand. In fact, working with companies around the world has interesting and complex effects on branding – “you'll get a multi-national brand but lose some efficiencies,” according to Steve. Genetics is the transcript of your own personal health. We often perceive it as something that dictates that health issues are hereditary but the truth is that no one was born with chronic illnesses. This story showcases how our clinicians today are able to prevent any possible illness by looking into a person's unique genetic makeup - even down to the body's smallest nuances.Onto the third epoch, the amount of information gathered must be handled with care so as not to dilute it in a soundbite that suggests that science is binary but rather give science its due right. See all the Healthcasts at https://www.biobalancehealth.com/healthcast-blog At Bio Balance Health I order blood tests to evaluate my future patients before I even have my first appointment with them. I use them to find a baseline for an individual patient, and to see if hormones will help them with their symptoms. Blood tests work well for establishing a diagnosis but are not the only factor in determining an ideal dose of hormone that works for that patient. Symptoms and medical history supply most of the information for determining dosage as well as help me find other medical problems to treat at the same visit. Many diseases in the early stages are not treated by primary care doctors because they are overwhelmed with their number of patients and the short-time they are allowed to see them. Our goal is to prevent disease that have been missed or treat conditions at an early stage before they become severe. Blood tests establish and confirm both hormone deficiencies and provide a baseline level before treatment and the optimal level for each individual patient. That blood level may or may not be within the range that is expected for a treatment success (written on the lab report). You are an individual and it is my job to find the ideal blood level of hormones for you. Why would the blood level not tell the whole story? Your body is genetically programed to both PRODUCE hormones from your endocrine glands and ACCEPT those same hormones in each cell. Every person is an individual and each person makes hormones based on their genetic map and environment. Everyone is programed genetically to accept hormones into their cells in an individual manner. This is the key to understanding the differences between patients' responses to an equivalent dose of hormones, either excreted from their own glands or absorbed through their chosen delivery system (oral, vaginal, transdermal or subdermal pellets) after their glands have aged and don't produce enough hormone, like estradiol and testosterone. I was in Cambridge in 2014 for the release of my first book, The Secret Female Hormone, when I visited a medical bookstore. I discovered a large red book that weighed no less than 10 lbs was titled Testosterone. I looked through this book and found that there was information that I had not discovered in my research of the American medical journals and books, so I bought it and read some of it on the long flight home to St. Louis, MO. I found that this book had answers to questions I had uncovered in my then,13 years of hormone medical practice. My biggest question at that time was why 2 people of the same sex having the equivalent blood level of free testosterone often feel completely different. For example, I was trained that if a man had a blood level of free testosterone that was above 129 pg/ml and under 350 pg/ml (using Quest Diagnostics lab), then he should feel normal, like he did when he was in his thirties (barring any other illnesses interfering). However, I observed that some men felt great at 110 pg/ml while others at 130 continued to have the symptoms of low T. This puzzled me, but at that time I had no answer. Then I read the first chapter of Testosterone. The answer is found in the individual differences in the receptors on each cell for that hormone, the receiving end of the hormone physiology. It informed me that receptor sites are genetically created differently in each individual and that one-size-does-not-fit-all! You can bathe a person's cells with what is considered an adequate blood concentration of testosterone for most people, let's say men since that was the subject of the first chapter, of the book Testsosterone, but some men would receive and use only a small portion of the circulating hormone. Their cells were “resistant”, therefore they required a higher concentration of hormone, to activate their cells. This very important fact in endocrinology has been ignored until recently when we began to use genetics to find the differences between individuals' receptor sites for all hormones and all peptide communicators. In the last 10 years we have found that receptor sites are the missing link in determining a person's required dose of a hormone, and they are genetically determined. Here is an example from the book in a very simplified translation. Think of a key and lock. Each cell has “locks” or receptor sites all over it, and the keys are the testosterone molecules. Some people's cells have locks that are easy to open and turn on a cell. They are “sensitive” to that hormone. Some cells have difficult “locks” and they must be bombarded with a hormone “keys” to turn the cells on. This led the researchers to look for the snips of DNA that were responsible for the difference. They found them and then tested men from all over the world to see if there were regional groups, genetic groups, that contained more sensitive receptor sites or more resistant. The results are quite telling and explain the differences between races and peoples in terms of how their bodies use the testosterone hormones available. The men with the most sensitive receptor sites live closer to the equator: Mediterranean, African, Brazilian, and those men with more resistant receptors had DNA from ancestors who came from closer to both North and South poles: Scandinavia, Canada, Europe, South Africa, Southern Argentina etc. The results mean that if you have a clear genetic line from one of these groups, we can predict whether you need a higher dose or lower dose to get the same resolution of symptoms. Most people in America are mixtures of genes so most patients don't give me any information from their ancestry. However, there are two catches to this system of determining an individual's receptor site sensitivity; 1) In this century we are able to travel and find a mate on the other side of the globe and have been able to do so for generations, therefore we are “mutts”, or people who have multiple genetic sources in our DNA and therefore how we look or where we are from is not a good predictable factor for how we will accept our testosterone. 2) There is no commercially available test to evaluate the sensitivity of our receptors. Currently, the best science has found is to test DNA for the snips that predict sensitivity or resistance to testosterone! This too will come to the practice of medicine, but “medicine” moves slowly and the government and people in control are always looking for a one-size-fits-all approach to illness and or aging, which makes me think this type of test will not be embraced by general American medicine. At this point all I can tell you about dosing and blood levels is that you have an individual level that is good for you, so I try to figure out what that is, and “mark it” with your “perfect blood level”. I proceed with is the knowledge that there are many other factors that affect the blood level of free-testosterone (active form) and the effect it has on individual patients. This leads me to practice individualized medicine like a tailor makes a dress or suit that fits one person only. I have a lot of experience to recognize resistant or sensitive T receptor patients and I now know how to approach their care. It is NOT by trying to adjust their hormones into the very inaccurate free T levels printed on the blood lab sheet, however I do look at those numbers to establish a baseline for that person, and I do listen to my patients when they tell me whether their symptoms are resolved or not. Resolving all SYMPTOMS is the real sign of a successful replacement of testosterone in both men and women.

Steven is currently the Executive Director of Genomic Services at Quest Diagnostics, which supports all advanced diagnostic testing at Quest Diagnostics and oversees and manages all activities of the genetic counselors. In his role, he acts as the liaison between Genomic Services and other areas of the company, such as Advanced Diagnostics, Corporate Medical, R&D, Laboratory Services, Finance, Legal, Compliance, and Commercial. Prior to joining Quest, Steven spent 14 years at Ambry Genetics, serving as Vice President of Clinical Diagnostics and 17 years as a clinical genetic counselor for Kaiser Permanente in Los Angeles. Steven is active in the National Society of Genetic Counselors, serving as the president of the society in 2009. In 2016, he was diagnosed with multiple myeloma and is currently on maintenance treatment and doing very well. ✨A few highlights from the show: 1. One of the causes of anemia is multiple myeloma. 2. Cancer survival statistics are out of date by the time you read them because they are based on people that were diagnosed 15 - 20 years ago. 3. It is important to know your family history.
