Dr. Devorah Segal earned her MD and PhD in Neuroscience at Icahn Mount Sinai School of Medicine. She completed her pediatrics and pediatric neurology training at Rutgers New Jersey Medical School and then completed a fellowship in Pediatric Neuro-Oncology at NYU Langone Medical Center.  She is a pediatric neurologist and neuro-oncologist at NYU Langone, where she is associate director of the Neurofibromatosis Center. She treats children with brain tumors and patients of all ages with neurofibromatosis, schwannomatosis, and other brain tumor predisposition syndromes. She also leads several clinical trials treating pediatric brain tumors, neurofibromatosis, and other genetic disorders with neurologic components. She lives in Northern NJ with her husband and 5 kids, all of whom were born during medical school. Lacerations Common causes: Broken glass Debris Improvised projectiles What to look for External bleeding Deep cuts with visible tissue Embedded debris Bleeding that soaks through clothing Basic treatment Put on gloves if available Apply direct pressure with gauze or cloth If bleeding continues, use a compression bandage For severe extremity bleeding, apply a tourniquet Cover the wound with a clean dressing Additional considerations Do not remove deeply embedded objects If the wound is large or continues bleeding, the patient needs hospital care Key reminder The vast majority of life-threatening bleeding can be controlled withpressure and time. Respiratory Irritants Common exposures: Tear gas (CS) Pepper spray (OC) Smoke from fires These agents cause severe irritation but are usually temporary. Common symptoms Burning eyes Tearing Skin irritation Coughing Shortness of breath Disorientation Basic treatment Move the person to fresh air immediately Encourage slow breathing Flush eyes with copious water or saline Remove contaminated clothing if heavily exposed Avoid rubbing eyes or skin Important notes Oils, lotions, or milk can sometimes trap irritants against the skin Most symptoms improve within 15–30 minutes once exposure stops Red flags requiring EMS Severe breathing difficulty Asthma attack Persistent confusion Heat and Dehydration Common causes: Long hours outdoors Heavy clothing or gear Stress and exertion Limited access to water Symptoms Dizziness Weakness Headache Nausea Muscle cramps Heavy sweating Basic treatment Move the person out of the sun or crowd Have them sit or lie down Provide water or electrolyte fluids Use cooling measures Shade Wet cloths Fanning Red flags for heat stroke Confusion Collapse Hot dry skin Seizures Heat stroke is a medical emergency. Common presentations: Panic attacks Hyperventilation Acute anxiety Disorientation What to look for Rapid breathing Shaking Crying Feeling unable to escape the crowd Basic treatment Move the person to a quieter, safer space Speak calmly and reassure them Encourage slow breathing Inhale through the nose Exhale slowly through the mouth Help them regain orientation and control Often, simply removing the person from the chaotic environment dramatically improves symptoms. “The key point here is that most injuries in these environments are not exotic trauma cases. They're the same things EMS treats every day — bleeding, falls, heat illness, and panic — but they're happening in a chaotic environment where help may take longer to arrive.” 4. Basic Medical Kit for High-Risk Gatherings Emphasize compact, discreet gear. Essentials Nitrile gloves Gauze / compression bandage Tourniquet Saline or water for eye irrigation Simple airway mask Electrolyte packets Small flashlight Optional but useful Chest seal Trauma shears Space blanket Eye protection Basic first aid medications Practical considerations Avoid large visible medical packs Keep supplies distributed in pockets Maintain mobility 5. This conversation offers a rare look inside one of the world's most recognised humanitarian medical organisations and the complex system that allows it to operate in some of the most dangerous and hard to reach places on earth. With around 70,000 staff working across more than 70 countries, the organisation provides emergency medical care to millions of people affected by armed conflict, disease outbreaks and natural disasters. In the past year alone, teams carried out more than 16 million outpatient consultations, alongside trauma surgery, treatment for malaria, tuberculosis and HIV, vaccination campaigns, and mental health support. Yet behind every clinic or hospital lies an intricate global operation that combines medicine, logistics, diplomacy and risk management. In this episode, MSF's Secretary General explains how humanitarian medicine works in practice. Teams must negotiate access with both state and non state actors, often in highly polarised conflict environments. Medical professionals work alongside logisticians, analysts and coordinators who ensure that drugs, equipment and staff can reach remote locations safely and reliably. The scale of the logistics alone is extraordinary. Medicines and vaccines must travel through complex supply chains while maintaining strict quality standards and often requiring temperature controlled storage. Equipment for surgery, sterilisation and treatment must arrive on time in places where infrastructure is limited or damaged. In many cases, care is delivered through mobile clinics operating from the back of a vehicle. Security is an ever present concern. Staff operate in environments where shelling, crossfire or kidnapping are real risks. Rather than promising safety, the organisation focuses on understanding risk, training staff and ensuring informed consent about the conditions in which they work. In 2025, eleven colleagues lost their lives while carrying out humanitarian work. The conversation also explores how knowledge gained in these extreme settings travels across the global health system. Experience with epidemic response, infection control and contact tracing developed in Ebola outbreaks later helped support hospitals and health ministries in Europe and the United States during the early stages of the Covid-19 pandemic. A defining feature of the organisation is its financial independence. Around 98 percent of funding comes from private donors, with more than 7.3 million donors contributing. This allows operations to be guided primarily by medical need rather than political priorities. Beyond funding, these contributions represent something deeper: a global expression of solidarity between people who will likely never meet but are connected through a shared commitment to helping others in crisis. A school administrator and massage therapist wants guidance on how to respond to a student who is reluctant to work with clinic clients who report a history of herpes. Is this a significant risk for massage therapists? The reality is that anyone with a history of herpes simplex can shed the virus intermittently, even without visible symptoms. However, the risk to massage therapists who follow proper hygiene and self-care protocols is extremely low. In that sense, herpes falls into the same category as other infections that may be transmitted through direct contact but are effectively managed with consistent hygienic practices.
