It's not intuitively obvious, but discarding DEI can result in even fewer medical professionals serving already underserved communities in rural Ohio. The Trump administration is doing its level best to eliminate DEI on the premise it fosters unlawful discrimination, but as Abraham Graber, Ph.D, points out, DEI promotes diversity and equity so as to achieve better outcomes, and that includes better outcomes in terms of getting more medical students interested in practicing medicine in rural Ohio. There's been a longstanding problem in rural Ohio suffering from inadequate healthcare. Some counties have just a handful of doctors. Some counties don't have a single doctor or hospital. What if you live in, say, Vinton County, and you need an oncologist? Well, like they say in the mob, “Fuggetaboutit.” You're pregnant and need prenatal care? Same thing.Who picks up the slack in these counties? EMT crews, but their services are only a stopgap.The absence of medical care for these communities means the people in these areas are not as healthy as those who live in metropolitan areas, and they have shorter life spans. To get more medical professionals in rural Ohio, we need a concerted recruiting program. And that means recruiting from the counties that are underserved, because the people who would want to practice in those counties will most likely come from those counties and not from the large metropolitan areas. And here's the challenge. If kids in those counties don't see doctors and don't see their friends becoming doctors, they'll never even think about becoming doctors. And for those few who might aspire to practice medicine, they likely don't have the same life experience or education as others and, thus, face barriers when tested and suffer from the negative stereotype many have about rural Americans. As Dr. Graber points out, if we want to start getting students in rural Ohio interested in and practicing medicine, we have to start thinking about how we find these young people. Focusing on test scores alone won't get us where we need to be. We need targeted recruiting, and that's DEI.
Originally expanded in 2014 under the Kasich administration, Ohio is one of 41 states, including the District of Columbia, that has implemented Medicaid expansion under the Affordable Care Act. However, the addition of trigger language into the current state budget bill would allow Ohio to disenroll from Medicaid expansion if federal assistance for the program drops below 90%. The trigger language comes as congressional Republicans in D.C. consider cutting the federal match for Medicaid expansion.rnrnFor Ohio, it's a move that could strip healthcare access from over 770,000 Ohioans; including about 362,000 in rural Ohio counties.rnrnAdvocates in health equity, maternal and infant vitality, mental health, and more have sounded the alarm on what it means to return to an era before Medicaid expansion. The United Democratic Movement (UDM) in the City of Johannesburg says it is appalled and disturbed by the inhumane and unethical treatment of a Johannesburg Metropolitan Police Department (JMPD) officer who was reportedly denied emergency medical care after being shot in the line of duty. UDM says it has noted with grave concern that Milpark Hospital allegedly refused to treat the officer citing an alleged municipal debt of R35 million. 446 Who am I? Medical Help that Saves Children's Lives Featuring Dr. Rachel Sewall: “I want to shout from the mountain tops!” Today we hear from Rachel Sewall, M.D., a Stanford pediatric endocrinologist who provides medically necessary care for transgender and gender diverse young people. She says that her work is highly emotional, with extreme highs and lows. People view this population with great fear and considerable ignorance as well. Valid medical information is powerful and important. Seeing how people view these young people is constantly on my mind. She says: When I was a medical student, I didn't have clear goals for what type of medical practice I would pursue. However, during my work in the emergency room, I realized, as did my colleagues, that I got very excited when working with children, so I decided to focus on pediatrics. I knew I'd be an advocate for LGBTQ+ young people. A colleague implied that they are rare, but I realized that wasn't true. In fact, surveys tend to underestimate the percent of trans individuals and LGBTQ+, do to underreporting due to feelings of shame and fear, but the surveys still suggest a trans population around 2% and an LBTQ+ population around 15%. So, together, we are likely looking at roughly 20% of the population fall into one or both of these two categories. As a first-year medical student, I did research on hormones. These are chemical messengers telling our bodies to do things. Figuring out the actions of hormones is like a complicated puzzle (Rachel explain a little, pleaee. I also went to a camp for kids with Type I diabetes. It was a perfect marriage for me! (Rachel fill in why please). My work with children and teens is a combination of medical and emotional support. More than anything else, these kids want to be listed to and respected. And the highs and lows I experience, as I mentioned earlier, can be extreme. Many begin sobbing when I talk to them, because they're so grateful that someone is listening! Reviewing basic definitions, your gender is usually assigned at birth, due to your sexual organs. They say, “Hurray! It's a boy!” Or “Hurray! It's a girl!” But as your identity and sense of our gender evolves, and it may not be the same as the gender assigned to you at birth. Gender identity begins to form around 18 months to 3 years old, but can emerge at any time later as well. Most of us “know” our gender identity by the time we are 5 or 6, but it can also emerge during puberty as well, or even later. They may suddenly say, “this is not whom I am.” And some, of course, say “I am neither male nor femail.” That's where the terms cis and trans come from. Cis means that your assignment at birth and your gender identity—your sense of who you really are—can be the same. If so, you are a cis male or a cis female. But if they are not the same, you are a trans. For example, a trans male was born with female genitalia but a male identity evolved. And a trans female was born with male genitalia but a female identity evolve. Many trans individuals experience great distress when taking showers, since they may feel repulsed when they see that they have the “wrong” genitals. Gender identity has overwhelming power. Some have told me, “I just want to tear my skin off!” Sexual preference and gender identity are not the same or linked in any particular way. Sexual preference simply refers to who you love, sexually and emotionally, and want to partner with. Rachel continued, Hormonal interventions can be life-saving. If you are a trans male, having periods can be profoundly disturbing, often to the point of triggering suicidal urges. Helping these kinds and their periods via hormonal interventions can bring enormous relief. They often say, “Dr. Rachel, you saved my life!” I almost always work with the family, and my goal is for the family to learn to function as a team, all working together to provide love and support for their child, so they can say, “this is my child and I love and accept them unconditionally!” I emphasize that there is no right or wrong on their gender journey. Metabolic and surgical interventions both have their place in treatment. The time to go public with who you really are is an important social and cultural decision, and this also varies tremendously. But regardless of age, gender-affirming medical care can always make a tremendous difference in your life! Rhonda and I are deeply grateful to Rachel for giving us this wonderfully patient and clear education in a field that was not even covered, to the best of my knowledge, when I was a medical student at Stanford. Currently, he is the Chief Behavior Analyst for the Lee Specialty Clinic and a Behavioral Consultant at Kramer Behavioral Health. In these roles, Brandon has implemented various interventions to help individuals with intellectual disabilities access routine medical and dental care. If you're not familiar with this topic, it might sound pretty mundane, but as you'll hear, these populations really struggle to attend and participate in routine medical appointments, which, not surprisingly, has downstream negative health consequences. As such, interventions that improve on the current statistics can truly be considered life-saving. In this podcast, we get into the following: Common phobias associated with medical treatments. Brandon's personal experience with aerophobia (the fear of flying), and what he did to improve it. The basics of desensitization strategies. Protest action is still going ahead in Napier this weekend. There's been community backlash after a leaked proposal showed the Government considering cutting the overnight urgent care nurse service. It's now choosing to retain it, but still with no GPs on-site. Napier Mayor Kirsten Wise told Mike Hosking the closest doctor is a 25-minute drive away at Hastings Hospital's busy Emergency Department. GPs say a Government cash injection into general practices is a good start. The Health Minister's announced $285 million in performance-based funding for practices over three years. Simeon Brown is also kick-starting initiatives to boost the number of nurses and overseas-trained doctors in healthcare. Royal College of GPs President Samantha Murton told Mike Hosking practices need a lot more money, but the Government has to start somewhere. Another measure announced was the 100 new placements for overseas-trained doctors. The Royal College of GPs says vocational training for the doctors was always done in hospitals. But Murton told Hosking primary care is now getting a slice of the pie. She says general practices offer a wider range of training in New Zealand healthcare than hospitals. The Prime Minister is defending how much nurses in New Zealand are being paid. Yesterday the Government announced multiple incentives to improve access to primary care, including a cash injection of $285 million over three years. Christopher Luxon says any narrative about health services being cut is completely untrue. He told Mike Hosking the Government's making good progress on recruitment, just not getting the results patients need. Luxon says the remuneration of nurses has increased to $127 thousand dollars, including allowances and overtime. He also says that those unhappy with his Government's refreshed school lunches should go make a marmite sandwich. A number of schools have complained about late deliveries and sub-standard offerings under the new cut-price regime. Luxon's acknowledged some delivery timeframes and meals haven't been up to scratch, but says some people will never be pleased. He told Hosking parents should be the ones feeding their kids. She cares for patients of all ages at the Cooper Family Medicine office at the Kroc Center in Camden. She loves working with medical students in preclinical and clinical educational settings. She teaches selectives in Narrative Medicine and Audio Storytelling and co-directs the Narrative Medicine Scholarly Concentration. UPDATE: Dr. Hussam Abu Safiya's mother has tragically passed from a heart attack. Dr. Thaer Ahmad, MD, is a board certified emergency medicine physician and a board member of the Palestinian American Medical Association. He has traveled to Gaza on several medical missions and recently spent three weeks volunteering at El Nasser Hospital in Khan Younis. Since returning, he has spoken out about the humanitarian crisis in Gaza and the need for humanitarian aid and services to reach the people. He is an assistant clinical professor at the University of Illinois at Chicago, and the global health director for his emergency department. Dr. Ben Thomson is a renowned public health expert, nephrologist and general internal medicine doctor, an award-winning educator, a board member of the Muslim Advisory Council of Canada and the Union of Medical Care and Relief Organizations, and a global humanitarian physician. Dr. Thomson's efforts have markedly improved healthcare in Indigenous communities in Ontario and globally in places including Uganda, and in Gaza through initiatives like the Keys of Health Fellowship and EmpowerGaza. Dr. Thomson envisions a world where resilient, compassionate healthcare is accessible to all communities globally. Ron Hira, an Economic Policy Institute research associate, is an associate professor in the department of political science at Howard University. His book, Outsourcing America, was one of the first to examine the economic and policy implications of the offshoring of high-skilled jobs. It was a finalist for the Benjamin Franklin awards in the best business book category. Hira has testified before Congress on offshoring and high-skilled immigration. He is frequently interviewed by the media about his work, which intersects STEM labor markets, immigration, globalization, and competitiveness policy. He is a licensed professional engineer. Quinn Slobodian is professor of international history at the Frederick S. Pardee School of Global Studies at Boston University. His books, which have been translated into ten languages, include, most recently, Crack-Up Capitalism: Market Radicals and the Dream of a World without Democracy. Forthcoming is Hayek's Bastards: Race, Gold, IQ and the Capitalism of the Far Right. He has been an associate fellow at Chatham House and held residential fellowships at Harvard and FU Berlin. He co-directs the History and Political Economy Project and is on the board of editors of the American Historical Review.
