"The accumulation of these malignant and reactive cells, that's what causes the lymph nodes to get big. That's why they grow. And that's why the classic presentation is persistent, painless, enlarged lymph nodes. So when we think of the pathophysiology of Hodgkin lymphoma, there are really two processes contributing to the presentation: One is the physical effects of enlarging lymph nodes or the involved organs. Highlights From This Episode "Hodgkin lymphoma can be diagnosed at virtually any age but is more common in younger patients as well as older adults. It kind of skips middle-aged people, but it occurs in our geriatric population as well. There are different prognostic implications depending on the group of patients that are diagnosed with Hodgkin lymphoma. As far as risk factors ... Hodgkin lymphoma is not considered a hereditary disease. There's not a specific gene we know of today that can be passed down to children and put them at risk. But if there is a first-degree relative affected by a lymphoma, we do find that noteworthy. We do consider it a risk factor. But that risk factor isn't necessarily a hereditary component so much as it is having that same genetic susceptibility to Hodgkin's or perhaps sharing common environmental exposures." TS 5:46 "Sometimes the subtypes of Hodgkin lymphoma can vary in terms of presentation. It's certainly not textbook how someone can present. But generally, we worry about enlarged lymph nodes, especially if they are notably enlarged to the point where someone can even see them when they're standing a few feet away from that individual. ... We also worry about what we call B symptoms, related to B lymphocytes, such as unexplained and persistent fevers, drenching night sweats, unexplained weight loss, and new progressive fatigue. ... Interestingly, there is a phenomenon where sometimes people who have an underlying Hodgkin lymphoma will experience lymph node pain or tenderness when they drink alcohol. It's kind of a unique presenting symptom. But I also want to point out that you don't have to have all of these symptoms. A lot of patients are relatively asymptomatic, meaning without symptoms." TS 8:37 "It's always important to consider if there is any very obvious infectious or inflammatory correlate that could explain the enlarged lymph node. Something I've seen in my practice is someone gets a flu vaccine in their right arm and then a couple of days later they notice a lump in their right armpit. And yeah, it might be a flared lymph node, but they also had a vaccine in their arm recently, which would stimulate your lymph node, because the purpose of the vaccine is to have an effect on the immune system. So I always want to make sure—is there something really obvious that could explain these lymph nodes being abnormal? A recent tattoo, recent injury, or infection proximal to where that enlarged lymph node is? So, you have to think of all of that when you're thinking of the differential diagnoses that come to mind." TS 19:19 "Nurses can be monumentally helpful with the education role with any patients who may have lymphoma. It would be helpful if nurses guide patients on the diagnostic process, providing support, guidance and encouragement, and of course, advocating for patients. Also, when you think of Hodgkin lymphoma and you think of the younger patients that are diagnosed with it, there's some additional supportive care that we need to consider, and we may need to refer them for fertility preservation. That can be very daunting for our young patients, so it's helpful to have nursing support in those situations." TS 22:55 "People think of Hodgkin lymphoma as a very treatable disease—and it can be very curable for certain patient populations—but it's not a 100% success rate. We do still have fatalities, so it's a very serious thing. I try to always educate my patients about seeking out care if they notice new lumps or bumps on their bodies. We can observe enlarged lymph nodes most easily on the neck, under the armpit, and in the groin area. If someone were to notice a lump in that area, short-term observation—keeping an eye on it for two weeks is fine—but if it persists past that, they should definitely seek care to get it evaluated." An unused door keeps opportunities standing outside. That blue throw pillow in the closet, the candlestick in the pantry — rearrange, repurpose, and shift how energy flows for free. (14:40) — Clutter, gently Clutter carries emotion — fear, holding on, "I might need this." Be kind to yourself, but make sure everything in your home is worthy of being there. Donate rather than landfill: do you really need nine coats, or would eight and a warm neighbor be better? (16:08) — One thing, not the whole house Skip the overwhelming weekend purge. Choose one thing that's blocking you — often that memory-laden box in the corner. Even forgotten clutter "pings" your subconscious like a phone notification. Balance them by purpose: bedrooms and reading rooms want watery, earthy calm; kitchens and workout spaces want fire and wood energy. (27:12) — Home office: routine, command, and focus Create a dedicated space — even a nook — and a physical open/close routine (blinds, lamp, chair) that trains your body into and out of work mode. Sit in the command position. And audit your field of view: cute trinkets directly behind your monitor are adorable distractions; move them to a side wall. (31:01) — The skeptic's challenge: six weeks Try the changes for six weeks — you can undo everything after. Lisa's clients call before the deadline: sleeping better, loving their home, finally watching movies together. Journal your emotions at the start and re-evaluate at the end. (32:44) — One thing to do this weekend: love on your center Using the Bagua map (nine energy centers laid over your floor plan), the centermost area connects to all the others. Find your home's center as a bird would see it, and make it beautiful: fresh flowers, a houseplant, swept floors, working bulbs, a scented candle. Lifting the center lifts every space. This is an ongoing excavation.The Sun in Virgo sextiles Mars in Cancer — Earth and water together, an opportunity for the guardian energy, the protective energy, the slow-and-meaningful energy to align with the Sun's call to purposeful, humble, stewardship-oriented action.And then: the Moon occults Venus.An occultation is like an eclipse — the Moon passes directly across the face of a planet, hiding it from view. A Note from James:Carlo Rovelli is one of the smartest people I know. He is a theoretical physicist and the author of a lot of great books, including Seven Brief Lessons on Physics, The Order of Time, Helgoland, White Holes, and now On the Equality of All Things: Lessons on Physics and Philosophy.He has been on the show before, and at the end of this conversation I told him I felt like he was just tolerating my questions. He was nice about it.But the first question I asked was: Do I actually exist?Because in Carlo's new book, nothing really exists on its own. Not a person, not a particle, not you, not me. Everything exists in relation to something else.So I spent an hour trying to understand what that means.Does absolute truth exist? Is free will fake? Are we just machines? Is consciousness quantum? Is everything doomed to decay? Is time real? Was there a before the Big Bang? And what does any of this actually do for us in ordinary life?Carlo's answer, again and again, is not that nothing is real. It is that we make a mistake when we try to imagine the world from the outside, as if we were standing somewhere beyond the universe looking in. We are not outside the universe. We are inside it. We are part of it. We know the world through relationships, perspectives, interactions, and limits.And oddly, that is calming.Because Carlo says the thing that brought him serenity was not finally knowing everything. It was giving up the need to know everything with absolute certainty.That is where this conversation goes: physics, quantum mechanics, philosophy, Buddhism, Taoism, Wittgenstein, consciousness, black holes, time, the Big Bang, free will, and the strange comfort of not needing the final answer.Episode Description:Carlo Rovelli returns to the show for a conversation about reality, uncertainty, quantum mechanics, time, consciousness, free will, and his new book On the Equality of All Things: Lessons on Physics and Philosophy.James begins with the central idea of the book: everything in the universe can only be understood in relation to something else. Carlo agrees that this is a good short version of the argument, but immediately pushes back against the need for “absolute” truth. Of course James exists. Of course Carlo exists. The question is why we need to add something heavier and more final than that.From there, the conversation becomes an exploration of relational reality. Carlo explains that motion is not an illusion just because it depends on perspective. When James says he is sitting still, that is true with respect to Earth. When someone says Earth is moving, that is true with respect to the sun or the galaxy. The mistake is asking which motion is the “real” one outside all relations.The same frame applies to quantum mechanics. Carlo argues that particles are not little objects with fixed properties sitting there independently of the rest of the world. A particle is what it manifests in interaction. It is a set of relations. This is why quantum mechanics has forced such a profound change in the way physicists think about reality.James keeps pressing the practical question: What do we gain from knowing this?Carlo's answer is partly scientific and partly philosophical. We gain better questions. We stop asking impossible questions like “Who is really not moving?” or “What is the final foundation of everything?” We become more aware of our own perspective. And we may become more capable of understanding people whose moral, political, or philosophical perspectives differ from ours.The conversation moves through the Big Bang and the possibility of a Big Bounce, with Carlo stressing that science is powerful precisely because it knows its own ignorance. Science is not the final story. It is an ongoing process of learning, correcting, testing, and replacing older models with better ones.James and Carlo then talk about time. Carlo explains that Earth is so small, compared with the universe, that we experience conversation as almost instantaneous. That gives us the illusion that time is the same for everyone. But physics has shown that time is not universal. Every star and galaxy has its own time, and the idea of “now” across the whole universe does not really work.The second half of the conversation turns toward philosophy and human life. Carlo connects his thinking to Eastern philosophy, Zhuangzi, Buddhism, Taoism, and Wittgenstein. For him, accepting the partial, perspectival, incomplete nature of knowledge is not depressing. It is serene.James also asks about consciousness, free will, and whether quantum mechanics can explain the mind. Carlo respects Roger Penrose but says he does not think consciousness is specifically quantum. The brain is physical. Choices are physical. But that does not make choice unreal. The mistake, Carlo says, is imagining a separate ghost or soul outside the physical brain that is supposed to make the decision.The episode closes with James trying to find one absolute: everything decays. Carlo says no. The universe also creates order. Galaxies form. Stars light up. Life emerges. Water and oil separate into structure. Civilization produces books. Nature is not simple decay. Jack Janasiewicz, lead portfolio strategist at Natixis Investment Managers, says that the market's ability to keep running depends on the "up and to the right" trend continuing, and he expects that to continue even if the slope of the line gets flatter. There's a difference, Janasiewicz says, between a slowing economy and a slow one, noting that slowing conditions can still be a good backdrop for corporate earnings — "which is really all that matters here" — rather than a slow market, which opens the door to recession. For that, his outlook is for a slower economy without a recession, even if long-term Treasury yields creep higher, as he believes the economy can withstand higher rates and higher oil prices so long as those conditions don't become extreme. Nicholas Epley, author, "A Little More Social: How Small Choices Create Unexpected Happiness, Health, and Connection" — who says he studies "mind-reading ... how we make inferences about each other's thoughts, beliefs and attitudes and mostly how we screw that up and misunderstand each other in lots of ways" — discusses America's loneliness/isolation epidemic and how people should choose to engage more often because those interactions are good for physical and mental health. He talks about a recipe for making your world friendlier, especially in times when that feels so difficult. In the Market Call, Mike Dickson, head of research and quantitative strategies at Horizon Investments, discusses the mix of art and science he puts into evaluating stocks, using valuation metrics, earnings quality, profit margins, balance-sheet health and more for determining the best relative companies across each industry group.  He notes that the market's trends on beating earnings estimates remains a very strong broad-based trend, going beyond artificial-intelligence and tech companies to the smaller sectors.
