What if the biggest revenue leak in your practice isn't a denial or a payer contract problem - it's the person processing your payments? In this episode, Dr. Heather Signorelli breaks down the four-step payment posting audit we run on every practice we onboard - and why practices doing $300K+ a month are routinely losing $8,000 to $25,000 of it to undetected posting errors. You'll learn: • How to catch unapplied patient payments before they generate angry calls • The ERA spot check that exposes systemic contractual adjustment errors • How to find payer underpayments before your billing team writes them off • The write-off audit that protects your revenue from unauthorized adjustments This is the final episode in our four-part payment posting series. If you've been following along, you now have more visibility into your revenue cycle than most practice owners ever get.

More management and insurance knowledge in my newsletter: https://www.odysseymgmt.com/newsletter Most dental patients think their only option is to pay out of pocket or rely on limited dental insurance, but what if thousands of dollars in treatment could actually be covered through medical billing instead? Although I don't teach it, I keep up with its impact on dentistry. For many offices, medical billing bridges the gap between what patients need and what they can afford. I'm joined by Danielle Brown and Dental Revenue Network member Sandy Buechler. They explain how dental conditions tied to medical issues like autoimmune disorders, diabetes, cancer treatments, and trauma can qualify for medical coverage when properly documented. The concept of medical necessity is key to coverage. They go over how telling the patient's full health story matters more than just listing procedures. Also discussed is how medical credentialing and pre-authorizations are different from what we see in dentistry. Beyond the technical side, Sandy and Danielle focus strongly on patient advocacy and empowerment, especially in large and involved cases like full mouth reconstruction and implants. My takeaway is that we are operating in a revenue cycle world where dental billing incorporates medical care. Are you ready for it? There's a number in your practice that looks precise but lies to you every single month: your AR report. We routinely audit multi-provider practices showing $400K, $600K, even $1M in AR — and 20–40% of that “asset” is already dead. Not collectible. Just trash left behind by bad payment posting. In this episode — the first in a 4-part series on payment posting — Heather walks through what payment posting actually is, why getting it wrong silently inflates your AR, and the seven specific things that change in your practice when posting is done right. Inside the episode: Why your AR report is a mirror of your team's posting accuracy, not what you're actually owed How a $400K/month practice dropped their AR by 30% in two weeks — without collecting a dollar more The 7 reasons clean payment posting transforms your revenue cycle The audit moment we found 200+ accounts a week being reworked that were already paid in full What's coming in Episode 176: the most common insurance-side posting mistake we see in almost every audit If you're a practice owner, billing lead, or operations director who has ever made a financial decision off an AR report — this is the foundation. "What if you train them and they leave?" It's the fear that quietly keeps most healthcare leaders from investing in their people. Matt Staub — CEO of Your Health — wants you to sit with the question his mentor once asked in return: What if you don't train them, and they stay? In this episode, Matt joins Jamie Preston for a conversation about why workforce education isn't a perk at Your Health — it's the culture. From nationally accredited apprenticeships, to a training pipeline built out of a licensing crisis, to the real people behind the success stories, this is a blueprint for leaders who want to grow something that lasts. Key topics covered: The lumberjack story: why sharpening your axe beats swinging harder every time How a shortage of licensed administrators became the catalyst for Your Health's training engine The shift from "education happens on your own time" to "this is how we behave" Real success stories — Olivia, Kristin, Taylor, McKinsey, Rebecca — and what they share Matt's three challenges for anyone ready to grow: show up, find your who, take your shot If you've ever wondered whether developing your people is worth the cost, this episode will change the math. Press play — then look around, and ask yourself who's looking at you.

Got questions? Send Ericka a Text!Your medical insurance may be able to cover wisdom teeth extractions, and the reason most people never find out is painfully simple: nobody asks the right way. We sit down with Jen from Hygiene Unlocked and unpack a real, recent moment where a medical insurance rep insisted “that's dental” until Jen pushed for an actual code check. Once the CPT and diagnosis pairing was reviewed, the answer changed, and so did the entire cost picture. We get practical about dental medical billing: how to think in medical necessity instead of “dental vs medical”, how ICD-10 diagnosis codes tell the story that justifies treatment, and why cross-coding matters when an oral surgery plan only lists dental codes. We also talk about the everyday realities of calling payers, including how reps default to in-network benefits, how to confirm out-of-network coverage, and why you should always get a call reference number so you can appeal bad information later. The takeaway is bigger than wisdom teeth. When we treat the mouth like it's separate from the body, patients lose benefits they pay for and offices miss legitimate reimbursement. If you want a clearer, more defensible process for wisdom teeth extraction billing, CPT coding, ICD-10 documentation, and CMS-1500 claims, this conversation will change how you look at your next treatment plan. Subscribe, share this with a biller or office manager, and leave a review with your biggest medical billing question. America spends nearly double what the fourth-ranked country spends on healthcare per capita — and still ranks among the worst in outcomes. So what exactly are we paying for? In this episode of the Experiencing Healthcare Podcast, Jamie Preston and Your Health CEO Matt Staub examine what happens when healthcare gets treated like gasoline: something people expect to be available, can't easily compare on quality, and ultimately choose based on price or convenience. When brand and price stop mattering, the only differentiator left is how patients are made to feel — and whether they trust the person across from them enough to actually change. What you'll hear in this episode: Why Matt ranks service above outcomes and access — and the patient story that changed how he thinks about both The "Chick-fil-A problem": how your healthcare experience is now being compared to your best service experience anywhere, not just the clinic down the street What provider burnout really looks like when a clinician closes their notes at 11pm wondering if their patient listened How insurance billing creates distrust that bleeds directly into the patient-provider relationship — and what healthcare organizations can do about it Why the most caring thing a doctor can do sometimes feels like the worst customer service in the room If you've ever felt like a number in a waiting room — or if you've ever been the one trying to help someone who wouldn't listen — this conversation will stay with you. Press play.

Taylor Bowker - Mindful Insurance Agency On Common Mistakes in Medical Billing: "She ended up getting a bill for $14,000 for a pee test." Health insurance in the United States is a pain. We all know that. As business owners, we often get stuck needing to shop for our own insurance. Only during certain times of year and only with all of these rules. But how do you buy health insurance, without going crazy? There is a marketplace, but it isn't exactly like going to buy grapes. It helps to have an expert on your side, as with most things. In this case, the expert offers their expertise at no additional cost to you. Taylor Bowker is the health insurance expert. She started Mindful Insurance Agency to help people navigate the murky waters of health insurance. As an entrepreneur, a business owner with multi-state employees, or someone simply trying to make sense of the marketplace, this episode breaks down the trends, challenges, and solutions in health insurance. Listen as Taylor offers tips on choosing the right coverage, using HSAs, and understanding group versus individual enrollment periods. Enjoy! You could owe, you know, $12,000, $13,000, $14,000, $15,000 back at tax time if you go even a dollar over. So that's kind of where the issue lies. James [00:00:35]: You have found Authentic Business Adventures, the business program that brings you the struggle stories and triumphant successes of business owners across the land. Downloadable audio episodes can be found in the podcast link found at drawincustomers.com. We are locally underwritten by the Bank of Sun Prairie, and today we are welcoming slash preparing to learn from Taylor Belker of Mindful Insurance Agency. So Taylor, How is it going today? Taylor Bowker [00:00:59]: Hello. Good. Staying warm or attempting to. James [00:01:02]: Wisconsin, here we are. So tell me a story. What is Mindful Insurance Agency? Taylor Bowker [00:01:07]: Oh, sure. So at Mindful Insurance Agency, we assist individuals and businesses get health insurance, health, dental, and vision. And we are a brokerage. So we are essentially the middle person between the members and the health insurance carriers and just try to advise and educate and help people get health insurance. James [00:01:30]: Right on. How do you end up in health insurance? Taylor Bowker [00:01:33]: Well, um, so I started in the insurance industry back in 2012. I was 5 years old. I'm just kidding, I was a little bit older than that. Um, yes, yes, yes. Um, so I started very entry-level job at an agency in Waunakee, around town here. Um, again, just doing very entry-level things. And then I shortly got promoted to a business development role. Um, and then from there, there there was a different agency hiring that was focusing on health insurance, and I was ready for a change. Taylor Bowker [00:02:11]: They needed a customer service rep, front desk person, so I went there to work, and within a year of me working there, they actually asked me if I wanted to become an agent or a broker, and I had never thought that that was something I wanted to do. Insurance, it can be a very just like cutthroat kind of a situation with sales and quotas and things like that. And that always kind of made me a little nervous. But I am very much a people person. So I, you know, I thought, why not? Let's give it a try. So I got licensed and became an agent back in 2016 now. And then, so started as an agent there doing health, dental, and vision, like I said. And then back in 2019, I decided to start my own business. James [00:03:05]: So that's awesome. Taylor Bowker [00:03:05]: Yeah, I've been doing that for 6 years now. James [00:03:08]: So what was the motivator to start your own gig versus just maintaining with where you were at? Sure. Taylor Bowker [00:03:12]: Yeah. Well, I guess I've always kind of just been somebody who works just, I prefer to kind of be on my own. Being my own boss sounded really great. I was in a group of individuals at the time and other business owners, the group that you're in with me now where we met. And again, there were just a lot of business owners there and they were doing a really good job of owning a business. And it just sounded like a feasible option for me and something that I wanted to do. I also felt like I kind of had learned everything I really could being at the place I was at previously. So I figured, you know, why not give it a shot and rip the Band-Aid off? And it's been, yeah, it's been working out well so far. Taylor Bowker [00:04:00]: So that's good. James [00:04:01]: Right on. Taylor Bowker [00:04:02]: Yeah. James [00:04:02]: So has health insurance changed much over the past, what are we talking, 9, 10 years? Taylor Bowker [00:04:08]: Sure. So I would say yes, yes and no. So I think the biggest thing that's changed is rates, insurance rates. James [00:04:19]: They keep going down. Taylor Bowker [00:04:20]: Yeah, that would be amazing. No, similar to a lot of other things, they just keep increasing and by a lot, especially the last couple years. So a lot of other products have come into play to try to help mitigate those premium costs for people. There's a lot of other products out there now that aren't necessarily health insurance as it is defined by, you know, the commissioner of insurance, if you will, or the government. But there are other products out there that are designed similarly to help people, you know, still get the care that they need and save money on their premiums and their out-of-pocket costs. So I would say prices have changed and other products have been developed in the market to try to help with those pieces. James [00:05:08]: And other products, you mean, I guess, help me what you mean by other understand products. Taylor Bowker [00:05:11]: Sure, yeah. So obviously you have typical health insurance. This would be either through, it's all kind of under the Obamacare umbrella as we've known it for a while now. It can be a group plan through an employer that you work with, or it can be an individual plan on the marketplace or direct with an insurance carrier. And that would kind of be your more typical, just average health insurance policy. But as of the last, say, maybe decade, a couple of new products have come out, one of them being referred to as medical cost sharing. So medical cost sharing in layman's terms is essentially a product that you pay a monthly subscription fee for, similar to a premium, and you choose what's called an initial and shareable amount, similar to a deductible, where essentially you're saying, I'm willing to pay this cost upfront in the event that something catastrophic were to happen to me. And so because it's covering you for more catastrophic occurrences and not the full shebang like a regular health insurance policy would,, they tend to be less expensive for certain people in certain situations. Taylor Bowker [00:06:24]: Those types of products also work really well for lucky individuals that are more healthy, that don't necessarily use their benefits all the time, but just want something there in case. That also works well with what's called direct primary care, which is a newer model, at least around the Madison area. It's where you work with a primary care doctor directly. So they're not affiliated with any any hospital systems. You do pay them a small monthly subscription fee to utilize their services. However, it's, it's really very worth it because a direct primary care doctor is always going to make sure they have same-day or next-day appointments available for their patients, which is very unheard of nowadays. Yeah, unfortunately. And then they can also do a myriad of things for you, you know, because you're paying that monthly fee. Taylor Bowker [00:07:18]: They can run labs for a couple of dollars, which is also very crazy to say. They can run prescriptions for people at cost. If you have an appointment with them, it'll be either a 30-minute or an hour-long time block, which again is not very common if you go to a clinic through a hospital system. They can help you with most acute and even urgent care needs as well. So that can just be, yeah, those two things specifically go hand in hand pretty well together. There's also things out there that have been out there for a while, like short-term policies, you know, through UnitedHealthcare or Allstate or something like that. Those can be less expensive for folks,

Terry kicks off 2026 by clearing up a major misunderstanding in the provider and manufacturer community. Some believed that CMS's last‑minute withdrawal of the LCD for skin substitute products would delay or stop the 2026 reimbursement changes. That's not the case. The LCD withdrawal has no impact on the Final Rule, and the new 2026 reimbursement methodology for skin substitutes will move forward exactly as finalized, using an incident‑to payment structure. Auditing split/shared encounters can become confusing when providers, auditors, and coders are not aligned. CPT and CMS have both issued guidance to help clarify how these services should be billed. In this episode of the CodeCast podcast, Terry breaks down where to begin when auditing and educating on Split/Shared visits, what payers are currently saying, and how to maintain compliance for physicians and NPPs to prevent payer audits or confidently address them when they occur. She also gives a shout out to Sonda Kunzi, CPC, for her NAMAS article referenced during the discussion. When auditing risk of management in an E/M note, how are over-the-counter (OTC) medications scored? Under the 1995/1997 guidelines, they were categorized in the “low” risk row. However, the 2021 guidelines provide no examples under minimal or low risk, relying instead on AMA and Medicare guidance. Terry explains this distinction and highlights the difference between an acute uncomplicated illness and an acute illness with systemic symptoms. Take a listen. Medical record signatures are more than the macro “electronically signed by Dr. Jack Jones.” A provider's signature is a legal attestation that the physician or provider performed, reviewed, and/or agreed with the documentation. Is this actually true, or are your EMR auto-signatures taking over? Terry discusses this critical aspect of medical record documentation compliance, with a shout-out to NAMAS for an article that also addresses the topic. Plus, a bonus update on the G0136 add-on code for 2026. Last month, the "Billing Boys"—Chris Jones and Phil Rodgers—joined the GeriPal podcast to demystify medical billing and coding in palliative care. This month, we're back with part two, shifting the focus to geriatrics. While billing and coding may not be the most exciting topic, they're essential for ensuring fair reimbursement for the complex care we provide and for supporting the work of our interprofessional teams, many of whom can't bill directly for their services. When we underbill or leave money on the table, we not only shortchange ourselves but also devalue the critical role of geriatrics in the healthcare system. This time, we're joined by experts Peter Hollmann, Ken Koncilja, and Audrey Chun to dive into key questions: Why does billing matter, and who does it benefit? What's the difference between CPT, E&M, and ICD-10 codes (if you need a refresher, check out our chat with the Billing Boys here)? We explore how to think about billing for complexity versus time, and unpack new and impactful codes like the Cognitive Assessment and Care Plan Services code (99483), advance care planning (ACP) billing codes, and G2211, which acknowledges the added work of managing patients with chronic conditions. We also highlight the new APCM G-codes for 2025, a set of HCPCS codes that could provide substantial financial support for interdisciplinary teams in geriatrics. Finally, we discuss the advocacy behind these codes. The American Geriatrics Society (AGS) plays a vital role on the AMA's RUC committee, helping to improve reimbursement for the complex care of older adults. Tune in to this week's GeriPal podcast for expert advice, practical strategies, and insights that will help you optimize your billing practices and sustain the future of geriatrics! Here are some of the resources we also talked about: The physician fee schedule look up tool Wwere you can find out CMS expected charge based off where you practice AGS's annual coding update Geriatrics at Your Fingertips, which has a one-pager on billing Medicare Claims Processing Manual
