Strategies for Leading an Academic Health System

Steering an academic medical center through a global health crisis demands remarkable humility, a relentless commitment to partnership, and true collaboration. However, what it perhaps doesn’t demand is a one-size-fits-all, traditional healthcare background. On that note, when Alan Kaplan, MD, first began his role as Chief Executive Officer at UW Health in Wisconsin five years ago, this was his first foray into academic healthcare – his prior academic experience limited to his experience as a student attending school. Alan, who’d been a senior healthcare executive for 25 years, learned he was actually the only candidate for the job without an academic background and was chosen for his know-how as a physician leader who understood how to run a business. To share his journey, Alan’s here on our podcast in a conversation hosted by Oliver Wyman partner, Deirdre Baggot, PhD, RN, to discuss physician leadership, provider-sponsored health plans, the importance of having a living strategic plan, what defines an effective partnership strategy, and why diversity of perspective and background among C-suite healthcare leaders serves as a competitive advantage.

Relentless Health Value
EP299: FFS (Fee for Service) Is a Whole Business Model—It’s Not Just a Way to Get Paid, With Alan Kaplan, MD, MBA, Assistant Professor of Urology at Georgetown University and a Practicing Urologist

Relentless Health Value

Play Episode Listen Later Nov 5, 2020 31:39

If you are a forward-thinking specialist right now, alarm bells may be going off, given COVID and/or the prospect of another COVID-style pandemic. Also, all of the capitated and advanced PCP (primary care provider) practices popping up. Also, virtual care models. FFS is a cushy status quo revenue model until it isn’t. One underappreciated point might be that FFS is not only a revenue/payment model. It’s also a business model. And as a business model, FFS very much drives how practices structure themselves to realize that FFS revenue. Consider that to earn a fee for a service, someone (a human person) has to physically do the service. So, all FFS-style businesses have an inherent incentive to add labor and not use technology in any way that actually reduces the amount of billable human hours involved in providing care to patients. But if that top-line revenue line goes down—wow!—you’ll find yourself as many did with way too many employees. An FFS business model has zero flexibility when it comes to revenue that isn’t consistently going up or, at a minimum, a flat line. If revenue plummets and payroll is big—big so as to power a way higher revenue number than is possible for whatever reason—you have a major financial problem on the quick. That is what I talk about in this health care podcast with Alan Kaplan, MD, MBA. Dr. Kaplan is assistant professor of urology at Georgetown University, and he is a practicing urologist. He recently cowrote a paper with Dan O’Neill in the publication NEJM Catalyst Innovations in Care Delivery. The article discusses COVID-19 and health care’s “productivity shock,” as they call it. Dan O’Neill, by the way, was on the show. Also, he was on EP287 and part of EP292. But in the article that Dr. Kaplan and Dan O’Neill wrote, they give some advice to specialists and hospitals who are looking to evolve with the changing marketplace. Spoiler alert: Conceptually, it’s a shock to move from a place where, every year, you can count on your billings going up and up and move to a model instead that assumes that this is not the case. So, yeah, there’s a little talk for sure about the joys and challenges of transitioning to value or a value-based payment model. But that’s only the very first consideration. It’s also about reconsidering the operating model and the strategic use of digital technologies. We talk about all of the above in this health care podcast. Quick sidebar: My interview with Dr. Steve Schutzer (EP294) might be a good follow-on for a very actionable work plan for specialists to implement some of the advice that Dr. Kaplan gives in this podcast.   You can learn more by contacting Dr. Kaplan via LinkedIn. Alan L. Kaplan, MD, MBA, is a practicing surgeon, innovator, and health services researcher. After finishing his urology residency, a health care administration fellowship, and an MBA, all at UCLA, Alan helped build a multispecialty medical group in a highly underserved area of South Los Angeles. Alan is currently an assistant professor of urology at Georgetown University; an attending physician at the Washington, DC, VA Medical Center; and a physician advisor at IDEO, a human-centered design firm. Alan’s work over the past 10 years has centered on value-based care redesign, aiming to transition to a more just, equitable, and sustainable health care system for all Americans. 03:51 Who are we actually discussing when we use the term specialist? 05:58 How does the PCP taking on more risk affect the specialists’ path to value-based care (VBC)? 09:42 “Technology leads to … a reduction in labor burden … but in health care, that really hasn’t been the case.” 11:36 “Technology … in health care … has never really been about making the bottom line more efficient. It’s been about expanding the top line.” 13:39 What do specialists need to be considering if they want to stay relevant in the next 5 years? 14:27 EP292 with Brian Klepper, PhD. 16:53 Is there a future where specialists can transition from FFS to VBC while skipping the messy middle of a transition? 18:37 “The way we always did things is not the way that we have to always do things in the future.” 25:20 “When all is said and done, the relationship between [PCPs] and the specialists that they refer … those relationships are really, really important.” 26:14 EP219 with Arshad Rahim, MD, MBA, FACP. 28:13 What’s going to be a big driver for providers to become more independent in the next 5 to 10 years? ‘Tis the season for Shakespeare al fresco so pack a picnic, grab a blanket and check out these North Bay productions: Marin Shakespeare closes out its season under the stars with Pericles, a play whose authorship by Shakespeare has fostered many a debate. Plot points include incest, assassination, famine, a shipwreck, marriage, maternal death, familial separation, attempted murder, kidnapping, pirates, prostitution, and a seemingly dead person rising from a watery grave. Who knew Shakespeare wrote a zombie play? And this is a comedy. Director Lesley Currier and her design team have taken all these elements, dressed them up in modern garb, added a few topical references, and come up with the theatrical equivalent of a “B” movie. It’s entertaining and even moving at the end, but it evaporates quickly in the night air. Artist-in-residence Dameion Brown brings his commanding stage presence to the title role. Fine supporting work is done by Cathleen Ridley as the loving Queen Simonedes and the treacherous Dionyza; Eliza Boivin as Marina, Pericles’s daughter; Rod Gnapp and Richard Pallaziol in a variety of roles; and Diane Wasnak, who is very engaging as the puckish storyteller Gower. 'Pericles' runs Thursday–Sunday through August 5 at Forest Meadows Amphitheater at Dominican University in San Rafael. The showtimes vary and the venue opens one hour before curtain for picnicking. For more information, go to marinshakespeare.org Santa Rosa’s Shakespeare in the Cannery ceases to exist after this season’s production as the property is being “repurposed.” Co-founder/director David Lear decided to go out on a lighter note so they’re presenting Shakespeare in Love, the stage adaptation of 1998’s Best Picture Oscar winner. Poor Will Shakespeare (John Browning) has writer’s block and can’t seem to finish his latest opus, Romeo and Ethel, the Pirate’s Daughter. A muse arrives in the person of Viola (Sydney McNulty), who disguises herself as ‘Thomas Kent’ so as to get around the ‘no women on stage’ rule. Shifty theatre producers, a loathsome Lord, a treacherous boy and a haughty queen all come into play before Romeo and Juliet sees the light of day. It’s a piffle but the cast has fun, with good comedic support from Alan Kaplan and Liz Jahren. Isiah Carter impresses in two roles and keep an eye out for Isabella, one of the moodiest, scene-stealing “bitch” characters I’ve seen on a North Bay Stage. 'Shakespeare in Love' runs Friday through Sunday through August 5 at the Cannery in Railroad Square in Santa Rosa. Show time is at 7pm with the gate opening at 5pm for picnicking. Today on The Neil Haley Show, The Total Tutor Neil Haley will interview Donny Most of Happy Days. Donny Most, a mature vocalist who loves Bobby Darin, Sinatra and others of the era, is a swinging crooner. He puts plenty of personality into the lyrics of the standards he sings. His voice is friendly and pleasing and he clearly conveys the love that he feels for these vintage songs. On Mostly Swinging, Most is joined by a big band filled with all-stars from the L.A. studio scene. The joyful arrangements of Willie Murillo are so spirited that they border on the riotous at times with the emphasis on faster tempos and extroverted ensembles. It helps that he has a killer trumpet section led by Wayne Bergeron plus plenty of notables including trombonists Andy Martin and Alan Kaplan and saxophonists Rusty Higgins and Brian Scanlon. Such songs as “Lover Come Back to Me,” a Latinized “Let's Fall In Love,” “After You've Gone” and “Day In Day Out” are given rousing treatment by Danny Most and the ensembles. His tribute to Bobby Darin on “Clementine” is a definite highlight.

"Macbeth" - July 13, 2016

“By the pricking of my thumbs, something wicked this way comes.” One of Shakespeare’s scariest lines comes from what is easily his scariest play, ‘Macbeth,’ a show so frightening most theater people refuse to speak the title aloud. In the third season of Shakespeare in the Cannery—in the old cannery ruins near Railroad Square—director David Lear unleashes an offbeat, seriously gonzo take on the Bard’s bloody masterpiece, blending modern jeans, combat boots, and bike leather with traditional elements like capes, cackling witches, blood drenched swords … and a head in a bloody bag—which never gets old. Overall, Lear’s vision is impressive. He springs masks, trapdoors, and a whole toolbox of other theatrical tricks, including the incorporation of drummers from Sonoma County Taiko, pounding out a live percussive score throughout the show. Shakespeare’s eerie supernatural tragedy tells the twisty-gory tale of the highly suggestible Scottish warrior Macbeth, played by Ben Stowe, who emphasizes the reluctance and fear beneath his character’s fighting façade. At Macbeth’s side, sort of, is his blindly ambitious wife, whom Shakespeare never gave a name to. She is always only ‘Lady Macbeth,’ played here with cruel coolness by Ilana Niernberger. Mr. and Mrs. M are goaded into traitorous action by the riddle-spouting Weird Sisters—you know, the ‘bubble, bubble, toil and trouble twins— who tell Macbeth he will become king. Anyone else might sit around and wait for the crown to fall in their laps, but Lady M. wants that crown, and Duncan, the king, just invited himself over for dinner so . . . why waste an opportunity? Screwing their courage to the sticking place—a line that here takes on a seriously sexy second meaning—the married first-time murderers argue a bit before finally offing Duncan, quickly shifting blame to his sons, and then taking his throne as king and queen. That would be a terrible end to the story. But fortunately for the audience, Macbeth—not to mention Shakespeare—just can’t seem to stop killing people, and the whole kill-the-king-and-then-retire-from-evil-deeds scheme soon collapses under of storm of mystery, madness and mayhem. On a pleasingly playground-like stage, the performances sometimes stray toward the big, and over-acted. That’s not necessarily a bad choice for an outdoor show. But in this case, what’s gained in terms of clarity and size is often lost in terms of subtlety and absorbing emotion. That’s a problem enhanced by a number of freaky visual ideas that—while interesting and certainly not boring—often distract from the drama rather than heighten it. Still, there are some very strong moments in this ‘Macbeth,’ fueled by several effectively surprising acting choices. As Duncan, the doomed King, Clark Miller very convincingly plays the monarch’s essential sweetness. Eric Thompson, as a servant in Macbeth’s castle, is hilarious, making the most of the play’s one comedic scene. Sam Coughlin, as Duncan’s vengeful son, is impressively complex in a very small part. And as Macbeth’s fellow warrior Banquo, Alan Kaplan brings a sense of affronted decency and a solid soldiers’ bearing to a role usually played by a much younger man. Though perhaps lacking in actual human drama and emotion, playing more like a Halloween pageant than an actual tragedy, this particular ‘Macbeth’ is probably an excellent production to introduce kids to Shakespeare with. Energetic and ambitious—like poor befuddled Macbeth himself—the show may sometimes stumble … but it definitely brings the sound and fury. ‘’Macbeth’ runs Friday & Saturday through July 23 at The Cannery. "Twelfth Night" - July 15, 2015

"Twelfth Night" is amongst Shakespeare’s most popular plays, in part because the story is so accessible, and the situations so universally funny. Typical of the Bard, humor and tragedy are never far from each other. A young woman, Viola, grieves for her twin brother Sebastian after surviving a shipwreck in which she believes he drowned. Cast ashore in a strange country, she disguises herself as a man, donning an outfit matching that of her brother, who soon shows up - surprise! - wearing the same thing. They are now identical. Viola, having taken the name Cesario, finds employment with the local Duke, whom she instantly falls for. The Duke’s in love with the wealthy Olivia, who’s mourning her own brother’s recent death, and swears she will never love any man, but changes her mind the first time she takes a look at Cesario, who of course is actually Viola. Wacky subplots involve of Olivia’s drunken cousin Toby Belch and the clueless Sir Aguecheek, Olivia’s servants Maria and the self-righteous Malvolio, and a freelance jester-singer-trouble-maker named Feste - all of whose paths begin to cross Cesario’s in increasingly complicated, potentially life-threatening ways. Meanwhile, believing his sister to have drowned, Sebatian can’t understand why everyone in this strange land seems to know him, some trying to kill him and others trying to take him to bed. It’s great stuff, all served up with Shakespeare’s patented sense of poetry and escalating crisis. What’s tricky about a play so well known and well loved is presenting it freshly. How do you bring something new to a show so thoroughly mined and milked that virtually everything findable in it has been found? That’s the task set before director David Lear and the Shakespeare in the Cannery crew, the relatively new company that last weekend opened its sophomore season in the old cannery ruins in Railroad Square. With a uniformly talented cast, and a strong vision by Lear, this production manages to both hit the notes we love to hear in a staging of "Twelfth Night," and somehow find a few more I, for one, have yet to have heard. Without altering the text, Lear has found a way to balance the play’s extremes, making the comedy funnier by attending to the details of the drama. Certainly, Lear - known for his innovations and sometimes unconventional approaches - does make a few changes, the most obvious being Feste the Jester, who here has been split into TWO, played as twins or clones or BFFs by Haley Bartels and Brandon Wilson, both delightful. The other obvious splashes of invention are largely stylistic, from the tennis shoes worn by most of the cast to the black lipstick and corsets worn by several of the supporting ensemble. As Viola and Sebastian, Kot Takahashi and Carmen Mitchell are not just wonderful at making you care about their plights, they look a lot more like each other than in most productions of "Twelfth Night" I’ve seen. I don’t have time to mention everyone by name, but special mention must go to April Krautner’s Olivia, arcing from gloom to puppy love with immense charm, Clark Miller and Brian Abbott as Toby and Aguecheek - managing to be silly and kind of sad at once - and the magnificent Alan Kaplan in the play’s trickiest role, that of Malvolio, who as the subject of one of literature’s most famous practical jokes, must transform from ridiculous to near-tragic, without altering the tone of the shenanigans all around him. Congratulations to Lear and his entire team. By finding the new in something so old, this is a Twelfth Night to celebrate and savor. Twelfth Night runs Fridays and Saturdays at 7pm through August 15.
