Topical estrogen is everywhere right now — but if you have melasma, rosacea, or acne, should you be worried? In this episode, Dr. Mary Alice Mina cuts through the fear-based noise and breaks down what the science actually says about topical estrogen and skin conditions like melasma and rosacea. She shares her own personal experience with melasma, explains why estrogen alone isn't enough to trigger it, and walks through what the current research — including a systematic review in Dermatologic Surgery — actually shows. She also covers who might want to proceed with caution, how to do a proper test spot, why sun protection is the real game-changer, and why rosacea patients have even less to worry about than they think. Today, I have the honor of interviewing Dr. Doris Day, a board-certified dermatologist who specializes in cosmetic and longevity dermatology. She is also a clinical professor of dermatology at NYU, recognized for her dedication and excellence in teaching, and is widely regarded as a leader in the aesthetic dermatology field. In addition to her clinical work, Dr. Day is a medical journalist and a prominent media figure. In our discussion, we examine the basics of perimenopause and menopause from a pathophysiology perspective, including the 30% decline in collagen synthesis that occurs during the first five years of menopause. We explore the skin microbiome, inflammation, and inflammaging, and why hydration, electrolytes, reducing processed sugars, and eating a nutrient-dense diet can have such a visible impact on skin health. Dr. Day also explains her approach to facial balancing and how our bones, fascia, muscles, and fat pads age. We discuss topical agents, advanced interventions including lasers, microneedling, PRF, PRP, stem cells, and growth factors, and even touch on emerging research suggesting that certain laser treatments may help reduce the risk of skin cancer. Throughout the conversation, I also openly shared what I have done to support my skin. Stay tuned for today's fascinating conversation about menopause, skin health, collagen loss, and the evolving science behind modern aesthetic dermatology. IN THIS EPISODE, YOU WILL LEARN: The sudden skin changes many women experience during early perimenopause Dr. Day explains how she personalizes skincare routines for each patient How collagen synthesis rapidly declines during the menopause transition How hydration, sleep, and nutrition can visibly improve skin tone and puffiness How inflammation, blood sugar issues, and poor metabolic health contribute to inflammaging and visible skin aging Why facial balance is more important than trying to erase every wrinkle or line Dr. Day shares how she combines lasers, topicals, and recovery protocols to support skin healing and collagen production How fillers and other cosmetic procedures can create an unnatural appearance when used incorrectly Bio: Dr. Doris Day, MD, is a board-certified dermatologist specializing in cosmetic and longevity dermatology. She is a Clinical Professor of Dermatology at NYU Langone Health, where she has been recognized with the Award for Dedication and Excellence in the Teaching of Dermatology. Her leadership in aesthetic dermatology has earned her the AAD Presidential Citation and the American Skin Association Award for leadership in dermatology. Ranked #3 in the United States and #1 in New York by Newsweek as a Top Cosmetic Dermatologist, Dr. Day is a recognized authority in her field. Her work has been cited in leading publications, including The New York Times and Castle Connolly's NY Magazine Top Doctors listings, for over 20 consecutive years. Dr. Day is also a medical journalist and a prominent media figure, providing expert commentary on major television programs and podcasts. She also hosts a popular dermatology show on SiriusXM's Doctor Radio, a platform where she has been educating the public for over 15 years, earning the AAD's Golden Triangle Award for Media Excellence. Her professional affiliations include the American Society for Dermatologic Surgery, where she has served on the board of directors; the American Academy of Dermatology, where she was recently elected to the nominating committee; the New York State Dermatology Society; and the American Honors Society of Dental and Facial Aesthetics. Check out my top picks for skin care here. Download the Free eBook 'Skincare Myths Busted' What do you do when the face in the mirror doesn't match how you feel inside? Welcome to the skin shift that happens in midlife — a direct result of hormonal changes, specifically the drop in estrogen that comes with perimenopause and menopause. And while it's common, it's not talked about enough. Until now. In this episode, I sit down with Dr. Pooja Rambhia, a board-certified dermatologist who's leading the conversation around what's been coined the “menopause makeover.” This is about understanding what's happening in your skin and body during menopause, and how to support both with science, compassion, and the right strategies. From collagen loss to skin laxity to the real reason your products aren't working anymore, we cover it all. We also talk about realistic solutions, from injectables and resurfacing lasers to hormone therapy and everyday skincare that actually supports your skin at this stage of life. This episode is not about “fixing” your face. It's about finally getting answers to the questions so many women are asking and doing so with a sense of clarity. Key Takeaways: Skin loses up to 30% of its collagen in the first five years after menopause. Estrogen is important for your skin's hydration, elasticity, thickness, and glow. It's normal to experience acne, dryness, sagging, and texture changes but that doesn't mean you have to just accept it. Bio-stimulatory injectables, topical actives, laser treatments, and even hormone replacement therapy can all play a role when used wisely and intentionally. Aging is not a decline. It's a transition that deserves support, not shame. Pooja H. Rambhia, MD, is a board-certified dermatologist with specialized fellowship training in cosmetic dermatology and laser surgery. She practices at UnionDerm in New York City and Greenwich, CT, providing comprehensive dermatologic care and specializing in laser treatments, injectables, and facial and body contouring for all skin types. Dr. Rambhia completed her medical training at Case Western Reserve University and dermatology residency at Zucker School of Medicine/Northwell Health, where she served as Chief Resident. She then completed the prestigious American Society for Dermatologic Surgery Fellowship at UnionDerm with Dr. Anne Chapas. Committed to advancing the field of dermatology, Dr. Rambhia has published numerous peer-reviewed articles in leading journals and presented her research at major national conferences including the American Academy of Dermatology and the American Society for Dermatologic Surgery. Her expertise has been featured in major media outlets including Vogue, Allure, Marie Claire, InStyle, Elle and The New York Times.
