Episode 025 | Dr. Teo Soleymani, a Los Angeles native, one of my Stanford residency mates, and founder of California Dermatology and Mohs Surgery Specialists (CDMS), completed his undergraduate studies in Evolutionary Biology at UCLA. Notably, Dr. Zitelli, was trained directly by Dr. Frederic E. Mohs, the founder and pioneer of Mohs micrographic surgery. This direct lineage from Dr. Mohs enables Dr. Soleymani to give his patients the highest-quality Mohs surgery experience. Dr. Soleymani's unique fellowship training was the only program in the world that included fellowship training in both Mohs Micrographic Surgery and Facial Plastic and Reconstructive Surgery as well. As a result he is one of only a handful of Mohs Surgeons in the world with dedicated specialized training in Facial Plastic and Reconstructive Surgery as well, allowing him to provide the best possible functional and aesthetic reconstructive results. This week we are honored to welcome pediatrician Dr. Daniel Ganjian to The Hamilton Review Podcast! Today, I am blessed to have here with me Dr. Steven Gundry. He is a cum laude graduate of Yale University with special honors in Human Biological and Social Evolution. [00:50] Dr. Gundry Gives His Definition of Ketosis Generating ketone bodies is done by free fatty acids entering the liver from circulation. The liver can convert free fatty acids into ketone bodies. Ketone bodies are short-chain fats that are water-soluble. Ketones have a distinct benefit in getting to the brain, which free fatty acids can't because they're too big to get through the blood-brain barrier. Ketosis should happen every night after about eight hours of not eating if you are metabolically flexible. 50% of ordinary people are not metabolically flexible. [03:50] Uncoupling To Survive: How The Mitochondria Release Pressure Dr. Gundry recommends reading Uncoupling To Survive: https://pubmed.ncbi.nlm.nih.gov/11053672/ When you're starving to death, mitochondria have to save themselves at all costs. If mitochondria die, then it's all over. Making energy is hard work; it's very damaging to mitochondria. Mitochondria have to have ways of protecting themselves, and one of the ways they protect themselves is to release pressure. There are five pressure release valves for the mitochondria. We couple oxygen and protons to make ATP. If we uncouple the burning of oxygen from making ATP, that's how we release pressure. [21:05] The Reason, Most People, Cannot Get Into Ketosis Right Away Most people can not get into ketosis by following a high-fat diet because they lack metabolic flexibility. Many people are insulin resistant. Their cells no longer listen to insulin, so insulin keeps rising and rising. It can take three to four weeks to liberate fat from fat cells on a ketogenic diet because insulin levels stay high for so long. The ketogenic diet came about to treat childhood epilepsy. If you put kids on an MCT oil-based diet, with only about 50% of their calories coming from fat, they would get the same benefit as the full-blown ketogenic diet. [32:25] The “Mito Club” - How Your Body Is Just Like A Nightclub The mito club has one entrance, and there is only one exit. If everything works out okay, a proton and electron will couple and exit the mito club. However, there are a lot more electrons than there are protons. Electrons will also couple with oxygen; it's a bad match. The mito club needs to be cleaned up every night after closing. [38:40] Calorie Restriction Is A Non-Starter, and It Doesn't Work The best way to expand lifespan and healthspan is calorie restriction. However, calorie restriction is a non-starter, and it doesn't work. Calorie-restricted animals are profoundly hungry. A study found that the animals with time-restricted eating lived 11% longer than the animals who ate the same amount of food but ate throughout the day. This concept has been proven in humans as well. [47:05] The Major Health Benefit of Taking MCT Oil or Powder 30% of sheep and goat dairy is MCT. MCTs are an excellent fat that goes directly to our liver, where they are automatically converted into ketones. You can have insulin resistance, take MCTs, and generate ketones. A tablespoon of MCT oil will have you generating adequate amounts of ketones to begin uncoupling mitochondria. MCTs are easy to take; you can mix them in salad dressing. Many females have issues with MCT oils and their stomachs, so start slow. Powered MCTs work well for women. [53:35] You Have Melatonin All Wrong: It May Have Nothing To Do With Sleep Pistachios actually have the highest melatonin content of any food. The Mediterranean diet gets melatonin from olive oil, red wine, and mushrooms. Melatonin isn't there to put you to sleep. Instead, it's there to repair mitochondria. Also, melatonin may be one of the secrets of cancer therapy. Dr. Gundry has cancer patients on up to 100 milligrams a day of melatonin. Dr. Paviol is from Pennsylvania and obtained his undergraduate degree at Franklin and Marshall College in Lancaster, Pennsylvania with a Bachelor of Arts in Neuroscience. While at F&M, he was a John Marshall Scholar, graduated Magna Cum Laude, and was inducted into Phi Beta Kappa. He was accepted by the Penn State College of Medicine following his sophomore year. In addition, he was one of 30 medical students selected for the nationally recognized Clinical Science Training Program at the National Institutes of Health in Bethesda, Maryland, where he spent a year performing individualized research on Type 1 Diabetes. He also spent a month in Ecuador as part of a program bringing medical care to underserved areas. Dr. Paviol did his internship at Scripps Hospital in San Diego, California and then completed his dermatology residency at University of Michigan, which is ranked as a Top 5 Dermatology program in the country. While at the University of Michigan, Dr. Paviol received specialized training in all types of skin cancers, including melanoma and more rare skin cancers such as merkel cell carcinoma and cutaneous T-cell lymphoma. Charles J. Yeo is a hepatopancreaticobiliary surgeon here at Jefferson. He was born in East Orange, New Jersey, and attended Spring Valley Senior High School in Spring Valley, New York. He received his undergraduate degree from Princeton University in 1975, summa cum laude with an A.B. in Biochemistry. Dr. Yeo graduated in 1979 from the Johns Hopkins University School of Medicine, being awarded the Upjohn Achievement Award and he was elected to Alpha Omega Alpha and Phi Beta Kappa. While completing his internship and residency in General Surgery at the Johns Hopkins Hospital, he pursued a one year Research Fellowship at the S.U.N.Y. Downstate Medical Center in Brooklyn, N.Y. Dr. Yeo joined the faculty of the Johns Hopkins University as an Instructor and Assistant Chief of Service in the Department of Surgery in 1985, and rose to the rank of Professor of Surgery in 1996. In 1997, he became a Professor in the Department of Oncology. Dr. Yeo directed the Pancreatic Cancer Interdisciplinary Working Group at Johns Hopkins, and served as the Surgical Clerkship Coordinator and Surgical Curriculum Consultant. In 2002, Dr. Yeo was named to an endowed chair at Johns Hopkins, becoming the inaugural John L. Cameron M.D. Professor for Alimentary Tract Diseases. On October 1, 2005 Dr. Yeo was named the 8th Samuel D. Gross Professor and he assumed the chairmanship of the Department of Surgery at Jefferson (now Sidney Kimmel) Medical College of Thomas Jefferson University in Philadelphia, Pennsylvania. He currently serves on the Board of Trustees of the Thomas Jefferson University Hospitals, and as the Senior Vice President and Chair of Enterprise Surgery for Jefferson Health (an 18 hospital system). Dr. Yeo has authored over 500 peer reviewed scientific papers, 105 book chapters, and over 15 books or monographs. Dr. Yeo is also known as the “Whipple King” having completed over 1700 Whipple procedures and treated 2100 patients with pancreatic and related cancers. The importance of estradiol in women is crucial for reproductive, brain, and bone health, as well as cardio protection. Decline in estradiol during menopause can lead to various diseases of aging. There is controversy around replacing estradiol in women, but the literature supports its necessity. Christy Huff, MD, is a cardiologist and director of Benzodiazepine Information Coalition, a nonprofit that educates about the adverse effects of prescribed benzodiazepines. Dr. Huff attended medical school at the University of Texas Southwestern where she graduated Alpha Omega Alpha. She completed an internal medicine residency at Washington University in St. Louis and a cardiology fellowship at U.T. Southwestern, with a focus in advanced cardiovascular imaging and noninvasive cardiology. Dr. Huff is a Fellow of the American College of Cardiology, and was a private practice cardiologist in Fort Worth,Texas until 2011, when she became a stay-at-home mom after the birth of her daughter. Dr. Huff speaks and writes frequently about her lived experience with benzodiazepines. In 2015, she experienced adverse effects after three weeks of prescribed Xanax for insomnia. Over a three-year period, she slowly tapered off benzodiazepines utilizing Valium, and suffered protracted and disabling symptoms. Her personal experience led her to realize the serious risks of these medications and the potential severity of benzodiazepine withdrawal syndrome, neither of which were emphasized during her medical training. Dr. Huff specifically advocates for better education of physicians regarding the adverse effects of benzodiazepines and how to safely taper patients off these medications. Her research interests include patient-centered benzodiazepine deprescribing and benzodiazepine-induced neurological dysfunction (BIND). In addition to her work at BIC, Dr. Huff is a member of the Colorado Consortium for Prescription Drug Abuse's Benzodiazepine Action Work Group.

After receiving a bachelor's degree from Baylor University, Dr. McCullough completed his medical degree as an Alpha Omega Alpha graduate from the University of Texas Southwestern Medical School. Jennifer received her undergraduate degree from Washington College in Chestertown, Maryland, where she graduated Summa Cum Laude. Today, I am blessed to have here with me Dr. Steven Gundry. He is a cum laude graduate of Yale University with special honors in Human Biological and Social Evolution. After graduating Alpha Omega Alpha from the Medical College of Georgia School of Medicine, Dr. Steven Gundry completed residencies in General Surgery and Thoracic Surgery at the University of Michigan and served as a Clinical Associate at the National Institutes of Health for years. [00:50] Dr. Gundry Gives His Definition of Ketosis Generating ketone bodies is done by free fatty acids entering the liver from circulation. The liver can convert free fatty acids into ketone bodies. Ketone bodies are short-chain fats that are water-soluble. Ketones have a distinct benefit in getting to the brain, which free fatty acids can't because they're too big to get through the blood-brain barrier. Ketosis should happen every night after about eight hours of not eating if you are metabolically flexible. 50% of ordinary people are not metabolically flexible. [03:50] Uncoupling To Survive: How The Mitochondria Release Pressure Dr. Gundry recommends reading Uncoupling To Survive: https://pubmed.ncbi.nlm.nih.gov/11053672/ When you're starving to death, mitochondria have to save themselves at all costs. If mitochondria die, then it's all over. Making energy is hard work; it's very damaging to mitochondria. Mitochondria have to have ways of protecting themselves, and one of the ways they protect themselves is to release pressure. There are five pressure release valves for the mitochondria. We couple oxygen and protons to make ATP. If we uncouple the burning of oxygen from making ATP, that's how we release pressure. [21:05] The Reason, Most People, Cannot Get Into Ketosis Right Away Most people can not get into ketosis by following a high-fat diet because they lack metabolic flexibility. Many people are insulin resistant. Their cells no longer listen to insulin, so insulin keeps rising and rising. It can take three to four weeks to liberate fat from fat cells on a ketogenic diet because insulin levels stay high for so long. The ketogenic diet came about to treat childhood epilepsy. If you put kids on an MCT oil-based diet, with only about 50% of their calories coming from fat, they would get the same benefit as the full-blown ketogenic diet. [32:25] The “Mito Club” - How Your Body Is Just Like A Nightclub The mito club has one entrance, and there is only one exit. If everything works out okay, a proton and electron will couple and exit the mito club. However, there are a lot more electrons than there are protons. Electrons will also couple with oxygen; it's a bad match. The mito club needs to be cleaned up every night after closing. [38:40] Calorie Restriction Is A Non-Starter, and It Doesn't Work The best way to expand lifespan and healthspan is calorie restriction. However, calorie restriction is a non-starter, and it doesn't work. Calorie-restricted animals are profoundly hungry. A study found that the animals with time-restricted eating lived 11% longer than the animals who ate the same amount of food but ate throughout the day. This concept has been proven in humans as well. [47:05] The Major Health Benefit of Taking MCT Oil or Powder 30% of sheep and goat dairy is MCT. MCTs are an excellent fat that goes directly to our liver, where they are automatically converted into ketones. You can have insulin resistance, take MCTs, and generate ketones. A tablespoon of MCT oil will have you generating adequate amounts of ketones to begin uncoupling mitochondria. MCTs are easy to take; you can mix them in salad dressing. Many females have issues with MCT oils and their stomachs, so start slow. Powered MCTs work well for women. [53:35] You Have Melatonin All Wrong: It May Have Nothing To Do With Sleep Pistachios actually have the highest melatonin content of any food. The Mediterranean diet gets melatonin from olive oil, red wine, and mushrooms. Melatonin isn't there to put you to sleep. Instead, it's there to repair mitochondria. Also, melatonin may be one of the secrets of cancer therapy. Dr. Gundry has cancer patients on up to 100 milligrams a day of melatonin.
