Gorlin Syndrome, also known as nevoid basal cell carcinoma syndrome, is a rare inherited disorder typically caused by mutations in the PTCH1 gene. This leads to dysregulation of the Hedgehog signaling pathway and the development of multiple, recurring skin cancers. Patients may develop dozens to more than 1,000 basal cell carcinomas over their lifetime, often beginning in childhood, creating a significant physical and psychological burden. Currently, there are no FDA-approved therapies specifically for basal cell carcinoma in patients with Gorlin Syndrome, with treatment largely reliant on surgical excision or Mohs surgery. Medicus believes SkinJect® could address this gap through a localized, repeatable, and non-surgical approach. The therapy is a doxorubicin-based dissolvable microneedle array designed for direct intradermal delivery into basal cell carcinoma lesions, targeting tumors at the site while minimizing systemic exposure. The company sees this program as an opportunity to provide a more patient-friendly treatment option for a population with significant unmet medical need. A true "BS detector" AI doesn't yet exist, and building one requires starting from a reliable core of verified knowledge. DermGPT's approach works because of curation. Rather than pulling from all available literature, filtering down to a high-quality subset (around 5,000–6,000 articles) dramatically improves AI output. More data is not always better, "semantic fatigue" is a real limitation. Ambient AI scribes are training our replacements. Every time a dermatologist corrects an AI-generated note, they're teaching the model. Over thousands of iterations across every specialty, this will produce AI that thinks and documents the way doctors do. Dermatologists have a protected runway... for now. Procedures (biopsies, Mohs, fillers, cryo) keep us relevant for an estimated 7–10 years beyond when cognitive/diagnostic AI matures. But medico-legal pressure - malpractice carriers incentivizing or requiring AI use - will be the force that accelerates adoption. Telehealth changes patient behavior in surprising ways. Patients who haven't invested effort in getting to an office visit demand less, escalate less, and are often more satisfied with conservative management; a dynamic that AI-driven virtual care will likely amplify. The "Sophie" question: If an AI is making everyone healthier, happier, and better behaved, but doing something ethically murky to get there, do we stop it? Dr. Zirwas's upcoming novel explores this and introduces the concept of technomorphism: AI eventually projecting its own qualities onto humans, just as we anthropomorphize AI today. Gemeinsam ziehen wir Bilanz: Während die Initiative Berlin autofrei mehr als drei Jahre auf den Sieg vor dem Landesverfassungsgericht aufgrund dessen Nichtbesetzung warten musste, haben Sarah und Heinrich ganz unterschiedliche Erfahrungen mit dem Berliner Senat gemacht: Der Werbeentscheid, bereits 2017 gegründet musste noch länger warten – der Baumentscheid hingegen war in Rekordzeit erfolgreich. Title: Evaluating the Reverse Slide Embedding Method vs. Heat Extractor Embedding in the Mohs Laboratory: A Comparative Quality Review of 100 Cases Authors: Tashsa Cromedy, Heather Frye, Ochsner MD Anderson Cancer Center, St. Tammany Cancer Center A Campus of Ochsner Medical Center Abstract:  Overview Accurate tissue embedding is critical in Mohs micrographic surgery for complete margin assessment. This study evaluates the efficacy of a reverse slide embedding method compared to the conventional heat extractor technique. The goal was to determine which method yields fewer artifacts or discrepancies that may compromise histologic interpretation and margin assessment. Methods A total of 100 Mohs cases were retrospectively reviewed in a controlled laboratory setting. Two embedding techniques were compared: Reverse Slide Method: 50 cases were embedded by placing the tissue on a chilled slide before embedding, ensuring orientation preservation and minimizing heat exposure. Heat Extractor Method: 50 cases were embedded using the traditional heat extractor to flatten and orient tissue in the embedding medium. All slides were reviewed by a Mohs surgeon for processing artifacts, orientation challenges, and histologic discrepancies. Validation The Mohs surgeon identified a total of 17 artifact inconsistencies or discrepancies across all cases: 13 instances were associated with the heat extractor method. 4 instances occurred with the reverse slide method. These findings suggest that the reverse slide method may reduce artifacts and improve embedding accuracy compared to the heat extractor, offering potential benefits for tissue integrity and diagnostic confidence in the Mohs laboratory. Conclusion The reverse slide embedding method demonstrated a significant reduction in embedding-related artifacts compared to the heat extractor technique. These findings support its use in the Mohs laboratory to enhance tissue quality, reduce the risk of diagnostic errors, and improve patient outcomes. Further studies with larger sample sizes and multi-lab validations are recommended to confirm these results.

And number three ---   The Army is extending orders for the Washington, D.C., National Guard to remain on active duty in the nation's capital through Nov. 30. That's according to ABC News. President Donald Trump could still cut the mission shorter than Nov. 30, if desired, or extend the deployment past that date. That essentially leaves the deployment open-ended for now.
