In 2013 he was elected to serve a 4 year term on the American Urologic Society Examination Committee. Dr. Khera has also served on the AUA Peyronie's Disease and Erectile Dysfunction Guidelines Panel. For the past several years he has taught numerous course in testosterone therapy and sexual dysfunction nationally and throughout the world. Dr. Khera freely shares his time and knowledge with the general public. We first learned of Dr. Aaron Spitz from The Game Changers film. In his book, aptly named The Penis Book, Dr. Spitz states that the most common reason for erectile dysfunction is a problem with blood flow to the penis due to narrowing of the arteries. In my conversation with Dr. Spitz, we discuss the causes and what you can do to prevent and even reverse erectile dysfunction by improving penis health. In this episode we discuss erectile dysfunction in young men. Did you know that 1 in 4 men under the age of 40 experience erectile dysfunction. Are the erectile problems faced by these men different from the typical erectile dysfunction experienced by middle aged and older men? Is erectile dysfunction in young men caused by different underlying problems? Do these men need a different evaluation? Should they be offered different treatment options? In order to answer these questions, we turned to Dr. Nannan Thirumavalavan. Dr. Thirumavalavan is Chief of Male Reproductive and Sexual Health at University Hospitals as well as Assistant Professor of Urology at Case Western Reserve University School of Medicine. Dr. Thirumavalavan earned his medical degree at Boston University School of Medicine and subsequently completed his residency in urology at Boston Medical Center. He then completed a National Institutes of Health-sponsored fellowship in Male Reproductive Medicine and Surgery at Baylor College of Medicine in Houston. Dr. Thirumavalavan has earned numerous awards in his career, including the AUA Men's Health Traveling Fellowship in 2014, the NIH Travel award in 2018 for the American Society of Andrology, and the Society for Male Reproduction and Urology Traveling Scholarship in 2018. Even though men contribute to more than 50% of instances of infertility, there is not the same culture around male fertility testing as there is around female fertility testing. Here to discuss this and many other topics around male reproductive health and fertility is the esteemed Dr. Natan Bar-Chama, Director of the Center of Male Reproductive Health at RMA of New York. He is a board-certified urologist and male infertility specialist and his practice is dedicated to the fields of male infertility and sexual medicine. He has published extensively and lectured throughout the world on these topics. Since 2005, Dr. Bar-Chama has been listed in New York Magazine’s Best Doctor list of urologists. This week, we are talking with Dr. Mohit Khera, M.D., M.B.A., M.P.H, Professor in the Scott Department of Urology at Baylor College of Medicine. Dr. Khera dedicates his clinical and research efforts to three main areas: testosterone, erectile dysfunction, and Peyronie’s disease. Dr. Khera earned his undergraduate degree at Vanderbilt University while subsequently earning his Masters Degree in Business Administration and his Masters Degree in Public Health from Boston University. While receiving his Medical Degree from The University of Texas Medical School at San Antonio, Dr. Khera also completed his Urology residency training in the Scott Department of Urology at Baylor College of Medicine. After finishing a six-year residency in Urology, he completed a one-year fellowship in Male Reproductive Medicine and Surgery. Currently a Professor in the Scott Department of Urology at Baylor College of Medicine, he specializes in male and female sexual dysfunction and hormone replacement therapy. Dr. Khera also serves as the Director of the Laboratory for Andrology Research, the Medical Director of the Baylor Executive Health Program, and the Medical Director of the Scott Department of Urology. Dr. Khera was appointed the F. Brantley Scott Chair in Urology at Baylor College of Medicine in 2019. Soon after completing the fellowship, he started the Laboratory for Andrology Research. His laboratory focuses on basic science research on testosterone and its effects on the prostate, stem cells to treat erectile dysfunction, the effects of finasteride on sexual function, and genetic markers of hypogonadism. In his laboratory, he has had the opportunity to train many residents and research fellows. In addition to his basic science research, he has initiated numerous FDA approved clinical trials. His basic science and clinic experiences have allowed him to thus far give over 200 lectures at scientific meetings, publish over 100 articles in peer-reviewed journals, complete 15 book chapters, and edit and write two books all in the field of sexual medicine. In 2007 he was awarded the American Urologic Association (AUA) Research Scholars Award to study the correlation between ED and BPH. In 2013 he was elected to serve a 4-year term on the American Urologic Society Examination Committee. Dr. Khera has also served on the AUA Peyronie’s Disease and Erectile Dysfunction Guidelines Panel. For the past several years, he has taught numerous courses in testosterone therapy and sexual dysfunction nationally and throughout the world. Finally, Dr. Khera currently serves as the Secretary of the Sexual Medicine Society of North America. Dr. Khera freely shares his time and knowledge with the general public. One of the biggest crises in health right now is the low levels of sperm count and fertility in young men, and the question of what the clinical community can do about it. Why are we experiencing such a decline in the reproductive potential of men who should be in their prime? Does testosterone therapy need to be at super-therapeutic levels all the time? How can men on testosterone therapy still maintain their fertility? On this episode, I’m joined by the Director of Male Reproductive Medicine and Surgery at the University of Miami, Dr. Ranjith Ramasamy, who talks about how to improve the reproductive health of men. Four Takeaways The reasons we’re experiencing low sperm count in young men are obesity, environmental influences, stress, and social media. Short-acting testosterone is better for fertility and reproductive potential because it mimics the body’s natural pulsativity. The reason testosterone therapy affects fertility and blocks metagenesis is because if it’s used over a long term, it blocks FSH and LH. The restorative protocol for people who took supra-physiological doses of testosterone for long periods of time is HCG and clomiphene to jumpstart testosterone and sperm production. At the start of the show, Dr. Ramasamy shared how he got started, and what’s causing the crisis in male fertility right now. Next, we talked about why the biggest battle in the clinical optimization space is misinformation, and why it’s equally important to educate consumers as much doctors. We also talked about why faster-acting testosterone is better for improving fertility, and why the insurance-attached prescribing community is so afraid of testosterone. Dr. Ramasamy completed his undergraduate degree at Rutgers University in 2003; completed his medical degree at Rutgers University Robert Wood Johnson Medical School in 2007; completed a urology residency at New York Presbyterian Hospital – Weill Cornell Medical College in 2013; then completed a fellowship, sponsored by the NIH, in Male Reproductive Medicine and Microsurgery at Baylor College of Medicine in 2015.
