Restless leg syndrome is a common movement and sleep disorder affecting 2-3% of the population. Certain neurologic disorders are associated with a higher incidence, and neuropsychiatric medications prescribed by neurologists and psychiatrists often exacerbate these symptoms. In January, the American Academy of Sleep Medicine published updated guidelines for the evaluation and treatment of RLS. The recommendations shifted from recommending the use of dopamine agonists, to prioritizing iron evaluation and supplementation, alpha-2-delta ligands (gabapentin, gabapentin enacarbil, pregabalin), and recommending against the long-term use of dopamine agonists. The guidelines also recommend opiates for moderate-severe medication refractory RLS. Dr. Winkleman is a Professor of Psychiatry at Harvard Medical School and chief of the Sleep Disorders Clinical Research Program in the Department of Psychiatry at Massachusetts General Hospital. He is the first-author on the new guidelines, and is here to discuss these changes and what is needed to effectively change practice. My guest today on the Online for Authors podcast is Robert J Wolf, author of the book Not a Real Enemy. Robert is a neuroradiologist who attended the University of Michigan Medical School and completed his training at Brown and Yale Universities. He grew up in a suburb of Detroit as the only child of Ervin and Judit Wolf. Their stories of their escape from communist Hungary, and his father's tragic history of escaping the Nazis twice but losing his own parents, inspired Robert to document his parents' tales and share those stories with Jewish groups, history aficionados, and biography lovers throughout the United States, Europe, and elsewhere. In Not a Real Enemy Robert shares his family saga—and the forgotten history of the nearly half million Hungarian Jews who were deported and killed during the Holocaust—through an epic and inspiring tale of daring escapes, terrifying oppression, tragedy, and triumph. In my book review, I stated 'Not A Real Enemy: The True Story of a Hungarian Jewish Man's Fight for Freedom is an amazing family Holocaust memoir that reads more like a novel. I was brought into the story through the main character, Ervin Wolf, the father of the author - and I turned the pages as quickly as I could until I reached the end. Ervin begins life as a doctor's son and ends life as a doctor in the United States. Everything in between shows the reader the brutalities of the Nazi regime and then the Soviet communists who came after. The story left me wondering if I would have survived? If I would have had the gumption to escape not once, not twice, but three times? It showed me that freedom is something we should value beyond almost everything else. History wants to bury the ugliness that was the Holocaust. Thankfully, memoirs like this and the bravery of authors like Robert make that impossible. Well done. What exactly is a “learning health system”? In several of our episodes, you've heard us talk about how data collection has modernized through new technologies and enhanced approaches to clinical trials. But now that we have all that data, we need to transform it into clinical practice. Learning health systems are all about completing this virtuous cycle from scientific discovery to implementation, and yet, there are few that exist and work well. In this episode, you'll hear us discuss (and perhaps debate) what a true learning health system looks like, as well as how informatics can help lead the charge. Up first on our panel of distinguished guests is Dr. Genevieve Melton-Meaux, a Professor of Surgery and Senior Associate Dean of Health Informatics and Data Science at the University of Minnesota. Among her many accolades, she is a Past President of the American College of Medical Informatics, current President of the American Medical Informatics Association, and Director for the Center of Learning Health System Sciences at Minnesota. Her research focuses on clinical natural language processing, surgical informatics, and optimizing AI best practices. We were also joined by Dr. Chuck Friedman, a Professor of Medical Education and Chair of the Department of Learning Health Sciences at the University of Michigan Medical School. He is also the editor-in-chief of the open-access journal Learning Health Systems. Drawing from his time at the Office of the National Coordinator for Health Information Technology in the U.S. Department for Health and Human Services, he helped transform Michigan's medical education department into one of the first in the nation to focus on learning health at all levels, including large-scale information infrastructure. Finally, we had Dr. Peter Embí, Professor and Chair of the Department of Biomedical Informatics and Senior Vice President for Research and Innovation at Vanderbilt University Medical Center. Prior, he served as President and CEO of the Regenstrief Institute, a not-for-profit health care research organization in Indianapolis, Indiana that seeks to develop, conduct, and disseminate scientific research across communities. A Past President of the American Medical Informatics Association, his research centers on data-driven learning health systems. Freedom was not given freely but had to be fought for and defended.” ~ Dr. Ben Carson Biography | Dr. Ben Carson In 1973, Ben Carson graduated from Yale University. He went on to receive his M.D. from the University of Michigan Medical School. At the age of 33, Dr. Carson became the chief of pediatric neurosurgery at Johns Hopkins Hospital, making him the youngest major division director in the hospital's history. This would be among an extensive list of outstanding firsts for Dr. Carson. Dr. Carson's accomplishments have earned him a place in medical history. He performed the first and only successful separation of craniopagus (Siamese) twins joined at the back of the head in 1987. He also performed the first fully successful separation of type-2 vertical craniopagus twins in 1997 in South Africa. In his career, Dr. Carson became a professor of neurosurgery, oncology, plastic surgery, and pediatrics at the Johns Hopkins School of Medicine, and directed pediatric neurosurgery at the Johns Hopkins Children's Center for over 29 years. In 2016, Dr. Carson accepted the position of Secretary of Housing and Urban Development under the Trump administration. After completing his tenure as the 17th Secretary of the Department of Housing and Urban Development in 2021, Dr. Carson founded the American Cornerstone Institute. This organization is focused on fighting for the principles that have guided him through life and that make this country great: faith, liberty, community and life. Dr. Carson and his wife, Candy Carson, co-founded the Carson Scholars Fund, which recognizes young people of all backgrounds for exceptional academic and humanitarian accomplishments. The organization has awarded more than 11,000 scholarships and paid out more than $8.9 million in scholarships. The Carson Scholars Fund is currently operating in 50 states and the District of Columbia, and since its founding, has installed more than 270 Ben Carson Reading Rooms around the country. Some parents may describe their child as very strong-willed. That child may show an extreme avoidance of everyday demands, expectations, and requests. But what if the behaviors are not an indication of strong will, but rather occur because a child feels overwhelmed by demands and perceives them as threats to their autonomy and self-control? Are there specific strategies that would be helpful when raising a demand-avoidant child or teen? Today, we're going to be talking about Pathological Demand Avoidance with Dr. Casey Ehrlich. She is the CEO and founder of At Peace Parents where she specializes in teaching parents and therapists practical skills in the home or clinical setting to accommodate neuroception-driven demand avoidance and nervous system differences through creative techniques. Show Notes: [3:10] - Pathological Demand Avoidance (PDA) is considered a nervous system disability. Dr. Ehrlich explains why this is and shares alternative names for it. [5:41] - Many parents and clinicians have found that the strategies they have tried have not worked or made things worse. [7:39] - Dr. Ehrlich describes a recent situation with her son that demonstrates sudden nervous system activation in PDA. [9:33] - We need to understand the root cause of behaviors that are interfering with your ability to connect with your child. [10:57] - PDA may show up as a regression or can surround a primary need. [12:58] - PDA is often misunderstood. [15:04] - With a background as a social scientist, Dr. Ehrlich explains some of the research about PDA and why many children and teens are not correctly identified. [17:45] - Accommodations include autonomy, equality, trauma-informed play, nervous system signals of safety, communication, lowering demands, humor, novelty, and more. [21:38] - PDA is happening on a subconscious level. It isn't about emotions. [23:37] - It can be confusing as a child gets older because they do begin using social strategies and we may not see escalation as nervous system dysregulation. [27:46] - Pausing to de-escalate is crucial. [29:24] - Is PDA only seen in people with autism? [30:42] - Connections can only be made when PDAers feel a sense of safety. [33:21] - Awareness is the first step. [35:55] - Dr. Ehrlich learned more about PDA as a mother than a clinician. Audrey Wells, MD attended the University of Michigan Medical School and went on to train at Washington University School of Medicine in pediatrics and pediatric pulmonary medicine. Dr. Ben Carson joins Kathy sitting in for Dawn! For a time, the likelihood of Benjamin S. Carson, Sr., M.D. fulfilling his childhood dream of becoming a physician seemed unlikely. Growing up in a single parent home with dire poverty, poor grades, a horrible temper, and low self-esteem appeared to preclude the realization of that dream until his mother, with only a third-grade education, challenged her sons to strive for excellence. She observed successful people and encouraged her sons to emulate their behaviors, including reading. This led to behavior changes which had a profound effect on their education. In 1973, Ben Carson graduated from Yale University. He went on to receive his M.D. from the University of Michigan Medical School. At the age of 33, Dr. Carson became the chief of pediatric neurosurgery at Johns Hopkins Hospital, making him the youngest major division director in the hospital's history. This would be among an extensive list of outstanding firsts for Dr. Carson. Dr. Carson's accomplishments have earned him a place in medical history. He performed the first and only successful separation of craniopagus (Siamese) twins joined at the back of the head in 1987. He also performed the first fully successful separation of type-2 vertical craniopagus twins in 1997 in South Africa. In his career, Dr. Carson became a professor of neurosurgery, oncology, plastic surgery, and pediatrics at the Johns Hopkins School of Medicine, and directed pediatric neurosurgery at the Johns Hopkins Children's Center for over 29 years. He became the inaugural recipient of a professorship dedicated in his name in May 2008. He is now the Emeritus Benjamin S. Carson, Sr., M.D. and Dr. Evelyn Spiro, R.N. Professor of Pediatric Neurosurgery, having retired on June 30, 2013.

Jennifer Crumbley becomes the first parent in the United States to be found guilty of involuntary manslaughter for a mass school shooting committed by her son, Ethan. She was convicted of four counts of involuntary manslaughter and faces a maxium sentence of 60 years in prison. Prosecutors said Jennifer Crumbley knew her son was struggling with mental health issues. They alleged instead of getting him the help he needed, Jennifer and her husband, James Crumbley, bought Ethan the gun he later used in the attack. The prosecution also showed the Crumbleys were attempting to flee when they realized they were going to be charged in the crime. James Crumbley faces the same charges as his wife, on March 5. School shooter Ethan Crumbley opened fire in a Michigan school, killing four students. He pled guilty to all charges and is now serving life in prison. His parents, James and Jennifer Crumpley, are also charged with involuntary manslaughter. Prosecutors say they should have been aware of their son's mental health and should not have made it possible for him to have access to weapons. The parents' trials are expected to begin later this month, but now Jennifer Crumbley has asked a judge to dismiss three witnesses who say their testimony could prejudice a jury. A judge has ruled, however, that two of the witnesses will be allowed to testify, with a narrow focus. The jury will also be allowed to see surveillance video from the school. A college secret is said to be the motive behind the murder of an Ohio Mom. Brenda Powell was bludgeoned with a frying pan and then stabbed more than 30 times. Daughter Sydney Powell, 19 at the time, was charged with murder. Prosecutors claimed Sydney Powell did not tell her parents that she was suspended from Mount Union University months earlier for poor academics. Brenda Powell was on the phone with college officials when her daughter struck. Defense experts argued that Sydney Powell suffered a psychotic break when she killed her mother. Three experts found she was schizophrenic and could not discern right from wrong. Experts for the prosecution asserted Powell had mental health issues, but was not insane when she committed the crime. What was supposed to be a fun-filled weekend with his son, turned tragedic as a New York father is killed at his hotel. Paul Kutz, 53, died after a man opens fire in the hotel lobby, firing as many as thirty shots. Two men with reported gang ties were arrested for the shooting, Roy Johnson Jr. and Devin Taylor. Johnson was the shooter and was ultimatelly sentenced with the maximum life sentence, 58-years-to-life in state prison. 24-year-old Rita Curran is found violently murdered in her Burlington apartment. The second grade teacher had recently moved out of her parents' home to share an apartment with three other roommates. Curran was working over the summer break as a maid while taking graduate courses. Curran turns down a dinner invitation from her roommates, planning to spent the night in. When the roommates return home, they hang out in the living room for a time. When they turn in for the night, that's when Curran's badly beaten body is discovered.
