Today, we have the pleasure of speaking with Dr. Marah Hehemann, an assistant professor of urology and the Associate Program Director of Andrology at the University of Washington in Seattle. Dr. Hehemann specializes in male sexual function and fertility and also practices clinically at the university. Hair loss can be a distressing experience for many men, leading them to seek out treatments like minoxidil (Rogaine) and finasteride (Propecia). However, these popular medications may come with potential side effects that impact sexual function. Minoxidil is unlikely to significantly affect erectile function when applied topically, as only about 1% of the medication is absorbed into the bloodstream. On the other hand, finasteride, which blocks the conversion of testosterone into dihydrotestosterone (DHT), has been associated with a 5-10% chance of negatively impacting sexual function, including reduced libido, erectile dysfunction, ejaculatory problems, and decreased sexual pleasure. These effects have been reported with both the higher 5mg dose used for benign prostatic hyperplasia (BPH) and the lower 1mg dose for male pattern baldness. We first learned of Dr. Aaron Spitz from The Game Changers film. In his book, aptly named The Penis Book, Dr. Spitz states that the most common reason for erectile dysfunction is a problem with blood flow to the penis due to narrowing of the arteries. In my conversation with Dr. Spitz, we discuss the causes and what you can do to prevent and even reverse erectile dysfunction by improving penis health. Aaron Spitz, MD: Dr. Spitz earned his medical degree from Cornell Medical College, completed his Urology residency at the University of Southern California, and then obtained fellowship training in infertility at the Baylor College of Medicine. In this episode we are giving our supplement spotlight to Coenzyme Q10, or CoQ10 for short. If you've been In the fertility grind for a while then you are likely already aware of this supplement. It's a good one. Let's spend this episode talking about why and how to use it. CoQ10 is a nutrient that occurs naturally in the body. Our highest levels of this compound are found in the liver, heart, kidneys, and pancreas. Cells use CoQ10 for various functions, such as growth and maintenance. CoQ10 also functions as an antioxidant by protecting cells in the body from damage caused by harmful molecules. It plays many vital roles in our diet and health and functions like a vitamin to boost energy production in the body, while also helping to fight the leading causes of cardiovascular disease. Not only has the substance been touted for its neurological and cardiac health benefits, modern studies have shown that coenzyme Q10 can have a significant impact on fertility.According to research published in the Journal of Fertility and Sterility, women who were given 600 mg of CoQ10 supplements per day saw improved egg health and fertilization rates.The consumption of CoQ10 supplements has also been found to enhance male fertility. A study published in the Journal of Andrology found that men who took 200 to 300 mg of CoQ10 daily experienced a noticeable increase in sperm motility and sperm count, as well as an overall boost in sperm health. Why are there such gaps in our knowledge when it comes to male reproductive health? Well as we continue with our Early Years theme we're in conversation with Rene Almeling - sociologist and author of a new book called Guynecology, a smart book with a smart title. The Fertility Podcast is dedicated to giving a voice to male fertility issues, both with the expert interviews we share and inviting men to come and share their experiences. We also champion the importance for Men to be included in ALL fertility discussions, which we know often isn't the case. Natalie came across Rene's book after reading an article in Elle magazine and didn't hesitate to get Rene on the show as we needed to hear more about Guynecology! Rene takes us back in history to explain why, until recently, men's reproductive health was largely ignored. At the end of the 19th century, a move by US doctors to develop a male fertility speciality was ridiculed, mainly due to the stigma surrounding what was then in focus - venereal disease. In the 1960s another group of doctors developed the speciality Andrology but to this day Andrology is in no way near the established speciality like its female reproductive health equivalent. A pet hate of Kate's is that fertility clinics rarely work alongside each other, and Rene tells us about emerging research looking into paternal effects and why it is important to have more joined-up working. This new research suggests that the impact of age, lifestyle and exposure to chemicals, in the 2-3 months that sperm is developing, can damage sperm in a way that can impact not just fertility, but importantly the health of the child. Rene talks about the importance of adding male fertility to the school curriculum and missed opportunities of talking to young men about their reproductive health. From her research into the topic, Renee concluded that we don't need a whole new speciality of male reproductive health. We just need to take our umbrella of reproductive health and stretch it to cover men too. In this episode we discuss erectile dysfunction in young men. Did you know that 1 in 4 men under the age of 40 experience erectile dysfunction. Are the erectile problems faced by these men different from the typical erectile dysfunction experienced by middle aged and older men? Is erectile dysfunction in young men caused by different underlying problems? Do these men need a different evaluation? Should they be offered different treatment options? In order to answer these questions, we turned to Dr. Nannan Thirumavalavan. Dr. Thirumavalavan is Chief of Male Reproductive and Sexual Health at University Hospitals as well as Assistant Professor of Urology at Case Western Reserve University School of Medicine. Dr. Thirumavalavan earned his medical degree at Boston University School of Medicine and subsequently completed his residency in urology at Boston Medical Center. He then completed a National Institutes of Health-sponsored fellowship in Male Reproductive Medicine and Surgery at Baylor College of Medicine in Houston. Dr. Thirumavalavan has earned numerous awards in his career, including the AUA Men's Health Traveling Fellowship in 2014, the NIH Travel award in 2018 for the American Society of Andrology, and the Society for Male Reproduction and Urology Traveling Scholarship in 2018. He teaches and mentors resident urologists and medical students.

In this episode we discuss penile enlargement. A recent study revealed that 45% of men wish that their penises were larger. That desire is further confirmed by the multitudes of creams, medications, and procedures promising to increase the length and/or the girth of the penis. But is it actually possible to increase the size of the penis? Do any of these treatments actually work? And, if so, to what extent? What are the risks of these penile enhancement treatments? And are there trade offs involved? To answer these questions, we are fortunate to be joined by Dr. Matthew Ziegelmann. Dr. Ziegelmann is an Assistant Professor specializing in male sexual dysfunction and infertility (Andrology) in the Department of Urology at the Mayo Clinic. He obtained his medical degree from the University of Minnesota and then completed a residency in urology at Mayo Clinic in Rochester, MN. He then continued his sub-specialty training in male sexual dysfunction and infertility (Andrology) at Rush University Medical Center in Chicago, IL. He is an active member of the American Urological Association, North Central Section, and Sexual Medicine Society of North America. He has published extensively in male sexual dysfunction and speaks nationally on topics related to male sexual dysfunction and other urologic conditions.

In this episode we discuss testicular and scrotal pain. Most guys have had pain in their testicles and scrotum at some point. This pain is usually short lived and goes away on its own. But for some men, the pain does not go away. It persists and begins to affect their day to day lives, limiting their activities, destroying their sex lives, and impacting their mental health. What causes this chronic pain in the scrotum? How should it be evaluated? And what treatment options are available? In order to answer these questions, we spoke with Dr Petar Bajic. Dr Bajic is a urologist in the Center for Men's Health at the Glickman Urological and Kidney Institute in Cleveland, Ohio, and Assistant Professor of Urology at the Cleveland Clinic Lerner College of Medicine of Case Western Reserve University. He specializes in men's health and sexual medicine. Dr. Bajic earned his medical degree from the University of Toledo College of Medicine. He then completed his residency training in urology at Loyola University Medical and a fellowship in Andrology and Genitourinary Reconstructive Surgery at Rush University Medical Center. Dr. Bajic is an avid researcher and physician-scientist. Dr. Bajic is Associate Editor for the medical journal Urology and is a regular reviewer for multiple other medical journals.
In Episode 13, we heard the fascinating career story of Biomedical Scientist in Haematology turned famous comic and actor Dave Spikey. Dave is best known for his stand-up comedy tours, starring in sitcom Phoenix Nights alongside Peter Kay and Paddy McGuinness, presenting Bullseye, Chain Letters and being a team captain on 8 out of 10 Cats. Before he moved over to comedy, Dave led a successful 30-year career in Biomedical Science – rising the ranks to become Chief Biomedical Scientist in Haematology at The Royal Bolton Hospital. Dave recollects his journey into Biomedical Science from school pupil to trainee medical laboratory technician to senior biomedical scientist. He discussed the various roles he held and the innovations in the field he brought about. We then challenged Dave, who is a former Mastermind champion to our very own Haematology quiz. He then took us through his move from Biomedical Science into comedy, writing, film production and television - from a local hospital pantomime performance to a national stand-up tour, starring in iconic sitcoms and hosting television game shows. It was an absolute pleasure to chat with Professor Sheryl Homa this week on the podcast. Sheryl Homa is an HCPC registered Clinical Scientist with a special interest in male fertility. She obtained her degrees in Biochemistry at Imperial College of Science and Technology and the Middlesex Hospital Medical School. She spent a large part of her career as an academic research scientist in the field of oocyte maturation, funded by the National Institutes of Health in the USA. For many years she served as Scientific Director of several fertility clinics in the UK working in both the public and private sectors. During this time, she became aware that male fertility is considerably marginalised and as a result, opened Andrology Solutions in 2007, the first and only HFEA licensed clinic of its kind, dedicated to male infertility. She has collaborated on many research projects and is the author of numerous scientific articles, abstracts and book chapters in the fertility field. Her studies include investigating calcium signaling in sperm and more recently, the role of oxidative stress and infection on sperm quality. Sheryl has been responsible for introducing and CE marking novel state of the art testing for male infertility, including a chemiluminescence assay for measuring seminal reactive oxygen species. Currently Sheryl is honorary Professor in Biosciences at University of Kent and consultant clinical lead for Andrology at The Doctors Laboratory. We chatted about how men are equally responsible for creating an embryo that would lead to a child, how half of the genetic material is from men and that the child is a reflection of the genetic material of both partners. That it isn't just about getting pregnant or staying pregnant but ultimately the health of a child. We talked about how sperm and eggs need to be as fit and healthy as they can be and how if the sperm or egg is damaged in anyway this passes to every cell of the child and this is an area that I talk about a lot with my clients: that we need to widen our vision looking further than just getting pregnant. This helps us make the changes necessary. Sheryl said something that really made sense to me and she said that she advises her male clients to act as though they are pregnant so in the few months leading up to conception to think about their sperm as that they are carrying pre babies. We know that 11 to 12 weeks is necessary for both the sperm and the egg to develop fully and so a preconception preparation plan is absolutely vital and she compared this to preparing for Wimbledon and saying that if you were going to compete at Wimbledon you wouldn't just rush on and play would you? No, you would prepare with training, health and with the right equipment. I’m a huge fan of an analogy myself, I’ve often used the example of how you wouldn’t ‘just run’ a marathon. We looked at how it's important to consider quality and not quantity and how this is important to maximise your chances of both natural and assisted fertility and we need the best quality egg and sperm. We talked about the sperm analysis and how this is a crucial first step and provides great information but it only tells you if there are potentially enough and we know the parameters are low, too low, and if sufficient are moving. However, it doesn't tell us whether the sperm is capable of getting to the egg or if it is capable of entering the egg or if it has the right triggers to ensure embryo development. Resistance infertility circles was something that we discussed as well and this letters onto the resistance amongst the fertility clinics to undertake two very specific tests that give it so much information about the sperm in terms of fertility but also importantly in terms of miscarriage and how this really should be offered to all couples experiencing recurrent miscarriage and those tests are the DNA fragmentation test and the oxidative stress test on the sperm. Something that comes up for me with my clients and with something that we discussed is how some Doctors and clinics will say that even if the test showed DNA fragmentation or oxidative stress there isn't anything that can be done about it, but Sheryl explained how there might be quite a bit you can do about it. We discussed also how a condition called Variocele maybe a huge factor in terms of sperm quality, how a full clinical history should be taken for male partners, how women have an ultrasound but that men aren’t examined at all. How underlying infections which are not looked at are an underlying cause of conception and potentially miscarriage as well and how a microbiology test on the sperm is a crucial test. Of course we discussed the recent documentary by Rhod Gilbert ‘stand up for infertility’ Which led us to chat about physical issues and emotional issues. Not only emotional issues in terms of how they may affect sperm physically but the lack of support for men who are going through a fertility journey. She mentioned a Facebook group the men's fertility support run by Gareth Down which I would urge you to point your male partners in the direction of We chatted about some basic general advice at the start of a couples fertility journey and we discussed as you've heard me talk about before why trying to conceive at or just before ovulation may in fact be too late and how important a regular ejaculate is because stored sperm is in effect rubbish sperm, and that again it's not just about numbers it's about freshness and quality She will also explained how we were missing a vital piece in the journey and that your GP referring you to the fertility clinic straight away which is ultimately for assisted fertility is like going to your doctor with heart palpitations and the doctor saying let's get you a heart transplant. She explains that IVF is circumvention, how it is not managing fertility and that after initial GP tests, further investigation should take place and treatments suggested to correct possible fertility factors. Further investigation can be undertaken by an Andrologist. Something of interest that was pointed out in Rhod Gilbert's documentary is that there are 8000 registered Gynaecologists in the UK and only 200 registered Andrologist. It is Gynaecologists with a specialism in Fertility that you are referred to at the Fertility clinic Considering an Andrologist looks at male fertility factors this clearly isn't representing half of the equation is it? Professor Bartke is the Distinguished Scholar and Professor of Internal Medicine and Physiology at Southern Illinois University (SIU) School of Medicine in Springfield, Illinois, USA. The focus of his research is on genetic and hormonal control of aging in mammals. His current work is aimed at identifying mechanisms that link reduced growth hormone action with delayed aging and extended longevity. For this work, he is using mutant mice that live longer than normal mice and show various symptoms of delayed aging, including retention of cognitive function and protection from age-related disease. His career includes work at the Jangiellonian University in Krakow, Poland, Worcester Foundation for Experimental Biology in Shrewsbury, Massachusetts, the University of Texas Health Center at San Antonio, and SIU-Carbondale. He is a past president of the Society for the Study of Reproduction, the American Society of Andrology and the American Aging Association. Dr. Bartke has published more than 750 research papers, review articles and book chapters dealing with reproductive endocrinology, prolactin, growth hormone and aging, and has received numerous awards, in addition to 45 years of continuous NIH funding. His laboratory was the first to show that mutation of a single gene can extend mammalian longevity and to suggest that the remarkable increase in the lifespan of Ames dwarf mice is due to growth hormone (GH) deficiency. During the last 20 years his lab has extensively characterized phenotypic characteristics of long-lived hypopituitary and GH-resistant mice. Contributions of his group to reproductive endocrinology and the hormonal and genetic control of aging include nearly 600 peer reviewed publications and more than 150 book chapters and review articles. According to the Web of Science Alert, his work has been cited on an average of 490 times per year since 1975 and more than 774 times per year since 2004.

In the 1960's this was 80% normal forms, today 4% is considered the norm. So 96% of sperm are not of healthy form. So 4% of 30 million is perhaps OK, but 4% of 3 million, 15million, or even 100 million is not so great. DNA fragmentation is not routinely tested. DNA fragmentation s a huge huge issue. Dr. Ranjith Ramasamy Dr. Ramasamy (www.ranjithramasamy.com) is the Director of Male Reproductive Medicine and Surgery as well as an Assistant Professor of Urology at the University of Miami. Dr. Ramasamy completed his undergraduate degree at Rutgers University in 2003; completed his medical degree at Rutgers University Robert Wood Johnson Medical School in 2007; completed a urology residency at New York Presbyterian Hospital – Weill Cornell Medical College in 2013; then completed a fellowship, sponsored by the NIH, in Male Reproductive Medicine and Microsurgery at Baylor College of Medicine in 2015. Dr. Ramasamy has been integral to the advancement of the field of male reproductive medicine and surgery. He has published over a 100 peer-reviewed articles and authored several book chapters on the topic; is on the editorial board of Urology and the Journal of Andrology, as well as a member of many other journals and societies; and has presented his work at over 25 national and international conferences from the United States to Indian, a remarkable feat considering that he only just completed his formal training two years ago. Dr. Ramasamy is also the founder of a company called Uro-Logic that created an app called Urology Flashcards that has over 10,000 users and that is available for free in iTunes and on Google Play. Of note, Dr. Ramasamy is a former television news anchor also having made appearances on the hit ABC series NY Med as well as ABC Nightly News to talk about foods with aphrodisiac qualities. This talk centres on Alan's work as Head of Andrology for Sheffield Teaching Hospitals where he directs the Andrology Service overseeing diagnostic semen analysis and the sperm banking facility. His research interests are in fertility issues with specific emphasis on male fertility. This includes work in the lab to understand sperm biology, but other recent projects have included psychological and sociological aspects of male fertility. Allan is a Senior Lecturer at the University of Sheffield School of Medicine and Biomedical Science and the Head of Andrology for Sheffield Teaching Hospitals where he directs the Andrology Service overseeing diagnostic semen analysis and the sperm banking facility.
