When you're going through fertility treatments, you never imagine they will end without a baby. I didn't. I believed that if I tried hard enough, followed every step, and stayed committed, eventually I would have the child I dreamed of. But when IVF failed and motherhood didn't happen, I was completely unprepared and unsupported. That gap, the silence that follows when treatments end without children, is why I became a member of ASRM, the American Society for Reproductive Medicine, and serve in their Mental Health Providers group.  In October I am attending the ASRM annual congress with thousands of reproductive Endocrinologists, fertility doctors, IVF nurses, and fertility specialists from around the world. My mission is to make sure the voices of women who are childless after infertility are part of the conversation. For an upcoming podcast episode I will be interviewing a fertility doctor who not only treats patients but has gone through IVF herself. Each month, we will host a discussion with the authors of "Views and Reviews" and "Fertile Battle" articles published in a recent issue of Fertility & Sterility.  This month, we welcome Dr. Lydia Hughes and Dr. Eric Widra to discuss the ethics of egg-sharing, or "split-cycles" for fertility preservation. This is where an egg donor freezes their eggs for their own future use for reduced or no cost in exchange for donating a portion of the cohort. Dr. Hughes is a second-year REI fellow at Northwestern University in Chicago, where she also completed her residency in Obstetrics and Gynecology. She earned her medical degree from the University of Alabama at Birmingham. Dr. Hughes's clinical and academic interests include reproductive ethics, PCOS, and ovarian aging.  Dr. Widra currently serves as Executive Senior Medical officer and Vice President, Development for Shady Grove Fertility and US Fertility, respectively.  He was formerly Chief Medical Officer of SG Fertility, and Associate Director of the Combined Federal Fellowship in Reproductive Endocrinology and Infertility, operated through the NIH, Walter Reed National Military Medical Center and SG Fertility. She has presented to audiences around the globe including at the United Nations, ASRM, ESHRE, and the national infertility association, RESOLVE. She has helped companies craft benefit packages and parental leave policies. Her professional experience in the field of journalism, marketing and events, and fundraising. She enjoys marathons and is a writer when not advocating.  Dr. Preston Parry completed residency in Ob/Gyn at Tufts and went south to Louisville for his fellowship in Reproductive Endocrinology and Infertility. He was in academic REI for 8 years, including as division chief, before opening his own practice, Positive Steps Fertility, in 2017. The first successful births from frozen eggs were twins, born in Australia in 1986. IVF serves a valuable role for those with persistent infertility issues or other conditions where natural conception is hindered. Although oocyte cryopreservation was initially used as a fertility preservation strategy for medical indications, currently, it is increasingly used to circumvent age-related infertility. This process of elective egg retrieval and cryopreservation- targeting women in their 20s- has gotten out of hand! Mainly due to social media, Gen Z women are panicking about their fertility. Should They Be? For decades, the age of 35 has been seen as a “demarcation line” for female fertility. Before 35, the theory often goes, most women will have little trouble conceiving, but at that point, fertility falls off a cliff. This misunderstanding of natural fertility processes, spurred on by false information on social media, has led to 20-somethings calling for egg retrieval and egg freezing. The dread of age 35 is so pervasive that its effect bleeds backward in time, with women in their early 30s—and yes, sometimes even in their late 20s—already feeling as if they are behind in the race against their “biological clock.” But the reality of fertility “loss” is much more complicated then just “falling off a cliff” and should be viewed more as of a natural “slope” rather than a “cliff”. About a decade after it shed its “experimental” label from the ASRM, oocyte retrieval and cyropreservation has become ubiquitous in our social media culture and has ballooned in popularity, with over a million frozen eggs or embryos stored in the United States today. It has done little, however, to materially change women's lives. The ASRM has an ETHICS COMMITTEE OPINION (2024) on this very thing. Is 35 really a fertility cliff? Or it it 32? Today on The Egg Whisperer Show podcast, I'm honored to be sharing a conversation all about endometrial analysis. Originally hosted by Tiffany Stankewicz and Nasser Al Asmar of Igenomix / Vitrolife Group, this interview included Dr. Allison Rodgers and Dr. Aimee  We did a deep dive on how endometrial analysis works, and the tools that are available for fertility patients to get a better understanding of what may be happening with their endometrial lining. We also talk about how endometrial receptivity testing can lead to better outcomes for patients. The truth is that up to 30% of women have pathogenic bacteria in their endometrial lining that can negatively affect an embryo's ability to implant, and the good news is that there are simple antibiotics that can improve the lining. Dr. Rodgers and I have both had a lot of experience using the tools that Igenomix has created to test the endometrium, and we have both found that they can improve a patient's chance for a successful pregnancy. They call the trio of tests “Endometrio” and it includes EMMA, ALICE, and ERA. These three tests do the following: ERA (Endometrial Receptivity Analysis) - The ERA test evaluates the stage of an endometrium to determine if a receptive or non-receptive genetic profile is present at the time of biopsy. EMMA (Endometrial Microbiome Metagenomic Analysis) - A screening test to evaluate the endometrium at the microbiological level. ALICE (Analysis of Infectious Chronic Endometritis) - ALICE detects the bacteria causing chronic endometritis to improve your patient's reproductive prognosis. Rachel Weinerman, MD is an Associate Professor at Case Western Reserve University and a Reproductive Endocrinology and Infertility specialist at University Hospitals in Cleveland, OH, where she is the Program Director for the REI Fellowship. She attended Harvard Medical School and completed OB/GYN training at NYU followed by REI fellowship at the University of Pennsylvania. Dr. Weinerman specializes in complex fertility and recurrent pregnancy loss care and is also the Director of Third-Party Reproduction at University Hospitals.  She is an active member of ASRM, SREI, and SART. Her research interests include the impact of ART procedures on implantation environment and neonatal outcomes, and she investigates these areas through clinical and translational methods.
