Despite concerns and recommendations, modified-release opioids are still commonly used for the management of acute pain following total hip and knee arthroplasty. There is a need to compare modified-release and immediate-release opioid use following these procedures, and to determine whether the use of modified-release opioids is safe or harmful. The primary objective of this study was to examine the impact of modified-release opioid use on the incidence of opioid-related adverse drug events compared with immediate-release opioid use among adult hospital inpatients following primary total hip or knee arthroplasty. In this multicentre propensity score matched cohort study, modified-release opioid use in the acute postoperative period is associated with an increased risk of experiencing opioid-related adverse events after total hip or knee arthroplasty compared with those given immediate-release opioids only. Further, patients given modified-release opioids experienced a higher incidence of constipation and in-hospital falls, as well as a longer length of hospital stay compared with patients given immediate-release opioids only. We recorded today's conversation on location at Warren Memorial Hospital with Mesfin Shibeshi DO, fellowship-trained orthopedic surgeon and Grace Speicher, Program and Operations Manager, Valley Health Orthopaedic Clinics, Southern Region. Dr. Shibeshi shared his knowledge and experience with respect to robot-assisted total knee replacement. Using this technology, Dr. Shibeshi is able to create a 3D model of a patient's knee before surgery, allowing for precise planning of incisions and implant placement. The robotic surgical technology offers a level of personalization and precision that can limit soft tissue damage, preserve bone and reduce post-surgical pain for some patients. Dr. Shibeshi is using this technology to aid in same-day total knee reconstruction procedures at Warren Memorial Hospital, the first Valley Health hospital to earn the Gold Seal of Approval® for Total Knee Replacement and Total Hip Replacement Certification from The Joint Commission. Grace explained why certification is important. Certification keeps everyone focused on providing high quality patient care by providing: Patient Education- patients are prepared for their total joint journey. This is accomplished by distributing a total joint book to each patient, along with teaching what to expect prior, during, and after surgery. This is a collaborative effort with the surgeon, nursing, therapy and case management involvement. Early Ambulation and Pain Control- Studies have shown the sooner a patient ambulates the better their recovery. In order to accomplish this, pain must be controlled. In an effort to keep narcotic use at a minimum, they have what is called a multimodal approach which includes a block placed by the anesthesia team during the surgical process, along with a periarticular injection placed by the surgeon. This approach allows for a longer pain control with less need for oral pain pills. This is easier on the patient's stomach, as well as less of a threat for narcotic dependence. Back to Basics 2.0: Enhanced Recovery After Surgery for Patients Undergoing Total Hip or Total Knee by AORNJournal
Surgery for degenerative foot and ankle conditions often results in a lengthy recovery. Current outcome measures do not accurately assess postoperative mobility, especially in older patients. The Life-Space Assessment (LSA), a questionnaire quantifying patients’ mobility after a medical event, was used in this study to assess perioperative mobility in total hip arthroplasty (THA) and foot and ankle surgery patients. We hypothesized that patients undergoing elective foot and ankle surgery would have greater postoperative mobility limitation than THA patients. Compared with THA, recovery of mobility after foot and ankle surgery was slower. THA patients exhibited improved mobility as early as 3 months after surgery, whereas foot and ankle patients did not show full improvement until 12 months. This work will assist the foot and ankle specialist in educating patients about challenges in mobility during their recovery from surgery. To view the article click here.
