Christian hails from Michigan and is a young Italian man, working as an engineer, who has a passion for soccer, being Italian and heavy metal music.. He describes himself as a formerly “happy-go-lucky” guy until spine surgery and metal allergies turned his life upside down. After the sudden onset of back trouble and searing pain, Christian worked through the traditional medical routes, until it was decided that he would need a spinal fusion.After surgery, Christian knew immediately that something wasn't right, waking with searing, intractable pain and a litany of symptoms: rashes, insomnia, an ulcerated tongue, vision, cardiac issues and more. After numerous consults and research,, he came to the hypothesis that he could be allergic to his spinal fusion hardware. Christian obtained blood metal lymphocyte transformation testing from Orthopedic Analysis, where testing determined he was severely allergic to the titanium alloy. Christian was able to gain the confidence of a surgeon and was able to have this fusion revised with stainless steel hardware, which contained components he did not test allergic to. All of his symptoms went away immediately with revision, then returned within months, almost as bad as the first go. Christian re-tested with Orthopedic Analysis and the new test came back showing him reactive to the new hardware. After fusion occurred the supporting hardware was removed and like the prior time, Christian experienced a large resolution of his symptoms….until slowly they returned. Christian discovered that the surgeons had utilized a large number of titanium surgical clips in the revision surgery. He is still fighting a battle to have them removed to this day. In this episode you will learn: Systemic reactions to titanium exist & can cause havoc throughout the body. Hypersensitivities to a given metal don't always present until an exposure occurs. Why self-advocacy is important - surgeons can still make mistakes. When patients are diagnosed with scoliosis, one thing that always comes into their mind is will I need scoliosis surgery? Scoliosis is a progressive condition that worsens during key phases of life, notably in adolescence and later stages. Surgical intervention becomes a consideration when curves become severe. What is Spinal Fusion Surgery? Spinal fusion is a major surgery aimed at stopping the progression of scoliosis by fusing the most tilted vertebrae. It involves rods, screws, and bone grafts to immobilize and fuse the spine. The primary goal of spinal fusion is to halt the progression of scoliosis by stabilizing and immobilizing the spine in the most tilted region. But just like any surgery, there are inherent risks involved such as: Decreased Range of Motion - Spinal fusion surgery often leads to a significant decrease in the range of motion in the fused area. Limitations in Activities - Fused areas impose restrictions on the types of activities a person can perform. Pain at the Site of Fusion - Patients may experience pain not only at the skin level but also at an internal joint level where the fusion has taken place. Premature Aging and Degeneration - Fusion concentrates forces and movement into specific areas of the spine, potentially causing premature aging and degeneration in those regions. Cosmetic Results - Issues like rib deformation may persist, contributing to ongoing cosmetic concerns. Non-Functional Approach - The focus is on stopping curve progression rather than maintaining natural spinal function. Limited Reversibility - The structural changes to the spine, including the insertion of rods and screws, make reversing the procedure complex. Risk of Complications - Associated risks include nerve damage, excess blood loss, infection, adverse reactions, hardware failure, and decreased range of motion. Choosing Between Surgery and Conservative Treatment Surgery is typically considered for curves of 40 degrees or greater in growing adolescents. For adults, surgery is an option if there's severe impairment and disability resulting from scoliosis. Patients should be aware of the trade-offs involved in spinal fusion surgery, considering not only the potential benefits but also the challenges and limitations that may arise post-surgery. The decision to undergo surgery should be well-informed, taking into account individual circumstances and preferences. When patients have their spine examined, the term lumbar lordosis often gets thrown around during discussions about spinal health. In today's episode, we discuss the world of spinal curvatures, demystifying their significance, and uncovering secrets behind lumbar lordosis. Lordosis vs Kyphosis These are different types of spinal curvatures, like the graceful bends of a bridge that give it strength and flexibility. In essence, these curvatures exist to help our spine better handle the everyday stresses of life. Imagine kyphosis as a gentle backward bend, while lumbar lordosis is its opposite, a subtle forward curve. Each curvature has its designated area in the spine – kyphosis in the mid-back (thoracic spine), and lumbar lordosis in the lower back (lumbar spine). It's like a choreographed dance of curves that contribute to our spinal health. But lumbar lordosis isn't just a one-size-fits-all deal. Just like people, spines come in various shapes and sizes. Ideally, lumbar lordosis should be around 40 degrees, but a bit of flexibility exists. Think of it like a normal range for blood pressure – there's a sweet spot, but some wiggle room is okay. What's interesting is that too much or too little of lumbar lordosis can lead to problems. Imagine if your blood pressure were sky-high or too low – not good, right? Similarly, if lumbar lordosis goes off-balance, it can result in various spinal issues, often causing discomfort and pain. And it's not just limited to the lower back; it can ripple through your entire spinal alignment. Causes of the loss of lumbar lordosis or hypo lordosis in the lumbar spine Spinal Fusion and Scoliosis Surgery: Procedures like spinal fusion, especially in cases of scoliosis surgery with devices like Harrington rods, can lead to a flattening of the spine's natural curvature. This flattening can result in a decrease or loss of lumbar lordosis. Trauma and Injury: Significant trauma or injury to the spine can disrupt its normal alignment, potentially causing a decrease in lumbar lordosis. Such incidents can shift the spine out of its proper position, impacting its curvature. Global Loss of Alignment: When the overall alignment of the spine is compromised, it can lead to a reduction in lumbar lordosis. This can occur due to various factors, including poor posture, muscle imbalances, and degenerative changes in the spine over time. Degenerative Disc Disease: The degeneration of spinal discs, which act as cushions between vertebrae, can contribute to a decrease in lumbar lordosis. As the discs lose their height and hydration, the spine's natural curvature may be affected. Ankylosing Spondylitis: An autoimmune disorder known as ankylosing spondylitis can lead to fusion of ligaments and discs in the spine. This fusion can alter the spine's curvature and result in a reduction of lumbar lordosis. It's important to note that any loss or abnormality in lumbar lordosis can have implications for spinal health and overall well-being. Maintaining proper spinal alignment through exercises, ergonomic practices, and proactive care is essential to prevent these issues and promote a healthier spine. Angèle est née en Guadeloupe et réside actuellement dans une vallée du Colorado. Dans cet épisode, elle vient nous parler de sa scoliose qui lui a été diagnostiquée lorsqu'elle avait 11/12 ans. Jusqu'à ses 18 ans, elle explique devoir porter un corset et être suivie par un neurologue de manière régulière. Par suite, les médecins lui conseillent de se mettre "à fond" dans le sport avec l'espoir que sa scoliose ne s'aggrave pas puisqu'elle avait arrêté de grandir et ses os étaient relativement stables... Malheureusement, ce ne fut pas le cas... Malgré une pratique du sport soutenue, chaque courbe de sa scoliose avait pris 10° et l'opération était de nouveau à l'ordre du jour... Kyra Condie is a professional rock climber who had a spinal fusion as a teen to stop a rapidly progressing scoliosis curve. On this episode, Kyra discusses what it was like to have scoliosis progress to needing surgery when her climbing career was starting to take off. Fast forward to the Tokyo Olympics and Kyra is the first athlete to compete in the Olympic Games with a spinal fusion. She shares a great story about the first surgeon she went to telling her that climbing wasn't going to be a big part of her life, so she should just not worry about it. She ended up having a second opinion from a surgeon who took her goals into account when doing the surgery. One of the huge takeaways from this episode is to advocate for yourself...even if you are a teenager discussing with your parents. Spinal fusion is a major surgery. It is where one or more spinal bones (vertebrae) are fused together using screws, bolts, and or plates. The hardware may be placed in the front (anterior) or the back (posterior) of the spine. The disc between the spinal bones is oftentimes removed and replaced with bone or a spacer. Spinal fusion can be done in the neck, thoracic, and lumbar regions. Lumbar fusion is a surgical treatment for patients with ongoing pain, low back pain, or leg pain that has failed conservative treatment. Lumbar fusion surgery has become increasingly popular. The long-term side effects of Spinal fusion complications long-term can be broken down into two major categories: Failure of the procedure itself (failed fusion) Complications arise as a result of the fusion. The most common causes of a failed lumbar fusion are nonunion, hardware breaking, and hardware becoming loose. Non-union occurs when the spinal bones that are bolted together fail to fuse or grow together. The spinal bones are stabilized by the use of screws, bolts, and plates during lumbar fusion. Significant stresses are applied to the low back and the hardware; as a result, the hardware breaks, resulting in spinal instability and discomfort. In lumbar fusion, screws are utilized to support the spine. The screws can come loose due to the same stresses that can break the hardware. What's the main benefit of doing this? When it comes to scoliosis, surgery is the most common and traditional treatment option in our healthcare system, thus the goal is to attempt to stop progression even though there is very little study on the potential adverse effects and why so many individuals actually choose to have it. The final consequence of patient management by a surgeon is surgery, as would be expected, but patient management by a person who deals with conservative care is likely to result in more conservative care rather than surgery. Surgery can be used to treat scoliosis, and it's more likely to obtain any kind of reduction, but you won't be able to do so with the curve. When it comes to surgery, it should be your last resort. This means that you've exhausted all other treatment options and now you're forced to consider surgery. At this point, it's similar to when it comes to scoliosis therapy. When are you a candidate for posterior spine fusion? Spinal stenosis is a reduced space around the nerves in the spinal canal; With this condition, patients typically have difficulty with standing and walking that amplifies their back and leg pain. Patients may also experience weakness and numbness. Degenerative disc disease is the condition where the discs in the vertebrae start to lose their cushioning over time and those discs can become painful. During the process of degenerative disc disease, a condition called spondylolisthesis can occur where the bony structure starts to slip and is not aligned properly. Nearly all humans will experience degenerative discs at some point in their lives; 90% of people are affected by age 50. Dr. Danko reviews the symptoms you may experience with it. What is the treatment for degenerative disc disease? The first step is usually physical therapy and the modalities they have to offer. Dr. Danko explains the indication to progress to imaging, injections, and beyond. What is the zip procedure? It is the alternative to the pedicle screw fixation procedure. Dr. Danko explains the implant and how it works. What is the recovery process for the zip procedure? What is Silo? When do patients need it? Fusion sounds like a scary procedure but Dr. Danko explains how it is a safe, straightforward, outpatient procedure. If you believe that you are a candidate for any of these procedures, your first step is to discuss your situation with a provider at the office. 3 Key Points: Many patients that have ongoing back pain are sent for imaging to help detect what exactly is the problem. At Premier Pain they are often having imaging performed to detect conditions like spinal stenosis, disc herniation, degeneration of the disc or vertebrae, and spondylolisthesis. The doctors use the images to piece together along with the patient symptoms to know where the target of exactly what they need to treat. Dr. Danko explains the process of outpatient zip procedure and how it works as well as what the recovery process looks like. The sacroiliac joint fusion performed with SILO is a safe and effective treatment. Dr. Danko explains how patients may end up being in need for the procedure. There is a progression in the conservative treatment plan including external supportive devices, anti-inflammatory medications, and injections before moving on to more durable solutions or permanent fixes. Diagnosed at age 11 with a 53 degree curve her orthopedic told her she was the quickest progression they had seen. Fused in 1997 with Harrington rods from T2-L2, Laura sustained a lot of trunk rotation. In 2012, she became a PT. During her education, she found out how little was discussed about Scoliosis. In 2017, she had her second child where she would have a lot of pain. This lead her to the importance of pelvic floor health & the connection it has with Scoliosis where she practices with clients. Growing up, she was never in pain, always fit, but would have headaches. She attributed this to her body compensation. As we continue the conversation, she shares that one the key focuses she starts conversation with her clients, we start with a strong back no matter where you are. It is the bottom of our pelvis, it's “the pelvic floor is the floor of our core.” Pregnancy is an amplifier to whatever movement we are used to using. Following a back injury, lots of athletes are choosing motion-preserving disc replacement surgery over a traditional spinal fusion - so much so that this story has been in the news several times recently with professional athletes. It's a topic that comes up often for athletes at all levels who want to preserve motion after spine surgery so they can get back to playing the sports they love. In this episode, we talk with spine surgeon Dr. Ehsan Jazini about why disc replacements are becoming more requested and popular with athletes and weekend warriors.
Let's Dive in! Sarah- born with Scoliosis, feeling limited, never interested in sports. In her mid 40's she became interested in exercise. Through her journey, she learned about how unlimited she has been.Trying a little bit at time, she discovered she's able to do almost everything. One day while out with her dogs in the woods, she picked up a stick & started moving it. After finding that it felt good, she made a 4 lb Gada. She currently swings 15lb. What she likes about the mace, “it's stable. My whole body is working to keeping me upright, controlling the weight. It's not just arms, it's my whole core.” Kay- growing up playing sports. At 18 he started going to the gym, lifting heavy. At 25, his hip started hurting, causing him to start limping. He started to deteriorate without being diagnosed. Eventually he went to the hospital. Eventually he was diagnosed with Scoliosis. He would later find out his neck was broken. Doctors surprised, telling him, you should be dead! Kay's discipline of going to the gym, is what kept him alive. Eventually having surgery, resulting in not being able to rotate. Continuing with traditional workouts, he was still in pain. He researched & found “rotational movement training” & Rik Brown. Kay started training with the mace & clubs. Using these modalities, he has put on weight, gaining strength. He now has a collection of mace's & clubs. Clea- diagnosed with Scoliosis at 12. She was fused at 14. Not being very athletic, he never liked running. She always tried to do some exercise, “to stay fit” but having Scoliosis she had ups & downs. During the pandemic, she found TOPS (Twisted Outreach Program). Being a cello player, she's sensitive about her shoulders and arms. “What If I tweak them? I won't be able to play.” Trying a few exercises with a broom from the membership portal, she saw that she could safely do them. She invested in a mace. For the 1st time, she has a daily exercise routine. She currently switches between the 5 & 7 lb mace. Before training with a mace, she “used to be closed up.” Her training has helped with her pain, increasing her strength & range of motion. Emily- In high school, she played field hockey. Complaining about her back, she was diagnosed with Scoliosis. After having a growth spurt, her curves were at 40/50 degrees. This would result in surgery. Post surgery, it was recommended that she work with a personal trainer. Someone who has a background in ballet/pilates. It was through her friend that she was able to find that match. While not having access to a mace, she substituted it with a Didgeridoo. One of the first things she found while working with a mace substitute, “it's fun! A lot of the moves are strengthening while stretching. This results in instant feedback of something that is feeling good to my body.” On a final note, they share advise when starting with a mace The weight of the mace is shockingly less than what you think you would use. Go slow learning & do not push yourself too hard too fast. Leave your ego at the door Have fun with it! Don't feel limited by what you think you can do It's worth it to commit, even if it's not a long practice Don't underestimate the mace, it's more a holistic practice. On this Episode we welcome Allie, a scoliosis surgery pilates teacher She shares her story of being diagnosed at 13 with 43/40 degrees & no pain Going to Emery spine center in Georgia & being a ginny pig for an experimental brace. After wearing the brace for 3 months, her curves increased to 52/48 In 2007 she when she was a high school freshman, she found out she would need surgery. In May she was diagnosed by December she was already fused being in the hospital for only 4 days! Bringing a pillow to school was necessary because sitting on school chairs was too painful. She has only recently started to process the whirl wind of what happened. This December will be her 14 year mark of being fused. She shares what 14 years post surgery has been like & what she has learned. Acceptance. It's not your fault Take this one day at a time Allow yourself to feel the feelings, all of them. How she turned to fitness and what it has done for her What makes her different, she lives with a fused spine making her relatable to spinal fused clients. Orthopaedic patients are increasingly using cannabinoids for pain relief, but there is a lack of evidence-based data on this topic. In this episode, Marc and Mo are joined by special guest Jason Busse (Associate Professor in the Department of Anesthesia at McMaster University, Director of the National Pain Centre, and Associate Director of the Michael G. DeGroote Centre for Medicinal Cannabis Research) in an illuminating discussion on the issues, opportunities, and challenges related to orthopaedic research into the use of cannabinoids. Some of the questions covered during the discussion include: What are the most popular claims among cannabis proponents? What evidence is there to support these claims? Is there a causal relationship between cannabinoid use and decreased use of opioids and other prescription medications (i.e., a “substitution effect”)? Much of the current evidence on cannabinoids is observational and anecdotal. What can we learn from this evidence, and how can such information inform future research?
