Today, we're joined by Tennessee Music Legend Ed Snodderly at his live music venue The Down Home, in Johnson City, Tennessee.    Ed describes himself as a songwriter with a strong Appalachian sense of place. His songs ring in the old, the odd and in a non-sentimental way Ed calls it American Southern, “cause that sounds really cool.” In 2020, he was awarded the lifetime achievement award from the Southern Region of Folk Alliance, and the third verse of his song “The Diamond Stream”, which he performs here, is permanently displayed at the Wall of Honor in the Country Music Hall of Fame in Nashville, Tennessee.  But today's not just a music performance. We sat on the stage with him, and as he played, we talked about life, music, and everything in between. I did want to have a look at the state of our primary healthcare – this is something we've looked at before, and I have absolutely no doubt that we will look at again. I do tend to agree with the GP advocacy group, General Practitioners Aotearoa, that the concept of the family doctor is dead. You're not going to get a Dr Finlay's Casebook again any time soon on the telly. You're not going to see a doctor who has not only looked after your primary health for much of your life, but also that of your family's. These are the kinds of doctors that have gone the way of the moa.   Where I would disagree with the GPA is that they said you wouldn't see the sort of queues for GPs that we saw in South Auckland, in Remuera. I would argue it's just as difficult for people in the blue chip suburbs to get in to see a GP as it is in the poorer areas. It's just the people with disposable incomes can have other options.   A briefing given to Dr Shane Reti when he took over as Health Minister warned that New Zealand is at least 485 GPs short across the country. Remember trying to find the GP in Tokoroa? A GP was looking for somebody to take over the practice - all sorts of offers were put out there, nobody was interested. This number's expected to grow to a shortage of between 750 and 1050 doctors in the next ten years. At least a quarter of a million Kiwis aren't enrolled with a practice. Many of them won't take on any new patients. 1,034,000 people said they struggled to access GP services because of cost in 2022/23, double the number of the previous year. And the impact of this of course is pressure on hospitals, emergency departments, specialist consultations and immunisation rates. Waiheke Island's only afterhours medical clinic closed its doors yesterday; 24 practices and clinics in Canterbury, the Southern Region, Hawke's Bay, and mid Central that provide after hours or urgent care experienced closures or reductions in hours in 2023 because there aren't enough GPs.   There is a tiny bit of good news... in March, the Health Minister pointed to work beginning on setting up a third medical school and record numbers of GP registrars as green shoot, but added, “I understand there are other parts of retention and remuneration we need to collaborate on.”   There is so much need everywhere, across every field, but GPs are in crisis. If a crisis can be something that continues for many, many years, because they have been saying for at least the past five years that they are struggling. Pre-Covid they were struggling. GPs were getting older, new doctors weren't training in the field, they were getting stressed and burnt out because they were seeing so many patients with so much need, and yet without them, they are such an important component of the country's overall health plan that you cannot have a healthy country without healthy GPs. Difficulty in accessing GPs results in pressure on EDs and poorer health outcomes once people do finally get treatment.   I've been with the same GP practice for about 25 years. I don't see the same GP; I've had a succession of really lovely, fabulous GPs come and go. The last one I was absolutely fabulous, but she now only works mornings because she's trying to manage herself, and her family, and her practice, and it's all just overwhelming. It was three weeks before I could get in to see a GP. You expect to have to wait. If it's urgent, they do their best. If it's urgent, you try and get into an afterhours clinic, but you have to have the money to pay and there has to be an afterhours clinic open near you.   So like I say, difficulty in accessing GPs no matter where you are in the country, but if you have money, if you have disposable income, you can get a result a lot more easily. Do we try and attract them from overseas? Do we try and attract young people, pay their student loans if they become a GP?  We've seen what happens when you try and attract somebody with money and all the add-ons and the bells and whistles to get to Tokoroa.  If they don't want to, they don't want to. In this Podcast, we are going to travel from Edinburgh down to the Borders passing through some medieval villages on route. We will be travelling on roads built by the Romans in the 1st and 2nd   centurie and we'll be looking at some scintillating castles and other fascinating buildings on our way. You will hear of true Thomas or Thomas the Rhymer whose prophesies were uncannily accurate and also of Maiden Lilliard, the warrior lady who had her legs chopped off during the battle of Ancrum and continued to fight against those who had killed her lover. We will also look into the ruined Border Abbeys at Dryburgh and Kelso and take a look at Roxburgh castle ruins where James 2nd of Scotland was killed by one of his own cannons. The ruins stand in the grounds of the fabulous Floors castle, home to the Duke of Roxburgh. Our conversation with Dr. Vanessa Banks, Medical Director for the Southern Region for Valley Health Urgent Care systems, and Kristy Shannon, Senior Director of Urgent Care for Valley Health focuses on differentiating between when to visit an urgent care center and when an emergency room visit is necessary. Dr. Banks and Kristy explain what constitutes urgent care services. They discuss the operational hours, facilities, and types of treatments available at Valley Health's Urgent Care locations compared to their Urgent Care Express locations. Dr. Banks provides insight into what an emergency room visit entails and shares her background in emergency medicine. They both emphasize that common ailments like mild dehydration, vertigo, minor fractures, and less severe injuries can be efficiently treated at urgent care centers. The robotic-assisted biopsy platform Ion is a “game-changer” for patients with lung cancer, as it provides a quicker, less invasive surgical method for conducting a lung biopsy, according to Richard Lazzaro, MD. In a conversation with CancerNetwork, Lazzaro, the chief of Thoracic Surgery at the Southern Region of RWJBarnabas Health, spoke about his experience with adopting the Ion robotic bronchoscopy platform for the early detection of lung cancer at Monmouth Medical Center. He highlighted how the tool may enable practices to acquire tissue and perform disease staging with fewer complications, which may particularly benefit those who plan to undergo induction chemotherapy or immunotherapy. In terms of other potential advancements in the lung cancer surgery field, Lazzaro discussed how he anticipates the use of video-assisted thoracoscopic (VATS) surgery to evolve. Specifically, he mentioned the development of technologies such as augmented reality as tools that may help minimize the variability of surgical procedures. Regarding his practice, Lazzaro highlighted how a multidisciplinary thoracic tumor board—including medical oncologists, radiologists, pathologists, and pulmonary physicians, among others—has helped in producing long-term survival improvements. He emphasized collective discussions and shared decision-making as part of determining appropriate courses of care for his patients. When it comes to a multidisciplinary approach, Lazzaro stated that “you want to take care of patients” like they were part of “your family.” Overall, Lazzaro noted how the lung cancer treatment landscape has changed over time. He emphasized referring patients for CT scans as well as evaluations at nodule or thoracic oncology clinics as part of a multidisciplinary strategy. “The management of lung cancer is different than it was even 5 years ago. “Octave displacement is a really big thing with adolescent singers. Make a game out of it - I'll have them match me, match me up an octave, match me down an octave - versus scolding when you're in the middle of rep and someone is singing too low and you just point and say ‘that's too low.' That's a little ambiguous for the average 13-year-old. To give them the strength to identify it themselves is practicing the skill we want to see played out in the repertoire.”Since 2006, Mr. Oakes has served as Director of Choral Music and Music Instructor at The Baylor School, a grade 6-12 independent day and boarding school in Chattanooga. Under his direction, the choral program has grown to include over 200 participants in four student choirs and a faculty choir. In 2015, he was awarded Baylor's Glenn Ireland Chair for Distinguished Teaching and starts his service as Chair of Baylor's Fine Arts Department beginning with the 2019-2020 school year.Mr. Oakes also serves as Artistic Director of the Chattanooga Boys Choir, a music education and performance organization founded in 1954 which now includes over 120 choristers ages 8-18 in five ensembles. Including innovative performance opportunities and collaborative community initiatives, the CBC maintains a performance calendar of thirty appearances annually. The choir has performed and toured extensively, including performance tours to Europe, Canada, and Cuba. Recording opportunities for the CBC have included commercially-released recordings with Stephen Curtis Chapman, Casting Crowns, and the grammy-nominated NAXOS recording of Maurice Ravel's L'enfant et les sortilèges with the Nashville Symphony Orchestra.He is the former President of the ACDA's Southern Region and in 2012, he was selected as one of seven conductors chosen to represent the United States at the inaugural ACDA International Conductor Exchange Program in Cuba.As a conductor/clinician, he has conducted numerous honor choirs and festivals throughout the United States, including ACDA regional honor choirs. André J. Thomas is an Associate Artist with the London Symphony Orchestra.Thomas is Professor Emeritus of Music at Florida State University. He was visiting Professor of Choral Conducting at Yale University from 2020-2022. He also served as faculty member at the University of Texas, Austin.Dr. Thomas received his degrees from Friends University (B.A.), Northwestern University (M. M.), and The University of Illinois (D.M.A). He is in demand as a choral adjudicator, clinician, and director of Honor/All-State Choirs throughout North America, Europe, Asia, New Zealand, Australia, and Africa Dr. Thomas has conducted choirs at the state, division, and national conventions of the Music Educators National Conference (NAFME) and the American Choral Directors Association (ACDA). His international conducting credits are extensive. They include conductor/clinician for the International Federation of Choral Musicians' summer residency of the World Youth Choir in the Republic of China and the Philippines. He was also the conductor of the World Youth Choir's winter residency in Europe and a premier performance by an American choir (Florida State University Singers) in Vietnam.He is a highly respected guest conductor who has led numerous prestigious orchestras and choirs around the world, including the Birmingham Symphony Orchestra in England, the Berlin Radio Choir and the North German Radio Choir in Germany, the Netherlands Radio Choir, The Bulgarian Radio Choir and Orchestra, the Seoul Metropolitan Chorus, Ansan City Choir, Jeju Provincial Seogwipo Chorale in South Korea, the Charlotte Symphony, China's People's Liberation Orchestra, the Czech National Symphony Orchestra, and the London Symphony. Some health professionals are describing Te Whatu Ora's data on emergency department wait times as "gobbledygook", saying the authority is either fudging the numbers, or has made a huge error. One example is Northland where monthly stats show in November and December, more than 99 percent of people who attended their Emergency Departments were seen within the target time of six hours. That's 20 percent better than previous months. It's the same story in the Southern Region and Whanganui, where, according to the figures, they were all of a sudden reaching their targets too. A closer look at the figures in these three regions show the number of people presenting at Emergency Departments is down by thousands for those months. For example in Northland in October, close to 5000 people were seen at ED's - in November it was just 361 and in December 318.  At the other extreme, in just one month attendence at West Coast hospitals ballooned from 700 to more than 8000. That's a 1144 percent increase in emergency department use. National Party health spokesperson Shane Reti says the numbers are either wildly optimistic or simply can't be real. He spoke to Māni Dunlop.

Her professional niche is in the area of medical student career development and work-life integration. In her current decanal role, she creates and oversees programs and resources available for all areas of medical student support and career guidance. She is a national leader in student affairs with a passion and commitment to medical student well-being. Dr. Mechaber has broad national involvement as she currently serves as the Chair-Elect of the National Group on Student Affairs (GSA), recently completing her term as past-Chair of the GSA Committee on Student Affairs (COSA) and previously as the Southern Region's Chairperson. She also represents the GSA as a member of ERAS National Advisory Board. At approximately 8:30 p.m. on March 9, 2021, Greensboro ATCT (GSO) member Noah Walker was working the approaches into Greensboro, N.C., from the tower cab, as the positions were combined and the radar position was moved to the tower. Walker was lining up a Lifeguard aircraft to the final for Runway 14. Just before he instructed the pilot to contact the tower, he noticed lights that appeared to be another aircraft in conflict. Walker asked the tower controller if he was talking to an aircraft, but the controller confirmed he was not. Walker determined it was a drone flying near the arrival corridor for runway 14. Since the controllers could not predict the erratic behavior of this very large drone, they had no choice but to suspend operations at GSO until they could be reasonably sure the drone no longer presented a threat. Walker's actions on this night prevented a collision and ensured the safety of the users in the airspace. NATCA is proud to recognize his professionalism and skill to identify the hazard even though it did not appear on radar. It led one pilot to comment on frequency, “Thanks for looking out for us.” Walker will be honored on June 17 in New Orleans with the Archie League Medal of Safety Award for the Southern Region.

Farmers work in a business where the finances keep getting tighter and the stakes keep getting higher. That kind of pressure takes a toll. According to the latest data from the Centers for Disease Control and Prevention, the suicide rate in the agriculture industry is nearly 10 points above the national average, and about one in four farmers struggles with mental health issues. But mental health doesn't typically get talked about a lot in the ag community. So in this episode, we meet Lowell Neitzel, a grower from Kansas who's trying to change that by sharing his personal story about mental health. Lowell also leads the NCGA Member and Consumer Engagement Action Team, which has targeted grower mental health as a priority. Lowell is joined by Dr. Heather Sedges, an associate professor from the University of Tennessee who's working to prioritize mental health resources for growers. Dr. Sedges serves as Principal Investigator and Lead of the USDA/NIFA-funded Farm and Ranch Stress Assistance Network in the Southern Region.  Together, they discuss how to change the conversation about mental health in rural America, what to do when a friend or loved one is in a mental health crisis, and how to create a one-stop shop for mental health resources that growers can use to protect their own well-being. We recorded today's conversation on location at Warren Memorial Hospital with Mesfin Shibeshi DO, fellowship-trained orthopedic surgeon and Grace Speicher, Program and Operations Manager, Valley Health Orthopaedic Clinics, Southern Region. Dr. Shibeshi shared his knowledge and experience with respect to robot-assisted total knee replacement. Using this technology, Dr. Shibeshi is able to create a 3D model of a patient's knee before surgery, allowing for precise planning of incisions and implant placement. The robotic surgical technology offers a level of personalization and precision that can limit soft tissue damage, preserve bone and reduce post-surgical pain for some patients. Dr. Shibeshi is using this technology to aid in same-day total knee reconstruction procedures at Warren Memorial Hospital, the first Valley Health hospital to earn the Gold Seal of Approval® for Total Knee Replacement and Total Hip Replacement Certification from The Joint Commission. Grace explained why certification is important. Certification keeps everyone focused on providing high quality patient care by providing: Patient Education- patients are prepared for their total joint journey.  This is accomplished by distributing a total joint book to each patient, along with teaching what to expect prior, during, and after surgery.  This is a collaborative effort with the surgeon, nursing, therapy and case management involvement. Early Ambulation and Pain Control- Studies have shown the sooner a patient ambulates the better their recovery. In order to accomplish this, pain must be controlled.  In an effort to keep narcotic use at a minimum, they have what is called a multimodal approach which includes a block placed by the anesthesia team during the surgical process, along with a periarticular injection placed by the surgeon.  This approach allows for a longer pain control with less need for oral pain pills.  This is easier on the patient's stomach, as well as less of a threat for narcotic dependence.
