A meniscus tear can feel confusing because the diagnosis does not always make the next step obvious.  In this episode, Ted explains what the meniscus does, the difference between degenerative and acute tears, why tear location matters, and how to think about physical therapy, surgery, and long-term knee health.  For men over 40 dealing with knee pain, injuries, or the rest-reinjury cycle, this episode offers a practical way to understand the options and make a more informed decision. Listen to the episode now.    You'll learn:  Why meniscus tear location matters when deciding between rehab and surgery  The difference between degenerative and acute meniscus tears  When physical therapy may be the best first step for knee recovery  Why surgery still requires a proper progression back to strength, sport, and activity    Chapters:  (00:00) Introduction  (00:50) Meniscus Tear Options  (02:26) What the Meniscus Does  (03:44) Degenerative vs Acute Tears  (06:14) Tear Location Zones  (08:08) Physical Therapy Path  (09:35) R3 Restore Rebuild Return  (11:14) When Surgery Makes Sense  (12:10) Surgery Types Explained  (13:41) Rehab Is Nonnegotiable  (14:38) Wrap Up and Next Steps    Links Mentioned:  Legendarylifepodcast.com/knee

Today we're sharing a conversation from the Down to Earth archive. We talk to Kevin Watt, who at the time was strategic advisor at TomKat Ranch in Pescadero, California, about the the practice and benefits of regenerative agriculture, how to incentivize it, and the dire long-term consequences of the degenerative practices of industrial agriculture—and the evolution toward new ways of thinking about productivity, healthy food, and thriving on a crowded planet. We recorded at the 2019 Regenerate conference, and the program posted in January 2020.

Spine specialist, Dr. Casey Johnston shares an interesting case of a 58 y.o. triathlete with severe back and leg pain. Dr. Casey Johnston, D.C. is the owner and Clinical Director for Desert Springs Chiropractic. He has been in full time private practice since 2001. He is an alumnus of Oklahoma State University and Texas Chiropractic College. Prior to his work at DSCC, he worked as the Lead Chiropractor for multi clinic medical organization in Houston, Texas. During his tenure he trained and established chiropractic and rehabilitation protocols for the Chiropractic, Physical Medicine and Physical Therapy Departments for the various clinics. Dedicated to continually offering the most cutting-edge care, Dr. Johnston consistently trains in new and advanced techniques. Using the most advanced research, Desert Springs offers a dynamic system for assessing and reassessing each patient as an individual. Designing individualized care for each person and their individual needs is our number one priority. Taking time to fully understand each patient's unique situation has often provided success in complicated cases when medicine, physical therapy, surgery and even other chiropractors have been unable to help. Dr. Johnston and our staff use Gonstead Chiropractic corrective adjustments, coupled with Titleist Performance Institute (analysis, strength and conditioning) rehab protocols to build an individualized and effective plan of action for our patients. Dr. Johnston employs both hands-on and/or low force mechanical adjusting techniques depending on the individual patients needs and requests. Dr. J, as he is often called, has significant experience with over 18 years working with injuries to the spinal discs (herniations, bulges, degenerative discs, sciatica, etc.). After injuring his own L5/S1 disc, he developed protocols to help others recover based on his personal experience and success. He often employs Cox Flexion/Distraction (using the Cox 8 table/instrument), decompression, ELDOA and the McKenzie method for disc conditions. Often, we work in combination with primary care physicians, pain management doctors and/or surgeons to achieve the best results for our patients. Thank you for joining us for our 2nd Cabral HouseCall of the weekend!   I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Scott: Hello Dr. Cabral! I've been dealing with persistent brain fog and low motivation, especially in the mornings. I sleep 7–8 hours but still wake up groggy. What tests or protocols do you recommend to pinpoint the root causes and improve mental clarity naturally?                                                                                                                                                             Nela: Hi Dr. Cabral, I recently came across information on a treatment called Inuspheresis, which is described as a blood purification technology that filters out inflammatory proteins, environmental toxins and other harmful molecules. The materials I reviewed suggest it may benefit a wide range of conditions, including: - Chronic inflammatory disorders - Autoimmune diseases - Degenerative conditions Since I haven't heard you mention this treatment on your podcasts, I wanted to ask: 1. What is your opinion on the efficacy of Inuspheresis for the conditions listed 2. What do you see as the potential pros and cons of this therapy? I'd really value your knowledge and perspective! Thank you :)                                 John: Good day Dr. Cabral, I've been experimenting with cold exposure (ice baths, cold showers) to improve recovery and immunity. I've noticed a boost in energy but sometimes feel overly stimulated afterwards. How do you recommend people safely use cold exposure for health benefits without stressing the body?                                                                Jenna: Hi Dr. Cabral- I have a question about my 8 year old son. He has struggled with emotional regulation, big emotions, anxiety for a while. School was a struggle for him, he doesn't like to be away from me and he has anxiety about school work. We had him tested for ADHD, anxiety, Autism etc.. and they said he mostly just had anxiety. I don't want to give him medication but someone suggested methylfolate. That it's “normal with age” and irreversible? If you're stuck in pain despite trying physical therapy, meds, injections, or even surgery—you're not alone. But here's what most people don't realize: Pain from disc degeneration isn't always caused by damage. It's often driven by a hypersensitive nervous system, muscle guarding, and fear-based movement patterns that keep your body locked in protection mode. In this video, I'll show you a new path to relief that doesn't rely on fixing the disc itself—but instead focuses on reducing the sensitivity of your pain system, building movement tolerance, and restoring full-body strength and confidence. It's the same process I used to heal my own disc issues and help thousands of others do the same. You'll learn:- Why degenerative discs don't always equal pain- How to stop triggering flare-ups during everyday movement- The 3-phase process of the Hybrid Relief Method: Desensitize → Build Tolerance → Reclaim Resilience Gentle movements to start today—even if you've been in pain for years Enjoy! -William LOVING THIS CONTENT? Is Scoliosis Hereditary? Exploring the Genetic Link and Why It May Not Matter for Treatment Receiving a scoliosis diagnosis often sparks a cascade of questions:“How did I get this?”“Did I inherit it?”“What can I do about it?” In this episode of the Scoliosis Treatment podcast, Dr. Tony Nalda breaks down one of the most common concerns—whether scoliosis is hereditary—and explains why discovering the “why” may not always impact how we treat the condition. Family History vs. Genetic Cause Patients frequently share that scoliosis “runs in the family.” But does that mean it's genetic? Dr. Nalda explains that scoliosis tends to cluster in families, but no single gene has ever been definitively linked to idiopathic scoliosis. Even identical twins—who share the exact same DNA—may have different outcomes, with one developing scoliosis and the other not. This strongly suggests that while there may be hereditary tendencies, scoliosis is not directly caused by genetics alone. Different Types of Scoliosis (And Their Causes) To understand where heredity may play a role, it's important to look at the four main types of scoliosis: Idiopathic Scoliosis Makes up 80% of cases Cause: Unknown (idiopathic) Most often seen in adolescents Believed to be multifactorial, possibly involving environmental, developmental, and minor genetic influences Neuromuscular Scoliosis Associated with conditions like cerebral palsy, Marfan syndrome, or Ehlers-Danlos syndrome More likely to have a genetic or systemic origin Congenital Scoliosis Present at birth due to malformed vertebrae (e.g., hemivertebra) Truly genetic/developmental in nature Degenerative or Traumatic Scoliosis Caused by injury or wear and tear Often appears in adulthood Not hereditary—these result from physical changes over time Why “Cause” Doesn't Always Change the Outcome Dr. Nalda uses a powerful analogy: “If an earthquake causes a building to collapse, we may study the cause—but the most urgent need is to rebuild the structure.” Similarly, scoliosis—regardless of the cause—becomes a structural problem that must be corrected. Many cases are diagnosed long after the original cause (if any) occurred, and chasing the “why” doesn't usually change how the curve affects the spine today. Can Idiopathic Scoliosis Be Treated Without Knowing the Cause? Yes.Dr. Nalda emphasizes that effective scoliosis treatment doesn't depend on knowing the exact cause. At the Scoliosis Reduction Center, idiopathic cases are treated with a multimodal approach, including: Scoliosis-specific chiropractic care Customized physical therapy and rehab Scoliosis-specific exercises Corrective bracing These strategies focus on correcting the structural changes in the spine—not just the symptoms—and help stop curve progression over time. Bottom Line: Focus on the Solution, Not Just the Source While the question "Is scoliosis hereditary?" is understandable, the better question might be: What can I do now to manage it? Whether the root is genetic, environmental, or unknown, scoliosis can be treated effectively—especially when addressed early and proactively.

Meniscus tears have historically been treated with arthroscopy. However, compelling data has shown that this surgery does not work for degenerative meniscus tears. Physical therapy, conversely, has been shown to be a more effective measure for the degenerative meniscus tear, and is a more reasonable first-line measure compared to arthroscopic menisectomy. Still, the public's perception, guided by decades of advice from Sports Medicine Specialists that arthroscopy is the answer for all meniscus tears regardless of the chondral condition in the knee, has not changed with current evidence. This can create uncomfortable conversations with patients who have a degenerative meniscus tear and expect arthroscopy. Coming to us from the University of Buffalo, Drs. Nomi Weiss-Laxer and Leslie Bisson discuss their study examining patient perceptions and expectations of nonoperative treatment for degenerative meniscus tears.

Unveiling Scoliosis: A Lifelong Journey Through Progressive Stages In this episode of the "Scoliosis Treatment with Dr. Tony Nalda," we explore the progression of scoliosis through different life stages, focusing on how growth, hormonal changes, and aging influence the condition, with a particular emphasis on the impact of menopause on women. Scoliosis Overview and Types Scoliosis is a spinal condition characterized by an abnormal lateral curvature and is primarily classified into several types depending on the age of onset and underlying causes: Idiopathic Scoliosis: Predominantly found in adolescents, this type lacks a definitive cause and accounts for approximately 80% of all cases. It is most noticeable and diagnosed during puberty when children undergo rapid growth spurts. Neuromuscular Scoliosis: Linked to conditions like cerebral palsy or Ehlers-Danlos syndrome, this type stems from muscle or nerve abnormalities that affect spinal alignment. Congenital Scoliosis: This form arises from spinal malformations present at birth, typically involving abnormally shaped vertebrae like a hemivertebra. Degenerative Scoliosis: Often developing in adults over 50, particularly post-menopausal women, this type results from spinal degeneration and asymmetrical wear, commonly affecting the lumbar spine. Traumatic Scoliosis: Caused by spinal injuries, this type can occur at any age following significant trauma to the spine. The Impact of Menopause on Scoliosis Menopause significantly affects women with or at risk of scoliosis due to hormonal changes that influence bone density and spinal health. As estrogen levels decline, bones become more susceptible to compression and wear, which can exacerbate or trigger the onset of scoliosis in later life, particularly: Degenerative 'De Novo' Scoliosis: This form occurs in individuals without a prior history of scoliosis. It arises as the spine begins to deteriorate asymmetrically due to aging, compounded by menopausal changes. Progression of Existing Scoliosis: For women who had adolescent idiopathic scoliosis, menopause can lead to a rapid progression of their condition due to decreased bone density and increased vulnerability to spinal compression. Gender Differences in Scoliosis Adolescence: Girls are significantly more likely to develop scoliosis than boys during adolescence, attributed to earlier and more rapid growth spurts, which can quickly exacerbate spinal curvatures. Adulthood: Women are also more prone to developing scoliosis as they age, especially post-menopause, due to hormonal changes that affect bone strength and spinal integrity. Proactive Management and Treatment Early detection and proactive management are crucial across all stages of life but become particularly important during key phases such as adolescence and menopause: Adolescence: Early intervention can prevent the progression of scoliosis during growth spurts. Smaller curvatures respond better to treatment, making early action essential. Menopause: For post-menopausal women, managing bone density and closely monitoring any changes in spinal alignment can help control the progression of scoliosis. Conclusion Understanding scoliosis as a condition that can evolve with a person's hormonal and biological changes underscores the importance of lifelong monitoring and tailored treatment strategies. Whether it's during the rapid growth of adolescence or the hormonal shifts of menopause, proactive and informed care can lead to better management of scoliosis, reducing the need for invasive treatments and maintaining a higher quality of life. Before she had her son, Serena was already a pediatric SLP, business owner, and disability advocate who spent her time empowering families to understand and become more comfortable with their children’s disabilities. So, she was prepared for anything, or so she thought. But as it turns out, no amount of education or experience can prepare you for the moment your child receives a diagnosis for a disease that will eventually kill them. In this episode, I sit down with Serena Murison, SLP and owner of Play Spark, as she shares the story of discovering that her son Rudy has Lama2 Muscular Dystrophy, a disease that is progressive and life-limiting. Serena walks us through the emotional rollercoaster of her son’s first few months, as well as the conflicting feelings that have come up for her in the way that she both embraces her son’s disability and grieves the pain of knowing his disease will likely take his life.
