What defines the unique mindset of an emergency clinician? It's not just the fast pace or the chaotic environment—it's the deliberate, top-down thinking that prioritizes patient safety over diagnostic certainty. This approach, though deceptively simple, often flies in the face of traditional medical training, which emphasizes comprehensive differentials and exhaustive workups. In emergency medicine, knowing what the patient needs often matters more than knowing exactly what they have. In this episode, we explore the emergency medicine mindset, the pitfalls of the bottom-up approach, and why experienced clinicians focus on acute interventions and dangerous conditions. Finally, we discuss how humility and strategic communication with patients can make all the difference in mitigating risk and building trust.

During the episode, they discuss the use of handheld ultrasound for quicker patient care, why ultrasound offers better benefits to pain management, and why handheld ultrasound is the future of medicine. Topics discussed: What initially attracted Dr. Rosenblum to handheld ultrasound, and why he said "This is the future" when he encountered it. The benefits of having handheld ultrasound in a practice, including the time saved treating patients by having a wireless device, better image quality, and more portability. The patient experience with ultrasound, and how patients notice the superior care they receive from doctors who use ultrasound for injections over doctors who don't. Where the industry needs to improve their ultrasound education, and how to address fear and misunderstanding to close the gap between those who know how to use ultrasound and those who don't. Why it's time to abandon the old fashioned way of "fishing around" for nerves and use ultrasound to get injections correct each time, without added pain for the patient. What the future of handheld ultrasound might look like, the role AI will play in ultrasound training, and dream features that will help with patient care. How to increase the adoption of handheld ultrasound among physicians, especially as patient demand for it will only rise. Resources Mention:  Painexam.com NRAPpain.org

Women who experience acute aortic dissection—a spontaneous and catastrophic tear in one of the body's main arteries—not only are older and have more advanced disease than men when they seek medical care, but they also are more likely to die, according to research published online recently in The Annals of Thoracic Surgery.Benjamin A. Youdelman, MD, from Maimonides Medical Center in Brooklyn, New York, who was not directly involved in this research, explained that these variances in clinical presentation indicate that women may be waiting longer to seek medical care compared to men. This may be due to female patients being “stoic,” not considering their symptoms as signs of a significant problem, and not prioritizing their care.Overall, female patients had increased mortality, although, in the last few years, mortality between the sexes was comparable, which suggests recent improvements in care. According to Dr. Gleason, better recognition, earlier diagnosis, faster and more efficient care delivery, new and improved surgical techniques—including brain perfusion and reconstruction procedures—and subsequent longitudinal surveillance have all contributed to more lives being saved.Dr. Youdelman joins us today to discuss these findings, and how family medical history is a critically important factor for identifying patients at risk and saving lives. Since people began getting sick from COVID-19, there's been concern about having enough medical supplies of all kinds, including ventilators for very ill patients. While no one wants to be in a position where we need a ventilator, many of us don't know the risks that come with being placed on a breathing machine. And as we learn more about how to treat the coronavirus, some preliminary evidence suggests that in some patients ventilators have done more harm than good. Dr. Reuben Strayer has seen this phenomenon up close. He's an emergency physician and associate medical director at Maimonides Medical Center in Brooklyn, New York, where COVID-19 has taken hold. He himself has COVID-19, and as an expert on managing critically ill patients with airway and breathing problems, he is the perfect expert to answer the question "What do I need to know about ventilators in light of COVID-19?"
