Whelp, goodbye folks!  Eric and I have been DOGE'd. In a somewhat delayed April Fools, Nancy Lundeberg and Annie Medina-Walpole have taken over podcast host duties this week. Their purpose is to interview me, Eric, and Ken Covinsky about your final AGS literature review plenary session taking place at the Annual Meeting in Chicago this May (for those attending, our session is the plenary the morning of May 10).  We discuss our favorite articles, parody songs, and memories from AGS meetings past, with a little preview of a song for this year's meeting.   We covered: The first parody song I wrote, for AGS 2018 in Orlando, about this article by Nancy Schoenborn on how to discuss stopping cancer screening. In this episode, Dr Kat Ralston chats with Dr Imogen Smith about palliative care in hospital inpatients. They discuss what is different about the older adult when considering palliative care, including thinking about trajectories, communicating uncertainty, and parallel planning. They explore symptom control including a practical approach to syringe drivers (continuous subcutanous infusions) for the non-specialist. Dr Imogen Smith was appointed as Consultant in General Internal Medicine and Medicine for the Elderly at St John's Hospital, Livingston, in April 2020. Her clinical interests are in Hospital at Home, and Hospital Based Complex Clinical Care/Palliative Care. She is an educational and clinical supervisor and also clinically supervises and educates Advanced Nurse Practitioners in HBCCC/H@H contexts. During training, she was appointed to the first cohort of NHS Lothian Chief Registrars (2018-2019). And she sat on the RCPE Trainees and Members Committee (2018-2020). Functional decline is the decrease in physical and/or cognitive functioning and occurs when our aging loved one is unable to engage in activities of daily living. ADL’s or activities of daily living is a term used to collectively describe fundamental skills that are required to independently care for themselves such as eating, bathing and mobility. After a certain age, some level of decline should be expected year after year. In your forties and fifties, this decline is gradual. It happens slowly, and while it can affect our physical and mental performance, most are still able to live their lives comfortably. But in our sixties and beyond, decline begins to speed up and can become a little more noticeable. Eventually, our aging loved ones can reach a point of functional decline, where care is required for our aging loved one to live comfortably and safely. Unfortunately, it can be hard to spot functional decline. While decline accelerates in seniors, it still happens gradually enough that functional decline can go unnoticed. As a result, our aging loved ones who require elderly care in some form — either from family caregivers, in-home elderly care professionals, or facility living — end up without the support they need. How to Notice Signs of Functional Decline in Seniors. To determine whether or not your aging loved one may need elderly care or another senior service, you need to know the signs of functional decline. Keep in mind that functional decline relates to physical and cognitive function, so you need to be aware of the signs of decline for both. To help prevent this from happening, family members should be making a conscious effort to monitor their aging loved ones and track changes in their functional abilities. How to Notice Signs of Functional Decline in SeniorsOne strategy is to take stock of the things your loved one has difficulty with over the course of a typical week or month. You can do this mentally, or you might consider taking private notes. This will allow you to compare how your loved one is functioning now compared to the past. Gradual changes become much more visible if you can compare how well they’re doing now to six months or a year ago.Should you determine that your loved one is suffering from functional decline, you may wish to explore what extra care they will be needing now and in the future as their function declines. Finding recourse in your loved one’s budget and community can seem overwhelming but I’ll be posting a podcast here soon to help navigate through that. Thank you so much for joining me this week. This episode features Deidre Morgan (Palliative and Supportive Services, College of Nursing and Health Sciences, Flinders University, Adelaide, SA, Australia).   Functional decline can be anticipated for people with life-limiting illnesses. Trajectories of functional decline differ in shapes and patterns. Understanding patterns of functional decline has implications for patient care and design of responsive health services. This prospective study identifies two contemporary trajectories of functional decline for patients receiving specialist palliative care in the last 120 days of life. Precipitous deterioration in functional decline for cancers, solid organ failure and cardiovascular disease occurs as cohorts of patients approach Australia-modified Karnofsky Performance Status (AKPS) of 40. The pattern of functional decline for the neurological and dementias cohorts is flatter, showing a prolonged period of low function. Study findings highlight that different types of care responses and resource allocation may be needed at different time points in different trajectories. This may require rapid mobilisation of carer support and modification of care plans preceding a precipitous functional decline (Trajectory 1). Extended periods of support to maintain patient function and support carers are required for those with a prolonged slow rate of functional decline (Trajectory 2).
