On this episode of the Aluna experience podcast I talk to Dylan who shares how at 23 years old he was faced with a medical illness which brought him close to death and bed ridden for 2 years, what he now calls a soul initiation. Dylan shares his exploration of alternative healing modalities and finding ayahuasca where he experienced two very different nights with the medicine. Night one was a stripping of identities, wounds, and any foundation of self that was built on false beliefs. A painful but necessary clearing to create space for what was to come the second night. Dylan passionately shares how night 2 illuminated the many layers of self for Dylan— from his physical, to mental to spiritual to everything in between, above and beyond. I have noticed that in the Kali Yuga, although the last pillar supposedly is truthfulness, that seems to have been eroding quite quickly. I have noticed, however, that oftentimes people make public statements in a political setting, but then, when they're under deposition, they completely change their story because they know that in deposition under oath, you're liable for telling the truth and can be held accountable for it. Well, those brave souls who are accepting initiation are putting themselves under scrutiny. Best practices for hospital-based initiation of medications for opioid use disorder: A consensus statement JAMA Network Open This survey study used a 2-round Delphi process to develop expert consensus on best practices for hospital-based MOUD initiation for patients with OUD, with a goal to provide guidance on changing inpatient addiction treatment in response to increased synthetic opioids in the unregulated drug supply. A total of 42 expert clinicians participated; clinicians were considered a national expert if they had cared for at least 100 hospitalized patients with OUD in the last two years. There was consensus that buprenorphine and methadone initiation in the hospital setting were appropriate, with less support for hospital-based naltrexone initiation. Consensus was also reached to support rapid methadone initiation; high- and low-dose buprenorphine initiation; and provision of non-MOUD full agonist opioids for treatment of opioid withdrawal during methadone initiation, as a bridge to buprenorphine initiation, and for those declining MOUD.   Read this issue of the ASAM Weekly Subscribe to the ASAM Weekly Visit ASAM

Podcast Notes: The Most Overlooked Sexual Skill in Marriage Many married couples want deeper sexual intimacy but struggle with one surprisingly common issue: initiation. In this episode of Operation: Thriving Marriage, Bryon and Jen Harvey discuss why initiating sex often feels awkward, vulnerable, or emotionally risky—even in healthy marriages. While culture often portrays sex as completely spontaneous and effortless, real marriage rarely works that way. Most couples were never taught how to initiate intimacy in healthy, safe, and mutually loving ways, which can create misunderstanding, fear of rejection, and emotional distance over time. The Harveys explain how differences in desire styles—especially spontaneous desire versus responsive desire—can create confusion between spouses. One spouse may feel rejected when the other does not immediately respond with excitement, while the other spouse may simply need emotional connection, affection, or mental preparation before desire develops. This does not mean the marriage is broken or that attraction has disappeared. Instead, couples often need healthier expectations and better communication around intimacy. The episode also explores practical tools that can reduce awkwardness and pressure, including agreed-upon cues, scheduling intimacy, lowering the emotional stakes of “no,” and viewing sexual initiation as an invitation to connection rather than a demand for performance. Drawing from Scripture such as Song of Solomon 1:4 and the biblical concept of yada (“to know”), the Harveys remind listeners that sex in marriage is ultimately about intimacy, unity, and becoming one.
