Known by many terms over the years, paradoxical insomnia (or sleep state misperception) is at once common and rare, This is unfortunate because more awareness could improve the overall recognition and diagnosis or insomnia as well as improve treatment outcomes. God broke the hearts of Tim and Dawn for the lost people of South Asia—and called them to go and live in the region as ambassadors for Christ. Guest Jeff Nussbaum Jeff Nussbaum is an adjunct professor of Public Communication. Nussbaum is a partner in the speechwriting and strategy firm West Wing Writers. He has previously served as a speechwriter for Vice President Joe Biden, Vice President Al Gore, and Senate Majority Leader Tom Daschle. Most recently Nussbaum oversaw all speechwriting operations for the 2012 Democratic National Convention. In addition to his speechwriting experience, Jeff was the co-author and collaborator with James Carville on the 2003 bestseller, Had Enough? Jeff also collaborated with Senator Bob Graham on his book Intelligence Matters, which was published to critical acclaim in September 2004 and updated and re-released in 2008. As a co-founder of The Humor Cabinet, Nussbaum has also worked on humor speeches for dozens of elected officials and corporate executives and has served as a creative consultant for the Kennedy Center Mark Twain Prize for American Humor. Host Michael Zeldin Michael Zeldin is a well-known and highly-regarded TV and radio analyst/commentator. He has covered many high-profile matters, including the Clinton impeachment proceedings, the Gore v. This is: My experience experimenting with a bunch of antidepressants I'd never heard of, published by Luisa Rodriguez on October 19, 2022 on The Effective Altruism Forum. This isn't professional medical advice, it's just my experience and amateur knowledge. Summary I got moderately (and occasionally, severely) depressed about four months into the pandemic. I tried a bunch of things to treat it, including therapy, antidepressants, meditation, and a long list of other things. I was prescribed the antidepressant sertraline (aka Zoloft) by my NHS GP, and it really helped! But it had a number of side effects that made me very unexcited to stay on it. I experimented with a bunch of different types of antidepressants to see if I could find one that worked well and didn't give me bad side effects. I knew this would probably be a long and grueling experiment... and it was. It took over a year, and was really hard, both emotionally and physically. But I ended up finding a great one for me, and it now feels well worth the costs. I think this kind of experimentation might be a good approach for some people who also experience depression and anxiety. Caveats: This was expensive. It's probably much harder to do this without savings or great insurance, or both. I have a particularly supportive work environment and partner. It's probably much harder to do this without those things. Things besides antidepressants were also really important to feeling better (and also allowed me to stick with the experiment). Therapy and cognitive behavioral therapy (CBT) were extremely effective for me in treating my imposter syndrome and perfectionism, and in making my depression manageable. Searching for an antidepressant that worked for me probably wouldn't have gone nearly as well if I hadn't done it alongside these other practices. Antidepressants have not solved all of my problems — and it's important to set reasonable expectations for how much they can help. Context I got moderately (and occasionally, severely) depressed about four months into the pandemic. This was triggered by: The pandemic — including things like: feeling very isolated, not having much personal space, shitty British weather, having to work from home, not being able to see my family for years. Genetics — my mom has suffered from moderate-severe depression since she was in her early thirties. Imposter syndrome at work — I'd gotten a dream job, but it felt like, sooner or later, my employer would realize I was a total phony. I was afraid to do really any task that might reveal I was actually a big idiot, which made me anxious for a lot of my day-to-day activities. My main symptoms were: Having continuous low mood or sadness Feeling hopeless Having very low self-esteem, feeling like I was letting everyone down all the time Feeling tearful Feeling guilt-ridden Feeling irritable and intolerant of others Having little motivation or interest in things — finding myself and everyone else really boring Finding it difficult to make decisions Not getting much enjoyment out of life, including out of things that were previously some of my favorite activities (hiking, seeing close friends) Feeling anxious and worried Non-pharmacological things I did to treat my depression: Weekly therapy with an excellent therapist, who taught me a bunch about CBT (helped a lot) Weekly therapy with a couples therapist (sometimes my depression made me especially angst-y about my relationship) (helped a bit) Taking time off work (sometimes helped, sometimes made things a bunch worse) My depression was a factor in switching jobs (kind of helped) Changing my role at work significantly to focus on tasks I found especially exciting and satisfying (helped a lot) Spending time in sunnier countries (helped a fair bit) Regular exercise (helped a fair bit when I actually had ...

That's the grim reality of those with Epidermolysis Bullosa, or EB, a rare genetic connective tissue disorder which results in blisters and tears to the skin being created from even minor contact or friction. The range of complications this causes for almost every normal activity – from eating to bathing to sleeping – is daunting, leading our guest today, Brett Kopelan, to call EB “the worst disease you've never heard of.”  He should know. Brett and his wife have the equivalent of fulltime jobs just managing daily care for their daughter Rafi, who suffers from a severe form of EB.  Advocating for Rafi led to Brett becoming Executive Director of debra of America, the only national not-for-profit providing all-inclusive support for patients battling the disease.  In this revealing discussion with host Shiv Gaglani, Brett opens up about his frustrations with the health insurance system, the challenges of raising money for rare diseases, and why he feels there is a need for more doctors to specialize in rare disorders for patients over twenty-three.  Brett shares some positive news as well about several promising gene therapies on the horizon. “I can say that the past two years has really been the first time that I've felt I'm working for my child's life, not for beyond her. In 400-plus episodes of The Dale Jr. Download, guests have brought a lot of Earnhardt stories to the table. But on this week's edition the mold has been broken as Dale Earnhardt Jr. and co-host Mike Davis welcome longtime MLB player, coach and team manager Ned Yost. After being drafted by the Milwaukee Brewers in 1974 and playing in the majors for a handful of years, Ned retired during the ‘85 season. When he was at home in Mississippi figuring out his next move in life, he received a call from the Hank Aaron that would change the trajectory of his life forever, inviting him to be a coach for the upcoming pitching prospects of the Atlanta Braves. It was during his time with the Braves organization that he first made the acquaintance of Dale Earnhardt Sr. Ned and friend Jody Davis had made plans to go deer hunting and Davis invited Dale Sr. to come along. Ned and Dale Sr. became fast friends, bonding over their love of hunting and the outdoors and Ned would become a confidant for the Intimidator for years to come. Dale Jr. and Mike pick Ned's brain about the transition from being a player to a coach, and he explains about how his time with Ted Simmons on the Brewers helped teach him the nuts and bolts of the game. When it came time for Ned to try his hand at team management, he leaned on his experiences of working with Bobby Cox, longtime manager of the Braves. And when he needed to turn a losing organization into a winning one, he depended on the lessons he learned from a brief stint of working on Dale Sr.'s race team in ‘94. Ned recalls the instance that occurred during the ‘94 MLB strike, where Dale Sr. invited him to come on the road with his Richard Childress Racing No. 3 team as they chased their seventh championship. While Ned merely worked as a “rehydration engineer”, he gained first person insight into what it takes to win at the highest level. The education came in handy as he went on to win his first World Series title with the Braves the following year as a bullpen coach. Another lesson that Dale Sr. instilled in Ned was that you never leave a winning team for a losing one. Ned explains that during a hunting trip in Texas, Dale Sr. was talking about his ongoing contract negotiations for the following season. When Ned suggested that he drive for his own start-up D.E.I., Dale Sr. explained that they weren't ready to win yet. Ned pondered eventually having to leave the Braves to get his shot at managing a major league team and Dale Sr. implored him to stay put, explaining that winning is everything. The time came for Ned to leave in 2002 when he was made team manager of the Brewers. Ned breaks down the time it takes for a young organization to grow confident in their ability and start winning consistently, about a two-and-a-half-year process. Unfortunately, he was let go by Milwaukee before this concept came to fruition, but he found another opportunity to prove the system when he became manager of the Kansas City Royals in 2010. Ned would have the last laugh, as the Royals would win the ALC Pennant in 2014 and the World Series championship in 2015. The first thing Ned thought of was his old friend and how proud he would be of the persistent road to success. This episode has many never before heard Earnhardt hunting stories, as well “the rest of the story” of some of the most memorable tales told on The Download.
