Erst mal SORRY für die verspätete Veröffentlichung. Aus privaten Gründen musste ich das hochladen auf Sonntag verschieben, aber unser Podcast-Provider macht grad zicken. Wir arbeiten dran! Und wenn ihr das hier lest haben wir eine Lösung gefunden. vermutlich aber auf Kosten der Qualität - schon mal sorry! Hoffentlich lösen wir auch die Design-Probleme mit dem Blog.... Heute ist einfach so ein Tag... Wir gehen an die Anfänge der Reihe. Die erste Folge von Regisseur Reiner Cluthe, die zweite Folge, die je aufgenommen wurde und die dritte Folge in der Chronologie: Das sicherste Gefängnis der Welt. Die zweite Folge, in der es um keinen Mord geht. Und eigentlich um gar kein Verbrechen. Vielleicht ist das der Grund, warum wir unsere unfreiwillige Rekordjagd, immer längere Folgen zu produzieren, eingestellt haben. Die Folge ist ja gerade zu ein Quickie im Vergleich zu den vorherigen. Aber keine Angst, wir haben alles im Detail besprochen was es zu besprechen gibt. Zum Beispiel: Holger hat einen durchschnittlichen Kopf. So saß Al Capone nun eigentlich ein? Wie sehen eigentlich die Rohre im Gefängnis aus? Sind Ratten gleich Ratten? Wie oft stört der Notdienst den Podcast. Warum gibt Erik eigentlich immer so wenig Doktorhüte? Hat das was mit der Abwesenheit von Mike zu tun? Was hat Koser von Futrelle übernommen? Und was nicht? Was sagt ihr zu der Folge? Was ist Euer Eindruck? Ich habe heute keine Worte, die ich für dich bzw. euch sprechen kann ... Denn diese Woche ist unerwartet ein zutiefst geliebter Mensch gestorben und ich kann nicht wie gewohnt weitermachen oder gar einfach funktionieren. Das Schreiben gelingt noch ganz okay. Aber viele Zeilen werden es nicht. Meine Idee ist: Du kannst in dieser Folge atmen und spüren, dass du am Leben bist. Und du kannst Anteil nehmen und mir, allen Trauernden und dem Universum eine Minute lang Kraft und positive Energie senden. Darüber freue ich mich sehr. Eigentlich wollte ich eine Minute Stille aussenden. Leider hat dabei der Podcast-Provider nicht mitgespielt. Du kannst dir genauso also auch einen Wecker auf eine Minute stellen und es ohne die Hintergrundmusik machen.

Relation of vascular tone to pleth wave amplitude and variability. Using the pulse ox waveform to confirm mechanical capture during transcutaneous pacing. Odds and ends... Pulse oximetry is more than just a measurement of oxygenation: The pulse oximetry plethysmograph is a pulsatile waveform that can be thought of as an arterial waveform. However, this waveform is influenced by cardiac output and systemic vascular resistance. Thinking of the SpO2 pleth as a pulsatile waveform, variations in the patients cardiac output should be transmitted to the fingertip arteries, which results in variations in the pulse ox waveform amplitude. The implications of this are far reaching. In patients who are mechanically ventilated, cyclical changes in intrathoracic pressures can produce measurable variations in the pulse oximetry waveform of a preload dependent patient, which suggesst a potentially fluid-responsive patient. Do we really need to remove fingernail polish? You can save your department some money because it's unlikely to affect the SpO2 measurement to any clinically relevant degree. However, if you are concerned, just turn it sideways. Is this the silver bullet of measuring fluid responsiveness, non-invasively? No. At least not yet. The majority of studies evaluating pulse oximetry utilization in this application were conducted under controlled conditions, such as mechanically ventilated patients with consistent respiratory changes in a proper sedation. Additionally, pulse oximetry waveform variations can be difficult to measure during increased systemic vascular resistance (How many patients on mechanical ventilation also require vasopressor support?). Then there's the issue with variations in vascular tone. The fingertip is much more susceptible to these changes as opposed to the vessels in the forehead, nose, or ear.
