Poyss, RN, Ph.D., CNL, APRN-BC, FAMSN works at Drexel University, College of Nursing & Health Professions since1992. She was the Track Director for the MSN Clinical Nurse Leader Track. Particular clinical expertise is gastrointestinal surgery, nutrition support and nutrition screening of adults and elderly. She received her postmaster`s certificate as an Acute Care Nurse Practitioner, and certification as an Acute Care Nurse Practitioner and Clinical Nurse Leader. She holds certification from ANA as a medical-Surgical nurse Specialist. She is co-founder of the Academy of Medical-Surgical Nurses, a specialty nursing organization created for medical-surgical nurses. In 2024, Dr. Poyss was inducted into the inaugural group of Fellows, of the Academy of Medical-Surgical Nurses (FAMSN). Dr. Poyss is currently practicing in a primary medical group managing palliative care for Elderly homebound patients. She has taught in both undergraduate and graduate Nursing programs in three Universities. Dr. Poyss's research interests in the clinical areas include nursing intervention/outcome studies, and nursing treatment/outcome studies. She has participated with the Iowa Nursing Intervention Classification project and authored two nursing interventional labels for the project. Other research interests include program evaluation, and effects of alternate teaching styles with student learning. Funded research includes Evaluation of an Alternative Care Delivery System in Critical Care Nursing and preventing readmission for CHF patients to Acute Care.   MEET OUR CO-HOSTS Samantha Bayne, MSN, RN, CMSRN, NPD-BC is a nursing professional development practitioner in the inland northwest specializing in medical-surgical nursing. In this podcast episode we want to introduce you to our BCEN Friend, Lionel Lyde. Lionel Lyde is a retired Military Nurse with 35 years of service in the armed forces. He holds his MHA MBA, and CEN. Lionel has taken on clinical, leadership and executive leadership positions during his 31-year nursing career, spending most of his clinical time as a critical care and ER/Trauma Nurse. His last position being an Air Force Critical Care Master Clinician and Special Military Consultant to the Air Force Surgeon General for Critical Care Nursing. He retired from military service 2022 but has certainly not retired from nursing. He's currently the Director of Nursing for a group home and manages a free clinic. His current professional goal is to network primary care clinics that provide care to low income and indigent patient populations. This goal is part of a greater aspiration to facilitate access to the healthcare system and decompress community emergency departments. Come along as Michael Dexter and Hollye Briggs talk with Lionel about his career in military nursing and leadership. Bloodstream infections (BSI) are the most deadly healthcare associated infections, and 35% of these are caused by intravenous (IV) therapy through various types of IV catheter [1] . Such ‘catheter-associated BSI (CABSI)' substantially increase risk of death, prolong ICU and hospital stay, and healthcare costs. Nurses are responsible for many aspects of catheter insertion, management, troubleshooting, and removal. To prevent CABSI, we must deny microorganisms entry into the blood via both the: (1) catheter insertion site, and (2) internal catheter lumen. This presentation reviews the concept of CABSI, core fundamentals of infection prevention, and reviews recent evidence from large randomised controlled trials. All patients are at risk for neurologic deterioration, whether from a primary neurologic process, or from metabolic complications associated with systemic organ failure. Notable change in the level of consciousness during serial neuro exams is the most sensitive indicator of neurologic decline. Nurses performing a thorough neurologic assessment will examine consciousness and cognition, language and speech, the cranial nerves, and movement and sensation. Although the neurologic assessment can be complicated, using a standardized approach allows the rapid identification of neurologic decline allowing for rapid intervention to prevent further neurologic deterioration. Gastrointestinal dysfunction occurs in up to 60% of intensive care patients and almost 20% suffer from three or more gastrointestinal symptoms. Persistent GDF in this setting results in the enteral feed delivery of less than half of the estimated caloric requirement in these patients putting them at risk of hospital-acquired acute malnutrition and adverse complications including organ failure. The role of the gut in the genesis of organ failure has been recognised for decades. Gut injury occurs in many intensive care patients, where splanchnic vasoconstriction is present to maintain perfusion of heart, lungs, kidney, liver and brain. The intestine is extremely sensitive to ischaemia, and this results in hyperinflammatory response secondary to gut hypoperfusion leading to mucosal atrophy, increase in intestinal permeability, bacterial overgrowth and translocation. Shortening the interval would increase the translation rate of scientific advancements into health care improvements. This presentation will review the pipeline for research translation to improve health, differentiate implementation science from other implementation activities, identify different approaches for implementation in critical care, and lastly, to identify competencies and strategies to enhance knowledge translation. Filipinos are known to be a proud and resilient race. We are highly regarded as hospitable, hardworking, compassionate, and caring. And we are deeply faithful and have strong family ties. Today, the Philippines continues to be the primary source of professional nurses to the US and other countries. Due to the pandemic, the healthcare industry is the most vulnerable when it comes to the virus since they care for and are in direct contact with COVID-19 patients. In September 2020, the largest nursing union in the US found that 67 out of 213 registered nurses who died of COVID-19 are Filipinos. And that is an astounding rate that shows how Filipino nurses are disproportionately affected by the pandemic.Despite the heightened risk and all the hardships, everyone in the front line continues to fight COVID-19. Professor Leanne Aitken is Professor of Critical Care at City, University of London. In this role she is responsible for leading research and scholarship in acute and critical care nursing as well as implementing her own programme of research that focuses on recovery after critical illness and injury and a range of clinical practice issues within critical care. Other responsibilities include teaching, supervision of research students and leadership of new developments within the discipline of nursing at City, University of London.   Professor Aitken holds a visiting appointment with Griffith University in Australia and is an Ambassador for the World Federation of Critical Care Nurses. She is a Fellow of both the American Academy of Nursing and the Australian College of Nursing as well as a Life Member and Fellow of the Australian College of Critical Care Nurses. She is also a Fulbright Alumnus after receiving a Fulbright Senior Scholarship to undertake research examining recovery after trauma at the University of Pennsylvania, Philadelphia.   Professor Aitken has published more than 120 original publications in peer reviewed journals and edits the textbook, Critical Care Nursing.

We are incredibly fortunate as registered nurses to work within a profession that can offer so much diversity. Whether you’re working in critical care areas such as intensive care, or emergency; in the operating theatre; on a cruise ship; in a prison; in rural and remote areas; as an educator; and the list can go on and on. There is one area of nursing that I believe a lot of people have thought of at one time or another; I know I certainly have; that can offer excitement, travel, autonomy, challenge and diversity, career progression, you name it, this job has it. On today’s episode, I'll be interviewing Nick Coleman-Hicks, who currently works as a Flight Nurse, with the Royal Flying Doctor Service. Hailing from Sydney, Nick has worked within areas such as Intensive Care, and Emergency, and has completed a variety of  post graduate qualifications including a Graduate Certificate in Critical Care Nursing; and a Graduate Diploma of Midwifery, and has gained some relevant experience practicing as a midwife, which is a vital skill to work with the RFDS. Today, Nick elaborates on how he got into nursing; his incredible career thus far; his experiences as a student midwife; the benefits of working in both ICU and ED; and goes into detail of what his job entails as a Flight Nurse with the RFDS.

I was first introduced to Reiki when the Director of Critical Care Nursing, and Associate Director of Integrative Nursing at the OSU Med Center came into a class and discussed the practical use of this alternative form of healing through a lecture. In my research I have found that Reiki is a vibrational, or subtle energy, therapy most commonly facilitated by light touch, which is believed to balance the biofield and strengthen the body's ability to heal itself. Although systematic study of efficacy is scant thus far, Reiki is increasingly used as an adjunct to conventional medical care, both in and out of hospital settings. I am very interested in alternative forms of medicine and healing and had to find the best Reiki practitioner in the area. After researching, I found Linda Haley who is a master reiki practitioner, teacher, and an incredibly passionate and fulfilled individual. She has been practicing for decades and has found a way to mix her passion, her purpose, and her profession all in one space that she has created from the ground up. I am incredibly grateful I had the opportunity to connect with such a wise leader who approached her work and life from an incredibly spiritual lens. I learned a lot recording this episode and hope you will appreciate Linda’s message as much as I did.

Odds and ends... Pulse oximetry is more than just a measurement of oxygenation: The pulse oximetry plethysmograph is a pulsatile waveform that can be thought of as an arterial waveform. However, this waveform is influenced by cardiac output and systemic vascular resistance.  Thinking of the SpO2 pleth as a pulsatile waveform,  variations in the patients cardiac output should be transmitted to the fingertip arteries, which results in variations in the pulse ox waveform amplitude. The implications of this are far reaching. In patients who are mechanically ventilated, cyclical changes in intrathoracic pressures can produce measurable variations in the pulse oximetry waveform of a preload dependent patient, which suggesst a potentially fluid-responsive patient. Do we really need to remove fingernail polish? You can save your department some money because it's unlikely to affect the SpO2 measurement to any clinically relevant degree. However, if you are concerned, just turn it sideways. Is this the silver bullet of measuring fluid responsiveness, non-invasively? No. At least not yet. The majority of studies evaluating pulse oximetry utilization in this application were conducted under controlled conditions, such as mechanically ventilated patients with consistent respiratory changes in a proper sedation. Additionally, pulse oximetry waveform variations can be difficult to measure during increased systemic vascular resistance (How many patients on mechanical ventilation also require vasopressor support?). Then there's the issue with variations in vascular tone. The fingertip is much more susceptible to these changes as opposed to the vessels in the forehead, nose, or ear.
