Patient from Hell
Episode 69: A Physician's Journey as a Cancer Caregiver with Dr. Jean-Luc Neptune

Patient from Hell

Cancer care is difficult enough to navigate as someone with little to no healthcare/biology background, but what does it mean when a physician faces similar challenges within the healthcare system? Dr. Jean-Luc “JL” Neptune shares his compelling caregiver story of caring for his mother and father's cancer diagnoses simultaneously with the help of his sister. We delve into how his experience impacted his career, his outlook into the flaws of the healthcare system, and his perspective on what patients (and caregivers) can do to get the most out of their care.  Key Highlights: 1. Access to contextualized information is not easy; information online without context can be overwhelming and not specific to the patient. 2. The best source of information about your diagnosis and prognosis is likely a doctor in the field, but finding one with the time to fully inform you is not an easy feat either. Fee-for-service care is the current healthcare model, which arguably promotes quantity over quality. However, the tide is slowly but surely shifting towards value-based care. 3. To achieve the highest quality care today, the best thing you can do is advocate for yourself and seek multiple expert opinions. Keeping track of your symptoms is a good idea for effectively and efficiently contextualizing your treatment plan with your provider(s). About our guest: Jean-Luc (“JL”) Neptune, MD MBA is a physician executive, digital health leader, investor and advisor with 20+ years of experience in the health innovation space. Dr. Neptune most recently served as the CEO, Chief Medical Officer, and co-founder of Suntra Modern Recovery, which provided early addiction intervention and treatment services at scale. Prior to Suntra Modern Recovery Dr. Neptune was the founder of Athletik Health Inc., which operated modern sports medicine clinics offering the “athlete in everyone” outstanding clinical care and customer service. Dr. Neptune served as entrepreneur-in-residence at the Hospital For Special Surgery (“HSS”), the #1 rated orthopedic hospital in the United States, working closely with the leadership team of the HSS Innovation Institute. Dr. Neptune was also General Manager at Blueprint Health where he oversaw investments into dozens of digital health companies (including RubiconMD, NexHealth, Moving Analytics, and CleanSlate UV), and continues as a partner in the Blueprint investment funds. Dr. Neptune earned a BA from Columbia College at Columbia University, an MD from the Columbia College of Physicians and Surgeons, and an MBA at the Wharton School. Dr. Neptune completed a residency in internal medicine at New York Presbyterian Hospital and is licensed to practice medicine in the state of New York. Key Moments: At 10:55 “I've often debated with people online about access to information and really as a patient, should you be like going on the internet and looking at all these sites? And my argument is you shouldn't because unless you can contextualize this information, make it specific to you and cut out all of the noise from the signal, you're probably only making yourself more stressed out, more confused by seeking information from all these places.” Visit the Manta Cares website  Disclaimer: This podcast is for general informational purposes only and does not constitute the practice of medicine, nursing or other professional health care services, including the giving of medical advice, and no doctor/patient relationship is formed. Tanya:  That’s Carlos Reines, Harvard grad and entrepreneur who raised $20 million for his startup RubiconMD, an eConsult platform that connects clinicians to top specialists. Now serving over 5,000 primary care physicians in 37 states in the US, RubiconMD has saved over 2.5 million days of patients waiting for specialist consults, which turns out to be almost 7,000 years. Carlos:  I grew up in Madrid. Actually, all my family is in Madrid. I was born and raised there, lived in the center of the city for the first few years, and then we went to the suburbs when I was about 5 or 6. I’ve really been in Madrid most of my life. When I was 22 is the first time that I went to study abroad. Through different work experiences and educational experiences, I’ve ended up spending time in the Netherlands, in Germany, and obviously, a lot of time in the US. Tanya:  If you were to describe yourself as a kid, how would you describe yourself? Carlos:  I was the only child, only nephew, only me, no cousins, for ten years. My mother is the oldest of six. On my dad’s side, his uncle never had children, so for about ten years, I was there by myself. I was getting a lot of love from the entire family. Tanya:  I can imagine.  Carlos:  At the same time, I was dying to get some folks around. I wanted to have cousins. I wanted to have siblings. In Spain, there is a big tradition in Christmas that the Three Magic Kings will bring you games and toys, and when I was a kid and I would write my letter, I would never ask for any games or toys. I just wanted to have a brother or a sister. I wanted to have that for a long time. It never happened, so I grew up as an only child. At some point, my parents had friends that lived outside of Madrid, and their son moved to Madrid for college. He was about five years older than I am, and he went to live with us. For three years, all of us had – I had an older brother, and it was an awesome experience.

Gil Ado and Carlos Reines are the co-founders of Rubicon MD, an innovative medical eConsult service that connects primary care doctors to same-day insights from top medical specialists. Their mission is to provide better health care through their online platform that enables primary care doctors to easily and quickly access medical specialists and have online consultations (eConsults) and ultimately provide better direct care to patients. With a fragmented US healthcare system, Gil and Carlos identified a need in the market for access to specialist medical care that didnt require patients to physically be in the same room with their specialist doctor. After a lot of networking and hustle, Rubicon MD was officially birthed in 2013. >> Show notes Link: https://www.businessgenerals.com/gilandcarlos (https://www.businessgenerals.com/gilandcarlos) >> Today Rubicon is a strong, growing Venture backed Healthcare startup, which has successfully raised over $19M in funding since 2014. The company is scaling at pace throughout the US now with over 5,000 primary care providers using the platform. The two Harvard Business School graduate co-founders partnered up to launch Rubicon on a mission to facilitate the provision of a higher level of care to patients who may not have access to specialist care due to them not having private healthcare or geographical limitations. The platform allows general practitioner doctors to have direct access to expert specialist help on a case by case basis through e-consult process. Doctor to doctor interaction via the e-consult platform is one of their biggest wins – patients that would have previously been referred to the emergency department for specialist consultation now receive access through the eConsult process through their primary doctors, and as Gil says this is real tangible benefit that clinicians and patients are excited about. The business launch and the first paying customer In 2013 the business was launched with the first key goal being to prove that there was value in the platform and that patients would be able to access greater healthcare through the platform They put a basic minimum viable product from a technology platform standpoint, they found a doctor who was willing to trial the platform and it run for a few months with a few hundred eConsults and the feedback was very positive The founders joined an accelerator program focusing on how to build a business around the idea and how to find the people who could potentially buy into your idea Talked and networked to as many people in the healthcare eco system  – learning, doing the research and also selling the idea at the same time Discovered that groups that are at risk, ie taking care of primary health care patients will be most interested in this Pivoted on the fact that there was limited supporting data for the idea so focused on finding the companies that were ready to take some risk in this space as well Their first paying customer contract was from Boston, Gil and Carlos share with excitement how this client has grown and stayed with Rubicon MD for over 4 years, a testament in their minds to the value RubiconMD brings to the clinician. Cash flow during the start up phase Carlos was still completing his https://www.hbs.edu/Pages/default.aspx (Harvard MBA) at the beginning but working full time in the business, Gil was also full time in the business but they were scraping by and keeping everything very lean financially It was a difficult beginning for the business, so they had to run their operations in a very lean way but they were determined to see the vision come to life The duo managed to live through the first startup phase through their savings, family support and just sheer tenacity to make it through the early days Access to funding 2013 accessed their first seed money from an angel investor, this was enough to run their pilot After this started...

Listen as Dr. Jeffrey Gold shares his vision for the future of primary care and passion for delivering care the way it was intended - through trust, openness, and investing in the doctor-patient relationship. The ModernMD: Dr. Jeffrey Gold Dr. Jeffrey Gold is the Founder of Gold Direct Care, one the first Direct Primary Care practices in Massachusetts. A family physician by training, he attended the University of Massachusetts Medical School. After completing his residency in Arizona, he moved back home to Massachusetts and worked for the North Shore Physicians Group. Jeff was continuously frustrated with the traditional system and with a passion for improving care deliver and patient experience he launched Gold Direct Care. Success Quote: “If opportunity doesn't knock build a door.” - Milton Berle White Coat to Business Suit: Jeff never saw himself as an entrepreneur, but grew up in small retail business family and was inspired by his parents and grandparents. Jeff completed his residency training in family medicine and joined a traditional primary care practice after graduation. During his work in the traditional system he saw the role of doctors as assembly line worker with limitations to perform their greatest skills of creative problem solve and building strong patient-doctor relationships. Jeff was frustrated with the current system and decided to launch Gold Direct Care! Why Direct Primary Care (DPC)? Traditional system of primary care is complex with increased demands on coding and shorter visit times. Direct primary care lets you build a high quality practice with simple payment, longer visit times, and better doctor-patient relationships. Advice for launching DPC practice: Physicians are not well training in the business of medicine. Surround yourself with people who you trust and can provide complementary skills and knowledge. Admit when you don't know something. You must have your WHY but you might need help with the HOW! Build two models scenarios when you are getting started:  Your Best & Worst case scenarios.
