Access to quality healthcare shouldn't depend on your zip code, yet for many underserved communities, barriers remain.In this episode of Straight Out of Health IT, Christopher Kunney interviewed Christopher Cruttenden, CEO of Safety Net Connect (SNC), a company providing digital healthcare tools like eConsult to improve access to specialty care for underserved populations. SNC connects medical, behavioral, and social service providers to enhance equitable healthcare access. Cruttenden highlighted major challenges in providing equitable care, including medical and food deserts, time constraints for working families, and long wait times. He emphasized the burden on patients, especially those with chronic illnesses and other socioeconomic challenges.Regarding technology's role, Cruttenden advocated for collaborative, patient-centered solutions integrated into healthcare workflows. He noted the importance of AI in supporting physicians, particularly through AI-assisted dictation, coding support, and quality assurance to enhance efficiency. By improving access to care, reducing ER visits, and enabling timely interventions through tools like eConsult, the cascading effect leads to better patient outcomes and system-wide savings. It facilitates remote consultations, allowing specialists to provide timely input, reducing wait times and preventing unnecessary specialist visits. In this month's episode we visit Dumfries and Galloway, the latest stop in our trip around the regions. We speak with Dr Neil Kelly, a familiar voice on the SNUG podcast, and discuss life in the South of Scotland and how things are going with preparations for GP system change, how the local project team and facilitators have become  confident in managing the change, and the importance of supporting staff during the transition.  Neil has reduced his GP leadership workload and developed a new interest in farming, but still has plenty of reflections on the evolving nature of general practice, the increase in telephone consultations, the occasional trivia of eConsult requests and why video consultations haven't taken off. We discussed some of the difficulties with integrating systems like digital dictation, ECGs, and spirometry into the new IT environment, due to budget limitations. He's impressed by Heidi Health, which shows promise in saving time through note-taking and digital dictation, and also discusses the potential benefits of Digital Dermatology, with his practice being one of the early adopters. Neil remains optimistic about the future of general practice in Scotland, contrasting it with the more corporate approach in England. He points out the positive impact of additional support staff like pharmacists, mental health nurses, and the importance of having GPs handle complex tasks. Despite fears of GPs being replaced by other practitioners, Neil believes GPs remain highly efficient and cost-effective. Listen to the end for some wine and book recommendations! One of the big areas of interest at the SNUG members' day in May was the increasing use of digital systems for communicating and consulting “asynchronously” and how these are changing the face of general practice. In this episode, we have highlights from two of the talks discussing issues around online consulting. Thomas Dodd is a training partner for AskmyGP and he discussed some of the popular myths about online consultations – for example that patients prefer face to face rather than online contact, yet online services will also submerge GPs with new demand. Dr David Cooper, the co-chair of SNUG and a user of eConsult, describes why he is a big advocate of online consulting and why his practice has found it hugely beneficial in maintaining services throughout the pandemic. We pose the question: are we now at a similar moment in weighing up the pros and cons of providing a new means of access to care online, as doctors were in the 1880s when the advent of the telephone also offered new and better access to medical care? I don’t think they had to break that too much. Tanya:  That’s great, so they followed the wonderful Spanish tradition on naming.  Carlos:  Yeah, absolutely. Carlos:  I grew up in Madrid. Actually, all my family is in Madrid. I was born and raised there, lived in the center of the city for the first few years, and then we went to the suburbs when I was about 5 or 6. I’ve really been in Madrid most of my life. When I was 22 is the first time that I went to study abroad. Through different work experiences and educational experiences, I’ve ended up spending time in the Netherlands, in Germany, and obviously, a lot of time in the US. Tanya:  If you were to describe yourself as a kid, how would you describe yourself? Carlos:  I was the only child, only nephew, only me, no cousins, for ten years. My mother is the oldest of six. On my dad’s side, his uncle never had children, so for about ten years, I was there by myself. I was getting a lot of love from the entire family. Tanya:  I can imagine.  Carlos:  At the same time, I was dying to get some folks around. I wanted to have cousins. I wanted to have siblings. In Spain, there is a big tradition in Christmas that the Three Magic Kings will bring you games and toys, and when I was a kid and I would write my letter, I would never ask for any games or toys. I just wanted to have a brother or a sister. I wanted to have that for a long time. It never happened, so I grew up as an only child. At some point, my parents had friends that lived outside of Madrid, and their son moved to Madrid for college. He was about five years older than I am, and he went to live with us. For three years, all of us had – I had an older brother, and it was an awesome experience.

Yee Jr, Ateev Mehotra, and Paul Giboney. The paper describes and analyzes data from an e-consult system that was rolled out to a network of hundreds of safety-net clinics in Los Angeles County. We are thrilled to have the lead author, Michael Barnett, join us for our discussion! Michael is an Assistant Professor at Harvard T.H. Chan School of Public Health and a primary care physician at Brigham and Women’s Hospital. This week, James is joined by Mark Harmon, who originally studied pharmacology before becoming a buyer for M&S - he was there for 7 years travelling the world. He then worked for Arcadia Group, which includes the likes of Miss Selfridge and TopShop, opening up flagship shops in places like Rio, and because loves sport, he moved to speedo to set up Speedo Beach, which was a business within a business that grew to about $20M in 3 years - he had a team in Australia and some other incredible beach locations around the world, before getting headhunted by Nike to run their $100M sports apparel business globally and worked with the likes of Wayne Rooney, Roger Federer and Rafael Nadal. Mark then had a very moving personal experience when his son born unwell and his priorities in life change pretty much overnight so he went on to become the oldest medical student through Kings College Medical School, graduated as a doctor and went on this great journey to combine his business and medical acumen to now work for eConsult - a healthtech company that streamlines primary and urgent care through an online consultation platform. www.econsult.net

The company is scaling at pace throughout the US now with over 5,000 primary care providers using the platform. The two Harvard Business School graduate co-founders partnered up to launch Rubicon on a mission to facilitate the provision of a higher level of care to patients who may not have access to specialist care due to them not having private healthcare or geographical limitations. The platform allows general practitioner doctors to have direct access to expert specialist help on a case by case basis through e-consult process. Doctor to doctor interaction via the e-consult platform is one of their biggest wins – patients that would have previously been referred to the emergency department for specialist consultation now receive access through the eConsult process through their primary doctors, and as Gil says this is real tangible benefit that clinicians and patients are excited about. The business launch and the first paying customer In 2013 the business was launched with the first key goal being to prove that there was value in the platform and that patients would be able to access greater healthcare through the platform They put a basic minimum viable product from a technology platform standpoint, they found a doctor who was willing to trial the platform and it run for a few months with a few hundred eConsults and the feedback was very positive The founders joined an accelerator program focusing on how to build a business around the idea and how to find the people who could potentially buy into your idea Talked and networked to as many people in the healthcare eco system  – learning, doing the research and also selling the idea at the same time Discovered that groups that are at risk, ie taking care of primary health care patients will be most interested in this Pivoted on the fact that there was limited supporting data for the idea so focused on finding the companies that were ready to take some risk in this space as well Their first paying customer contract was from Boston, Gil and Carlos share with excitement how this client has grown and stayed with Rubicon MD for over 4 years, a testament in their minds to the value RubiconMD brings to the clinician. Mr. Ahmed is a true innovator and pioneer in healthcare information systems. He has unparalleled experience in all aspects of using computer technology and web-based applications to deliver the best quality of medical care to patients, facilitate collaboration among multiple and diverse medical care providers and systems, and increase the efficiency of healthcare environments. He is also a highly respected consultant on innovation, and frequently lectures about entrepreneurism and information technology for healthcare environments. Mr. Ahmed was formerly the director of health information technology and innovation for L.A.
