Tommy John is a name associated with the game of baseball not just because of the pitcher's Hall of Fame career, but because it has become synonymous with a devastating injury to the Ulnar Collateral Ligament of the elbow that continues to plague baseball pitchers often resulting in reconstructive surgery colloquially known as the Tommy John Surgery. Multiple risk factors have been postulated to put the UCL at risk of rupture including increased velocity, torque across the elbow, baseline stiffness in the shoulder and/or elbow, pitch volume, and pitch type. Well known for their extensive work in baseball science, Dr. Glenn Fleisig, PhD from the famed American Sports Medicine Institute in Birmingham, AL and Dr. Peter Chalmers, MD from the University of Utah in Salt Lake City join us to discuss their study that looks at these risk factors in professional baseball pitchers.

It's July 17, 1974 and Dodgers pitcher Tommy John is in the midst of his best season in baseball. He's 13-3 with an ERA of 2.59 and helping lead Los Angeles to first place in the NL West. And then in the 3rd inning that day against the Montreal Expos, one pitch changed his season, his career and the course of baseball history. One pitch. Tommy John was hurt, and while they tried resting his sore left elbow, that wasn't doing the trick and on September 25, 1974, Dr. Frank Jobe performed the surgery and when he was asked what it was called he said, “Ulnar collateral ligament reconstruction of the elbow using ipsilateral palmaris longus tendon graft”. Understandably, he grew tired of having to say that and referred to it as "the surgery I performed on Tommy John." And soon enough, Tommy John surgery was part of the American lexicon. Now, 50 years after undergoing this groundbreaking surgery, Tommy joins us on the Past Our Prime podcast and talks about that time in his life, the rehab, and the comeback that allowed him to win 164 games after returning from the surgery named after him. His 288 career wins is more than anyone but Roger Clemens who is not in the Hall of Fame, and the 188 No Decisions he endured are the most in baseball history... but the true irony of TJ not being a Hall of Famer -- yet -- is that had he not had to have Tommy John surgery, he certainly would have gotten to 300 wins... the golden ticket to Cooperstown. Answering a Question About a Deep Ulnar & Median Nerve Laceration Injury. In this Podcast we talk about Nerves! I received a question about a 33 year old male with a deep laceration affecting his ulnar nerve, artery and median nerve as well. Become a CHT Click Here Lacerations of the ulnar and median nerves can cause significant injury and functional impairment. The ulnar nerve and median nerve are two major nerves in the arm that provide sensation and motor control to the hand and fingers. When these nerves are lacerated, it can result in a variety of symptoms depending on the location and severity of the injury. Symptoms of ulnar nerve laceration may include: Loss of sensation: Laceration of the ulnar nerve can lead to numbness or tingling in the little finger and half of the ring finger. Weakness or loss of muscle control: Damage to the ulnar nerve can cause weakness or paralysis of the muscles that control finger movements, resulting in difficulty with gripping objects or manipulating the hand. Claw hand deformity: Severe ulnar nerve injury can lead to a claw-like deformity, where the fingers are unable to fully straighten. Watch on YouTube!

You compared wrist injuries to low back pain Low back pain has so many different causes and ulnar wrist pain is so similar You do not always have to have all of the answers on your first interaction. Using a differential diagnosis is super helpful. I think it is either this or this…here are some options for both of these. What are the red flags for ATs to be referring? Ulnar wrist pain is fairly forgiving If you think the DREJ is unstable then you really need to get fixed quickly On the ulnar side are the differences in age? Yes, all of our joints wear down over time and those needs to be considered. We see traumatic injuries as well as chronic issues. Recovery in age differences In older patients we are astarting to see some wear in the joint and lengthens the recovery time. The wrist has to be unloaded in some of the older patients the you would not with younger patients. Reinjury risk? Pediatric patients are more likely due to the constant motion This can be trick since the imaging show the repair. You really have to look critically at this point. How can we help prevent reinjury of the TFCC? Strengthening helps Pronator strengthening ECU work Most of the stuff we do not have control over except for the gymnastic population due to the constant weight bearing on their wrists. Should we be limited movement in gymnastics? It would help but its nearly impossible. These kids tend to live with some amount of pain and ignore but it eventually catches up with them. What should we be teaching students about interprofessional communication? Develop a personal relationship with the people around you. ATs can benefit by connecting with hand therapists to help improve the outcome for the patients. Getting a congruent plan benefits all parties.
