Our next AAOS 2024 Annual Meeting poster is titled The Statistical Fragility of Platelet-Rich Plasma in Lateral Epicondylitis: A Systematic Review and Simulated Fragility Analysis of Randomized Controlled Trials. Our podcast tries to stay far away from statistical analysis and methodology breakdown; however, this poster draws attention to an important issue. Often in orthopedics we try to use statistically significant findings –defined as those with a p value of less than .05 – from Level I randomized controlled trials to guide our clinical decision making. This poster suggests that is not all we should be looking at, particularly as it pertains to the use of PRP for lateral epicondylitis.

Platelet-rich plasma (PRP) has been used extensively in clinical practice to treat patients with symptomatic knee osteoarthritis (OA). Leukocyte-poor PRP (LP-PRP) has been clinically preferred over leukocyte-rich PRP (LR-PRP); however, it is unclear which cytokine mediators of pain and inflammation are present in LR-PRP and LP-PRP from patients with mild to moderate knee OA in order to rationalize a specific formulation.   In conclusion, although numerous studies have demonstrated the excellent safety profile of PRP in treating patients with knee OA symptoms, a specific formulation has yet to be determined. Although challenging to conduct, clinical trials are needed that incorporate a mechanistic approach in all study arms in order to assess the effect of all components in PRP formulations (growth factor profile, leukocytes, RBCs, platelet dose) that not only are responsible for improving symptoms but also could contribute to disease-modifying effects. Our results expand on the current literature and demonstrate novel findings in that LR-PRP that was neutrophil-rich was predominantly anti-inflammatory compared with LP-PRP with reduced neutrophil concentration in patients with knee OA.   To read the article, click here.

Robert Bowers, DO, PhD, joins the show to discuss PRP (platelet-rich plasma), baseball medicine, the role of the team physician, and the future of orthobiologics and sports medicine. Dr. Bowers is an attending physician at Emory Sports Medicine and the director of the ⁠Emory Baseball Medicine Program⁠. As a former Division 1 college athlete himself, he brings a unique understanding of sports-related injuries to his patients. He currently serves as team physician for the Atlanta Braves and the Georgia Tech baseball team. He is also the head team physician for the College Park Skyhawks, the G League affiliate of the Atlanta Hawks, and Woodward Academy.  He attended Furman University in South Carolina where he played Division 1 college baseball and majored in health and exercise science. He then attended graduate school at Auburn University where he earned a Master of Education in exercise science and a PhD in exercise physiology. Following graduate school, Dr. Bowers earned his medical degree from Philadelphia College of Osteopathic Medicine. He completed his residency training in physical medicine and rehabilitation at Emory University School of Medicine, where he served as chief resident.
