Ben Zimmer, CEO of Priovant Therapeutics, focuses on developing therapies for rare autoimmune diseases specifically dermatomyositis, which affects the skin, muscles, and organs, and non-infectious uveitis, a severe ocular inflammatory condition. While these conditions are symptomatically different, mechanistically, they have features in common related to the underlying pathology of the diseases. The drug in development is an oral once-daily therapy that addresses the inflammatory conditions and cytokines driving the pathology. Ben explains, "During COVID, there was a lot of talk about the cytokine storm and these are molecules involved in immune cell signaling. So, they're basically ways that different types of immune cells signal to each other to do different things. There's a large variety of different cytokines and some autoimmune diseases. There are only maybe one or two cytokines or a small number that are driving the pathology of the disease."   "Both dermatomyositis and non-infectious uveitis are highly inflammatory conditions with a lot of different pathogenic cytokines, a lot of different inflammatory axes involved. And so, one of the neat things about our drug, brepocitinib, is that it works on a set category of enzymes called JAKs, and then there are four different types of JAKs, JAK1, JAK2, JAK3, and you might guess JAK4, but it's actually called TYK2. And our drug inhibits TYK2 and JAK1. And by doing that, it suppresses the signaling of a large number of different cytokines." "Uveitis and dermatomyositis have a number of overlapping pathogenic cytokines involved, as well as some different cytokines, but they are both suppressed through the inhibition of TYK2 and JAK1, which our drug does. Today, I have a special guest called Peggy joining me to share her story of overcoming the health challenges she faced with her horse Cami.Peggy lives in Alabama. She has two horses, one of which is a 36-year-old Arabian Quarter Horse cross. The other is Cami, a 13-year-old Oldenburg Quarter Horse mare. I began working with Peggy in 2021 when Cami had several challenging health conditions.Initial Contact and Reason for AppointmentAfter hearing me on a podcast, Peggy initially contacted me to help her 36-year-old horse, Sterling, who was struggling with FICO water syndrome. During our discussions, she realized that her other horse, Cami, who was dealing with chronic uveitis, could also benefit from my understanding.Background on CamiPeggy bought Cami in 2020 during the pandemic. She found her online and learned that Cami had only been away from her breeder for about eight months. Initially, she had a stress-free life on 40 acres with a herd, but her next home was stressful. She was kept in a stall 24/7 with little outdoor time and no herd contact, which was a drastic change for her.Initial Health ConcernsWhen Peggy brought Cami home, she knew she had eye issues, and the vet confirmed a minor irritation. Peggy initially managed it with medication, even considering the possibility of removing the eye if necessary. However, as Cami settled in, various other health issues emerged, including weight gain, crusty neck, and anxiety.Treatment and MedicationsPeggy tried multiple medications to manage Cami's eye issues, including oral anti-inflammatories and ointments. Taking her to an ophthalmologist was challenging due to her anxiety around trailering, so Peggy had to manage treatments at home with my help and with that of her vet. It required significant teamwork and research.Initial Attitude and GoalsPeggy was determined to save Cami's eye, feeling that surgery was not a viable option due to her reactive state and the stress it would cause her. She began using positive reinforcement and clicker training to help her administer the medication, which allowed us to manage the symptoms but not solve the underlying problem.Holistic Approach and RealizationsReducing stress and holistically addressing equine health is essential for managing chronic conditions like uveitis. Peggy explains that Cami's health conditions worsened in a less stressful environment as her body began to express the underlying imbalances.Health Deterioration and ProgressPeggy recalls how Cami developed a cataract and went blind in one eye after a severe reaction to annual vaccinations. That led to a visit from an ophthalmologist and required intense medication to save the eye. That period was the beginning of our understanding of how her hypersensitivity led to complex health issues.Managing Multiple Health IssuesCami's immune system was hypersensitive. She reacted to various triggers like vaccinations, ant bites- and even organic fly sprays. Although the increased sensitivity was a sign of her body healing, it also added a layer of complexity to her care.Initial Treatment StrategyMy first treatment program for Cami focused on improving her digestion, regulating her insulin and blood sugar, and reducing oxidative stress rather than directly addressing the eye issue. Peggy began to notice improvements in her overall health, including weight loss, a shinier coat, and healthier hooves.Importance of Detailed ObservationRegular photos helped Peggy monitor changes and effectiveness of treatments and helped her make informed decisions regarding Cami's care. Daily observation and documentation were also required to track her progress.Milestones...

Uveitis is a big concern for horse owners because it can lead to blindness and, in severe cases, even result in the horse losing an eye. Even in traditional medicine, the exact cause of this complex and chronic condition remains elusive. So, today, I approach uveitis from a holistic perspective, exploring its various facets and potential treatments.Understanding UveitisUveitis, indicated by the suffix itis, denotes inflammation, specifically chronic inflammation of the uvea, which is one of the layers of the eye. The uvea comprises three structures- the iris, ciliary body, and choroid, which could get damaged in uveitis cases.Possible Causes and Holistic ApproachThe causes of uveitis are diverse and often unknown. A holistic approach considers dietary, lifestyle, and nutritional interventions to reduce inflammation and potentially save the affected eye. Many believe uveitis is triggered by infections or autoimmune responses, leading to continuous inflammation and tissue damage.Genetic PredispositionAppaloosa horses are eight times more likely to develop equine recurrent uveitis, indicating a genetic predisposition. Despite that, horse owners should focus on mitigating the condition with an appropriate diet and lifestyle changes to reduce inflammation and support tissue repair.Stress and ToxicityStress and toxicity are significant triggers for uveitis flare-ups. Stressful events, like separation from the herd or lifestyle changes, can exacerbate the condition. Toxins from medications or vaccinations can also overload the immune system, leading to increased inflammation.Free Radical DamageFree radicals cause considerable damage to tissues, contributing to conditions like uveitis. All right now on to the show.   01:37 Hey everybody, it's Dr. Sam and I want to take a question today from a listener. She's got high eye pressure, but she doesn't have glaucoma. So I want to go into the causes and what you can do to bring your eye pressure down. So in ocular hypertension, there's a consistently high eye pressure reading that the doctor measures, but there's no optic nerve damage or visual field loss. Usually it's a problem with either the aqueous production or the drainage in the eye. And this is what causes the high eye pressure. So even though I am a believer in the epigenetics model, which says that the genes are expressed, based on our environment, our diet lifestyle, there is a link genetically if your parents or grandparents have had glaucoma, you do have a higher risk of having either glaucoma or in this case, ocular hypertension. I've also seen that as you get older, and you have more free radical damage or inflammation in the eye, there's a higher risk of ocular hypertension and or glaucoma. And in ethnicity, African Americans tend to have more glaucoma or ocular hypertension. And so these are things to note if you fall into any of those categories. So I've done many video blogs and podcasts on primary open angle glaucoma, narrow angle glaucoma, you can find those on my website or YouTube or Facebook. But today because the question is about ocular hypertension, I want to talk about some other causes. One of the causes could be taking either oral or topical corticosteroids. So when you take steroids, this can artificially raise the eye pressure. And so you have ocular hypertension, if you're taking any kind of corticosteroids. I've also found that eye trauma or eye injury can cause ocular hypertension. And this trauma can change the fluid dynamics and flow in the aqueous. And this is another reason why I've seen ocular hypertension. I've also seen that certain eye inflammatory diseases like iritis and Uveitis can drive the eye pressure up. And even secondary glaucoma like pseudo exfoliation, glaucoma, which has these flakes that can deposit in the drainage areas of the eye, which also cause ocular hypertension. You know, another thing that I asked a lot about is what kind of medications you're taking, because certain medications can actually cause your eye pressure to go up. So let's go through the list. We have things like decongestants anti depressants, any histamines so in treating this condition and I want to talk more holistically are using integrative medicine. Check out today's questions:    Heather: Hi, me again! I also had a quick question about the natural flavors listed on the ingredient list for the Daily Fruit and Vegetable powder. What is it? I was under the impression that natural flavors are just another word for artificial flavors but I doubt you would put that in any of your products. Thanks for taking the time to answer our questions!   Amal: Hi Dr Cabral, I've been diagnosed with an inflammatory eye condition called Uveitis in my left eye. Doctors have not looked into what's causing it but I think it is autoimmune. I also seem to have something called lipedema. Everytime I try to detox my body, it will accelerate the inflammation in my eye, causing further damage to the eye. I tried to take iodine and it would help my eye but cause me to rapid fat gain everywhere. I then tried N acetyl cysteine and the same rapid weight gain but it would lower inflammation in the eye and clear my vision. Then I tried chlorella and spirulina and I would feel coolness in my eyes in 10 mins and it would clear my floaters, inflammation and improve vision. However I will gain a lot of body fat within 2 days of taking the chlorella & spirulina. I came across gynostemma herbal tea and I started losing weight within days but my eye would start hurting and I could also feel weakness in my right eye. I'm not sure what to do. I cant lose the weight without hurting my eye and I can't detox my liver or the rest of my body without gaining weight. My hair has become damaged and dry and I have developed a lot of cellulite on my legs and excess fat around my ankles and everywhere else. I am trying to work on gut health Any advice would be helpful.    Alicja: Hi Dr. Cabral, I would love to get your opinion about inducing period with prescription drugs. I lost my periods a while ago, but I'm only 38 so probably too early to be menopausal. 4 years ago I was pregnant and then was breastfeeding my baby for a while. It's been about 2 years since I stopped breastfeeding, but my cycles don't want to return. After having my baby I developed gut issues. Later I also found out I have hypothyroidism and Hashimoto. I've been addressing both. Overall I feel now much better and stronger comparing to where I was a year ago (thanks to moving to a new house and thanks to your protocols. I've done para pro and currently I'm in the muddle of CBO protocol) I should mention that I am underweight (BMI: 16.2) Since I've been feeling better recently, do you think I could now "induce" my period with a medication? Or do you think I should still wait longer until my body decides it's time. I'm not sure if it's good for me to be that long without cycles. I often hear from other women that once first period is induced, next ones come on their own. I tried Vitex in the past but stopped because I didn't feel well taking it.
