I get into why physicians are so far behind on this, historically we've been independent, and you can't unionize as a practice owner or contractor, and why we're catching up now that companies like Apollo MD, Sound Physicians, SCP Health, and even Optum are employing us. After the break, another eye drop recall. This one is a big one. 2.5 million bottles of prednisolone from Lupin Pharmaceuticals recalled after a foreign substance was found. Prednisolone is one of the most commonly prescribed drops in ophthalmology and standard after cataract surgery, so this one is going to be felt. I almost finished my July 4th call week. Good news! Portland did not blow its face off this year. No open globes, no eyelid lacerations, no thermal injuries, no sclopetaria. Bad news: a Friday holiday means every unanswered call from every patient at every one of our offices lands on the on-call doctor, and I earned every one of them. I walk through three cases. First, a cataract surgery my partner had to abort mid-procedure because the patient had six clock hours of zonule laxity. I explain what zonules actually do, why pseudoexfoliation ruins your day, and how a viscoelastic-driven pressure spike at 1 AM sent me to clinic to "burp the wound" instead of shipping the patient to the ED. Second, an inpatient consult for suspected optic disc swelling, and why nobody outside ophthalmology reliably examines an optic nerve. Third, the classic infectious disease vs. ophthalmology debate: does every asymptomatic patient with candidemia need a dilated eye exam? IDSA 2016 says yes. The American Academy of Ophthalmology says no. I'll tell you where I actually land and why I still show up anyway. Also: my full case for licensing fireworks rather than banning them, and a little pushback on the once-a-year concern for veterans and dogs. Takeaways: Zonule laxity, loose or broken suspensory fibers that hold the natural lens in place, can force a cataract surgeon to abort mid-procedure and refer to a retina specialist for a safer staged approach; pseudoexfoliation is one of the most common causes The viscoelastic gels used during cataract surgery can plug the eye's drainage system and cause an IOP spike that lasts up to 72 hours; oral Diamox is standard, but if the patient can't keep it down, options are IV Diamox in the ED or manually burping the wound in clinic Ophthalmologists are effectively the only clinicians reliably trained and equipped to examine the optic nerve, which is why hospitalist consults for suspected disc swelling should be treated as a normal part of the job, not an imposition The IDSA (2016) recommends dilated exams for all non-neutropenic candidemia patients within one week of diagnosis; the American Academy of Ophthalmology recommends against routine screening in asymptomatic patients. This week we're joined by Kendra Cunningham and Kayla Messana, sisters and co-hosts of the podcast Flatlined and Fine, and their stories are almost impossible to believe. Kayla was 31, healthy, three kids, husband just back from deployment, when she woke him up with agonal breathing in the middle of the night. He dragged her to the floor, did CPR for 10 minutes, and she was shocked twice by an AED before going on to have cardiac arrest approximately eight more times in the next 24 hours. Four years later, she's been shocked by her ICD three additional times and still has no clear diagnosis. Then, a few years after Kayla's event, Kendra had her own cardiac arrest while awake and feeding her newborn twins, told her husband she was about to faint, and was gone within seconds, saved by a man whose only CPR training was a few American Heart Association videos he'd watched before their first child was born. Kristin and I swap notes with them on everything: what it's like to wake up in an ICU with no memory of what happened, how you explain cardiac arrest to kids who are five, six, and eight years old, the psychological weight of an idiopathic diagnosis that gives you no answers and no closure, and the very real experience of being a 31-year-old in a cardiology waiting room surrounded by people forty years older than you. We also get into the insurance disaster of a $15,000 whole exome sequencing test that Blue Cross Anthem denied, why Kendra couldn't pick up her newborn twins for months after her ICD surgery, and why I was cleared to perform eye surgery but not allowed to drive for six months. I also get publicly shamed for not having plugged in my ICD data transmitter in two years. It's earned. Takeaways: Cardiac arrest in young, healthy adults often has no identifiable cause. CPR saves lives even when the person doing it is terrified and untrained. The co-survivor experience is its own separate, underserved story. Young cardiac arrest survivors often get inadequate guidance from the healthcare system. I'm on call July 4th for the first time in what feels like forever, and I have fully lost perspective on what call should feel like. 2018 me, fresh out of an Iowa trauma center, would be horrified at how nervous private practice me has gotten about it. I'll report back after the holiday. Today's main event is World Cup eye injuries. Every professional sport needs an ophthalmologist on the sideline, soccer included, and I walk through three I'd be ready for. First, a 50 MPH ball off Erling Haaland the Viking's foot straight to the face, hyphema, open globe risk at the equator and limbus, retinal dialysis in young patients. Second, a player smashing their face on a goalpost, orbital floor fractures, the workup for extra-ocular muscle entrapment, and a full breakdown of eyelid laceration repair: above brow, below brow, eyelid margin, and canalicular. Third, the soccer flop, where grass exposure can deliver a corneal abrasion or an allergic reaction, and the drops I reach for. I've got more on deck if the tournament keeps giving me material. In practice, you can't slash physician pay without first addressing the $500,000 in debt that gets people into the door. The Australian model, a real public safety net coexisting with a private system, makes more sense to me, but only if we fix the training pipeline first. After the break, ophthalmology. Our guest this week is Mark Kerstens, a PhD student at OSU, who studies black-backed woodpeckers with the hope of improving forest management before and after wildfires. Black-backed woodpeckers are ecosystem engineers, creating habitat for many other forest-dwelling species, and Mark's work is helping to inform natural resource managers about what these charasmatic birds may be doing.

Victor Wembanyama, the 7'5" alien playing for the Spurs, apparently refuses to look at blue light after 9pm, which allows me to get into my love-hate relationship with blue light glasses, the misinformation campaign that has them treating everything from dry eye to macular degeneration, and the one claim that actually holds up: sleep and circadian rhythm. Then a great Patreon question drops me into retinopathy of prematurity. What it is, why preemies are vulnerable, and how oxygen, vascularization, and laser treatment all fit together. From there, the bigger problem: 90% of U.S. counties don't have a single pediatric eye specialist. Four states, New Mexico, North Dakota, South Dakota, Vermont, have zero pediatric ophthalmologists. And surprise, this comes right back to private equity, because every Knock Knock Eye episode does. Takeaways: Blue light blocking glasses have one evidence-based benefit: improving sleep by helping regulate circadian rhythm. Claims about reducing dry eye, eye strain, or preventing macular degeneration are not supported by the data. Retinopathy of prematurity is caused by a mismatch between an underdeveloped retina and the oxygen exposure premature infants receive — too much supplemental oxygen damages fragile capillaries and can lead to neovascularization, retinal detachment, and blindness. 90% of U.S. counties have no pediatric eye specialist, and New Mexico, North Dakota, South Dakota, and Vermont have zero pediatric ophthalmologists at all, driven by low reimbursement rates and consolidation that deprioritizes the subspecialty. Private equity-owned ophthalmology practices aren't expanding into pediatric care because it isn't profitable, kids need Medicaid coverage, not cataracts and LASIK, leaving rural and Medicaid families with the fewest options. A 2025 JAMA Ophthalmology study on AI screening for ROP found the model was sensitive but not specific, it can flag disease, but it still can't determine which babies actually need laser treatment, so human ophthalmologists remain essential. I'm not in a great mood and I'll tell you why. A mini fridge in our family room sprang a slow leak, and before I knew it I was $5,000 deep in a restoration black box with no idea whether any of those numbers were real. Which, of course, got me thinking about healthcare cost transparency, because everything eventually does. From there I get into Oregon's SB 951, the strongest corporate practice of medicine law in the country, and what it might mean in 2029 for the private equity firms (and one suspiciously friendly-sounding Canadian pension fund) currently buying up ophthalmology. After the break I cross over into veterinary land to talk about the cow eye dissection I ran with about 50 middle schoolers at my kids' school. These aren't flattering portraits, yet they reveal a profound truth—God isn't annoyed by our persistence; He's honored by it. The sermon unpacks six potential barriers to answered prayer: unconfessed sin, unforgiveness, dishonoring our spouse, failing to actually pray, praying with wrong motives, and missing that God might have something better planned. Each barrier invites honest self-examination. Are we quick to confess? Do we harbor bitterness? Do we honor our spouse as a co-heir of grace? On this week's Talkhouse Podcast, we've got an interesting pairing—two guys that hadn't met before, and who come from different parts of the arts world, and who are both pretty damn great at their jobs. It's Bill Callahan and Michael Imperioli. Bill Callahan made music under the name Smog for about 15 years, releasing an incredible catalog of gorgeous, mostly minimal, often wandering songs delivered in his unmistakable baritone. He's one of the most profound lyricists of the past 30 years, crafting careful little worlds that are often funny and heartbreaking at the same time. In 2005, he dropped the Smog moniker and started releasing records under his own name, but there's no obvious break in the continuity. It's a fantastic body of work and you can start almost anywhere: I'd recommend 1999's Knock Knock as an entry point, or maybe 2007's A River Ain't Too Much Too Love, which comes up in this chat. But you could just as easily jump in with Callahan's fantastic new one, My Days of 58, which was finished after a scary cancer battle. In spite of that—or maybe because of it—it's one of Callahan's most relaxed and charming ever. Check out “Why Do Men Sing” right here. Today's other guest is instantly recognizable to fans of The Sopranos as Christopher Moltisanti, the tragic character that surely wouldn't have been nearly as indelible without Michael Imperioli's genius-level portrayal. You've seen him in a million other things, too, from Goodfellas to Summer of Sam—which he also co-wrote—to The White Lotus. What perhaps you didn't know about Imperioli is that he's also a musician. He's the singer/guitarist in a band called ZOPA, which plays catchy, straight-ahead rock that sounds deeply rooted in New York City, which Imperioli still calls home. He also wrote a novel, which comes up in this chat with Callahan, and he's a practicing Buddhist, another topic of conversation. Even though Callahan and Imperioli didn't know each other, they were clearly fans of each other's work. In this chill chat, Michael tells Bill about the exact moment he first heard one of his songs. They also get into Bill's health scare and how it inspired his new record, Michael's kids and what they're up to, and there's a lot of talk about Lou Reed—a guy that inspired them both.
